Described are three male patients showing bizarre manifestations of granulomatous dermatophytosis, all in the hand region. One had a small infiltrated nodular lesion near the thumb which was mistaken for a common insect bite. The second had a violaceous-yellowish plaque, also on the thumb, composed of 2-3 small subcutaneous nodules. The third case had five reddish intracutaneous nodules on the dorsum of the hand. Puncture of these nodules revealed the presence of fungal elements and culture yielded T. rubrum in all cases. Each patient was also found to have a fingernail affected by the same agent. Histologic sections in each case showed granulation tissue in the dermis and hypodermis, mainly in the vicinity of hair fragments with spores. Hereby described is a case of a young girl suffering from widespread dermatophytosis caused by Trichophyton violaceum which proved intractable to accepted methods of therapy throughout a number of years. The girl was also found to have a history or recurrent respiratory tract infection and pyoderma in addition to verrucae vulgares, and Giardia lamblia in the gastrointestinal tract. Investigation revealed a marked immunological deficit seen in both the cellular and humoral system. It is suggested that in similar cases resistant to therapy it may be helpful to carry out a comprehensive investigation of the immunological system. [ABSTRACT FROM AUTHOR]

Clinical and mycological data on 1000 cases of dermatophytosis seen from 1970 to 1975 are reported. Tinea pedis was present in 44%, T. rubrum being the main causative agent; nail involvement was present in 41% of these cases. Tineainguinalis was present in 13%, 38% of them females, with T. rubrum isolated in 87% and the swimming-pool incriminated as the main source of contamination. There were various manifestations of tinea corporis in 128 patients, with T. rubrum responsible in 91%. Among the 21 cases of tinea capitis, there was one adult female with a kerion-type due to T. violaceum, 4 cases due to M. gypseum and 2 to M. canis. Tinea favosa was seen in 6 patients, all of them females and one with a spreading fungus on the soles and palms. Amonh the 10 cases with tinea barbae, T. rubrum was found in 4, T. tonsurans in 3, T. mentagrophytes in 2 and M. gypseum in one. There were widespread lesions of T. rubrum located on large areas of the body and including the legs, arms, groins and nails, in 11 patients with other disorders, including diabetes, Cushing's syndrome and lymphoma. It was apparent that the dermatophytic flora of Israel comprises 10 dermatophytes, T. rubrum being the predominant agent of infection (in more than 80%), with a gradually increasing incidence evident. The other agents found in our survey were T. mentagrophytes, T. violaceum, T. schoenleinii, T. tonsurans (found sporadically), E. floccosum and Microsporum spp. M. canis and M. gypseum, the latter being reported for the first time in this country. One hundred and fourteen soil samples collected from various areas of Israel were screened for the presence of keratinophilic fungi. Five species were isolated from the 59 positive samples, viz.: C. keratinophilum - 22 strains, C. evolceanui - 18, C. serratus Eidam - 10, M. gypseum - T. terrestre - 4. Scalp involvement by M. gypseum was found in 22 children (15 boys and 7 girls), ranging from 1 till 12 years of age, most cases coming from urban areas. Kerion-like manifestation was observed in 9 cases, the rest presenting the superficial form of tinea capitis infection. The main type of hair invasion was ecto-endothrix, but endothrix type was also observed in 4 cases with favus-like aspect. The parasitised hair exhibited no fluorescence. The soil was proved to be the source of contamination in 40 % of the cases, where a trauma had favoured the transmission of the causative agent. [ABSTRACT FROM AUTHOR]

In 525 patients with onychomycoses by dermatophytes, collected in Bucharest (45% of the cases being females), the toe-nails were found to be affected in 65% of the cases, while the finger-nails only in 20%. In order of frequency, the following agents were isolated: T. rubrum (55%), T. mentagrophytes (interdigitale - 26%), T. violaceum (12%), E. floccosum (3%), T. schonleinii (1.3%). Only sporadically were found other species, viz.: M. canis (4), T. tonsurans (3), T. verruccosum (2), T. quinckeanum (1) and M. audouinii (1). Dermatophytoses of animals in Romania are reviewed. Ringworm infections have been found in cattle (where T. verrucosum was the only isolated agent), horses (the causative organisms being T. verrucosum, T. mentagrophytes and M. equinum), cats (dermatophytes isolated: M. canis and T. quinckeanum), dogs ( M. canis and M. gypseum), fowls T. gallinae). From the laboratory animals: guinea pigs ( T. mentagrophytes, M. audouinii, T. rubrum), mice ( T. quickeanum and T. mentagrophytes), rats ( T. mentagrophytes), rabbits ( T. mentagrophytes and M. canis) and hamster) T. mentagrophytes). Among the wild animals, only the chamois had a ringworm infection by M. gypseum, other large or smaller wild animals harboring only saprophytically some poozhilic and geophilic dermatophytes. Out of 750 soil samples collected from various regions of Romania, during 1958-1967, a positive percentage of 86 was positive for keratinophilic fungi. Among the 13 isolated species, the most prevalent were M. gypseum (54.6 %), C. keratinophilum (40.1 %) and K. ( T.) ajelloi (27 %). Beside T. terrestre, found in 8.9 % of the examined soil samples, the percentage of the remaining species is low, not exceeding 4 %. In order of frequency: C. ( T). evolceanui (3.2 %), M. cookei (2.2 %), T. mentagrophytes (2 %), T. quinckeanum (0.9 %), C. ( T.) indicum (0.6 %). There have been also isolated, but only once; M. canis, A. multifidum, T. georgiae and T. vanbreuseghemii. The studies carried out on the immunobiological properties of the Romanian soil isolates (including intracutaneous tests, serological reactions and therapeutic efficiency), have showed a close relationship between these fungi and the common pathogenic dermatophytes. 20 strains of 10 different species of keratinophilic fungi isolated from Romanian soil have been tested for in vitro griseofulvin sensitivity. The most resistant was M. cookei, whose growth was inhibited only by 64 µg/ml concentration of griseofulvin. The highest sensitivity was observed for C. keratinophilum, which is completely stopped in its development by 4 µg/ml concentration. The actual state of dermatophytes in Romania is briefly reviewed. From the 20 species discovered, up to now, 14 belonged to the group of human and animal origin, the other 6 to 'geophilic' dermatophytes. The most frequently isolated were T. mentagrophytes, (var. interdigitale), M. audouinii, T. violaceum and T. rubrum, which altogether caused more than 50 % of the human ringworm found in the last 15 years. While T. mentagrophytes still remains the agent no. 1 of tinea pedis in Romania and T. rubrum keeps the first place in the nail involvements by dermatophytes, the first organism of tinea capitis are M. audouinii and T. violaceum, the former causing mainly the epidemic foci. Such epidemics may also be due to T. ferrugineum. The number of cases by T. schönleinii is more and more reduced. The infections by T. verruccosum are increasing. The rarity of cases due to T. gallinae, to T. equinum and to T. mengninii are pointed out. From the so called geophilic dermatophytes, M. gypseum, K. ajelloi and Chrysosporium keratinophilum are mostly isolated from the Romanian soil. During the last 12 years, T. tonsurans has been isolated from 250 cases of various mycotic infections, 80% in the adults, the rest in children. The age of infected patients varied from 1 to 66 years, the female being represented by 10%. Most of the cases (93%) involved the hair follicles, 45 of them affecting the bearded area, in which 4 clinical types could be distinguished: the superficial form, the kerion type (in 11% of the cases) and 2 intermediate forms. In 16% of cases, T. tonsurans attacked the scalp, simulating various nonmycotic dermatoses as impetigo, seborrheic dermatitis, tinea amiantacea etc. 5 cases of the scalp infection were found in the adults (3 of black-dot type, the other 2 of superficial tinea capitis). The same variety of clinical manifestations was observed in the glabrous skin, which this species affected in 7% of the cases. It also involved the nails in 2 cases only. T. tonsurans is in continuous decrease in Romania for the last ten years (4% of all the dermatophytes). This dermatophyte remained, till now, the main agent of tinea barbae in Romania. A case of mycetoma, with abscess-like lesions which appeared on the right forearm of a 43-year-old male, is briefly reported. A few whitish granules were detected in the oily-like discharge, the same as in the histologic examination. No bone involvement was discovered in X-ray examination. Actinomadura was identified in the culture. A daily dose of 4 g bactrin brought significant improvement to the patients continuing the treatment. [ABSTRACT FROM AUTHOR]

During a period of 8 years 300 cases of dermatophytoses involving both hairy areas and the glabrous skin were found to be caused by M. canis. There was scalp involvement in 60%, including 8 infants and 27 adults; most of the adults presented Kerion-like lesions and presented various clinical aspects such as seborrhea capitis, folliculitis and discois lupus erythematosus. In the 21 patients showing invasion of the beard the clinical manifestations included superficial erythematosquamous patches with hyperemic slightly elevated margins, folliculitis or abscess-like lesions and Kerion-like lesions. Among the lesions found on the glabrous skin there were unusual aspects of tinea faciei in 19 adults, mimicking lymphocytic infiltration, granuloma faciale or discoid lupus erythematosus. Some of the cases of tinea corporis found in 70 patients also had lesions simulating various other dermatological entities, including erythema multiforme, psoriasiform eruption, pityriasis rosea and seborrheic dermatitis. The hands were invaded in 5 adults patients, with involvement of the finger nails in one. Repeated mycologic examinations were necessary to establish the true etiology in many of these cases. Thirty-four psoriatic patients (23 males, 11 females) were found to have skin manifestations of dermatophyte infection. Tinea pedis was observed in 20 cases, tinea cruris in 6 and tinea mannum in 2. T. rubrum was the causative agent in all of these with the exception of 2 cases caused by E. floccosum. Lesions of tinea corporis were found intermingled with psoriatic plaques in various areas of the body skin in 6 patients (4 males, 2 females); T. rubrum was isolated from 5 of these and M. canis from one. Twenty-one of these psoriatic patients also had lesions caused by C. albicans in the toe-webs and interdigital aspects of the fingers, the latter being associated with paronychia in 9 cases. These findings indicate that we should remain aware of the possibility of fungus manifestations in patients with psoriasis, which would not appear to be an exceptional occurrence. A second case of actinomycetoma due to Nocardia caviae contracted in Israel is reported. A 39-year-old female of Yemenite origin, resident in Israel for more than 15 years, presented with a sclerotic tumor mass on the dorsum of the right foot which discharged an oily purulent fluid containing minute yellowish granules. Seen in the microscopic examination of smears were thin Gram-positive filaments and bacillary elements. Culture of the crushed granules yielded small pale colonies identified as Nocardia caviae. In vitro testing of the isolate to various antibiotics, isoniaside and sulphonamides showed the organism to be more sensitive to isoniaside and tetracycline. Isoniaside was administered for one year with marked improvement and was then replaced by tetracycline due to side effects, this treatment still being continued. [ABSTRACT FROM AUTHOR]

Three cases of scalp involvement by M. canis in infants are briefly reported. The first was a 2-month-old female with a few erythematous spots on the scalp, the second an 8-month-old male with lesions resembling seborrhoeic dermatitis and the third an 11-month-old male having areas of folliculitis of a kerion type. A group of 83 patients (45 females, 38 males) suffering from various cancerous diseases were investigated for the presence of fungi, specimens being obtained from the intertriginous areas of the body and toe and fingernails as well as areas of apparently healthy skin at sites less prone to fungal infection, including the abdomen, thorax and interscapular area. A group of 25 patients with non-cancerous disease served as controls Examination of the KOH-treated scrapings and cultures revealed that the nails were affected in 85 % of the neoplastic patients, the toe-webs and soles in 79 %, the groin in 72 % and the inframmammary fold in 33 %. T. rubrum was the predominant organism found in the nails and soles, C. albicans in the groin and breast T. mentagrophytes (interdigitale) was second in order of frequency and E. floccosum was found in only a few cases. Of note was the finding of these fungi in areas with no visible changes, particularly on the abdomen (in 23 patients), the back (in 17 cases) and thorax (in 11 cases). The latter areas also were found to harbour the blastospores of Pityrosporum spp. in 22 patients. There was a negative response to trichophytin and candidin antigens in all the patients. The tests for cell-mediated immunity performed in 22 patients provided no significant data. In the control group of non-cancerous patients there was no evidence for any of the above fungi on the back, abdomen or thorax area. The findings of this study indicate that the apparently healthy skin of cancerous patients shows a higher incidence of fungal invasion in both areas usually affected by fungi as well as those usually found not to be affected in non-cancerous individuals. A group of 199 healthy laboratory animals, comprising 63 guinea pigs, 58 white mice. 47 rats and 31 rabbits, was sampled for the presence of pathogenic dermatophytes. T. mentagrophytes, var. granulare, was isolated in 10% (5-guinea pigs, 6 mice, 6 rats and 2 rabbits) and M. gypseum was found in 7 animals (3 guinea pigs, 3 mice and one rat).
