Although there is a current ban in many countries on the injection of liquid silicone, paraffin or polyacrylamide hydrogels or other gel or liquid materials into the breasts for the purpose of increasing size, there are still a large number of non-professionals who use these injections and offer them as a cheap alternative to plastic surgery. The results, at first, may seem cosmetically favorable for the patient, but these injections often lead to painful complications like mastitis (inflammation of the mammary gland) and can make it incredibly difficult to diagnose breast cancer early. This new volume fills the gap in the literature as it will guide urologists and pathologists in the proper utilization of a variety of laboratory methods that are currently available to determine the presence, persistence or progression of tumors of the lower urinary tract. Because of its relative rarity and favorable outcome, it has not been feasible to assess medical interventions for thyroid cancer using randomized prospective trials. The approach to diagnosis and treatment relies to a great extent on information derived from retrospective studies. Overall prognosis and survival rates have been edging upward over the past two decades. This is attributed to a wider acceptance of total thyroidectomy as the primary surgical strategy. The appropriate indication of radioiodine therapy remains controversial, and physicians must be familiar with staging criteria to make educated decisions. We are now beginning to understand the genetic mechanisms of thyroid tumor initiation and progression. There are still major challenges ahead. Ovarian carcinoma continues to be responsible for more deaths than all other gynecologic malignancies combined, due to a continued inability to achieve detection of early (rather than advanced) stage disease and the lack of effective tumor-specific therapeutics. Ovarian carcinogenesis, invasion, and metastatic dissemination require a complex cascade of interrelated genetic, molecular, and biochemical events that regulate the neoplastic transition of normal ovarian surface epithelium. This updated second edition includes exciting new advances in ovarian cancer detection and treatment and provides an analysis of current research into aspects of malignant transformation, growth control, and metastasis. A more detailed understanding of these processes may ultimately translate into the development of novel approaches for the detection and control of ovarian cancer.

Without metastasis, prostate cancer would be both tolerable and treatable. The high incidence of indolent and organ confined disease is testament to this sweeping generalisation. Equally, if molecular markers of metastatic spread can be identified, then the choice of treatment for many patients would be easier and more radical, even curative. However, should prevention and treatment of the primary tumors prove difficult or impossible, then a knowledge of the phenotype of advanced metastatic tumors should allow us to target these lesions for destruction by conventional (drug based) or more innovative means such as gene and/or immunotherapy (1). In der Öffentlichkeit existiert das weit verbreitete Vorurteil, der medizinische Fortschritt spare das Thema »Krebs« vollständig aus und während der letzten Jahrzehnte habe es auf diesem Feld keine wesentliche Verbesserung der Behandlungsmöglichkeiten gegeben. Kaum ein Vorurteil geht an den vorhandenen Tatsachen weiter vorbei wie dieses, denn es gibt ihn eben doch, den Fortschritt bei der Behandlung der bösartigen Tumoren und es gibt ihn speziell bei den vier wichtigsten Tumoren des Urogenitaltraktes, nämlich dem Hodentumor, dem Prostatakarzinom, dem Nierentumor und eben auch beim Blasenkarzinom. Den dramatischsten Wandel haben wir sicherlich bei den bösartigen Tumoren des Hodens erlebt, wo die Heilungswahrscheinlichkeit im Laufe der letzten zwei bis drei Jahrzehnte fast an 100% herangebracht werden konnte. Beim P- statakrebs wurde seit der Jahrtausendwende in vielen Industrieländern ein deutlicher Rückgang der tumorbedingten Sterblichkeit dokumentiert, während sie in den Jahrzehnten zuvor stetig angestiegen war. Auch wenn endgültige Beweise ausstehen, so scheint es nicht unwahrsche- lich, dass diese erfreuliche Entwicklung wesentlich durch die verbesserten Möglichkeiten der Früherkennung und der dadurch weitverbreiteten kurativen Therapie bedingt ist. Beim Nier- tumor erlaubt die Früherkennung durch den Ultraschall sogar immer häufiger, die betroffenen Patienten bei ausgezeichneter Heilungswahrscheinlichkeit organerhaltend zu operieren. Demgegenüber hat das Blasenkarzinom während der letzten Jahre deutlich weniger A- merksamkeit auf sich gezogen, was aber keineswegs bedeutet, dass es nicht auch hier erhebliche Fortschritte gegeben hat: Gerade beim infiltrierenden Blasenkarzinom, einem hochmalignen Tumor, wurden die Behandlungskonzepte zunehmendbesser standardisiert.
