Abstract

In the majority of the patients with myeloproliferative neoplasm-associated myelofibrosis (MPN-MF), the disease interferes with the quality of life and social activities. A great proportion of patients suffer of general symptoms, such as fatigue, constitutional symptoms, or pruritus, that reflect the biological activity of the underlying disease. The greatest impact on the patients’ well-being is offered by severe anemia and progressive splenomegaly, and the therapy of these manifestations represents a major medical needs. Some of the disease complications, such as hemorrhagic or thrombotic events and infections, need treatments that are not specific for MPN-MF. On the contrary, treatment of extramedullary nonhepatosplenic hematopoiesis, treatment of pulmonary or portal hypertension, and therapy for the accelerated phase and of blast transformation of the disease are MPN-MF-specific medical needs.

Abstract

When normal circulating lymphocytes and thymic lymphocytes are cultured with phytomitogens for up to 72 h, a large proportion of cells are transformed to blasts. Phytohemagglutin (PHA), concanavalin A (Con A) and pokeweed mitogen (PWM) result in blast transformation of lymphocytes which develop different surface features than their nontransformed counterparts (examined before or after incubation for 72 h without the addition of mitogens) [70, 96]. Peripheral blood lymphocytes transformed by PHA, Con A or PWM showed a variety of surface microprojections [70, 96]. These were most frequently finger-like or cone-shaped microvilli but occasional cells showed ruffled membranes and others had elongated uropods terminating in tuft-like extremities. Smooth thymic cells transformed by Con A become markedly villous [76] in nature but still maintain their T-cell markers [96], while nontransformed cells remain spherical in shape with relatively few microvilli.

Abstract

An immunosuppressive effect of L-asparaginase seems to be likely. Therefore it is interesting to investigate the effect of L-asparaginase in the treatment of human autoimmune disorders. We have treated a patient with dermatomyositis with L-asparaginase (Crasnitin) at a daily dose of 1000 units/kg body weight over a period of 35 days. Previously he did not respond to steroids at a dose of about 50 mg daily.
