The article presents a study which examined the effectiveness of the failure mode and effect analysis (FMEA) model to identify possible chemotherapy process failures. Also cited are some areas where failures could occur including during ordering, preparation, and compounding of medications, as well as some possible errors in the chemotherapy process like underdosing, improper drug preparations, and timing errors.
