Most soundfield synthesis approaches deal with extensive and regular loudspeaker arrays, which are often not suitable for home audio systems, due to physical space constraints. In this article, we propose a technique for soundfield synthesis through more easily deployable irregular loudspeaker arrays, i.e., where the spacing between loudspeakers is not constant, based on deep learning. The input are the driving signals obtained through a plane wave decomposition-based technique. While the considered driving signals are able to correctly reproduce the soundfield with a regular array, they show degraded performances when using irregular setups. Through a complex-valued convolutional neural network (CNN), we modify the driving signals in order to compensate the errors in the reproduction of the desired soundfield. Since no ground truth driving signals are available for the compensated ones, we train the model by calculating the loss between the desired soundfield at a number of control points and the one obtained through the driving signals estimated by the network. The proposed model must be retrained for each irregular loudspeaker array configuration. Numerical results show better reproduction accuracy with respect to the plane wave decomposition-based technique, pressure-matching approach, and linear optimizers for driving signal compensation. How can an intentional movement be distinguished from the same movement done nonintentionally? How can this distinction be drawn without asking the subject, or in patients who are unable to communicate? Here we address these questions, by focusing on blinking. This is one of the most frequent spontaneous actions in our daily life, but it can also be done intentionally. Furthermore, blinking is often spared in patients with severe brain injuries, and for some, it is the only way to report complex meanings. Using kinematic and EEG-based measures, we found that intentional and spontaneous blinking are preceded by different brain activities, even when they are indistinguishable. Unlike spontaneous ones, intentional blinks are characterized by a slow negative EEG drift, resembling the classic readiness potential. We investigated the theoretical implication of this finding in stochastic decision models as well as the practical significance of using brain-based signals to improve the discrimination between intentional and nonintentional actions. As proof of principle, we considered three brain-injured patients with rare neurological syndromes characterized by motor and communicative impairments. In recent years, the adoption of deep learning techniques has allowed to obtain major breakthroughs in the automatic music generation research field, sparking a renewed interest in generative music. A great deal of work has focused on the possibility of conditioning the generation process in order to be able to create music according to human-understandable parameters. In this paper, we propose a technique for generating chord progressions conditioned on harmonic complexity, as grounded in the Western music theory. More specifically, we consider a pre-existing dataset annotated with the related complexity values and we train two variations of Variational Autoencoders (VAE), namely a Conditional-VAE (CVAE) and a Regressor-based VAE (RVAE), in order to condition the latent space depending on the complexity. Due to improvements in medicine, the figures of patients with disorders of consciousness (DoC) are increasing. Diagnostics of DoC and prognostication of rehabilitation outcome is challenging but necessary to evaluate recovery potential and to decide on treatment options. Such decisions should be made by doctors and patients' surrogates based on medico-ethical principles. Meeting information needs and communicating effectively with caregivers as the patients´ most common surrogate-decision makers is crucial, and challenging when novel tech-nologies are introduced. This qualitative study aims to explore information needs of informal DoC caregivers, how they manage the obtained information and their perceptions and experiences with caregiver-physician communication in facilities that implemented innovative neurodiagnostics studies. In 2021, we conducted semi-structured interviews with nine caregivers of clinically stable DoC patients in two rehabilitation centers in Italy and Germany. Participants were selected based on consecutive purposeful sampling. Caregivers were recruited at the facilities after written informed consent. All interviews were recorded, transcribed verbatim and translated. For analysis, we used reflexive thematic analysis according to Braun & Clarke (2006). Caregivers experienced the conversations emotionally, generally based on the value of the information provided. They reported to seek positive information, comfort and empathy with-in the communication of results of examinations. They needed detailed information to gain a deep understanding and a clear picture of their loved-one's condition. The results suggest a mismatch between the perspectives of caregivers and the perspectives of medical profession-als, and stress the need for more elaborate approaches to the communication of results of neu-rodiagnostics studies. Background and purpose: Stroke-associated pneumonia (SAP) affects 10 to 38% of patients in the acute phase of stroke. Stroke patients diagnosed with dysphagia have an 11-fold higher risk of developing SAP. Thus, identifying dysphagic patients through a highly accurate screening tool might be crucial in reducing the incidence of SAP. We present a case–control study designed to evaluate efficacy in reducing the risk of SAP between two swallowing screening tools, the classic water swallow test (WST) and a recently validated tool such as the GLOBE-3S (the Sapienza GLObal Bedside Evaluation of Swallowing after Stroke), which is a highly sensitive swallowing screening tool particularly accurate in detecting silent aspiration as well. Methods: We analyzed the occurrence of dysphagia in 100 acute stroke patients distributed in two groups: half were screened with WST and the other half with GLOBE-3S. Results: Dysphagia was diagnosed in 28 patients. The main result is that, among patients who passed the dysphagia screenings, none of those screened with the GLOBE-3S method developed pneumonia compared to 31.82% in the WST group. Discriminant function analysis (DFA) showed that NIH Stroke Scale (NIHSS) score and the dysphagia screening method (i.e., GLOBE-3S vs. WST) were the two main factors in the SAP's predicting model and the only significant ones per se. The response of the San Pietro monumental bell-tower located in Perugia, Italy, to the 2016 Central Italy seismic sequence is investigated, taking advantage of the availability of field data recorded by a vibration-based SHM system installed in December 2014 to detect earthquake-induced damages. The tower is located about 85 km in the NW direction from the epicenter of the first major shock of the sequence, the Accumoli Mw6.0 earthquake of August 24th, resulting in a small local PGA of about 30 cm/s, whereby near-field PGA was measured as 915.97 cm/s (E-W component) and 445.59 cm/s (N-S component). Similar PGA values also characterized the two other major shocks of the sequence (Ussita Mw5.9 and Norcia Mw6.5 earthquakes of October 26th and 30th, respectively). Despite the relatively low intensity of such earthquakes in Perugia, the analysis of long-term monitoring data clearly highlights that small permanent changes in the structural behavior of the bell-tower have occurred after the earthquakes, with decreases in all identified natural frequencies. Such natural frequency decays are fully consistent with what predicted by non-linear finite element simulations and, in particular, with the development of microcracks at the base of the columns of the belfry. Microcracks in these regions, and in the rest of tower, are however hardly distinguishable from pre-existing ones and from the physiological cracking of a masonry structure, what validates the effectiveness of the SHM system in detecting earthquake-induced damage at a stage where this is not yet detectable by visual inspections. In Italy fluoroquinolones (FQs) are extensively prescribed in empirical therapy of uncomplicated urinary tract infection (UTI) despite recommendations in national guidelines and widespread antibiotic resistance in community. To survey the dissemination of plasmid-mediated quinolone resistance in a peak area of FQs consumption, E. coli strains from 154 community and 41 local hospital patients were collected; low level ciprofloxacin resistance qnrA, qnrB, qnrS, and aac(6)′-Ib-cr genes were screened by PCR and patterns of transferable resistances were determined. Clinical ciprofloxacin resistance in hospital doubled community value, while overall rates of FQ resistance genes were similar (31.6% and 27.8%). Prevalence of aac(6′)-Ib-cr gene was 11% in outpatients (21%, inpatients) and risk of harbouring this variant was significantly associated with gentamicin resistance; linkage to ceftazidime resistance was significant ( P = 0.001) and six out of eight strains produced CTX-M-15 and TEM-1 beta lactamases. In transconjugants, the unique pattern ampicillin/kanamycin-gentamicin/ ESBL + was associated with aac(6′)-Ib-cr gene presence and with an increase of ciprofloxacin MIC value. Data highlight the need to monitor the resistance risk factors in the local community to provide clinicians with well-grounded guidelines for UTI therapy. [ABSTRACT FROM AUTHOR]

Hepatitis B virus (HBV) replicating in patients with serum hepatitis B surface antigen and antibody to hepatitis B e antigen (cryptic HBV replication) can be detected by a spot hybridization technique for serum HBV-DNA and by immunoperoxidase staining of hepatitis B core antigen in the liver. These methods allowed us to study the effects of chronic (13 to 82, mean 30 months) administration of low doses (10 or 15 mg/day) of prednisone to 17 patients with chronic active hepatitis and cryptic HBV replication. Liver biopsies performed before treatment demonstrated that 1 to 50% (mean 12%) of the liver cells were infected. After therapy, infected cells had disappeared in 5 (29%), were considerably reduced in 9 (53%) and remained unchanged in 3 (18%) patients. The mean percentage of infected cells in the liver biopsies performed at the end of the follow-up was 3.2±5.5% (p<0.005). Serum HBV-DNA was present in 12 of 13 and in 5 of 12 patients investigated before treatment and at the end of the study, respectively. Five patients harboring HBV in the liver developed cirrhosis during treatment. Our data indicate that, despite steroid therapy, HBV replication either ceased or was decreased in two thirds of the patients, while in no case it flared-up. Unresponsive wakefulness syndrome (UWS) patients may retain intact portions of the thalamocortical system that are spontaneously active and reactive to sensory stimuli but fail to engage in complex causal interactions, resulting in loss of consciousness. Here, we show that loss of brain complexity after severe injuries is due to a pathological tendency of cortical circuits to fall into silence (OFF-period) upon receiving an input, a behavior typically observed during sleep. Spectral and phase domain analysis of EEG responses to transcranial magnetic stimulation reveals the occurrence of OFF-periods in the cortex of UWS patients (N = 16); these events never occur in healthy awake individuals (N = 20) but are similar to those detected in healthy sleeping subjects (N = 8). Crucially, OFF-periods impair local causal interactions, and prevent the build-up of global complexity in UWS. Our findings link potentially reversible local events to global brain dynamics that are relevant for pathological loss and recovery of consciousness. Many brain-injured patients retain large cortical islands that are intact, active and reactive but blocked in a state of low complexity, leading to unconsciousness.
