For more than two decades there has been a continuous rise in opioid overdose related deaths. The majority of the deaths include the age range when, traditionally, individuals are likely to attend college or university. As a result, Vassar College has taken the important initiative and created and implemented a new opioid overdose intervention strategy and action plan called AED+. AED+ expands on the Model of Greater Awareness, Training and Increased Availability of and Accessibility to Life Saving Intervention Devices; a model that was created based on AED devices improving outcomes of out-of-hospital cardiac arrest emergencies. Similar to AED's improving out-of-hospital cardiac arrest outcomes, the + component of the AED+ initiative increases awareness and provides basic, targeted education about naloxone and its use. Furthermore, the education includes information about naloxone's greater availability and its more immediate access across the campus by students, staff, faculty, administrators, and visitors in the event of a suspected opioid overdose. Starting in May 2023, members of the school's Health Service and senior administrative leaders identified it necessary to be proactive and not reactive to managing an opioid overdose in the campus community. Although Vassar College has not recently experienced an opioid overdose, it is confidently projected that these targeted actions will proactively and positively reduce the likelihood of opioid-related fatalities on campus. Furthermore, it is the purpose of this article to share the AED+ model so other colleges and universities can modify it to best fit their unique setting in order to improve opioid overdose outcomes. Herbaceous isolated wetlands in the North American Southeastern Coastal Plain are important breeding sites for many imperiled amphibians. However, most are degraded from alterations to historic fire disturbance and hydrologic regimes. Without fire, encroaching woody vegetation can transition wetlands to more terrestrial conditions and negatively impact amphibian breeding habitat, yet few studies have experimentally tested the efficacy, cost, or temporal requirement of current methods to restore herbaceous wetland vegetation. Here, we tested the interaction of manipulating wetland canopy and leaf litter/duff to promote herbaceous vegetation within one year (i.e., one breeding season) in degraded herbaceous wetlands in South Carolina. We assessed plant response via herbaceous cover, composition, and species similarity to the wetland seed bank and then related treatment performance to treatment cost. Removing trees combined with burning, disturbing, or removing duff significantly increased herbaceous cover and proportions of wetland plants and graminoids. Removing trees alone did not improve herbaceous cover compared to closed-canopy controls, and manipulating duff alone had limited positive effects on plant cover and composition. The most expensive yet effective treatment was Tree Removal-Duff Removal, while Tree Removal-Duff Disturbance was the most cost-effective. At a minimum, we recommend removing trees and burning to kickstart herbaceous recovery. Promisingly, comparisons of our data with previous seed bank studies from these same wetlands indicate there was limited seed bank attrition during 30 years of woody encroachment. Salt marshes are globally important ecosystems, but many have been lost or transformed due to the impacts of global change. There have been attempts to broadly quantify salt marsh communities, especially the ubiquitous grasses which serve as foundation species such as Spartina alterniflora and Spartina patens, the latter of which is being lost due to sea-level rise. However, few researchers have used high-resolution geospatial imagery to quantify fine-scale changes in the distribution of grasses or to track losses of S. patens. To address this issue, we utilized a simple and rapid method of classifying geospatial marsh imagery with cloud-based machine learning in Google Earth Engine (> 94.59% accuracy for S. patens across all models for 2006 and 2019). Our methods allowed us to characterize large landscapes (two geospatially proximal areas, > 7000 ha each) of critical salt marshes on the New Jersey coast and to evaluate fine-scale (1 m) community transformations in response to global change with imagery from 2006 to 2019. Notably, one marsh experienced very little change while the other experienced an 81.17% (1087 ha) loss of S. patens, illuminating disparate patterns of change for two geographically proximal ecosystems. Further exploration revealed an association in the loss of S. patens with increases in streamflow and total nitrogen content in the rivers that run through each marsh. These results signify the importance of broad-scale ecological studies that evaluate fine-scale community transformations and for management strategies that do not generalize across landscapes of an ecosystem type. Context is critical for conceptual processing, but the mechanism underpinning its encoding and reinstantiation during abstract concept processing is unclear. Context may be especially important for abstract concepts—we investigated whether episodic context is recruited differently when processing abstract compared with concrete concepts. Experiments 1 and 2 presented abstract and concrete words in arbitrary contexts at encoding (Experiment 1: red/green colored frames; Experiment 2: male/female voices). Recognition memory for these contexts was worse for abstract concepts. Again using frame color and voice as arbitrary contexts, respectively, Experiments 3 and 4 presented words from encoding in the same or different context at test to determine whether there was a greater recognition memory benefit for abstract versus concrete concepts when the context was unchanged between encoding and test. Instead, abstract concepts were less likely to be remembered when context was retained. This suggests that at least some types of episodic context—when arbitrary—are attended less, and may even be inhibited, when processing abstract concepts. In Experiment 5, we utilized a context—spatial location—which (as we show) tends to be relevant during real-world processing of abstract concepts. We presented words in different locations, preserving or changing location at test. Location retention conferred a recognition memory advantage for abstract concepts. Thus, episodic context may be encoded with abstract concepts when context is relevant to real-world processing. The systematic contexts necessary for understanding abstract concepts may lead to arbitrary context inhibition, but greater attention to contexts that tend to be more relevant during real-world processing. Brain metastases comprise 40% of all metastatic tumours and breast tumours are among the tumours that most commonly metastasise to the brain, the role that epigenetic gene dysregulation plays in this process is not well understood. We carried out 450 K methylation array analysis to investigate epigenetically dysregulated genes in breast to brain metastases (BBM) compared to normal breast tissues (BN) and primary breast tumours (BP). For this, we referenced 450 K methylation data for BBM tumours prepared in our laboratory with BN and BP from The Cancer Genome Atlas. Experimental validation on our initially identified genes, in an independent cohort of BP and in BBM and their originating primary breast tumours using Combined Bisulphite and Restriction Analysis (CoBRA) and Methylation Specific PCR identified three genes (RP11-713P17.4, MIR124-2, NUS1P3) that are hypermethylated and three genes (MIR3193, CTD-2023M8.1 and MTND6P4) that are hypomethylated in breast to brain metastases. In addition, methylation differences in candidate genes between BBM tumours and originating primary tumours shows dysregulation of DNA methylation occurs either at an early stage of tumour evolution (in the primary tumour) or at a later evolutionary stage (where the epigenetic change is only observed in the brain metastasis). Epigentic changes identified could also be found when analysing tumour free circulating DNA (tfcDNA) in patient's serum taken during BBM biopsies. Native biodiversity decline and non-native species spread are major features of the Anthropocene. Both processes can drive biotic homogenization by reducing trait and phylogenetic differences in species assemblages between regions, thus diminishing the regional distinctiveness of biotas and likely have negative impacts on key ecosystem functions. However, a global assessment of this phenomenon is lacking. Here, using a dataset of >200,000 plant species, we demonstrate widespread and temporal decreases in species and phylogenetic turnover across grain sizes and spatial extents. The extent of homogenization within major biomes is pronounced and is overwhelmingly explained by non-native species naturalizations. Asia and North America are major sources of non-native species; however, the species they export tend to be phylogenetically close to recipient floras. Australia, the Pacific and Europe, in contrast, contribute fewer species to the global pool of non-natives, but represent a disproportionate amount of phylogenetic diversity. The timeline of most naturalisations coincides with widespread human migration within the last ~500 years, and demonstrates the profound influence humans exert on regional biotas beyond changes in species richness. Human-driven movements and extinctions of species have made plant communities across biomes more homogenous. Desiccation tolerance was a key trait that allowed plants to colonize land. However, little is known about the transition from desiccation tolerant non-vascular plants to desiccation sensitive vascular ones. Filmy ferns (Hymenophyllaceae) represent a useful system to investigate how water-stress strategies differ between non-vascular and vascular stages within a single organism because they have vascularized sporophytes and nonvascular gametophytes that are each capable of varying degrees of desiccation tolerance. To explore this, we surveyed sporophytes and gametophytes of 19 species (22 taxa including varieties) of filmy ferns on Moorea (French Polynesia) and used chlorophyll fluorescence to measure desiccation tolerance and light responses. We conducted phylogenetically informed analyses to identify differences in physiology between life stages and growth habits. Gametophytes had similar or less desiccation tolerance (ability to recover from 2 days desiccation at − 86 MPa) and lower photosynthetic optima (maximum electron transport rate of photosystem II and light level at 95% of that rate) than sporophytes. Epiphytes were more tolerant of desiccation than terrestrial species in both life stages. Despite their lack of greater physiological tolerances, gametophytes of several species occurred over a wider elevational range than conspecific sporophytes. Our results demonstrate that filmy fern gametophytes and sporophytes differ in their physiology and niche requirements, and point to the importance of microhabitat in shaping the evolution of water-use strategies in vascular plants. Invasive species are a major threat to biodiversity and ecosystem function. Thus, understanding their spread and ecological impacts is critical for management and control. Strawberry guava (Psidium cattleianum Sabine) is an aggressive invader across the tropics and has been rapidly spreading throughout the eastern rainforests of Madagascar. However, both the mechanisms of its spread on the island and the consequences of its invasion on native floral and faunal communities remain largely unexplored. By surveying multiple sites across Madagascar's eastern rainforests, we demonstrate that the introduction of P. cattleianum significantly correlates with changes in forest structure—namely tree/shrub size, taxonomic richness, and taxonomic diversity. Further, at a local scale, the presence of P. cattleianum was associated with an increase in frugivore species richness; its primary dispersers during our study period were lemurs. Moreover, we identified species-specific effects of lemur gut-passage on the germination of P. cattleianum seeds. Finally, microsatellite analysis of P. cattleianum from a variety of locations across Madagascar demonstrated three distinct, highly differentiated, genetic population clusters, each with high levels of intra-population diversity, suggesting multiple independent introductions of P. cattleianum into Madagascar followed by long-distance dispersal. Collectively, these findings illuminate the complex status of strawberry guava invasion in Madagascar, which poses a growing threat to the island's unique flora and yet provides important forage for threatened and charismatic animal species. Meningiomas are the commonest types of tumours in the central nervous system (CNS). It is a benign type of tumour divided into three WHO grades (I, II and III) associated with tumour growth rate and likelihood of recurrence, where surgical outcomes and patient treatments are dependent on the meningioma grade and histological subtype. The development of alternative approaches based on attenuated total reflection Fourier-transform infrared (ATR-FTIR) spectroscopy could aid meningioma grade determination and its biospectrochemical profiling in an automated fashion. Herein, ATR-FTIR in combination with chemometric techniques is employed to distinguish grade I, grade II and grade I meningiomas that re-occurred. Ninety-nine patients were investigated in this study where their formalin-fixed paraffin-embedded (FFPE) brain tissue samples were analysed by ATR-FTIR spectroscopy. Subsequent classification was performed via principal component analysis plus linear discriminant analysis (PCA-LDA) and partial least squares plus discriminant analysis (PLS-DA). PLS-DA gave the best results where grade I and grade II meningiomas were discriminated with 79% accuracy, 80% sensitivity and 73% specificity, while grade I versus grade I recurrence and grade II versus grade I recurrence were discriminated with 94% accuracy (94% sensitivity and specificity) and 97% accuracy (97% sensitivity and 100% specificity), respectively. Several wavenumbers were identified as possible biomarkers towards tumour differentiation. The majority of these were associated with lipids, protein, DNA/RNA and carbohydrate alterations. These findings demonstrate the potential of ATR-FTIR spectroscopy towards meningioma grade discrimination as a fast, low-cost, non-destructive and sensitive tool for clinical settings. The constant failure of single-target drug therapies for ischemic stroke necessitates the development of novel pleiotropic pharmacological treatment approaches, to effectively combat the aftermath of this devastating disorder. The major objective of our study involves a multi-target drug repurposing strategy to stabilize hypoxia-inducible factor-1 α (HIF-1α) via a structure-based screening approach to simultaneously inhibit its regulatory proteins, PHD2, FIH, and pVHL. Out of 1424 Food and Drug Administration (FDA)-approved drugs that were screened, folic acid (FA) emerged as the top hit and its binding potential to PHD2, FIH, and pVHL was further verified by re-docking, molecular dynamics (MD) simulation and by Drug Affinity Responsive Target Stability (DARTS) assay. HIF-1α stabilization by FA was demonstrated by the nuclear translocation and increased green fluorescence emission of HIF-1α using HIF1α-GFPSpark tag vector. Further, FA treatment enhanced the cell survival following oxygen glucose deprivation and its neuroprotective mechanism was elucidated by measuring the expression of BAX, NFE2L2, VEGF, and EPO genes in a time-dependent manner (5 and 11 h following FA treatment). VEGF and EPO expressions were significantly increased by 5.41- and 1.35-folds, respectively, whereas BAX expression reduced by 4-fold at 11 h post-FA treatment. NFE2L2 expression was elevated (1.65-fold) at 5 h with no major difference at 11 h post-FA treatment. The chicken chorioallantoic membrane (CAM) assay demonstrated the pro-angiogenic potential of FA as evidenced by an increased blood vessel density and branching. The present study elucidates for the first time that the post-ischemic neuroprotection exerted by FA may be attributed to its HIF-1α stabilization and pro-angiogenic properties. Necroptosis or programmed necrosis is evident in various neurological disorders such as ischemic stroke. Receptor interacting serine/threonine protein kinase 3 (RIPK3) is one of the crucial targets of necroptosis and inhibition of this protein exerts neuroprotection. However, knowledge regarding the three-dimensional structure and binding site information of this protein is lacking. In the present study, structure-based in silico methods were implemented to identify the key amino acids in the RIPK3 binding site that might be responsible for ligand interactions. Further, novel RIPK3 inhibitors were identified through a dual ensemble screening strategy. The ability to diagnose cancer rapidly with high sensitivity and specificity is essential to exploit advances in new treatments to lead significant reductions in mortality and morbidity. Current cancer diagnostic tests observing tissue architecture and specific protein expression for specific cancers suffer from inter-observer variability, poor detection rates and occur when the patient is symptomatic. A new method for the detection of cancer using 1 μl of human serum, attenuated total reflection-Fourier transform infrared spectroscopy and pattern recognition algorithms is reported using a 433 patient dataset (3897 spectra). To the best of our knowledge, we present the largest study on serum mid-infrared spectroscopy for cancer research. We achieve optimum sensitivities and specificities using a Radial Basis Function Support Vector Machine of between 80.0 and 100 % for all strata and identify the major spectral features, hence biochemical components, responsible for the discrimination within each stratum. We assess feature fed-SVM analysis for our cancer versus non-cancer model and achieve 91.5 and 83.0 % sensitivity and specificity respectively. We demonstrate the use of infrared light to provide a spectral signature from human serum to detect, for the first time, cancer versus non-cancer, metastatic cancer versus organ confined, brain cancer severity and the organ of origin of metastatic disease from the same sample enabling stratified diagnostics depending upon the clinical question asked. A 62-year-old white woman with a history of hepatitis C and type 2 diabetes mellitus developed urticaria during treatment with ustekinumab for plaque psoriasis. The patient received two 45-mg ustekinumab injections in her first 2 months and then one 45-mg injection every 3 months for her psoriasis. After 10 months, she developed a round red rash on her skin diffusely on her body. She also complained of joint pain in her hands. Rheumatology became involved, and investigations revealed that her antinuclear antibody titer was negative, but her rheumatoid factor, erythrocyte sedimentation rate, and liver function enzymes were elevated. She was diagnosed with urticaria, and ustekinumab was discontinued. A Naranjo assessment score of 6 was obtained, indicating a probable relationship between the patient's urticaria and her use of ustekinumab. Ustekinumab was subsequently discontinued, and the patient received a course of Harvoni (Gilead Sciences, Inc., Forest City, California, United States) (ledispasvir/sofosbuvir) with clearance of the hepatitis C virus. The ability to diagnose brain cancer rapidly from serum samples is of great interest; such a diagnosis would allow for rapid testing and time to results providing a responsive diagnostic environment, ability to monitor treatment efficacy, early detection of recurrent tumours and screening techniques. Current methods rely upon subjective, time-consuming tests such as histological grading and are particularly invasive with the diagnostic test requiring hospitalisation of 2-3 days. A rapid diagnostic method based upon serum samples would allow for a relatively non-invasive test and open up the possibility of screening for brain cancer. We report for the first time the use of a Bioplex immunoassay to provide cytokine and angiogenesis factor levels that differ between serum from glioma and non-cancer patients specifically angiopoietin, follistatin, HGF, IL-8, leptin, PDGF-BB and PECAM-1 providing sensitivities and specificities as high as 88 % and 81 %, respectively. We also report, for the first time, the use of serum ATR-FTIR combined with a RBF SVM for the diagnosis of gliomas from non-cancer patients with sensitivities and specificities as high as 87.5 % and 100 %, respectively. We describe the combination of these techniques in an orthogonal diagnostic regime, providing strength to the diagnosis through data combinations, in a rapid diagnostic test within 5 h from serum collection (10 min for ATR-FTIR and 4 h for the Bioplex Immunoassay). This regime has the ability to revolutionise the clinical environment by providing objective measures for diagnosis allowing for increased efficiency with corresponding decreases in mortality, morbidity and economic impact upon the health services. This paper focuses on agenda change affecting the politics of 'fracking operations' in the US, a process of extracting natural gas from underground shale formations. We examine how the movement of this policy issue between the state and federal levels of government has become increasingly contentious because of rising public concern about pollution impacts. Using information obtained from documentary sources and media content analysis, we found that the natural gas policy coalition has largely focused on a political strategy based on maintaining fracking regulatory controls at the state level, while the environmental policy coalition has pushed for increased regulation of drilling practices in general, including a larger policy and oversight role for federal agencies such as EPA. Introduction: Cranial expansion has been the mainstay of initial management of children with Apert syndrome. Surgical timing is a balance between the risk of relapse if performed too early and the possibility of raised intracranial pressure and ossification defects if performed later. Primary occipital expansion has been proposed as a method to delay the timing of frontal surgery. We have applied the principal of spring expansion of patent sutures to expand the posterior cranial fossa without a cranial osteotomy. Patients and methods: All new Apert syndrome patients seen during the 3-year period December 2004-December 2007 underwent initial occipital expansion without osteotomy using spring expansion of the patent lambdoid suture. Results: Four new Apert syndrome patients underwent posterior spring expansion of the patent lambdoid suture. Good occipital expansion was achieved in all cases. Standard frontal advancement was performed 5-18 months later. No relapse after frontal advancement has been seen after mean follow-up of 41 months. Conclusions: Spring expansion of the patent lambdoid suture is an alternative technique to expand the posterior cranial fossa. Compared to current techniques it has very low morbidity. Occipital expansion is thought to treat several of the mechanisms responsible for raised intracranial pressure in Apert syndrome. When performed at 6 months of age it has enabled us to delay the time at which we would normally perform frontal advancement surgery until a time when the surgical result is likely to be more stable. During spring-assisted cranioplasty, the spring transmits forces through adjacent cranium. We have previously demonstrated that the ectocranial–endocranial thickness of cranial sutures increases significantly over time in the presence of continuous spring forces. We wished to investigate if cranial bone showed similar adaptational responses. New Zealand white rabbits were randomized into a treatment group [a spring was placed across a posterior frontal suture (PFS) suturectomy and a control group (PFS suturectomy)]. Animals ( n = 6) were euthanized from each group at 4, 7, and 10 weeks, respectively. A sham group ( n = 6) was euthanized at 10 weeks. Frontal bone thickness was recorded at five reproducible anatomical points on the frontal bone. Histological analysis of the bone architecture was performed. Frontal bone thickness was significantly greater than controls at all five sites at weeks 7 and 10. There were multiple significant differences between the 4-, 7-, and 10-week groups with each site progressively thickening over time. Histological analysis revealed a uniform increase in thickness of the endocranial and ectocranial cortical bone in the treatment groups. Cranial bone adapts to the presence of continuous spring cranioplasty forces by progressively thickening over time. This property is beneficial in craniosynostosis cases with very thin and poor quality bone and may partly explain the observed lack of spring erosion through bone. [ABSTRACT FROM AUTHOR]

Scaphocephaly is usually due to sagittal synostosis. Scaphocephaly may also be seen in the presence of a nonsynostosed sagittal suture. In this situation traditional surgery is controversial due to the altered risk–benefit profile. This paper reports the first known series of patients with nonsynostotic scaphocephaly treated using spring assisted expansion of the sagittal suture. All patients referred to our craniofacial program over the period February 2005–February 2008 were retrospectively reviewed. Eleven patients were seen with nonsynostotic scaphocephaly. Seven patients underwent spring expansion of the sagittal suture without osteotomy. Four female and three male patients had spring expansion of a patent sagittal suture. Four patients were born prematurely. Two patients had Beckwith–Weidemann syndrome. The ages ranged from 6 to 26 months (mean 12 months). The average preoperative cranial index was 66 (range 63–67). This improved to 76 at the time of spring removal (range 73–78). Springs were kept in situ for an average of 7.25 months. The mean blood loss was 7 ml and the mean operative time 36 min. Spring cranioplasty for sagittal synostosis is ideally performed before 6 months of age however in nonsynostotic scaphocephaly older children can be considered due to the absence of frontal bossing. Significant aesthetic improvement was achieved in all cases and normalization of the cranial index was achieved in 86% of cases with minimal morbidity and no significant complications. “Business development” is a corporate entrepreneurial capability (or competence) that has emerged in the Information Technology industry to support that industry’s practice of co-creation of value with customers and complementors. As a set of practices that link the firm’s value creating processes with its external environment, business development capabilities are a key factor in the success of IT SMEs. This article examines business development functions and business developer attributes in SMEs in the Information Technology Industry in Eastern Canada. The principal business development functions are finding profitable opportunities in business networks, developing and maintaining partnerships, providing support for new product development, and recognizing and responding to customer needs. We examined the concurrent validity properties of the Facial Discrimination Task (FDT), an instrument for the assessment of facial emotion recognition by comparing it with the widely used Pictures of Facial Affect (PFA). In Study 1, 100 adults with heterogeneous psychiatric diagnoses were administered items of the FDT Emotion Task, the FDT Age Task, and PFA. In Study 2, 25 normally developing preschool children were instructed to label happy, sad, or neutral facial expressions from the FDT and the PFA. Despite methodological differences between the two studies, very similar and high correlations were found between the FDT and the PFA overall correct scores (r = .79 and r = .77, respectively). The data suggest that the FDT and the PFA measure similar competencies in preschoolers and in adults with psychiatric disorders. Purpose. To describe the pharmacokinetics of SK&F 107647, a synthetichematoregulatory peptide, in healthy volunteers and in patientswith adenocarcinoma. Methods. SK&F 107647 pharmacokinetics were evaluated in 2dose-escalation studies. Volunteers received SK&F 107647 as single15-minute iv infusion doses of 1, 10, 100, 500, and 1000 μg/kg. Cancerpatients received 2-hour iv infusions of 0.001, 0.01, 0.1 and 1μg/kg once daily for 10 days. Drug concentrations were quantified in plasmaand urine of healthy volunteers and on days 1 and 10 in plasma ofcancer patients receiving the two top dose levels. Results. In volunteers, mean clearance (CL) ranged from 76.7 to 101ml/hour/kg; mean volume of distribution at steady-state (Vss)rangedfrom 175 to 268 ml/kg. Most of the administered dose was renallyexcreted as intact peptide within 24 hours postinfusion. In patients,mean CL was 57.6 ml/hour/kg, mean Vss ranged from 128 to 150ml/kg and terminal half-life from 2.1 to 3.4 hours. There was littleaccumulation of drug. The article discusses the long-term benefit prospects of seclusion and restraint techniques in patient psychiatric facilities. Seclusion and restraints prevent injury to both patients and staff. In addition, these procedures are assumed to prevent imminent harm to patients and others around them, reduce disruption of treatment programs, and decrease sensory stimulation. Age, gender, race, length of stay, etc. race, diagnosis, presence of psychotic symptoms, length of stay have been found to be both significant and nonsignificant predictors of seclusion and restraint use. In terms of patient perspectives, those patients who have experienced seclusion often feel angry and fearful when others are secluded. When patients themselves are secluded, feelings of humiliation are prevalent and the seclusion is seen as a punishment that lasts much longer than needed. Other adverse consequences of these techniques include the fact that they may cause harmful physical and psychological effects on both the patient and staff.

This study examines public opinion in Costa Rica and El Salvador regarding regional integration in Central America. Recent efforts at regional integration as well as the response of the governments of the above countries to those efforts are reviewed. Public opinion toward regional integration is significantly more positive but less structured in El Salvador than in Costa Rica. Differences in the international and domestic contexts help to explain the differential responses in these two countries. Although urinalysis is one of the most frequently ordered tests in primary care, its usefulness in screening has not been demonstrated. A retrospective review of 1,607 admission urinalyses for inpatients in a referral/community hospital identified 861 as clinically indicated and 746 as routine. Routine urinalyses were abnormal less frequently than clinically indicated urinalyses (18.1% vs 39.6%) and when abnormal, were responded to less often (33.3% vs 75.4%). Forty-five (6.0%) of the routine urinalyses yielded an abnormality that led to diagnostic action. Of these, 18 were normal on repeat testing and 17 were considered unlikely to represent significant disease. Therefore, only ten (1.3%) of the routine urinalyses affected patient therapy. In eight of these cases, the abnormality was pyuria, of which six proved to be asymptomatic bacteriuria. The objective of this research was to analyze the retention of acid precipitation as a viable policy issue on the Congressional agenda during the 1980s. Issue maintenance (a term borrowed from Barbara Nelson's discussion of the four stages associated with agenda decision making) was examined in relation to a set of issue characteristics originally developed by Roger Cobb and Charles Elder, i.e., concreteness, social significance, temporal relevance, complexity, and categorical precedence. Each issue attribute was found to be somewhat useful in explaining the longevity of acid rain as an agenda item although the direction of influence for two factors, complexity and temporal relevance, was contrary to expectations. It was suggested that a conceptual merger of this sort could contribute to the comparative analysis of agenda policy decisions by providing a clearer and more restricted set of decision points to be explained. This research considers the question of changing environmental values within the leadership of firms responsible for the management of pollution as an unwanted byproduct. Information was obtained from a pair of surveys mailed to the chief executive officers (CEOs) of the 50 largest firms listed within the mining and manufacturing directories of Colorado, Montana, Utah, and Wyoming in 1976 and again in 1986. The authors found that industry CEOs were more supportive of environmental concerns in 1986 than 1976, suggesting that ecological values have become institutionalized to some extent. Yet, there is little indication that this attitudinal shift in environmental concern among CEOs has been accompanied by a willingness to spend a larger proportion of the company budget on pollution control or to improve working relationships with federal regulatory officials. The ability of commerical human immune serum globulin (HISG) to inhibit the adherence of urinary tract infection isolates to rat bladder epithelial cells was investigated utilizing an in vitro adherence system. Significant decreases in adherence were noted when strains of Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, and Enterobacter cloacae were tested against five HISG preparations. An enzyme-linked immunosorbent assay indicated that all five HISG preparations also contained antibodies against type-1 pili isolated from Klebsiella pneumoniae. The presence of antibodies directed against a bacterial adhesin and the effectiveness of HISG in inhibiting the attachment of a wide range of urinary pathogens to bladder cells suggest that HISG may have practical therapeutic values in the prophylaxis of diseases where bacterial adherence is a prerequisite for the initiation of infection. Hemagglutination of human type A and guinea pig erythrocytes showed that both mannose-sensitive and mannose-resistant adhesive mechanisms (pili) were detectable in broth cultures of Escherichia coli while only a mannose-sensitive mechanism (type 1 pili) was noted with Klebsiella pneumoniae. The degree of hemagglutination was shown to be related to differences in erythrocytes, bacterial concentration, and growth media constituents. Escherichia coli grown in human urine produced both types of hemagglutinins, while K. pneumoniae cultured in urine continued to express only a mannose-sensitive hemagglutinin. [ABSTRACT FROM AUTHOR]

The interaction of bacteria belonging to several genera with the bladder epithelium was studied. An in vivo rat bladder model system was used to evaluate the cytopathic effects of members of eight representative genera of bacteria and six strains of Escherichia coli on the bladder surface. Alterations were noted mainly with the Gram-negative bacteria ( Escherichia, Klebsiella, Enterobacter, Proteus, Pseudomonas), although some strains were associated with more cytopathology than others. Foci of cytopathology consisted of swelling, exfoliated epithelial cells, strand formation, and ulceration. Aggregates of bacteria, epithelial cells, and debris were shed into the bladder lumen. In contrast, members of three genera of Gram-positive organisms ( Lactobacillus, Streptococcus, Staphylococcus) caused little or no cytopathology. The study strongly suggests that the bladder responds in a relatively uniform manner to viable Gram-negative organisms, while the response to Gram-positive organisms is minimal. In an attempt to understand the relation between the outcome of chemonucleolysis treatment of lumbar discs and the presence of spondylosis (degenerative process) of the lumbar spine, 40 cases of disc herniation with subsequent intradiscal chymopapain injection (chemonucleolysis) were reviewed. Thirty-eight patients had plain radiographs, 25 had myelography, 28 had computed tomography (CT), and 15 had both myelography and CT. A review of these examinations revealed that 24 patients had minimal or mild spondylosis (group 1) and 13 patients had moderate spondylosis (group 2). Only one patient had a severe spondylosis (group 3). Chemonucleolysis failed in 8/24 (33%) patients from the first group and 2/13 (15%) cases from the second group. Therefore, the presence or absence of even moderate spondylosis did not affect the treatment success rate. A review of the 11 failed cases revealed spinal canal stenosis in three cases and a large disc herniation in two (one case with a myelographic block). Another case had a sequestered disc fragment. In two further cases, the disc herniation was far lateral with complete occlusion of the lateral foramen; another case exhibited severe degenerative changes of facet joints. In two other cases, treatment failure could not be explained by any radiographic finding. Of three patients with large disc herniation, only one responded successfully to intradiscal injection of chymopapain. Chemonucleolysis, however, failed in the only patient in the third group who had severe spondylosis of the facet joint. [ABSTRACT FROM AUTHOR]
