Objectives: Current studies on the treatment of adolescent patients with disc displacement without reduction (DDWOR) are limited by short follow-up periods and small sample sizes, and there are few comparative studies on the efficacy of conservative treatment with and without disc reduction for acute DDWOR. This study compared the therapeutic effects of two conservative treatment methods: physical therapy alone and physical therapy combined with non-surgical manual disc reduction and anterior repositioning splints (ARS), in adolescent patients with acute DDWOR. Materials and methods: This retrospective study included adolescent patients with DDWOR who underwent physical therapy at the Temporomandibular Joint Rehabilitation Department of the Shanghai Ninth People's Hospital from January 2018 to December 2021. Patient assessment data were collected before and after treatment. Patients were followed up through telephone and online questionnaires from March to August 2023. Results: The results indicate that compared to physical therapy alone, the combination of physical therapy and non-surgical manual disc reduction with ARS showed better short-term efficacy, improved mouth opening, and better long-term pain control. Also, it may be effective in preventing degenerative joint disease. Conclusions: This combination therapy is recommended for clinical application in adolescent patients with acute DDWOR. Clinical relevance: The present research demonstrates the superior efficacy of physical therapy and non-surgical manual disc reduction combined with anterior repositioning splint in adolescent patients with acute DDWOR. Background: Temporomandibular joint disc displacement disorders are a group of temporomandibular disorders beside from other inflammatory disorders and growth related disorders of joint. The purpose of this analysis was to evaluate the efficacy of arthrocentesis procedure in comparison with stabilization splints used for disc displacement disorders without reduction. Materials and Methods: A systematic search was done in electronic databases (PubMed, Cochrane Central, Web of Science). In addition to this hand search of references and grey literatures was done. Qualities of randomized controlled clinical trials were assessed by Cochrane's tool for Systematic Reviews of Interventions and the Newcastle–Ottawa Scale was followed to assess the prospective and retrospective studies. Outcome variables pain (VAS) and maximum mouth opening were assessed by the software review manager 5.03. Results: A total of five studies were included in the review. Three studies showed greater improvement of symptoms in patients of arthrocentesis group in terms of maximum mouth opening, pain (VAS) value. Two other studies found no significant advantage of arthrocentesis over the other treatment protocol. The meta-analysis resulted in statistically significant difference between outcome variables favouring arthrocentesis group (VAS 1–10) (Mean Difference: 3.10; 95% CI 1.74, 4.45; P ≤.00001, Mean difference: 2.00; 95% CI 0.29, 3.71; P = 0.02). Conclusion: Arthrocentesis showed effective result in terms of increase in mouth opening and reduction of pain level compared to stabilization splint and other non-invasive approaches in patients with disc displacement disorders without reduction. Objectives: To predict temporomandibular joint (TMJ) anterior disc displacement with reduction (ADDWR) from condylar shape, position, and dimensions obtained from CBCT images. Methods: This cross-sectional study was performed on 17 patients suffering from temporomandibular disorders diagnosed by history taking according to the chart of the American Association of orthodontists, clinical examination according to the Helkimo index and MRI. CBCT and MRI examinations were performed within one-week interval. Disc position, diagnosed by MRI was used as the gold standard. TMJs with posterior disc displacement or anterior disc displacement without reduction were excluded. Qualitative and quantitative analyses were performed on CBCT images to find the correlation between condylar variables and ADDWR. A logistic regression model was created to estimate ADDWR from condylar dimensions (height, width and depth). Results: Condylar shape and condylar position in the glenoid fossa were significantly correlated with ADDWR (P < 0.05). Condylar width, height and depth were significantly smaller in condyles with ADDWR compared to condylar dimensions in normal disc position. Logistic regression analysis could be used to predict the probability of anterior disc displacement with reduction from condylar dimensions. Conclusion: Condylar shape, position, and dimensions assessed by CBCT are significantly correlated with ADDWR of the TMJ. Background: Static MRI was used as an effective tool for diagnosis of temporomandibular joint dysfunction instead of invasive techniques such as arthroscopy and arthrography. The purpose of this study was to detect whether dynamic MRI can be used instead of static MRI in diagnosis of TMJ dysfunction or not. Results: According to disc displacement, anterior disc displacement was detected in 29 joints (36.25%) by both static and dynamic MRIs, and medial and lateral disc displacements were detected only by static MRI. Regarding disc mobility, dynamic MRI detects stuck disc in 4 joints versus 2 joints detected by static MRI (p value 0.008). Condylar translation was abnormal in 18 joints (22.5%) by static MRI and in 26 joints (32.5%) by dynamic MRI (p value < 0.001). The detection rate of articular disc for dynamic MRI was 87.5% versus static MRI 92.5% (p value 0.038), and the detection rate of condylar head for dynamic MRI was 97.5% versus static MRI 100% (p value 0.012). Conclusion: Dynamic MRI cannot replace static MRI in evaluation of TMJ dysfunction; both of them support each other for accurate diagnosis and better image quality. [ABSTRACT FROM AUTHOR]

Background: The purpose of this study is to introduce our modified disc plication technique using MITEK mini anchors and to evaluate the clinical outcome for patients with internal derangement (ID) of the temporomandibular joint (TMJ). Patients and methods: We evaluated 65 joints in 46 patients, comprised 32 women and 14 men, who first visited the Asan Medical Center from December 2012 to December 2016. The age of the patients ranged from 14 to 79 years, with a mean age of 36.6 years. The patients presented with joint problems including pain, joint noise, and mouth opening limitation (MOL). Patients who met our inclusion criteria underwent unilateral or bilateral disc repositioning surgery with our minimally invasive disc plication technique using MITEK mini anchors and No. 2-0 Ethibond® braided polyester sutures. The variables taken into account in this study were the range of maximum mouth opening (MMO), painful symptoms (evaluated with the visual analog scale, VAS), and the type of noise (click, popping, crepitus) in the TMJ. Results: Preoperative examination revealed painful symptoms in 50.7% (n = 35) of the operated joints (n = 69) and the presence of clicks in 56.5% (n = 39). Postoperative examination revealed that 4.3% (n = 3) of the operated joints had painful symptoms with lower intensity than that in the preoperative condition. Additionally, 17.4% (n = 12) had residual noise in the TMJ, among which two were clicking and the other 10 had mild crepitus. The intensity of the postoperative residual noise was significantly decreased in all cases compared to that in the preoperative condition. Among patients with MOL below 38 mm (n = 18), the mean MMO was 31.4 mm preoperatively and 44.2 mm at 6 months postoperatively, with a mean increase of 13.8 mm. A barely visible scar at the operation site was noted during the postoperative observation period, with no significant complications such as facial palsy or permanent occlusal disharmony. Conclusion: Subjective symptoms in all patients improved following the surgery. TMJ disc plication using MITEK mini anchors with our minimally invasive approach may be a feasible and effective surgical option for treating TMJ ID patients who are not responsive to conservative treatment. Purpose: Temporomandibular joint (TMJ) disc derangement is defined as a malpositioning of the articular disc relative to the condyle and eminence. Arthrocentesis of the TMJ is considered by many as the first-line surgical treatment for patients who do not respond to conservative treatment. The aim of this study is to assist needle insertion for temporomandibular joint arthrocentesis using ultrasonography. Materials and method: Twenty patients who required arthrocentesis of the TMJ were randomly assigned to two groups A and B undergoing single-puncture arthrocentesis with modified double-lumen single-barrel needle and ultrasound-guided single-puncture arthrocentesis using modified double-lumen single-barrel needle (n = 10 in each group). The number of attempts of needle manipulation, duration of the operative procedural time, and visual analog (VAS) scale score for pain to assess surgical discomfort were the main outcome variables. Results: None of the patients in either group developed any complication with no significant difference in VAS score for pain between the two study groups. The number of attempts for needle manipulation (mean ± SD) for group A was 2.20 ± 0.789, and for group B, it was 1.10 ± 0.316 (p value, 0.0007); operative procedural time (mean ± SD) for group A was 18.5 ± 3.171 min, and for group B, it was 13.1 ± 1.663 (p value, 0.0002) which was significantly lesser in group A than in group B. The sentence signifies that the number of attempts that was required to re-insert the needle to enter the joint space in Ultrasound guided and conventional method of arthrocentesis. Conclusion: Ultrasound-guided single-puncture arthrocentesis using a customized needle is a promising method to perform joint lavage with minimal trauma and in a precise manner. Purpose: Temporomandibular disorders (TMDs) affect 8-12 % of the adolescent and adult population, resulting in patient discomfort and affecting quality of life. Despite the growing incidence of these disorders, an effective screening modality to detect TMDs is still lacking. Although magnetic resonance imaging is the gold standard for imaging of the temporomandibular joint (TMJ), it has a few drawbacks such as cost and its time-consuming nature. High-resolution ultrasonography is a non-invasive and cost-effective imaging modality that enables simultaneous visualization of the hard and soft tissue components of the TMJ. This study aimed to evaluate the correlations between the clinical signs and symptoms of patients with chronic TMJ disorders and their ultrasonographic findings, thereby enabling the use of ultrasonography as an imaging modality for screening of TMDs. Methods: Twenty patients with chronic TMDs were selected according to the Research Diagnostic Criteria for TMDs. Ultrasonographic imaging of individual TMJs was performed to assess the destructive changes, effusion, and disc dislocation. Fisher's exact test was used to examine the correlations between the findings obtained from the ultrasonographic investigation and the clinical signs and symptoms. Results: There was a significant correlation between pain and joint effusion as well as between clicking and surface erosion. Conclusions: The present findings suggest that ultrasonography can be used as a screening modality to assess the hard and soft tissue changes in patients presenting with signs and symptoms of TMDs. Temporomandibular joint (TMJ) disc derangement is defined as a malpositioning of the articular disc relative to the condyle and eminence. Objectives: The aim of this investigation was to assess the association of magnetic resonance imaging (MRI)-diagnosed temporomandibular joint (TMJ) disorders [i.e., disc displacement with reduction, disc displacement without reduction (DDNR), osseous changes (OC), joint effusion] on the same side as well as in the joints of the two sides of the same individual. Materials and methods: A total of 199 patients undergoing bilateral MRI of the TMJs were included in the study. A single variable correlation matrix was created to assess the within- and between-side correlation of single diagnoses. Then, based on 12 possible combinations of diagnoses per each side, a contingency table was created to assess the chi-square values of the differences between the observed and expected frequencies of the different cross-combinations. Multiple variable permutation test was performed to assess the null hypothesis that the diagnoses in the right and left joints are not related. Results: Within the signs of the same side, DDNR was positively correlated with OC. As for combination of diagnoses, the presence of a specific combination of signs on one side implied the same combination of signs on the other side. The global multivariate permutation test with Tippett combination was significant at p < 0.001, showing that the null hypothesis of independence between diagnoses of the two sides was rejected. Conclusions: It can be suggested that disc displacement without reduction is associated with osseous changes of the same joint and that joints of the two sides are likely to be affected by the same combinations of MRI signs. We conducted a systematic review of the literature from 1965–1994 to assess the value of history and physical examination in the diagnosis of sciatica due to disc herniation; we also included population characteristics and features of the study design affecting diagnostic value. Studies on the diagnostic value of history and physical examination in the diagnosis of sciatica due to disc herniation are subject to important biases, and information on numerous signs and symptoms is scarce or absent. Our search revealed 37 studies meeting the selection criteria; these were systematically and independently read by three readers to determine diagnostic test properties using a standard scoring list to determine the methodological quality of the diagnostic information. A meta-analysis was performed when study results allowed statistical pooling. Few studies investigated the value of the history. Pain distribution seemed to be the only useful history item. Of the physical examination signs the straight leg raising test was the only sign consistently reported to be sensitive for sciatica due to disc herniation. However, the sensitivity values varied greatly, the pooled sensitivity and specificity values being 0.85 and 0.52, respectively. The crossed straight leg raising test was the only sign shown to be specific; the pooled sensitivity and specificity values were 0.30 and 0.84, respectively. There was considerable disagreement on the specificity of the other neurological signs (paresis, sensory loss, reflex loss). Several types of bias and other methodological drawbacks were encountered in the studies limiting the validity of the study results. Magnetic resonance imaging (MRI) of the temporomandibular joint (TMJ) has been shown to be of great value in the detection of displacements and delineation of the disc configuration in patients suspected of having internal derangements. The present study was conducted in temporomandibular joints of 144 patients in order to investigate whether an association exists between disc position, disc reduction and disc configuration as seen on MRI. Sagittal proton density-weighted MRIs of 288 TMJs (144 patients) were taken with the mouth closed and opened, and the position and configuration of the articular disc were assessed. The results showed that joints with a superior disc position barely demonstrated disc deformation while in joints with anteriorly displaced dises, disc deformation occurred more frequently (p<0.001). Non-reduction of the disc was associated with disc deformation whereas disc reduction was associated with the normal biconcave disc configuration (p<0.001). These data suggest that an association exists between disc displacement and disc deformation and between disc deformation and the degree of anterior disc displacement. [ABSTRACT FROM AUTHOR]
