Abstract Low‐flow vascular malformations are congenital overgrowths composed of abnormal blood vessels potentially causing pain, bleeding and obstruction of different organs. These diseases are caused by oncogenic mutations in the endothelium, which result in overactivation of the PI3K/AKT pathway. Lack of robust in vivo preclinical data has prevented the development and translation into clinical trials of specific molecular therapies for these diseases. Here, we demonstrate that the Pik3caH1047R activating mutation in endothelial cells triggers a transcriptome rewiring that leads to enhanced cell proliferation. We describe a new reproducible preclinical in vivo model of PI3K‐driven vascular malformations using the postnatal mouse retina. We show that active angiogenesis is required for the pathogenesis of vascular malformations caused by activating Pik3ca mutations. Using this model, we demonstrate that the AKT inhibitor miransertib both prevents and induces the regression of PI3K‐driven vascular malformations. We confirmed the efficacy of miransertib in isolated human endothelial cells with genotypes spanning most of human low‐flow vascular malformations.

Objective: To assess the effect of modeling free water (FW) on the identification of white matter (WM) microstructure alterations using diffusion Magnetic Resonance Imaging (dMRI) in episodic migraine without aura patients compared with healthy controls. Background: Diffusion tensor imaging (DTI) studies examining WM in migraine patients previously overlooked the potential influence of FW partial volume effects. Correcting FW effects could offer a clearer understanding of WM changes in migraine. This study is the first to incorporate FW effects when evaluating alterations in WM tracts in migraine patients, offering a comparison to standard DTI analysis. Methods: A group of 14 patients with low-frequency episodic menstrual-related migraine without aura and 15 healthy controls matched for the phase of the menstrual cycle were recruited and underwent dMRI acquisitions. FW partial volume fraction was estimated, the diffusion signal corrected and the diffusion parameters calculated from both FW-corrected and uncorrected signals. Tract-Based Spatial Statistics (TBSS) and WM skeleton regions of interest (ROI) analyses were used to compare between groups. Results: Comparisons between control subjects and migraine patients with TBSS and ROI analyses revealed significantly lower axial diffusivity (AD), both with and without FW correction, as well as altered FW values in migraine patients in some WM tracts. TBSS detected MD changes only after FW correction. Conclusions: These findings suggest WM alterations in these migraine patients in comparison with control subjects, in accordance with other migraine studies. Background: Changes in large-scale brain networks have been reported in migraine patients, but it remains unclear how these manifest in the various phases of the migraine cycle. Case-control fMRI studies spanning the entire migraine cycle are lacking, precluding a complete assessment of brain functional connectivity in migraine. Such studies are essential for understanding the inherent changes in the brain of migraine patients as well as transient changes along the cycle. Here, we leverage the concept of functional connectome (FC) fingerprinting, whereby individual subjects may be identified based on their FC, to investigate changes in FC and its stability across different phases of the migraine cycle. Methods: We employ a case-control longitudinal design to study a group of 10 patients with episodic menstrual or menstrual-related migraine without aura, in the 4 phases of their spontaneous migraine cycle (preictal, ictal, postictal, interictal), and a group of 14 healthy controls in corresponding phases of the menstrual cycle, using resting-state functional magnetic resonance imaging (fMRI). We propose a novel multilevel clinical connectome fingerprinting approach to analyse the FC identifiability not only within-subject, but also within-session and within-group. Results: This approach allowed us to obtain individual FC fingerprints by reconstructing the data using the first 19 principal components to maximize identifiability at all levels. We found decreased FC identifiability for patients in the preictal phase relative to controls, which increased with the progression of the attack and became comparable to controls in the interictal phase. Using Network-Based Statistic analysis, we found increased FC strength across several brain networks for patients in the ictal and postictal phases relative to controls. Conclusion: Our novel multilevel clinical connectome fingerprinting approach captured FC variations along the migraine cycle in a case-control longitudinal study, bringing new insights into the cyclic nature of the disorder. Leishmaniasis, a challenging disease to treat due to the intracellular location of the parasite and host immune modulation, requires innovative treatment solutions. This study evaluates a chloroquine-based hydrogel for topical use, targeting a localized and less invasive approach to leishmaniasis treatment. Thus, this study aimed at evaluating the skin permeation potential of the antimalarial chloroquine and also to evaluate the less invasive, topical treatment in murine models infected by Leishmania (Leishmania) amazonensis. For this end, a formulation was created, containing chloroquine (CQ), which had its release profile assessed through the ex vivo skin permeation assay using Sus scrofa domesticus skin. Afterward, the susceptibility profile of L. (L.) amazonensis in vivo in BALB/c mice was assessed over a 15-day treatment period. This study highlights the potential of chloroquine hydrogel as a topical treatment for tegumentary leishmaniasis, with significant reductions in parasite load and minimal side effects observed. The formulation demonstrated a 15% permeation rate during 12 h, with 20.26% on the 42nd day. A significant reduction in parasite load was observed in the treatment group. Thus, the results presented here point to the need for further pre-clinical trials with a higher concentration of the drug, other formulations and against other Brazilian species of Leishmania. Future studies should explore higher drug concentrations, alternative formulations, and other Leishmania species to expand the therapeutic potential of this approach. [ABSTRACT FROM AUTHOR]

Vascular hamartoma is an abnormal development of endothelial tissue that rarely affects horses. Histopathological examination remains the gold standard for definitive diagnosis, yet the presence of abnormal vascular structures must be carefully differentiated from other vascular anomalies and neoplasms. This report describes two cases of vascular proliferation affecting the skin on the lateral side over the left tarsus and the dorsolateral region over the fetlock joint of two fillies. The clinical history, presentation, and radiographic and sonographic findings in both patients suggested a neoplasm of unknown origin. Surgical excision of the mass was successfully performed in both patients. In one patient, a second surgery was performed six months after the first surgery because of the recurrence of the mass. Histopathological examination revealed a disorganized proliferation of well-differentiated, tortuous, and varied-sized blood vessels hemmed in myxoid tissue with abundant fibrocollagenous bands. Immunohistochemical examination of the endothelium markers factor VIII and ERG revealed positive staining of the mass samples in both cases, confirming their vascular origin. The combination of these markers was effective, providing a reliable option for differentiating vascular proliferation in horses. Purpose: Malignant melanoma is an aggressive cancer, and there is a notable dearth on epidemiology, clinical and treatment characterization within the Portuguese population. We performed a scoping review to identify real-world evidence studies focused in Portuguese adult patients with malignant melanoma. Methods: A comprehensive search was conducted. After screening, we described the studies by design, sample size, geographics, setting, population, and outcomes reported. Results: The search yielded 54 studies, mainly retrospective (79.6%). The population assessed was heterogeneous varying from patients with melanoma in general to specific types of melanoma, or even more restricted to patients with specific conditions. The evidence found was mostly concerning clinical outcomes (n=46), patients' clinical profile (n=44) and demographic characterization (n=48). Treatment information was described in 30 studies whereas only 18 reported epidemiological parameters. Studies were mainly performed by the major oncology centers in Lisbon, Oporto and Coimbra, and only two evaluated the entire Portuguese population. To allow comparability, only studies including patients with cutaneous malignant melanoma were considered (13 of the 54) for outcomes evaluation analysis. Median OS varied from 18 to 36 months, assessed after melanoma treatment. Incidence was the most reported epidemiological parameter, confirming the increasing number of cutaneous malignant melanoma patients over the years. Only one study reported prevalence and four reported mortality rates. Conclusions: The evidence found confirms the lack of information about malignant melanoma in Portugal, highlighting the need of real-world studies to assess melanoma prevalence and incidence rates, current treatment approaches, and clinical characterization of these patients. Hereditary diffuse gastric cancer is characterized by an increased risk of diffuse gastric cancer and lobular breast cancer, and is caused by pathogenic germline variants of E-cadherin and α -E-catenin, which are key regulators of cell-cell adhesion. However, how the loss of cell-cell adhesion promotes cell dissemination remains to be fully understood. Therefore, a three-dimensional computer model was developed to describe the initial steps of diffuse gastric cancer development. In this model, we have implemented a cellular Potts approach that contemplates cell adhesion to other cells and to the extracellular matrix, cell extrusion from the gastric epithelia, and subsequent proliferation. We demonstrate that early disease features are determined by decreased adhesion of mutant cells to their normal epithelial neighbors, with concomitant increased attachment to matrix components. Importantly, our simulation shows how mechanical pressure and uncontrolled proliferation of mutant cells lead to modifications in cell shape and in gastric gland morphology. In conclusion, this work underscores the potential of computational models to elucidate the role of cellular and noncellular components in gastric cancer that may be relevant targets in therapeutic interventions. Purpose: After radical prostatectomy (RP), it is rarely acknowledged that several sexual dysfunctions can arise. These include issues in the orgasmic domain (e.g., decreased orgasm intensity, dysorgasmia), problems with ejaculation (e.g., absence of ejaculation), the development of penile deformities, and low sexual desire. This article aims to report the occurrence of orgasmic and ejaculatory dysfunction when actively investigated, documenting those rates and characterizing specific features of these conditions following RP. Methods: This study has analyzed retrospective data from men who underwent RP and were experiencing erectile dysfunction. During a structured visit, patients were systematically questioned about dysorgasmia, altered orgasmic sensation, climacturia, and arousal incontinence. Continuous variables were analyzed using the Student T or ANOVA tests, while categorical variables were analyzed using Chi-squared or Fisher's exact tests. The associations were described as odds ratios with precise confidence intervals. All tests were two sided; a p value < 0.05 was considered statistically significant. Results: Sixty patients were included, out of which 3 (5%) reported dysorgasmia, while 33.3% presented a decrease in orgasm intensity. Climacturia was reported as the most common orgasmic disorder in 40 (66.6%) patients. However, only 14 patients (35%) reported that it frequently occurs, i.e., more than half of the time. Among the patients who reported climacturia, 72.5% classified it as mild losses. Additionally, arousal incontinence (AI) was noted in the study by 19 (26.3%) patients. Conclusion: Our study highlights the importance of discussing orgasmic dysfunctions after RP, which can be frequent and bothersome but are often overlooked in preoperative counseling. Objective: Tools for classifying adverse drug reactions (ADRs) have not yet been validated in the context of the neonatal intensive care unit (NICU). The study aims to investigate the inter-rater reliability of the Hartwig tool and the Liverpool avoidability assessment tool (LAAT) in assessing the severity and avoidability of ADR cases in hospitalized neonates. Methods: An observational and prospective study was conducted in the NICU of a maternity hospital in Natal, Brazil. The Hartwig tool and LAAT were employed to assess the severity and avoidability of ADRs, respectively. Three experienced clinical pharmacists independently assessed all ADR cases. Inter-rater reliability was measured using Cohen's kappa coefficient (k) with corresponding 95% confidence intervals (CI). Results: Among 79 ADR cases, the mean gestational age was 29.7 ± 4.4 weeks, and the birth weight averaged 1446.0 ± 1179.3 g. The assessment of ADR severity using the Hartwig tool revealed a significant overall correlation (overall k = 0.573; 95% CI 0.395 to 0.753) with exact agreement (EA) and extreme disagreement (ED) rates between evaluators of 86.5% and 2.5%, respectively. However, no statistically significant correlation was observed for determining avoidability using the LAAT (overall k = 0.017; 95% CI − 0.048 to 0.082), with an EA rate of 83.6% and ED rate of 10.1%. Conclusion: The Hartwig tool demonstrates good reproducibility among different evaluators in determining the severity of ADRs, unlike the LAAT for assessing avoidability. [ABSTRACT FROM AUTHOR]

Suicidality is a multifaceted phenomenon affected by several multisystemic vulnerabilities, including demographic, psychopathological, and family factors. However, few studies have explored the contribution of specific features of deliberate self-harm (DSH) to suicide risk, particularly DSH diversity, while also accounting for other relevant explanatory family and mental health dimensions. The main goal of this study was to examine if DSH diversity predicts the risk for suicide in adolescents in the presence of sociodemographics, depressive symptoms, and family relationship characteristics. This study consisted of 437 Portuguese adolescents (53.8% male, 42.8% female, 3.4% non-binary; Mage= 15.2, SD = 1.87), of which 220 confirmed having a lifetime history of DSH. The participants completed a self-report questionnaire measuring depressive symptomatology, interparental conflicts, family communication, DSH, and suicidal ideation. A logistic regression analysis was conducted. Our multivariate model revealed that depressive symptoms, family communication, and DSH diversity are predictors of suicide risk in adolescents. The strongest predictor was depressive symptoms. The analyses indicated that our global model has excellent predictive validity to discriminate between those with and without suicide risk, and the inclusion of DSH diversity enhances the identification of these vulnerable youth. In addition, our results suggest that non-binary gender adolescents are particularly at risk. The set of characteristics we found that predict suicide risk highlights the multisystemic nature of this phenomenon: individual/ psychopathological and relational dimensions. We also underscore the need to be vigilant of adolescents' number of used DSH behaviors. Background: Resistant and refractory migraine are commonly encountered in specialized headache centers. Several comorbidities, mostly psychiatric conditions, have been linked to migraine worsening; however, there is little knowledge of the comorbidity profile of individuals with resistant and refractory migraine. Methods: REFINE is a prospective observational multicenter international study involving individuals with migraine from 15 headache centers. Participants were categorized into three groups based on the European Headache Federation criteria: non-resistant and non-refractory (NRNRM), resistant (ResM), and refractory (RefM). We explored the prevalence of 20 comorbidities at baseline in the three groups. Results: Of the 689 included patients (82.8% women), 262 (38.0%) had ResM, 73 (10.4%) had RefM and 354 (51.4%) NRNRM. A higher prevalence of psychiatric comorbidities, trigger points, temporomandibular joint disorders, thyroiditis, and cerebrovascular diseases was observed in the RefM group, followed by ResM and NRNRM. Multiple comorbidities were more common in the RefM group, followed by the ResM group and by the NRNRM group (41.6% vs. 24.5% vs. 14.1% respectively; p < 0.001). At the sensitivity analysis, exploring participants with chronic migraine, significant differences among the NRNRM, ResM, and RefM groups were found in the prevalence of anxiety (p < 0.001), asthma and rhinitis (p = 0.013), bipolar and other psychiatric disorders (p = 0.049), cerebrovascular diseases (p < 0.001), depression (p < 0.001), obesity (p = 0.002), thyroiditis (p < 0.001), and trigger points (p = 0.008). Conclusion: REFINE data indicate that individuals with ResM and RefM have a higher burden of comorbidities than those with NRNRM. It can be postulated that those comorbidities may have an impact on the progression of migraine from a form that is easy to treat to a form that is resistant or refractory to treatments. Longitudinal studies are needed to understand the direction of the association between ResM or RefM and those comorbidities and if proper treatment of comorbidities might help overcome treatment resistance or refractoriness. Introduction: Anacanthorus silvoi n. sp. (Dactylogyridae, Anacanthorinae) is described from the gills of Hoplias aff. malabaricus (Bloch, 1794) from the Salgado River, Ceará state, Brazil. Materials and Methods: The monogeneans were affixed onto slides using Gray and Wess's medium for examination of their sclerotized structures. For analysis of internal organs, a single specimen was preserved in 5% formalin, stained with Gomori's trichrome, and mounted in Gray and Wess's medium. Results: Anacanthorus silvoi n. sp. is characterized by having a short broad tube MCO with a medial constriction (i.e., MCO with distal region wider than the proximal region, and flexed lateral flap in the distal region in A. cururutuiensis and a MCO with a small projection in the form of a hook in the distal region in A. siphonocommus). Conclusions: The present study corroborates previous studies that the absence of an accessory piece is a characteristic shared by all Anacanthorus members parasites of Erythrinidae. Loneliness and depression are serious public mental health problems. Meaning in life (MIL) is associated with reduced loneliness and depression. This study aimed to: (1) investigate associations between loneliness, MIL, and depression, differentiated by sex in individuals aged ≥ 50 years, residing in 26 European countries and Israel, and (2) to examine in men and women separately whether MIL mediated the relationship between loneliness and depression. We included 41,372 individuals (23,789 women) who responded to wave 8 of the SHARE project. The variables analyzed were loneliness (UCLA), depression (EURO-D scale), and MIL (CASP-19). The analytical procedures included regression analysis and exploratory mediation analysis. Among men and women, the odds of loneliness increasing depression were 3.6 and 3.3 times higher, respectively. Among men, feeling MIL sometimes or often had odds for reducing depression by 0.53 and 0.21, respectively. In women, feeling MIL sometimes or frequently reduced the odds of depression by 0.37 and 0.19, respectively. Regardless of sex, mediation analyses showed a positive association between loneliness and depression, while MIL was negatively associated with loneliness and depression. MIL partially mediated the association between LON and depression in male and female models by approximately 83.2% and 80.7%, respectively. No differences were found between men's and women's mediation models. Regardless of sex, high levels of MIL seem to be effective in benefiting the mental health of Europeans aged 50 and over. MIL proved to be a significant mediator of the relationship between loneliness and depression, while loneliness and depression strengthened each other. [ABSTRACT FROM AUTHOR]

Considering the small percentage of blood donors and that it is common for individuals who donate blood for the first time not to repeat the donation in the future, the continuous need for blood remains a global challenge. For these reasons, blood donation centres need to understand the aspects that motivate people to donate blood and those that discourage this behaviour. Hence, our research goal is to identify the aspects that contribute to donation and those that demotivate it. A total of 540 questionnaires were gathered, and t-tests were used to compare dimensions of Altruism regarding donors, and motivations and inhibitors, as well as the perception regarding incentives to blood donation, among nondonors and potential donors. We conclude that donors are mainly motivated by the attainment of warm glow from benefiting others and that they show preferential helping towards family. Our study suggests that potential blood donors are willing to donate, but may not do so due to perceived barriers, primarily the fear of falling unconscious, becoming nauseous/weak and of the transmission of infectious diseases –, and the lack of knowledge regarding the existence of incentives to blood donation. Therefore, social marketing campaigns for donor retention should be focused on the emotional reward gained by benefiting others, particularly family, and recruitment should be aimed at potential donors, with focus on changing perceptions regarding the identified inhibitors and on giving more information about incentives for blood donors. Our study contributes to previous research by identifying some particular aspects related to nondonors that have higher potential to become donors, and kinship as a strong motivation for donors, unlike previous studies in this area. Does artificial intelligence (AI) exhibit consciousness or self? While this question is hotly debated, here we take a slightly different stance by focusing on those features that make possible both, namely a basic or fundamental subjectivity. Learning from humans and their brain, we first ask what we mean by subjectivity. Subjectivity is manifest in the perspectiveness and mineness of our experience which, ontologically, can be traced to a point of view. Adopting a non-reductive neurophilosophical strategy, we assume that the point of view exhibits two layers, a most basic neuroecological and higher order mental layer. The neuroecological layer of the point of view is mediated by the timescales of world and brain, as further evidenced by empirical data on our sense of self. Are there corresponding timescales shared with the world in AI and is there a point of view with perspectiveness and mineness? Discussing current neuroscientific evidence, we deny that current AI exhibits a point of view, let alone perspectiveness and mineness. We therefore conclude that, as per current state, AI does not exhibit a basic or fundamental subjectivity and henceforth no consciousness or self is possible in models such as ChatGPT and similar technologies. The complexity of treating neurological diseases has meant that new strategies have had to be developed to deliver drugs to the brain more efficiently and safely. Intranasal drug delivery is characterized by its ease of administration, safety, and rapid delivery directly from the nose to the brain. Several strategies have been developed to improve the delivery of drugs to the brain via nasal administration. These include the use of mucoadhesive and thermoresponsive polymers and their combination into polymer blends, as well as the use of liposomes, niosomes, and nano- and microemulsions. Therefore, this review focuses on technologies for developing pharmaceutical systems aimed at delivery via the nose to the brain, contributing to new treatments for difficult neurological disorders. Some of the most common and difficult-to-treat neurological conditions, the intranasal route of administration, and the anatomy of the nasal cavity have been discussed, as well as factors that may influence the absorption of drugs administered into the nose. The types of intranasal formulations and the devices that can be used to administer these products are also discussed in this review. Strategies for improving the transport of bioactive agents and increasing bioavailability are highlighted. The technologies discussed in this review can facilitate the development of formulations with improved properties, such as drug release and mucoadhesiveness, which have several advantages for patients requiring complex neurological treatments. Non-indigenous Species (NIS) pose significant threats to marine biodiversity globally, especially in ecologically sensitive habitats such as Marine Protected Areas (MPAs). This study uses data collected in the Autonomous Region of Madeira (North Atlantic), Portugal, to create a spatial model aiming to (1) develop a spatially explicit index of NIS dispersal from known hotspots such as ports, harbours, marinas, and anchoring areas; (2) assess the relative vulnerability of Madeira's MPAs to local NIS dispersion and establishment; and (3) provide insights for a scalable NIS monitoring framework and evaluating invasion risks. The spatial model integrates maritime traffic intensity and proximity to NIS hotspots, using a straightforward approach that can be applied in data-limited contexts. While designed to address Madeira's regional challenges, the model is adaptable to other biogeographic contexts and can incorporate additional complexity, such as species-specific traits or ecological layers, to suit different settings. Our findings underscore the role of maritime infrastructure and vessel traffic in NIS spread, revealing the vulnerability of Madeira's MPAs due to insufficient ecological monitoring and the absence of NIS monitoring and early detection programs. This study provides practical recommendations for improving MPA management and mitigating NIS risks, contributing to regional conservation efforts. Additionally, it establishes a baseline risk assessment approach that can be customised and expanded to guide NIS management and biodiversity conservation in other regions, particularly those with similar challenges. Eco-labels are market tools that provide consumers with relevant information, enabling more informed and sustainable purchasing decisions. As fisheries operate on a high production scale, it is possible to dilute the costs of implementing and maintaining these initiatives. However, as the production scale decreases, small producing communities may need help participating in product certification processes. In global contexts, where consumer markets become increasingly demanding, the need for certification can translate into a barrier to selling such products. In this context, the present article aims to investigate existing certifications in the literature for large- and small-scale fisheries and their benefits and challenges for the fisheries. The systematic literature review was conducted to achieve the research objectives. A total of 38 articles were analyzed for this study. The Marine Stewardship Council emerged as the most recognizable certification body worldwide. Market access was the benefit of this certification scheme. On the other hand, small-scale fisheries face barriers in obtaining this market access tool, with costs being the primary challenge cited. Certification schemes can significantly enhance the fisheries value chain by fostering better interactions between fisheries stakeholders and reshaping the structure of small-scale fisheries. Microtubules are dynamic filaments that assemble spindles for eukaryotic cell division. As the concentration profiles of soluble tubulin and regulatory proteins are non-uniform during spindle assembly, we asked if diffusiophoresis - motion of particles under solute gradients - can act as a motorless transport mechanism for microtubules. We identify the migration of stable microtubules along cytoplasmic and higher concentration gradients of soluble tubulin, MgCl2, Mg-ATP, Mg-GTP, and RanGTP at speeds O(100) nm/s, validating the diffusiophoresis hypothesis. Using two buffers (BRB80 and CSF-XB), microtubule behavior under MgCl2 gradients is compared with negatively charged particles and analyzed with a multi-ion diffusiophoresis and diffusioosmosis model. Microtubule diffusiophoresis under gradients of tubulin and RanGTP is also compared with the charged particles and analyzed with a non-electrolyte diffusiophoresis model. Further, we find that tubulin and RanGTP display concentration dependent cross-diffusion that influences microtubule diffusiophoresis. Finally, using Xenopus laevis egg extract, we show that diffusiophoretic transport occurs in an active cytoplasmic environment. The authors show microtubule motion along concentration gradients of proteins and salts similar to those found in the cytoplasm. Background: There has been an increase in studies examining the demands of soccer relative to each playing position in recent years. Understanding the physical, physiological, and technical demands on soccer players according to their positional role during competitive matches is necessary to understand match requirements and develop position-specific training practices. Thereby, there is a clear need to synthesize the information on the different profiles of each playing position. Objective: This review aimed to organize the literature investigating physical, physiological, and technical demands according to playing positions and provide a framework to identify gaps and suggestions for future studies. Methods: A systematic search was conducted in October 2023 using four electronic databases: Web of Science, SPORTDiscus, PubMed and Scopus. The review followed PRISMA 2020 guidelines and the PRISMA-ScR extension for Scoping Reviews. The studies were included if the sample comprised adult male soccer players categorized from Tier 3 to Tier 5 (i.e., highly trained/national level, elite/international level, or world class) and compared the physical, physiological, or technical parameters across playing positions. Results: A total of 178 studies met the inclusion criteria and were included in the review. The number of teams, players, and matches analyzed per study varied considerably. Although a range of classifications were reported across studies, 59% of studies classified players as central defenders, full-backs, central midfielders, wide midfielders, and forwards. The findings suggests that central and external midfielders, and external defenders cover greater total and high-speed distance than forwards or central defenders. Sprint distance was higher in external midfielders versus all other positions. Defenders and central midfielders perform more passes than external midfielders and forwards. Heart rate was the most commonly reported physiological variable across playing positions. When expressed as a percentage of maximal heart rate, midfielders presented higher mean values than all other playing positions. Conclusion: This scoping review demonstrates that there are differences in the demands on players across playing positions in soccer. Training practices in soccer should be based on the specific requirements of each positional role to ensure players can fulfill their tactical responsibilities during the game. Corncob biochar was produced by pyrolisis (400 °C, N2 flow of 3 L/min, yield of 42%) and applied as an alternative adsorbent in solid-phase extraction (SPE) for pesticide control in water. Part of the biochar was activated with KOH (3 mol/L) and HCl (3 mol/L). The non-activated biochar (NB) and activated biochar (AB) were characterized by Fourier-transform infrared spectroscopy (FTIR) and point of zero charge (PZC) analyses, with PZC pH values of 7.37 and 6.07 obtained for NB and AB, respectivey. For AB, Boehm titration, energy-dispersive X-ray spectroscopy (EDX), scanning electron microscopy (SEM), and Brunauer-Emmett-Teller (BET) analyses indicated the presence of acidic groups, high silicon content, surface area of 99.59 m2/g, and pore volume of 0.265 cm3/g, respectively. The adsorption of pesticides in water (1 μg/mL) was performed by SPE using C18 and AB. The eluents that provided the best recoveries were acetonitrile and dichloromethane/methanol (50:50). Pesticide analysis was performed by LC/MS, with a C18 column and a mobile phase of water (5 mmol/L NH4HCO2) and methanol (5 mmol/L NH4HCO2). The effects of the variables adsorbent amount, sample volume, and eluent volume were evaluated using 23 factorial design for SPE with AB. Among 13 pesticides, 10 presented recoveries between 75.9 and 117%. Statistical analysis (Pareto chart, response surface, and ANOVA) showed that the best condition was obtained with 100 mg of AB, 50 mL of sample, and 15 mL of eluent, while the use of 5 mL of eluent also provided satisfactory results. Under the optimal conditions, the SPE method exhibited good linear relation (r2 ˃ 0.99) in the linear range of 1–1000 μg/L. The KATITA-25 (Kidney AlloTransplant Immunosuppressive Therapy Adherence) Questionnaire is a multidimensional self-administered scale developed in Brazilian Portuguese that measures the predisposition for non-adherence to immunosuppressive medication in candidate patients for kidney transplantation, in the pre-transplant setting. The aim of this study was the cross-cultural validation of the Spanish version of the KATITA-25 scale. The translation/back-translation method was used, followed by cognitive interviews and a pre-test. The Spanish version of KATITA-25 was administered to 163 candidate patients for kidney transplantation from 2 kidney transplant centres and 2 dialysis centres in Catalonia, Spain. The scale was re-administered to the first 79 patients included into the study, after a 2 to 4 weeks interval. Internal consistency was assessed using Cronbach's alpha, test–retest reliability with the intraclass correlation coefficient (ICC); construct validity with goodness of fit indices after confirmatory factor analysis using structural equation modelling. The Cronbach's alpha of the Spanish version was 0.83, the ICC was 0.86. Confirmatory factor analysis showed adequate fit of the structural model. Overall, the analytical results closely resembled those obtained in the validation of the original scale. Heatwaves (HWs) pose a severe threat to human and ecological systems. Here we assess the projected changes in heatwaves over Latin America using bias corrected high-resolution regional climate simulations under two Representative Concentration Pathway scenarios (RCPs). Heatwaves are projected to be more frequent, long-lasting, and intense in the mid-century under both RCP2.6 and RCP8.5 scenarios, with severe increases under the RCP8.5 scenario. Even under the low emissions scenario of RCP2.6, the frequency of heatwaves doubles over most of the region. A three- to tenfold rise in population exposure to heatwave days is projected over Central and South America, with climate change playing a dominant role in driving these changes. Results show that following the low emission pathway would reduce 57% and 50% of heatwave exposure for Central and South American regions respectively, highlighting the need to control anthropogenic emissions and implement sustainable practices. Energy efficiency is vital for energy transitions, and energy-poor, vulnerable, and hard-to-reach groups are at risk of being left behind. In this context, local middle actors have been suggested as partners in deploying targeted energy support. Nevertheless, scarce research has engaged with them to assess if they are willing and capable of contributing, if they can reach households, and if they have unmet needs. In this research, we draw on the mapping of 198 local organizations and 34 semi-structured interviews conducted after deploying a pilot energy support service in Setúbal, Portugal. Interviews characterized the organizations and target audiences, assessed energy literacy, explored potential collaborative roles, and recognized drivers, barriers, and solutions. Results show that most organizations are willing to disseminate activities, forward citizens, facilitate contacts, co-organize events, and participate in training. Half can identify vulnerable families for proactive support. Drivers include environmental concerns, community participation, and social support. However, few seem able to conduct energy support, hire/redirect staff, or participate in coordination. Scarce human resources, lack of time, other priorities, and financing concerns severely constrain local organizations. This case study finds some potential for local organizations to act as intermediaries in energy support, building on their communication channels and trusted relationships to fulfill specific roles. However, they are often hard-to-reach themselves and burdened with demanding activities. Thus, we suggest that fostering collaborations with local organizations is challenging but possible. Introduction: In soccer, most studies evaluate metabolic profile changes in male athletes, often using data from a single match. Given the current landscape of women's soccer and the effects of biological sex on the physiological response and adaptation to exercise, more studies targeting female athletes and analyzing pre- and post-game moments throughout the season are necessary. Objectives: To describe the metabolomics profile of female soccer athletes from an elite team in Brazil. The study observed the separation of groups in three pre- and post-game moments and identified the discriminating metabolites. Methods: The study included 14 female soccer athletes. Urine samples were collected and analyzed using Nuclear Magnetic Resonance in pre-game and immediate post-game moments over three national championship games. The metabolomics data were then used to generate OPLS-DA and VIP plots. Results: Forty-three metabolites were identified in the samples. OPLS-DA analyses demonstrated a progressive separation between pre-post conditions, as supported by an increasing Q2 value (0.534, 0.625, and 0.899 for games 1, 2 and 3, respectively) and the first component value (20.2% and 19.1% in games 1 and 2 vs. 29.9% in game 3). Eight out of the fifteen most discriminating metabolites appeared consistently across the three games: glycine, formate, citrate, 3-hydroxyvalerate, glycolic acid, trimethylamine, urea, and dimethylglycine. Conclusion: The main difference between the three games was the increasing separation between groups throughout the championship. Since the higher VIP-scores metabolites are linked to energy and protein metabolism, this separation may be attributed several factors, one being the accumulation of fatigue. [ABSTRACT FROM AUTHOR]

Extreme weather events have become more frequent and severe with ongoing climate change, with a huge implication for the agricultural sector and detrimental effects on crop yield. In this study, we compare several combinations of climate indices and utilized the Least Absolute Shrinkage and Selection Operator (LASSO) to explain the probabilities of substantial drops in silage maize yield (here defined as "yield shock" by using a 15th percentile as threshold) in Germany between 1999 and 2020. We compare the variable importance and the predictability skill of six combinations of climate indices using the Matthews Correlation Coefficient (MCC). Finally, we delve into year-to-year predictions by comparing them against the historical series and examining the variables contributing to high and low predicted yield shock probabilities. We find that cold conditions during April and hot and/or dry conditions during July increase the chance of silage maize yield shock. Moreover, a combination of simple variables (e.g. total precipitation) and complex variables (e.g. cumulative cold under cold nights) enhances predictive accuracy. Lastly, we find that the years with higher predicted yield shock probabilities are characterized mainly by relatively hotter and drier conditions during July compared to years with lower yield shock probabilities. Our findings enhance our understanding of how weather impacts maize crop yield shocks and underscore the importance of considering complex variables and using effective selection methods, particularly when addressing climate-related events. Microalgae are increasingly recognized as a valuable resource for bolstering sustainability in agriculture. However, current research and patents primarily focus on Chlorella spp., Scenedesmus spp., and Spirulina spp., thus leaving the vast diversity of microalgae relatively unexplored for agricultural applications. Euglena gracilis (Euglenophyta) is a microalga renowned for its resilience to diverse environmental stressors and capability to produce a variety of bioactive metabolites. This study investigated the potential of cultivating E. gracilis in piggery wastewater for nutrient recycling and as a source of beneficial biomolecules, particularly for biostimulant use. Utilizing raw wastewater diluted to 25% (P25) and pre-treated wastewater with photo-Fenton (PF), the research found that E. gracilis exhibited elevated cell density, biomass concentration, and overall cell health in both wastewaters compared to a synthetic medium (BG11-NPK). This was due to its efficient removal of nutrients, especially ammoniacal-nitrogen and phosphate, resulting in a biomass rich in polyunsaturated fatty acids, amino acids, and paramylon content. The whole-cell biomass significantly enhanced the germination index of lettuce and tomato seeds compared to the water control. Additionally, it promoted cell expansion and root formation in cucumber cotyledons, exhibiting similarities to phytohormones such as gibberellin, cytokinin, and auxin. Furthermore, it is suggested that E. gracilis biomass contains molecules related to resistance to environmental stresses, particularly in tomatoes, given the enhancement in the seedling vigor index. E. gracilis exhibited remarkable adaptability to piggery wastewater, recycling nutrients and yielding biomass rich in bioactive molecules with potential as plant biostimulants. Background: Heart failure (HF) often disrupts the protein quality control (PQC) system leading to protein aggregate accumulation. Evidence from tissue biopsies showed that exercise restores PQC system in HF; however, little is known about its effects on plasma proteostasis. Aim: To determine the effects of exercise training on the load and composition of plasma SDS-resistant protein aggregates (SRA) in patients with HF with reduced ejection fraction (HFrEF). Methods: Eighteen patients with HFrEF (age: 63.4 ± 6.5 years; LVEF: 33.4 ± 11.6%) participated in a 12-week combined (aerobic plus resistance) exercise program (60 min/session, twice per week). The load and content of circulating SRA were assessed using D2D SDS-PAGE and mass spectrometry. Cardiorespiratory fitness, quality of life, and circulating levels of high-sensitive C-reactive protein, N-terminal pro-B-type natriuretic peptide (NT-proBNP), haptoglobin and ficolin-3, were also evaluated at baseline and after the exercise program. Results: The exercise program decreased the plasma SRA load (% SRA/total protein: 38.0 ± 8.9 to 36.1 ± 9.7%, p = 0.018; % SRA/soluble fraction: 64.3 ± 27.1 to 59.8 ± 27.7%, p = 0.003). Plasma SRA of HFrEF patients comprised 31 proteins, with α-2-macroglobulin and haptoglobin as the most abundant ones. The exercise training significantly increased haptoglobin plasma levels (1.03 ± 0.40 to 1.11 ± 0.46, p = 0.031), while decreasing its abundance in SRA (1.83 ± 0.54 × 1011 to 1.51 ± 0.59 × 1011, p = 0.049). Cardiorespiratory fitness [16.4(5.9) to 19.0(5.2) ml/kg/min, p = 0.002], quality of life, and circulating NT-proBNP [720.0(850.0) to 587.0(847.3) pg/mL, p = 0.048] levels, also improved after the exercise program. Conclusion: Exercise training reduced the plasma SRA load and enhanced PQC, potentially via haptoglobin-mediated action, while improving cardiorespiratory fitness and quality of life of patients with HFrEF. Objective: Diffusional kurtosis imaging (DKI) extends diffusion tensor imaging (DTI), characterizing non-Gaussian diffusion effects but requires longer acquisition times. To ensure the robustness of DKI parameters, data acquisition ordering should be optimized allowing for scan interruptions or shortening. Three methodologies were used to examine how reduced diffusion MRI scans impact DKI histogram-metrics: 1) the electrostatic repulsion model (OptEEM); 2) spherical codes (OptSC); 3) random (RandomTRUNC). Materials and methods: Pre-acquired diffusion multi-shell data from 14 female healthy volunteers (29±5 years) were used to generate reordered data. For each strategy, subsets containing different amounts of the full dataset were generated. The subsampling effects were assessed on histogram-based DKI metrics from tract-based spatial statistics (TBSS) skeletonized maps. To evaluate each subsampling method on simulated data at different SNRs and the influence of subsampling on in vivo data, we used a 3-way and 2-way repeated measures ANOVA, respectively. Results: Simulations showed that subsampling had different effects depending on DKI parameter, with fractional anisotropy the most stable (up to 5% error) and radial kurtosis the least stable (up to 26% error). RandomTRUNC performed the worst while the others showed comparable results. Furthermore, the impact of subsampling varied across distinct histogram characteristics, the peak value the least affected (OptEEM: up to 5% error; OptSC: up to 7% error) and peak height (OptEEM: up to 8% error; OptSC: up to 11% error) the most affected. Conclusion: The impact of truncation depends on specific histogram-based DKI metrics. Background: The pathophysiology of migraine remains poorly understood, yet a growing number of studies have shown structural connectivity disruptions across large-scale brain networks. Although both structural and functional changes have been found in the cerebellum of migraine patients, the cerebellum has barely been assessed in previous structural connectivity studies of migraine. Our objective is to investigate the structural connectivity of the entire brain, including the cerebellum, in individuals diagnosed with episodic migraine without aura during the interictal phase, compared with healthy controls. Methods: To that end, 14 migraine patients and 15 healthy controls were recruited (all female), and diffusion-weighted and T1-weighted MRI data were acquired. The structural connectome was estimated for each participant based on two different whole-brain parcellations, including cortical and subcortical regions as well as the cerebellum. The structural connectivity patterns, as well as global and local graph theory metrics, were compared between patients and controls, for each of the two parcellations, using network-based statistics and a generalized linear model (GLM), respectively. We also compared the number of connectome streamlines within specific white matter tracts using a GLM. Results: We found increased structural connectivity in migraine patients relative to healthy controls with a distinct involvement of cerebellar regions, using both parcellations. Specifically, the node degree of the posterior lobe of the cerebellum was greater in patients than in controls and patients presented a higher number of streamlines within the anterior limb of the internal capsule. Moreover, the connectomes of patients exhibited greater global efficiency and shorter characteristic path length, which correlated with the age onset of migraine. Conclusions: A distinctive pattern of heightened structural connectivity and enhanced global efficiency in migraine patients compared to controls was identified, which distinctively involves the cerebellum. These findings provide evidence for increased integration within structural brain networks in migraine and underscore the significance of the cerebellum in migraine pathophysiology. Purpose: To evaluate the prognostic impact of hyperreflective foci (HRF) on spectral-domain optical coherence tomography (SD-OCT) in nonsyndromic retinitis pigmentosa (RP). Methods: Retrospective, single-center cohort study including genetically-tested RP patients with a minimum follow-up of 24 months. Clinical data including demographics, genetic results and best-corrected visual acuity (BCVA) at baseline and follow-up were collected. Horizontal and vertical SD-OCT scans were analyzed by 2 independent graders. Outer nuclear layer (ONL) thickness and ellipsoid zone (EZ) width were manually measured in horizontal and vertical scans. HRF were classified according to location: outer retinal layers within the central 3mm (central-HRF), outer retinal layers beyond the central 3mm (perifoveal-HRF), and choroid (choroidal-HRF). Central macular thickness (CMT), central point thickness (CPT) and choroidal thickness (CT) at baseline and follow-up were also recorded. Results: A total of 175 eyes from 94 RP patients (47.9% female, mean age 50.7±15.5 years) were included, with a mean follow-up of 29.24±7.17 months. Mean ETDRS (early treatment diabetic retinopathy study) BCVA decreased from 61.09±23.54 to 56.09±26.65 (p=0.082). At baseline, 72 eyes (41.1%) showed central-HRF, 110 eyes (62.9%) had perifoveal-HRF and 149 eyes (85.1%) exhibited choroidal-HRF. Central-HRF and perifoveal-HRF were associated with worse final BCVA, as well as greater BCVA deterioration (all p<0.0029). Only central-HRF were associated with a worse final CMT (p<0.001). Shorter EZ widths were associated with all types of HRF (p<0.05). Perifoveal and choroidal-HRF predicted smaller final EZ areas (p<0.01). In this paper, we argue that one way to approach what is known in the literature as the “Trust Gap” in Medical AI is to focus on explanations from an Explainable AI (xAI) perspective. Against the current framework on xAI – which does not offer a real solution – we argue for a pragmatist turn, one that focuses on understanding how we provide explanations in Traditional Medicine (TM), composed by human agents only. Following this, explanations have two specific relevant components: they are usually (i) social and (ii) abductive. Explanations, in this sense, ought to provide understanding by answering contrastive why-questions: “Why had P happened instead of Q?” (Miller in AI 267:1–38, 2019) (Sect. 1). In order to test the relevancy of this concept of explanation in medical xAI, we offer several reasons to argue that abductions are crucial for medical reasoning and provide a crucial tool to deal with trust gaps between human agents (Sect. 2). If abductions are relevant in TM, we can test the capability of Artificial Intelligence systems on this merit. Therefore, we provide an analysis of the capacity for social and abductive reasoning of different AI technologies. Accordingly, we posit that Large Language Models (LLMs) and transformer architectures exhibit a noteworthy potential for effective engagement in abductive reasoning. By leveraging the potential abductive capabilities of LLMs and transformers, we anticipate a paradigm shift in the integration of explanations within AI systems. This, in turn, has the potential to enhance the trustworthiness of AI-driven medical decisions, bridging the Trust Gap that has been a prominent challenge in the field of Medical AI (Sect. 3). This development holds the potential to not only improve the interpretability of AI-generated medical insights but also to guarantee that trust among practitioners, patients, and stakeholders in the healthcare domain is still present. Accurate and up-to-date information about the building stock is fundamental to better understand and mitigate the impact caused by catastrophic earthquakes, as seen recently in Turkey, Syria, Morocco and Afghanistan. Planning for such events is necessary to increase the resilience of the building stock and to minimize casualties and economic losses. Although in several parts of the world new constructions follow more strict compliance with modern seismic codes, a large proportion of existing building stock still demands a more detailed and automated vulnerability analysis. Hence, this paper proposes the use of computer vision deep learning models to automatically classify buildings and create large scale (city or region) exposure models. Such approach promotes the use of open databases covering most cities in the world (cf. OpenStreetMap, Google Street View, Bing Maps and satellite imagery), Therefore providing valuable geographical, topological and image data that may cheaply be used to extract valuable information to feed exposure models. Our previous work using deep learning models achieved, in line with the results from other projects, high classification accuracy concerning building materials and number of storeys. This paper extends the approach by: (i) implementing four CNN-based models to perform classification of three sets of different/extended buildings’ characteristics; (ii) training and comparing the performance of the four models for each of the sets; (iii) comparing the risk assessment results based on data extracted from the best CNN-based model against the results obtained with traditional ground data. In brief, the best accuracy obtained with the three tested sets of buildings’ characteristics is higher than 80%. Moreover, it is shown that the error resulting from using exposure models fed by automatic classification is not only acceptable, but also far outweighs the time and costs of obtaining a manual and specialised classification of building stocks. Finally, we recognize that automatic assessment of certain complex buildings’ characteristics compares to similar limitations of traditional assessments performed by specialized civil engineers, typically related with the identification of the number of storeys and the construction material. However, the identified limitations do not show worse results when compared against the use of manual buildings’ assessment.Article Highlights: Implement an AI/ML framework for automating the collection of buildings’ façades pictures annotated with several characteristics required by Exposure Models. The goal of this study was to check the direct and moderating effects of the dimensions of the national culture, based on the Cultural Dimensions Theory, about the offer of E-Government services. The direct effect of the technological infrastructure installed in the countries on their E-Government levels was also tested. Data from 55 countries were analyzed and indicated technological infrastructure and individualism are direct antecedents of E-Government. This research shows policymakers and practitioners should prioritize the enhancement of technological infrastructure and consider the cultural orientation of individualism in their strategies to promote the adoption and efficiency of E-Government services. Anaerobic and membrane technologies are a promising combination to decrease the energy consumption associated with wastewater treatment, allowing the recovery of resources: organic matter as biomethane, nutrient assimilation by microalgae and reclaimed water. In this study, domestic wastewater was treated using a combination of an upflow anaerobic sludge blanket sludge reactor (UASB) and a membrane photobioreactor (MPBR). The outdoor facilities were operated continuously for three months under unfavourable environmental conditions such as lack of temperature control, winter season with lower solar irradiation and lower daylight hours which was a challenge for the present work, not previously described. The energetic valorisation of the organic matter present in the wastewater by biomethane produced in the UASB would contribute to reducing overall facilities' energy requirements. The ultrafiltration (UF) membrane facilitated the harvesting of biomass, operating at 10 L·h−1·m−2 during the experimental period. Although the main contribution to fouling was irreversible, chemical cleanings were not necessary due to effective fouling control, which prevented the final TMP from exceeding 25 kPa. In addition, microalgae-bacterial consortium developed without prior inoculation were harvested from the MPBR using membrane assistance. Objective: The aim of this paper is to critically re-appraise the published trials assessing propranolol for migraine prophylaxis. Methods: We report methods and results following the Preferred Reporting Items for Systematic Reviews (PRISMA), by searching MEDLINE, EMBASE, Cochrane CENTRAL, and ClinicalTrials.gov for randomized trials of pharmacologic treatments for migraine prophylaxis. We included randomized trials that compared propranolol with placebo for migraine prophylaxis in adults. The outcomes of interest were informed by the Core outcome set for preventive intervention trials in chronic and episodic migraine (COSMIG) and include the proportion of patients who experience a 50% or more reduction in monthly migraine days, the reduction of monthly migraine days, and the number of adverse events leading to discontinuation. We assessed risk of bias by using a modified Cochrane RoB (risk of bias) 2.0 tool and the certainty of evidence by using the GRADE approach. Results: Our search yielded twenty trials (n = 1291 patients) eligible for data synthesis and analysis. The analysis revealed a moderate certainty evidence that propranolol leads to a reduction in monthly migraine days versus placebo (-1.27; 95% CI: -2.25 to -0.3). We found moderate certainty evidence that propranolol increases the proportion of patients who experience a 50% or more reduction in monthly migraine days, compared to placebo with a relative risk of 1.65 (95% CI 1.41 to 1.93); absolute risk difference: 179 more per 1,000 (95% CI 113 to 256). We found high certainty evidence that propranolol increases the proportion of patients who discontinue due to adverse events compared to placebo with a risk difference of 0.02 (95% CI 0.00 to 0.03); absolute risk difference: 20 more per 1,000 (95% CI 0 to 30). Conclusions: The present meta-analysis shows that propranolol has a prophylactic role in migraine, with an overall acceptable tolerability profile. Combining these results with its long-standing use and its global availability at a low cost confirms its role as a first line agent in the prophylaxis of migraine. [ABSTRACT FROM AUTHOR]
