Purpose: Since childhood exposure to radiation has been demonstrated to increase cancer risk with increase in radiation dose, reduced radiation exposure during computed tomography (CT) evaluation is desired for adolescent idiopathic scoliosis (AIS). Therefore, this retrospective study aimed to investigate the radiation dose of dual-source CT using a spectral shaping technique and the accuracy of the thoracic pedicle screw (TPS) placement for posterior spinal fusion (PSF) in patients with AIS. Methods: Fifty-nine female patients with thoracic AIS who underwent PSF using CT-guided TPSs were included and divided into two groups comprised of 23 patients who underwent dual-source CT (DSCT) with a tin filter (DSCT group) and 36 who underwent conventional multislice CT (MSCT group). We assessed the CT radiation dose using the CT dose index (CTDIvol), effective dose (ED), and accuracy of TPS insertion according to the established Neo's classification. Results: The DSCT and MSCT groups differed significantly (p < 0.001) in the mean CTDIvol (0.76 vs. 3.31 mGy, respectively) and ED (0.77 vs. 3.47 mSv, respectively). Although the correction rate of the main thoracic curve in the DSCT group was lower (65.7% vs. 71.2%) (p = 0.0126), the TPS accuracy (Grades 0–1) was similar in both groups (381 screws [88.8%] vs. 600 screws [88.4%], respectively) (p = 0.8133). No patient required replacement of malpositioned screws. Conclusion: Spectral shaping DSCT with a tube-based tin filter allowed a 75% radiation dose reduction while achieving TPS insertion accuracy similar to procedures based on conventional CT without spectral shaping. The videofluoroscopic swallowing study (VFSS) is a recognized standard diagnostic imaging technique that is used to investigate swallowing disorders and dysphagia. Patients were assessed in a seated posture on a chair or wheelchair. Using X-ray fluoroscopy, the state of patients' swallowing was checked by eating and drinking according to the physician's instructions. VFSS procedures are prolonged, and VFSS staff members are exposed to radiation. Therefore, we evaluated original lead shielding device (OLSD) that can be attached to the handrail of a table and placed vertically. The OLSD has a lead-equivalent thickness of 0.3 mmPb, weighs about 6 kg, and has the dimensions 50 cm × 50 cm × 8.0 mm. We used a human phantom and a radiation survey meter with and without protection from scattered radiation at the positions of the physician and medical staff at the height of 150 cm above the floor (i.e., the height of the eye's crystalline lens). After measuring the scattered radiation, we created radiation maps with and without the OLSD. The dose rate at the physician's position without and with the OLSD was 190 µSv/h and 92 µSv/h, respectively, and a dose reduction of 51.6% with the plate. Moreover, the radiation maps added clarity to the distribution of the scattered radiation. Such information should lead to greater awareness about exposures to physicians and other medical staff. Thus, the OLSD effectively provided protection from scattered radiation at the physician's position during fluoroscopy. It may contribute to the reduction of staff exposure for VFSS. Protection against occupational radiation exposure in clinical settings is important. This paper clarifies the present status of medical occupational exposure protection and possible additional safety measures. Radiation injuries, such as cataracts, have been reported in physicians and staff who perform interventional radiology (IVR), thus, it is important that they use shielding devices (e.g., lead glasses and ceiling-suspended shields). Currently, there is no single perfect radiation shield; combinations of radiation shields are required. Radiological medical workers must be appropriately educated in terms of reducing radiation exposure among both patients and staff. They also need to be aware of the various methods available for estimating/reducing patient dose and occupational exposure. When the optimizing the dose to the patient, such as eliminating a patient dose that is higher than necessary, is applied, exposure of radiological medical workers also decreases without any loss of diagnostic benefit. Thus, decreasing the patient dose also reduces occupational exposure. We propose a novel four-point policy for protecting medical staff from radiation: patient dose Optimization, Distance, Shielding, and Time (pdO-DST). Patient dose optimization means that the patient never receives a higher dose than is necessary, which also reduces the dose received by the staff. The patient dose must be optimized: shielding is critical, but it is only one component of protection from radiation used in medical procedures. Occupational eye dose monitoring during interventional radiology and interventional cardiology is important to avoid radiation-induced cataracts. The aim of this study was to assess the eye dose correlation with neck dose and patient-related quantities for interventional cardiology physicians and nurses. The originality of this study lies in obtaining correlations between the location of the dosimeter and eye dose radiation readings among different procedures and practitioners. The doses were measured for each procedure (18 procedures of coronary angiography and 16 procedures of percutaneous coronary intervention) using an active personal dosimeter. The eye dose for physicians was not correlated with the neck dose. The eye dose for nurses had a good correlation with the neck dose during both coronary angiography (R2 = 0.91) and percutaneous coronary intervention (R2 = 0.93). Kerma-area product values may be used for a rough estimation of the eye dose for physicians during routine coronary angiography procedures (R2 = 0.76). For nurses, the neck dose is a good proxy for the eye dose during coronary angiography and percutaneous coronary intervention procedures. [ABSTRACT FROM AUTHOR]

This study aimed to determine the placement distance, number, and position of the bismuth shield for developing a lens protective device for cone-beam computed tomography (CBCT). To determine the dose reduction rate, the lens doses were measured using an anthropomorphic head phantom and a real-time dosimeter. The image quality assessment was determined by analyzing the change in the pixel value, caused by the bismuth shield, and the artifact index was calculated from the pixel value and image noise within various regions of interest in the head phantom. When the distance between the bismuth shield and the subject was increased, the image quality deteriorated less, but there was also a decrease in the lens dose reduction rate. Upon changing the number of bismuth shields from 1-ply to 2-ply, the dose reduction rate increased; however, there was a decrease in the image quality. Additionally, placing the bismuth shield outside of the subject improved the dose reduction rate without deteriorating the image quality. The optimum placement conditions of the bismuth shield were concluded as follows: positioned outside, placed 10 mm from the surface of the subject, and used a 1-ply bismuth shield. When these placement conditions were used, the lens dose reduction rate was 26.9 ± 0.36% (right–left average) for the "bismuth shield: separate". Ionizing radiation exposure may not only cause acute radiation syndrome, but also an increased risk of late effects. It has been hypothesized that induction of chronic oxidative stress mediates the late effects of ionizing radiation. However, only a few reports have analyzed changes in long-term antioxidant capacity after irradiation in vivo. Our previous study demonstrated changes in whole-blood antioxidant capacity and red blood cell (RBC) glutathione levels within 50 days after total body irradiation (TBI). In this study, seven-week-old, male, C57BL/6J mice exposed to total body irradiation by X-ray and changes in whole-blood antioxidant capacity and RBC glutathione levels at ≥ 100 days after TBI were investigated. Whole-blood antioxidant capacity was chronically decreased in the 5-Gy group. The RBC reduced glutathione (GSH) level and the GSH/oxidative glutathione (GSSG) ratio were chronically decreased after ≥ 1 Gy of TBI. Interestingly, the complete blood counts (CBC) changed less with 1-Gy exposure, suggesting that GSH and the GSH/GSSG ratio were more sensitive radiation exposure markers than whole-blood antioxidant capacity and CBC counts. It has been reported that GSH depletion is one of the triggers leading to cataracts, hypertension, and atherosclerosis, and these diseases are also known as radiation-induced late effects. Radiation exposure during interventional radiology (IR) procedures is a critical issue. We have developed a wireless real-time dosimeter for IR patients that use nontoxic phosphor (four sensors). We evaluated the basic performance parameters (such as dose linearity, batch uniformity, reproducibility, and wireless-communication conditions) of the developed system using an IR X-ray system. Further, we investigated the influence of noise from other medical equipment on our wireless real-time dosimeter in the IR X-ray room. Overall, our wireless system exhibited excellent performance in terms of uniformity, reproducibility, and linearity; moreover, the wireless communication performance was better. The developed system enabled real-time visualization of patient radiation dose, without noise contamination from other medical equipment. In addition, the wireless system can be easily installed in a location where the PC screen (display) can be readily viewed by the IR physician. Hence, we developed a wireless system that can display the patient radiation dose data in real time; the system performed satisfactorily upon application in radiation dosimetry. Purpose: Cardiac resynchronization therapy (CRT) often requires a long fluoroscopic time and protection from scatter radiation. This study reports on scatter radiation levels during CRT, with and without additional shielding, and using standard or low pulse rate fluoroscopy.Materials and Methods: Additional lead-shielding drape (0.35-mm lead equivalent) was used on the left side of the table and pulsed fluoroscopy was performed at rates of 10 pulses/s (usual rate) and 7.5 pulses/s (low pulse rate). Fluoroscopy scatter radiation was measured for both pulse rates using an acrylic phantom with a radiation survey meter, both with and without the additional lead-shielding drape.Results: With the additional lead-shielding drape, the fluoroscopy scatter radiation was reduced by 74.3% at 10 pulses/s and 78.6% at 7.5 pulses/s. If the fluoroscopy was changed from 10 pulses/s to 7.5 pulses/s, the scattered radiation at the primary physician's position was reduced by 24.0%. The combined use of additional shielding drape and low pulse rate fluoroscopy reduced scatter radiation by over 80%.Conclusion: Additional lead-shielding drape and low pulse rate fluoroscopy are effective in reducing the scattered radiation dose to physicians and nurses during CRT. Radiation dose management is important in interventional radiology (IR) procedures, such as percutaneous coronary intervention, to prevent radiation-induced injuries. Therefore, radiation dose should be monitored in real time during IR. This study evaluated the fundamental characteristics of a novel real-time skin dosimeter (RTSD) developed at our institution. In addition, we compared the performance of our new and old radiation sensors and that of a skin dose monitor (SDM), with ion chamber reference values. We evaluated the fundamental characteristics (e.g., energy dependence, dose dependence, and angular dependence) of the RTSD developed by us in the diagnostic X-ray energy range. The performance of our RTSD was similar to that of the SDM. In particular, the new radiation sensor of our RTSD demonstrated better dose rate dependence compared to the old sensor. In addition, the new sensor had the advantage of being small in size and thus minimally affecting the X-ray images compared to the old sensor. Therefore, the developed skin dosimeter and radiation sensor may be useful in real-time measurement of patients’ exposure to and multi-channel monitoring of radiation in IR procedures. Interventional radiology procedures often involve lengthy exposure to fluoroscopy-derived radiation. We therefore devised a videofluoroscopic swallowing study (VFSS) procedure using a human phantom that proved to protect the patient and physician by reducing the radiation dose. We evaluated a new lead-shielding device and separately attached additional filters (1.0-, 2.0-, and 3.0-mm Al filters and a 0.5-mm Cu filter) during VFSS to reduce the patient's entrance skin dose (ESD). A monitor attached to the human phantom's neck measured the ESD. We also developed another lead shield (VFSS Shielding Box, 1.0-mm Pb equivalent) and tested its efficacy using the human phantom and an ionization chamber radiation survey meter with and without protection from scattered radiation at the physician's position on the phantom. We then measured the scattered radiation (at 90 and 150 cm above the floor) after combining the filters with the VFSS Shielding Box. With the additional filters, the ESD was reduced by 15.4-55.1%. With the VFSS Shielding Box alone, the scattered radiation was reduced by about 10% compared with the dose without additional shielding. With the VFSS Shielding Box and filters combined, the scattered radiation dose was reduced by a maximum of about 44% at the physician's position. Thus, the additional lead-shielding device effectively provided protection from scattered radiation during fluoroscopy. The purposes of this study were (1) to compare the radiation doses for 320- and 80-row fetal-computed tomography (CT), estimated using thermoluminescent dosimeters (TLDs) and the ImPACT Calculator (hereinafter referred to as the 'CT dosimetry software'), for a woman in her late pregnancy and her fetus and (2) to estimate the overlapped fetal radiation dose from a 320-row CT examination using two different estimation methods of the CT dosimetry software. The direct TLD data in the present study were obtained from a previous study. The exposure parameters used for TLD measurements were entered into the CT dosimetry software, and the appropriate radiation dose for the pregnant woman and her fetus was estimated. When the whole organs (e.g., the colon, small intestine, and ovaries) and the fetus were included in the scan range, the difference in the estimated doses between the TLD measurement and the CT dosimetry software measurement was <1 mGy (<23 %) in both CT units. In addition, when the whole organs were within the scan range, the CT dosimetry software was used for evaluating the fetal radiation dose and organ-specific doses for the woman in the late pregnancy. The conventional method using the CT dosimetry software cannot take into account the overlap between volumetric sections. Therefore, the conventional method using a 320-row CT unit in a wide-volume mode might result in the underestimation of radiation doses for the fetus and the colon, small intestine, and ovaries. Videofluoroscopic swallowing study (VFSS) is considered the standard diagnostic imaging technique to investigate swallowing disorders and dysphagia. Few studies have been reported concerning the dose of radiation a patient receives and the scattering radiation dose received by a physician during VFSS. In this study, we investigated the dose of radiation (entrance skin dose, ESD) estimated to be received by a patient during VFSS using a human phantom (via a skin-dose monitor sensor placed on the neck of the human phantom). We also investigated the effective dose (ED) and dose equivalent (DE) received by a physician (wearing two personal dosimeters) during an actual patient procedure. One dosimeter (whole body) was worn under a lead apron at the chest, and the other (specially placed to measure doses received by the lens of the eye) outside the lead apron on the neck collar to monitor radiation doses in parts of the body not protected by the lead apron. The ESD for the patient was 7.8 mGy in 5 min. We estimated the average patient dose at 12.79 mGy per VFSS procedure. The physician ED and DE during VFSS were 0.9 mSv/year and 2.3 mSv/year, respectively. The dose of radiation received by the physician in this study was lower than regulatory dose limits. However, in accordance with the principle that radiation exposure should be as low as reasonably achievable, every effort should be made (e.g., wearing lead glasses) to reduce exposure doses. As the use of diagnostic X-ray equipment with flat panel detectors (FPDs) has increased, so has the importance of proper management of FPD systems. To ensure quality control (QC) of FPD system, an easy method for evaluating FPD imaging performance for both stationary and moving objects is required. Until now, simple rotatable QC phantoms have not been available for the easy evaluation of the performance (spatial resolution and dynamic range) of FPD in imaging moving objects. We developed a QC phantom for this purpose. It consists of three thicknesses of copper and a rotatable test pattern of piano wires of various diameters. Initial tests confirmed its stable performance. Our moving phantom is very useful for QC of FPD images of moving objects because it enables visual evaluation of image performance (spatial resolution and dynamic range) easily. Today, interventional radiology (IR) X-ray units are required for display of doses at an interventional reference point (IRP) for the operator (IR physician). The dose displayed at the IRP (the reference dose) of an X-ray unit has been reported to be helpful for characterizing patient exposure in real time. However, no detailed report has evaluated the accuracy of the reference doses displayed on X-ray equipment. Thus, in this study, we compared the displayed reference dose to the actual measured value in many IR X-ray systems. Although the displayed reference doses of many IR X-ray systems agreed with the measured actual values within approximately 15%, the doses of a few IR units were not close. Furthermore, some X-ray units made in Japan displayed reference doses quite different from the actual measured value, probably because the reference point of these units differs from the International Electrotechnical Commission standard. Thus, IR physicians should pay attention to the location of the IRP of the displayed reference dose in Japan. Furthermore, physicians should be aware of the accuracy of the displayed reference dose of the X-ray system that they use for IR. For reducing the risk of skin injury during interventional radiology (IR) procedures, it has been suggested that physicians track patients’ exposure doses. The metal-oxide semiconductor field effect transistor (MOSFET) dosimeter is designed to measure patient exposure dose during radiotherapy applications at megavoltage photon energies. Our purpose in this study was to evaluate the feasibility of using a MOSFET dosimeter (OneDose system) to measure patients’ skin dose during exposure to diagnostic X-ray energies used in IR. The response of the OneDose system was almost constant at diagnostic X-ray energies, although the sensitivity was higher than that at megavoltage photon energies. We found that the angular dependence was minimal at diagnostic X-ray energies. The OneDose is almost invisible on X-ray images at diagnostic energies. Furthermore, the OneDose is easy to handle. The OneDose sensor performs well at diagnostic X-ray energies, although real-time measurements are not feasible. In pediatric patients, the lower body weight limits the volume of contrast medium. Conventionally, angiocardiography is carried out with a single large bolus injection of contrast material. Angiocardiograms are used in pediatric patients with complex congenital heart diseases. In patients with complex congenital heart disease, especially with MAPCA, the volume of contrast medium used may be excessive. This would allow further injection to provide additional information. To reduce contrast medium used in the angiocardiogram in pediatric patients, we decided to use electrocardiogram (ECG)-gated multiple diastolic injection (EMDI). Three small boluses were injected during the diastolic phase of three consecutive cardiac cycles using a commercially-available power injector. Seventy-eight ventriculograms (47 left ventriculograms and 31 right ventriculograms) using EMDI were carried out on 53 patients with congenital heart disease. Total contrast medium volume with EMDI ventriculograms (mean [±SD] per body weight: 0.72 [±0.25] ml/kg) was significantly smaller than with conventional injection (1.01 [±0.36] ml/kg) (p <0.001). The grades of ventriculograms with EMDI tended to be slightly better than those with conventional injection (statistically not significant, p = 0.478). No short-run type premature ventricular contraction (PVC)s or intramural injection occurred in the ventriculograms with EMDI. PVCs tended to be less frequent in the ventriculograms with EMDI than in those with conventional injection (statistically not significant, p = 0.131). EMDI may be worthwhile in reducing ventricular ectopy when checking ventricular function by angiography, since hemodynamic conditions are less affected by small quantities of contrast medium and only during the filling phase of the ventricles.
