Aims Recent UK legislation allows dental therapists or jointly-qualified dental hygienist-therapists to work in the general dental service. This study aimed to investigate the extent of dentists' knowledge of the clinical remit of jointly qualified hygienist-therapists, their willingness to consider employing such a professional, and factors associated with these two measures.Materials and methods A postal questionnaire was sent to 616 NHS-registered dentists in South-East Scotland. Dentists' knowledge of the clinical remit of the dually-qualified hygienist-therapist was found to be limited, reflecting a restricted and inaccurate view of the professional remit of a hygienist-therapist. The majority (64%) said they would consider employing a hygienist-therapist in their practice, rising to 72% amongst dentists already working with a hygienist. Reasons given by dentists who were negative about this prospect were sought. Those who worked with a hygienist tended to refer to lack of physical space, whilst those who did not tended to cite reservations on clinical skills, competence and responsibilities, or on the costs involved.Conclusions This study identified considerable ignorance and negativity among dentists about the nature and clinical remit of this group of professionals. Dually-qualified hygienist-therapists will be in a position to treat much of the routine disease that exists within the population, and dentists may benefit from education in relation to the substantial contribution these individuals could potentially make to patient care. Poverty, poor housing and poor health are complexly interconnected in a cycle that has proven resistant to intervention by housing providers or policy makers. Research often focuses on the impacts of the physical housing defects, particularly upon rates of (physical) illness and disease. There has been comparatively little research into the ways in which housing services can underpin the generation of positive health and, especially, wellbeing. Drawing on qualitative data from 75 tenants in the social and private rented sectors, this paper describes the findings of a research project that tracked tenants' experiences across their first year in a new tenancy in Greater Glasgow, Scotland. The project collected data on tenants' perceptions of housing and housing service quality, financial coping and health and wellbeing, which was analysed using the principles of Realist Evaluation to elucidate impacts and causal pathways. Being able to establish a sense of home was key to tenants' wellbeing. The home provided many tenants with a recuperative space in which to shelter from daily stressors and was a source of autonomy and social status. A sense of home was underpinned by aspects of the housing service, property quality and affordability which are potentially amenable to intervention by housing providers. These findings raise questions about the extent to which social housing providers and the private rental market in the UK are able to meet the needs of vulnerable tenants. They suggest that approaches to housing provision that go beyond providing a basic dwelling are needed to successfully intervene in the cycle of poverty, poor housing and poor health. Organised crime groups' involvement in illicit markets is a common focus of law enforcement and governments. Drug, weapon, human and wildlife trafficking (and others) are all illegal activities with link to organised crime. This paper explores the overlooked illicit market of puppies. We detail the state of knowledge about the organisation of the UK puppy trade, which includes irresponsible and illegal breeding of puppies throughout Europe and their often-illegal movement into the UK. In 2017, we conducted an analysis of hundreds of online advertisements in Scotland, 12 expert interviews, a stakeholder survey of 53 participants, and 40 focus groups across Great Britain. Our data suggest an organised illicit market running in parallel to the legal trade. We speculate as to whether at some point along the supply chain organised crime groups are responsible for the suffering and death of the puppies and the economic and emotional damage to 'consumers'. Online monitoring and physical scrutiny at the ports must be improved to reduce non-human animal abuse. People buying puppies must also be made aware that their purchase could be profiting organised crime. [ABSTRACT FROM AUTHOR]

The paper discusses the explanatory use of the concept of transnationalism in relation to contemporary patterns of intra-European mobility, drawing on evidence from two qualitative studies of new labour migration from Poland to Scotland. It suggests that the transnational way of life of migrants is leading to the creation of a European space conceived in terms of a new kind of socio-cultural configuration. The paper draws on the concept of capitals (economic, social and cultural) derived from the work of Pierre Bourdieu. In particular, it uses these concepts to reflect on the gains and losses in capital of Polish migrants in Scotland and in their home country. It questions the classical brain drain approach to labour migration by giving more nuance picture of the migrants' lives characterised by circulation of capitals and simultaneous commitment to more than one country. This paper sets out to examine the place of theory in preschool education, considering the theories to which practitioners and providers have access and which provide a rationale for everyday practices and shape the experiences of young children. The paper reflects the circumstances of preschool provision, practices and thinking in the UK in general and in Scotland in particular. The central argument is that while there may be little obvious recourse to theorising and limited exposure to explicit theory about children's development, learning and education, practitioners and those responsible for provision have tacit understandings or implicit theories which can be seen to shape their practices. The paper begins by considering the explicit theory which practitioners are exposed to through the development of practice guidance and professional education. This is followed by an examination of the explicit theories adopted from psychology and more recent theoretical developments which have their roots in other disciplines and perspectives. Some influences on practitioners' implicit theories are discussed before considering two key aspects of the dominate consensus about good practice: that play is the medium through which children learn and that practice should be child-centred. The paper concludes with a discussion of the 'identity' of preschool education theory and the benefits of articulating the implicit theory and folk beliefs on which local practices are based. In this paper we develop a typology of 'non-commercial' approaches to farming, based on a survey of a representative sample of farmers in Scotland, United Kingdom. In total, 395 (16.6% of the sample) farmers indicated that they do not seek to make a profit on their farms. We estimate that these non-commercial approaches to farming are utilised on at least 13% of agricultural land in Scotland. As such, non-commercial farming (NCF) is not a marginal practice, nor are NCF limited to small-scale 'hobby' farms: NCF exist across the scale of agricultural holding sizes and are operated by a wide range of socio-demographic cohorts. We identify 6 types of NCF: agricultural residences, specialist smallholdings, horsiculture holdings, mixed smallholdings, amenity mixed farms, and large farms or estates. These types were differentiated primarily by the scale of farm size, presence of diversification activities and types of animal present. The analysis demonstrates a number of emergent patterns of land management: de facto land abandonment, transition towards 'horsiculture', and management differences between retiring and new entrant NCF. We argue that the types identified reflect a number of intersecting issues in contemporary agrarian transitions, particularly the aging farmer population; generational renewal; and gendered implications of agricultural restructuring. Deaths from air pollution in the UK are higher by a factor of 10 than from car crashes, 7 for drug-related deaths and 52 for murders, and yet awareness seems to be lacking in local government. We conducted an 18-year retrospective cohort study using routinely collected health care records from Ninewells Hospital, Dundee, and Perth Royal Infirmary, in Tayside, Scotland, UK, from 2000 to 2017. Hospitalisation events and deaths were linked to daily nitric oxides (NOX, NO, NO2), and particulate matter 10 (PM10) levels extracted from publicly available data over this same time period. Distributed lag models were used to estimate risk ratios for hospitalisation and mortality, adjusting for temperature, humidity, day of the week, month and public holiday. Nitric oxides and PM10 were associated with an increased risk of all hospital admissions and cardiovascular (CV) admissions on day of exposure to pollutant. In the short term at least, the outcome of the Scottish Independence referendum has settled the constitutional status of Scotland as part of the United Kingdom. During the referendum campaign, however, the major UK wide parties committed themselves to conferring a package of enhanced devolved powers upon Scotland in the event of a rejection of independence and the exact scope of these powers is in the process of being finalised. Although this ignited calls in Wales and Northern Ireland for an expansion of their own devolved powers, arguably more strongly felt pressures in the aftermath of the referendum have concerned the 'West Lothian' question; the fact that decisions on matters which only affect England are taken by a UK Parliament that comprises representatives from all four constituent parts of the UK, whereas the same matters are often legislated upon locally in those other parts of the UK without the involvement of representatives from England. There is much consensus on the need to address this anomaly of the current UK constitutional framework, but less agreement on how this ought to be done. This paper considers the debate over 'English only votes' at Westminster and identifies several flaws to such an initiative, which it is argued make it a dangerous mechanism that threatens the integrity of the UK. A new constitutional framework based upon regional or federal mechanisms represents the most logical and workable step forward, but it is doubtful whether sufficient political or public support exists for such a development to take place. The Scottish referendum campaign highlighted, if anything, the extent to which the UK's future integrity is threatened by ideas of political difference and constitutional reform must take account of such realities. Loch Leven is a large, shallow lake in lowland Scotland, UK. Scientific research began here almost 200 years ago. Early research characterised the biodiversity and physical characteristics of the loch, providing an important historical background for future research. In the mid-1960s, this ad hoc approach was superseded by a more structured research programme under the umbrella of the International Biological Programme. This was the beginning of the Loch Leven long-term monitoring programme. Today, the results of these studies form one of the longest and most comprehensive limnological datasets for shallow freshwater lakes in the world, comprising more than 500 physical, chemical and biological variables collected at two-weekly intervals. To celebrate the 40th anniversary of the start of the long term monitoring programme, and to highlight the scientific investigations still being conducted at Loch Leven, the NERC Centre for Ecology & Hydrology (CEH) organised a symposium entitled "Loch Leven: 40 years of scientific research" in Kinross, Scotland, UK, on 11 December 2008. This examined the role of long-term monitoring in developing our understanding of the links between pollution, climate change and ecological responses in shallow lakes. At the beginning of the nineteenth century, the medical concept of “necessaries of life”—i.e., food, heat, clothing, and shelter—became controversial in Britain. The new domain of political economy, in both its Smithian and its Malthusian modes, challenged the view that necessity was either a measurable concept or a deliverable right. For the political economists, “necessity” was relative, a standard erected by class and culture. Many doctors and surgeons resisted that relativism, insisting that the fever epidemics they were responding to could be directly or indirectly attributed to the absence of such “necessaries.” The first half of the article reviews the distinct debates between doctors and economists on these issues in England, Scotland, and Ireland during the first half of the nineteenth century. By mid-century these so-called “necessaries” had largely been de-medicalized and transformed into the status of consumer commodities. The second half of the paper describes the quite different list of necessities that replaced them in public health campaigning and administration during the second half of the century. In 2001, the United Kingdom government ratified the European Charter for Regional or Minority Languages, recognising Scottish Gaelic, Welsh and Irish under Part III of the Charter, but Scots, in Scotland and Ulster, only under Part II. It may be that this distinction is representative of the dialectalised nature of this language in relation to its near relative, Standard English. Nevertheless, this paper demonstrates that implementation of language policy on Scots at all levels of government – European, United Kingdom, Scotland and local – has been half-hearted, ill thought-out and buried in a swathe of other ‘cultural’ issues. Whilst it would be impossible to prove actual animus against the language by governmental actors, it is likely that prevailing sociolinguistic attitudes towards the vernacular’s status have encouraged the ineffectual nature of policy towards Scots. Clinical coding is the task of transforming medical information in a patient's health records into structured codes so that they can be used for statistical analysis. This is a cognitive and time-consuming task that follows a standard process in order to achieve a high level of consistency. Clinical coding could potentially be supported by an automated system to improve the efficiency and accuracy of the process. We introduce the idea of automated clinical coding and summarise its challenges from the perspective of Artificial Intelligence (AI) and Natural Language Processing (NLP), based on the literature, our project experience over the past two and half years (late 2019–early 2022), and discussions with clinical coding experts in Scotland and the UK. Our research reveals the gaps between the current deep learning-based approach applied to clinical coding and the need for explainability and consistency in real-world practice. Knowledge-based methods that represent and reason the standard, explainable process of a task may need to be incorporated into deep learning-based methods for clinical coding. Automated clinical coding is a promising task for AI, despite the technical and organisational challenges. Coders are needed to be involved in the development process. Student and pre-registration pharmacist performance in a UK Prescribing Assessment': room for improvement and need for curricular change Background Increasingly the global policy direction is for patient-facing pharmacist prescribers. The 'UK Prescribing Safety Assessment' (PSA) was developed for medical graduates to demonstrate prescribing competencies in relation to the safe and effective use of medicines. Objectives To determine PSA performance of final year undergraduate student pharmacists (year 4) and pre-registration pharmacy graduates (year 5) and explore their opinions on its suitability. Setting Scotland, UK Methods Final year undergraduates (n = 238) and pre-registration pharmacists (n = 167) were briefed and undertook the PSA. PSA questions were mapped to specific thematic areas with 30 questions over 60 min. Data was analysed using descriptive statistics. A questionnaire was completed to gauge opinions on appropriateness of the PSA. Main Outcome Measure PSA scores Results Mean total PSA score for pre-registration pharmacists (64.4, SD 10) was significantly higher than for undergraduates (51.2, SD 12.0,) (p < 0.001). Pre-registration pharmacists performed significantly better across all question areas (all p < 0.001 other than 'adverse drug reactions', p < 0.01). Hospital pre-registration pharmacists performed statistically significantly better than community with higher overall scores (67.4, SD 9.8 v 63.2, SD 9.8, p < 0.05). Positive views on the appropriateness of the approach and the usability of the online interface were obtained from participants. Conclusion Hospital pre-registration pharmacists performed better than the undergraduates, but there is a need to improve prescribing skills in all, most notably in diagnostic skills. The PSA is acceptable to the participants. These results will help inform pharmacy curricula development and provides a cross-disciplinary method of assessment of prescribing competence. Empirical analysis of social mobility is typically framed by outcomes recorded for only a single, recent generation, ignoring intergenerational preconditions and historical conferment of opportunity. We use the detailed geography of relative deprivation (hardship) to demonstrate that different family groups today experience different intergenerational outcomes and that there is a distinct Great Britain-wide geography to these inequalities. We trace the evolution of these inequalities back in time by coupling family group level data for the entire Victorian population with a present day population-wide consumer register. Further geographical linkage to neighbourhood deprivation data allows us to chart the different social mobility outcomes experienced by every one of the 13,378 long-established family groups. We identify clear and enduring regional divides in England and Scotland. In substantive terms, use of family names and new historical digital census resources are central to recognising that geography is pivotal to understanding intergenerational inequalities. Intergenerational preconditions and historical conferment of opportunity play a role in social mobility. Meeting global targets that maintain temperatures at 1.5 °C above pre-industrial levels while adapting to the growing impacts of climate change requires significant and rapid societal change. Within this context, there has been growing interest in building community resilience to shocks and stressors and as a forward-looking process. Yet while there has been extensive attention to conceptual aspects, there has been much less on how this can be achieved in practice. This research worked with three communities in Scotland (UK) regularly exposed to flooding and other integrated challenges to learn from action about community resilience building. A carefully developed four-tiered transdisciplinary approach was applied that included: relationship-building; enhancing capacities to work with interconnections; enabling processes; and supportive action-oriented research. The findings of the analysis of the system dynamics that were occurring during the resilience-building process highlight that it is a complex and messy social process. Yet, it also shows that if quality and sufficient quantity of support and time to help surface and deliberate on underlying assumptions about communities and change is provided, it can be possible to stimulate emergence of beneficial reinforcing social dynamics that begin to support collaborative and systemic action. To further advance know how about resilience building, much greater focus will be needed on the 'how' of resilience. This, in turn, will require new framings of, and approaches for, community resilience and new framings of research, knowledge and knowing. [ABSTRACT FROM AUTHOR]

The final provisions of the UK's Companies Act 2006 have now been in force for 10 years. Part of this regime included a new form of model constitution, known as the Model Articles. This article uses empirical data to establish whether the Model Articles have been used in practice or not. To do so, it tracks the constitutions of a sample of companies (those incorporated in Scotland in October 2009) from their incorporation until December 2017. It undertakes a leximetric methodology to code 12 variables across the constitutions, with a 0 being coded for convergence to the default regime and 1 being coded for divergence from the default regime. The results show that the majority of companies do not deviate from the default regime, other than in one respect: most allowed for the ability to appoint alternate directors. More importantly, however, the dataset shows that few of the sample companies amended their articles of association following incorporation, and that there is a strong correlation between certain coding patterns and the presenter, or formation agent, used to incorporate the company. The future status of peatlands as carbon stores/sinks is uncertain given current and predicted environmental change. Several factors can affect the magnitude of the peatland carbon sink including disturbances such as wildfire. There is at present little evidence of how wildfire affects the emission of carbon dioxide (CO2) and methane (CH4) via perturbation to aerobic and anaerobic respiration. The greatest effects, which are likely to vary according to wildfire severity, would be expected in the immediate post-fire stages when little recovery has taken place. Here, we investigate five UK peatland wildfires (2011–2012) in the immediate post-wildfire period measuring CO2 and CH4 fluxes using static chambers. Fire severity was described using a modified form of the composite burn index. A hierarchical partitioning approach indicated time since fire was the most strongly associated variable that fluxes of both CO2, and CH4 followed by soil temperature for CO2 and fire severity for CH4. Using a liner mixed modelling approach to account for repeated measures; fire severity was a significant term for CH4 and borderline significant for CO2. Mean fluxes of CH4 were consistently lower on burnt sites. In contrast, data from a fire in the north of Scotland appeared to show the opposite relationship for CH4 with higher fluxes on the burnt sites. These results suggest that wildfire can affect gaseous carbon fluxes, but the responses can be variable in both space and time and that disruption to anaerobic processes may be site and/or fire dependent. Roadside dust can contain particulates enriched with potentially toxic elements (PTEs) as a result of the degradation of mechanical vehicular parts, tyre wear and combustion processes. To assess the potential accumulation of these metals in roadside areas, a snapshot study was carried out, investigating metal content at rural and urban locations in central Scotland. Samples of road dust were collected at six sites representing low, medium and high traffic intensity at rural and urban locations. The samples were separated based on particle size and analysed for heavy metal content using inductively coupled plasma optical emission spectroscopy (ICP-OES) after acid digestion. The metals analysed were aluminium (Al), cadmium (Cd), chromium (Cr), copper (Cu), iron (Fe), magnesium (Mg), manganese (Mn), lead (Pb) and zinc (Zn). The rural area measurements were carried out in West Lothian, approximately 13 to 17 miles west of the city of Edinburgh (UK). The urban area measurements were carried out in the southern part of the Edinburgh city district (UK). Concentrations of Cu, Cr and Zn were found to correlate with traffic intensity, although only Cu and Zn concentrations exceed recommended EC directive 86/278/EEC guidelines for urban runoff materials. The metal concentrations of small particles (0.45–20 μm) were exceedingly high in both Cu and Zn at areas of high traffic intensity, indicating potential areas of concern regarding health impacts for pedestrians and cyclists who are exposed to roadside dust on a regular basis. The article focuses on the Nintendo Wii technology, developed by Pearse Hannigan, David Lagan, and Adam Gray, final year dental students at the University of Glasgow Dental School. The concept, which intends to aid dental students practice their operative skills, won the Dental Innovation Technology Ideas Award. The Nintendo Wii console will be used via a software to simulate operative techniques. Dr. David Watson of the University of Glasgow Dental School cites the advantages of the idea.

Background: The Cancer Drugs Fund (CDF) was established in 2010 to improve access to treatments not routinely available. Having widely overspent, stricter budgeting rules were introduced in 2016. The CDF can now only include treatments with potential to be cost effective once sufficient data are collected.Objectives: Our objective was to explore the process and criteria used for consideration of treatments under the new CDF framework and to describe the extent of evidence collection.Methods: We identified CDF list, UK National Institute for Health and Care Excellence (NICE) and Scottish Medicines Consortium documents (10 May 2018). Data were collected on drugs and indications, reasons for inclusion in the CDF, data collection, incremental cost-effectiveness ratios (ICERs), and corresponding recommendations for Scotland.Results: In total, 12 drugs were listed on the CDF in 17 indications, 12 of which were considered end-of-life care. The most common cancers were non-small-cell lung (n = 4), urothelial (n = 3), lymphocytic leukaemia (n = 2) and multiple myeloma (n = 2). The companies' ICERs were generally lower than those from the committee and the evidence review group. Drugs were included in the CDF for 6-42 months, with the majority included for ≥18 months. Data were frequently collected on overall survival (n = 16) and progression-free survival (n = 5) using NHS systems and, frequently, ongoing trials.Conclusions: Data collection frequently included overall survival and exceeded the 2 years recommended in the CDF strategy. It appears the CDF is allowing patients access to drugs long before they may become available for routine use. In response to the impact of demographic change on the healthcare system, 'ageing in place' was introduced as a national policy to support elderly people living independently in their homes. Housing adaptation is essential for successful independent living and has been given increased political priority. However, adaptation policies and practice vary regionally, reflecting statutory limits, policy choices and local planning. This study investigated the current status of adaptation provision in different regions in the UK and assessed the effectiveness of local service planning and management. A mixed-methods sequential explanatory research strategy was employed. In the first quantitative phase, a questionnaire survey was carried out involving all 378 local authorities in England, Scotland and Wales. This was followed by a second qualitative phase involving individual interviews with five professionals and two clients and a focus group meeting with six key stakeholders. The study found that the current number of adaptations was relatively small compared with potential demands in most local areas, as was funding for adaptations. On the operational side, the adaptation process was fragmented, involving different service groups in many local authorities. There were disconnections between these groups, which often caused inefficiencies and poor effectiveness. Moving forward, local authorities need to have a clear vision of the overall need for adaptations and allocate sufficient resources. Grassland silage management is generally semi-organised with no conscious attempt to re-use wheel ways as with arable fields. The total number of machine passes can be 15 or more with normal traffic (NT) systems resulting in potentially large areas of a field suffering from direct damage to the crop and soil. Literature suggests there can be grass dry matter yield reductions of 5 to 74% under NT through compaction and sward damage, with a mean of 13% in the UK. Commercially available grass forage equipment with widths of 3 to 12 m set up for controlled traffic farming (CTF) could reduce trafficked areas (which is typically 90% to 80% for NT) to 40% to 13% for CTF. This study compared grass dry matter yield between CTF and NT for a three-cut silage system based on a 9 m working width in a permanent silage field in the southwest of Scotland, UK in 2015. Results showed a 13.5% (0.80 t ha−1) increase in yield for CTF for the 2nd and 3rd cuts combined. The CTF trafficked area covered was 57% less than the NT system (30.4% compared to 87.4%) over the three silage cuts. An economic analysis based on a 13% increase in dry matter yield (for 2- and 3-cut systems) and a reduction in trafficked area from 80% (for NT) to between 45% and 15% (for CTF), increased the yield by between 0.53 t ha−1 and 1.36 t ha−1 for 2- and 3-cut systems, respectively with an equivalent grass value of between £38 ha−1 and £98 ha−1. Purpose: The aims of this study were to (1) determine the association between diet quality using the Dietary Approaches to Stop Hypertension (DASH) score and cardiometabolic risk in a British working population and (2) identify employee characteristics associated with reporting a poorer quality dietary pattern.Methods: British police employees enrolled (2007-2012) into the Airwave Health Monitoring Study (n = 5527) were included for sex-specific cross-sectional analyses. Dietary intakes were measured using 7-day food records. DASH score was calculated to determine diet quality. Logistic regression evaluated associations between (1) diet quality and increased cardiometabolic risk (defined as ≥ 3 risk markers: dyslipidaemia, elevated blood pressure, waist circumference, CRP or HbA1c), and (2) poor diet quality (lowest fifth of DASH score distribution) and employee characteristics.Results: Employees recording a poor diet quality had greater odds (OR) of increased cardiometabolic risk independent of established risk factors (demographic, lifestyle and occupational) and BMI: men OR 1.50 (95% CI 1.12-2.00), women: OR 1.84 (95% CI 1.19-2.97) compared to the healthiest diet group. Characteristics associated with reporting a poor quality diet were employment in Scotland vs. England: men OR 1.88 (95% CI 1.53-2.32), women: OR 1.49 (95% CI 1.11-2.00), longer working hours (≥ 49 vs. ≤40 h) men: OR 1.53 (95% CI 1.21-1.92) women: OR 1.53 (95% CI 1.12-2.09). For men, job strain (high vs. low) was associated with reporting a poor diet quality OR 1.66 (95% CI 1.30-2.12).Conclusions: The general population disparities in diet quality between England and Scotland were reflected in British police employees. Rapid expansion in scientific knowledge, changes in medical practice, and greater demands from patients and society necessitate reform of the medical curriculum. In recognition of this, medical educators across the world have recommended the adoption of competence-based education. This is intended to increase the rigour and relevance of the curriculum, move students beyond a focus on the memorisation and regurgitation of scientific facts, and better enable them to understand scientific principles and apply them to the practice of medicine. Experience from 40 years' use of competence-based curricula across the world suggests that the uncritical application of this approach to the medical curriculum may not achieve its intended aims. There are valuable lessons to be learnt from the history of competence-based education. By taking on board these lessons, confronting the pitfalls of this approach, and devising new and creative solutions to the problems inherent in this methodology, medical educators can better achieve their aim of providing a strong foundation for the practice of medicine in the twenty-first century. It is only through such a strategy-rather than the uncritical adoption of this educational approach-that we will have real movement and progress both in competence-based education in general, and in its applications to medicine in particular. Despite the importance of periphyton-metal interactions in bioremediation schemes and in phosphorus (P) cycling, the processes controlling metal accumulation in periphytic mats are still poorly understood. Iron (Fe) accumulation in periphytic mats was examined across a Fe settlement lagoon receiving mine drainage in Scotland, UK, between March and June 2008. Quantification and mapping of intracellular and extracellular Fe concentrations in periphyton samples using scanning electron microscopy-energy dispersive spectroscopy suggested that Fe accumulation was dominated by the association of Fe-rich precipitates with the extracellular polymeric substances matrix, rather than biotic uptake. Intracellular Fe concentrations were significantly higher in periphyton samples exposed to the highest dissolved Fe concentrations. Neither intracellular nor extracellular Fe concentrations were significantly affected by light availability or cell density. While diatoms dominated the periphyton communities there was no significant association of diatom functional groups with Fe accumulation, indicating that community composition may not affect the function of periphytic mats with respect to Fe removal. Scale-up calculations based on the mean measured Fe accumulation rate by periphyton substrates of 0.021 g m day showed that exposure of large surface areas of periphyton substrate in the settlement lagoon would only increase the Fe removal efficiency of the lagoon by c.1%. Rivers, lakes and wetlands are good examples of ecosystems that provide multiple, concurrent, services to mankind. Human society has often exploited these systems by enhancing one ecosystem service at the expense of another. Loch Leven, Scotland, UK, is a good example of this. Over the past 150 years, the lake has been subjected to hydrological modification, fish stocking and pollution control to improve the delivery of key goods and services. This study uses historical records to explore the results of these interventions on the ecosystem services that were targeted for improvement and the knock-on effects on other services provided by the lake. The results suggest that, when management changes are being considered to enhance particular ecosystem services, the potentially damaging effects on other ecosystem services should be taken into account. This requires a better understanding of the role of ecosystem function in delivering ecosystem services, and of the links between multiple ecosystem services, than is currently available. While further research is clearly needed, the value of long-term datasets in providing knowledge and understanding through 'hindsight' should not be underestimated. Loch Leven has been designated as a UK Ramsar Site (1976), a Site of Special Scientific Interest (1985) and a Special Protection Area (2000) due to its importance as a site for overwintering waterfowl. However, no comprehensive assessment of trends in waterfowl at the local versus national scale has been conducted at the site. Coherence between trends in 5-year mean species abundance for Loch Leven and Underhill Indexing Method values for Scotland (or GB in the case of geese) were assessed using principal components analysis for ten study species between 1968 and 2006. Five species showed trends at Loch Leven that were coherent with those at the Scottish scale (Eurasian Teal, Mute Swan, Great Cormorant, Pink-footed and Greylag geese). These species may not respond positively to local scale management. However, the other five species (Mallard, Coot, Great Crested Grebe, Tufted Duck and Pochard) showed distinct differences between local and national trends. A study of the feeding ecology for these species, the re-establishment of ringing effort and an assessment of waterfowl–wetland relationships are recommended to determine how changes in local food resources and habitat quality interact with macro-scale population dynamics to influence local and regional patterns of abundance. This article reviews historical changes in the total phosphorus (TP) inputs to Loch Leven, Scotland, UK. Data derived from palaeolimnological records suggest that inputs in the early 1900s were about 6 t TP year−1 (0.45 g TP m−2 year−1). By 1985, this had risen to about 20 t TP year−1 (1.5 g TP m−2 year−1) due to increases in runoff from agricultural land and discharges from point sources. By the late 1970s, increased TP inputs were causing serious degradation of lake water quality. Most noticeably, there had been an increase in cyanobacterial blooms. A catchment management plan was implemented in the early 1990s. This resulted in a 60% reduction in the annual TP input between 1985 (20 t TP year−1/1.5 g TP m−2 year−1) and 1995 (8 t TP year−1/0.6 g TP m−2 year−1). The main reduction was associated with better control of point source discharges, but attempts were also made to reduce inputs from diffuse sources. Background: Completeness of ascertainment is a very important aspect of cancer registration. There is no recent published estimate for childhood cancer in Britain.Methods: We estimated completeness of ascertainment by the National Registry of Childhood Tumours for cancer diagnosed under age 15 years in residents of Britain during 2003-04. Stratified two-source capture-recapture was applied to notifications from general cancer registries (CRs) and specialist clinicians. Variation in notification patterns was assessed by logistic regression. Results were verified by cross-checking with Hospital Episode Statistics for leukaemia patients from England born in 1998 and diagnosed before 2005.Results: CRs notified 92-96% of registrations, and specialist clinicians 93%. Notification patterns varied slightly according to registry region, age at diagnosis, diagnostic group, socioeconomic status, and whether the patient had died. Irrespective of stratification by these factors, the overall completeness estimate was 99-100% (assuming independence of sources). Estimated completeness was at least 99% within all subgroups, except for one region (Thames 98-99%) and two small diagnostic groups (germ-cell and gonadal cancer 98-99%, melanoma and non-skin cancer 97-98%).Interpretation: The independence assumption cannot be fully justified, as both sources used records from treatment centres. This exploratory study examines the early impact of the Unfair Commercial Practices Directive 2005/29/EC on enforcers in Scotland within the context of current policy developments regarding the use of civil and criminal enforcement mechanisms within the UK. The Directive has been implemented in the UK by means of the Consumer Protection from Unfair Trading Regulations 2008 SI 1277 and the duty to enforce the Regulations falls primarily to local authority trading standards services. Twenty-one in-depth interviews were conducted with a range of officers employed within the trading standards service based in eight local authorities in Scotland on their perceptions of the new Regulations. The main findings were that officers' views of the new Regulations and the civil and criminal enforcement routes available to them was affected by structural, operational, and cultural issues which varied from one authority to another. A number of officers were finding the Regulations a challenge to old ways and there was evidence that many felt unprepared for the Regulations. A number of positive views were expressed regarding the flexibility of the new Regulations and the advantages of the provisions relating to misleading omissions. ObjectivesTo investigate, by questionnaire, various aspects of primary dental care provision in the North West of England and Scotland.MethodA questionnaire containing 79 questions was sent to 1,000 practitioners, selected at random, in the North West of England and Scotland. Non-responders were sent another questionnaire after a period of 4 weeks had elapsed.ResultsOverall a response rate of 70% was achieved. The majority of practitioners were practice principals (65%), working in a group NHS practice (80%) located in a city or town centre (49%). On average 10-20 patients were treated each session with fewer patients treated per session under private arrangements. Many practitioners were found to lack hygienist support (44%) and to employ unqualified dental nurses (82%). Younger practitioners were more likely than senior colleagues to have access to up-to-date computers whilst 37% and 74% of respondents never used CAL programmes or magnification respectively. Contemporary cross-infection control standards were used by the majority of practitioners, although 3% of practitioners reported only autoclaving their handpiece once a day.ConclusionsThe majority of practitioners, involved in this study, worked under National Health Service (NHS) regulations as principals in a group practice where the workload was greater than the private/independent sector. Contemporary cross-infection procedures were used routinely. In contrast, computer-aided learning programmes and magnification were not used routinely. Background UK dental graduates complete a 12-month vocational training programme, but only in Scotland is there mandatory assessment.Aim To evaluate the effect of mandatory assessment.Method Pragmatic experimental design. UK dental trainees (Cohort 1: 2003-2004; Cohort 2: 2004-2005; Cohort 3: 2005-2006) completed questionnaires assessing stakeholder-identified clinical and non-clinical outcomes pre- and post-training.Results Response rate = 84%: Scotland (n = 247); other UK (n = 308: 156 (England) + 102 (Wales) + 50 (Northern Ireland)). Compared to other UK trainees, trainees in Scotland were significantly more confident in clinical skills (F(1,555)= 15.49, p <0.001), continuing professional development (F(1,552) = 4.37, p <0.05), felt better prepared for keeping track of new technology (F(1,551) = 9.03, p <0.01) and new evidence (F(1,551)= 3.71, p <0.05); showed more positive attitude toward training (F(1,554) = 5.82, p <0.01), continued professional development (F(1,552) = 4.71, p <0.05), and their job (F(1,553) = 6.93, p <0.01). They also found mandatory assessment significantly more beneficial (Scotland mean = 5.6 (1.9); other UK mean = 4.0 (1.9); F(1,546) = 60.87, p <0.001), more useful (Scotland mean = 5.3 (1.5); other mean = 4.9 (1.7); F(1,549) = 7.474, p <0.01) and more comforting (Scotland mean = 4.1 (1.6); other UK mean = 2.7 (1.6); F(1,547) = 57.47, p <0.001).Conclusion Results suggest it does matter where dental graduates spend their training year in the UK. Support for training under the Scottish mandatory assessment system was robust over three cohorts. As part of the impetus to increase and widen participation in the UK, and in Scotland in particular, there has been considerable effort put into creating links between Further Education Colleges (FECs) and Higher Education Institutions (HEIs). However, because no unique identifier is used to track students between the two sectors, little is known in quantitative terms of the nature of this progression. In this article using substantial datasets, and an approach known as Probability Matching, we are able to provide novel data on the proportions of Scottish domiciled former FEC students within Scottish HEIs, and are able to compare the characteristics of this subset of all students with the whole cohort within HE in a given year, 1999–2000. Objective: To examine the association between body weight measures across the lifecourse and the risk of adult-onset diabetes.Methods: We analysed data from the Glasgow Alumni Cohort and the British Women's Heart and Health Study (BWHHS). The former included 5,571 men and women who had height and weight measured at university, and reported birthweight, mid- and later-life weight in a postal questionnaire. The BWHHS analysis included 4,280 women who had height and weight measured in later adulthood and recalled their birthweight and early adult height and weight. Adult-onset diabetes was defined as doctor-diagnosed disease after age 30, either self-reported or abstracted from medical records.Results: Thirty nine women and 209 men (Glasgow Alumni study) and 314 women (BWHHS) had diabetes. Those with diabetes had lower mean birthweight than those without, although the differences were small. Individuals with diabetes were also shorter and heavier at all ages than those without diabetes. Being overweight during at least one time period in adult life was associated with an increased risk of diabetes, compared to those who were never overweight. While there was no age at which being overweight was particularly detrimental, the risk associated with being overweight was cumulative across the lifecourse.Conclusions: Being overweight at any point during life is associated with an increased risk of adult-onset diabetes. Historic sulphidic Pb–Zn mining catchments at Leadhills and Wanlockhead, in the south of Scotland, UK have a legacy of mining of PbS (galena) and ZnS (sphalerite) from the twelfth century to the 1930s. The mining activities created tailing piles, ponds, adits and contaminated soils that contribute leaching and surface runoff of potentially toxic elements, particularly lead (Pb), which impact on the surface water and groundwater and are rapidly diluted in the wider catchment area. Studies by environmental regulators have shown that Pb, Cd and Zn in water can locally exceed the Environmental Quality Standards (EQS), particularly at Leadhills. To evaluate geochemical controls on release, 20 water sources (adits, surface water and near-surface groundwater) were sampled over four seasons (spring, summer, autumn and winter) over a 1-year period and characterized. Samples were circum-neutral pH from 6.3 to 7.9 (with average total dissolved solids < 55.0 mg/L), with no characteristics of acid mine drainage. The concentrations of PTEs in the water exceed UK EQS and WHO standards (and non-compliance on the Water Framework Directives). Geochemical modelling (GWB and PHREEQCv2) predicted mineral control on solubility which identified PbSO4 (anglesite), Fe2O3 ferric oxide (haematite), Fe3O4 (magnetite), FeCO3 (siderite), CaMg(CO3)2 (dolomite), CaCO3 (calcite) and Ca(Fe·Mg)(CO3)2 (ankerite) to be important. These were confirmed in solid phases analysed from tailings and sediments in contact with the hydrological cycle at the sites. Multivariate statistical analysis (PCA) of water samples associated with leaching through mine tailings showed strong seasonal variation with some elements (Fe, Cu, Cd, Pb, Na, Ca and Zn) with higher variance. The strong negative association of pH with soluble Zn, Cu, As, Cd and Pb highlights typical sulfide oxidation processes are taking place and supported by a positive correlation with temperature. Dissolution processes of mineral phases indicated by positive association of TDS and EC with Na, Ca and Zn. The data from PCA suggest contributions with potential for active generation of acid mine drainage and dissolution of solid phases influencing the release of PTEs into surface waters. Objective The purpose of this observational study was to investigate the relationship between deprivation and the delivery of primary care NHS orthodontic services across Scotland.Method Deprivation was measured using the Scottish Index of Multiple Deprivation (SIMD). The Information Services Division, NHS National Services Scotland, supplied data on all claims for orthodontic treatments in Scotland for the years 2008 and 2009. Each claim was assigned to a SIMD quintile (SIMD 1 being the most deprived, and SIMD 5 the least deprived), and odds ratios were calculated.Results Uptake of orthodontic services is highest in the least deprived areas. Patients from the least deprived areas are nearly twice as likely to receive orthodontic treatment as those from the most deprived areas (odds ratio of 1.90 with a 95% confidence interval (CI) 1.86 to 1.94).Conclusion Patients from more the most deprived backgrounds are less likely to receive orthodontic treatment than those from more affluent backgrounds, which does not necessarily reflect need.
