The necessity of determining stone density by non-contrast computerized tomography (NCCT) before extracorporeal shock wave lithotripsy (ESWL) is a controversial topic due to the radiation exposure. We aimed to investigate whether stone density is helpful in predicting the success of ESWL in pediatric patients or not. In this retrospective study, database of a single center was used to identify 232 children aged between 2 and 16 years. Patients with abnormal renal anatomy, distal obstruction, a known cystine stone disease, a previous history of an intervention regarding stone, and an insufficient follow-up period (< 3 months) were excluded from the study. A total of 209 patients were included in the study (94 with NCCT, 115 without NCCT). Groups were compared in terms of stone size, stone location, and stone-free rate at 3 months after a single ESWL session. The mean age was 6.17 ± 3.27 years and 120 (57.4%) of the patients were male and 89 (42.6%) were female. Mean stone size was 11.7 mm in NCCT group and 12.3 mm in non-NCCT group (p 0.128). The complete stone clearance rate in NCCT and non-NCCT group at 3 months after ESWL was 57.4% (54/94) and 54.7% (63/115), respectively, and there was no statistically significant difference (p 0.316). In conclusion, unnecessary NCCT use should be avoided before ESWL considering the similar success rates after ESWL and the risk of exposure to radiation., (© 2022. In this study we aimed to determine the effects of previous open renal surgery, percutaneous nephrolithotomy and extracorporeal shockwave lithotripsy (ESWL) on the results and complications of subsequent percutaneous nephrolithotomy (PCNL) in pediatric patients. We reviewed the files of all 105 patients with 116 renal units (RU) undergoing PCNL at two main institutions between December 2000 and February 2014. The 116 RUs were divided into four groups: primary PCNL patients with 44 RU (37.9%) were categorized as group 1, 29 RU (25%) with a history of failed ESWL on the same side were categorized as group 2, 23 RU (19.8%) with previous PCNL surgery were categorized as group 3 and 20 RU (17.2%) with open renal surgery were categorized as group 4. Patient characteristics, mean operative time, mean fluoroscopy time, time to access the collecting system, hemoglobin change, number of more than 1 access, stone-free rates, postoperative hospitalization time and complications in four groups were compared. There were no differences between the groups in age, sex, mean stone size and stone laterality. Mean operative time, mean fluoroscopy time, time to access the collecting system, hemoglobin change, complication rates, number of more than one access hospitalization times were similar in the each group (p > 0.05 for each parameter). The stone-free rates after PCNL were 81.8% in group 1, 79.3% in group 2, 78.3% in group 3 and 80 % in group 4 (p = 0.67). Percutaneous nephrolithotomy is effective and safe in children who have previously had open nephrolithotomy, history of ESWL or PCNL without no more complications than are seen with primary PCNL of kidneys and with similar success rate.

Objective: To compare dorsal and ventral dartos flap outcomes for tubularized incised-plate urethroplasty (TIPU) in terms of success, complication rates and cosmetic appearance in adult circumcised hypospadic men., Methods: Forty-two circumcised adult men underwent primary distal or midpenile hypospadias repair for cosmetic reasons by a single surgeon in a prospective randomized trial. Twenty-two patients were randomly selected for TIPU repair with dorsal (Group1, mean age 22.3 years) dartos flap as second layer. Twenty patients were randomly selected for TIPU repair with ventral dartos flap (Group2, mean age 21.1 years). Satisfaction questionnaires were answered by patients before surgery and at postoperative 6 weeks. The operating surgeon also filled in the form blindly at the same postoperative period. The differences between pre and postoperative satisfaction scores for each group were also evaluated. Preoperatively, all cases were evaluated with uroflowmetry and compared with postoperative results measured at 6th week of surgery. P < 0.05 was considered as significant., Results: Mean follow-up was 24 months (4-28). Complications were encountered in 18 and 20% of the patients in groups 1 and 2, respectively. Complications were urethrocutaneous fistula, meatal stenosis, retrusive/proximal meatus and residual curvature. Success rates were 82 and 80%, respectively. No differences were seen in overall satisfaction between the two groups (P = 0.07)., Conclusion: Outcome of hypospadias repair in circumcised adults for cosmetic reasons is similar to uncircumcised hypospadics previously mentioned in the literature. Both dorsal and ventral flaps can be used in previously circumcised hypospadic adults with comparable satisfaction and complication rates and uroflow findings.

Purpose: To compare the results of a new disposable clamp (SCD) used for routine circumcision in our department, compared with the conventional dissection tecnique (CDT) in infants and children., Methods: The SCD and CDT were evaluated prospectively, in terms of the duration, complication rate and postoperative pain assesment. The cosmetic result and parents' satisfaction were evaluated after 6 weeks. A total of 200 boys were included in the study (with a median age of 4.45 years)., Results and Conclusions: The median operative duration was 10 min less for the SCD (18 vs 8 min; P < 0.001). There was no difference in complication rates for both groups. The cosmetic results assessed by a blinded urologist were better for the SCD group (P < 0.001). The parents' satisfaction score for the procedure was similar in both groups, as 8 out of a scale up to 10 filled in by the parents. Circumcision with the SCD is quicker and leads to a better cosmetic results than with the CDT, without increasing morbidity.
