Background: Tooth wear is an important mechanism for reducing dental dimensions and, consequently, dental crowding. The objective of this cohort study was to examine the relation of tooth wear, adjusted for covariates (age, tooth loss, arch perimeter and intercanine width), on tertiary crowding in Amazon Indigenous populations. Methods: A sample of 40 Indigenous people in permanent dentition at T0 (baseline) and after 13 years (T1) were evaluated. The sample included 16 males and 24 females belonging to two villages, Arara (n = 22, mean ages 16.6 and 29.9 years) and Assurini do Xingu (n = 18, mean ages 16.0 and 29.6 years). Clinical, intraoral photograph and dental cast evaluations were performed at both times. The anterior crowding was measured using Little's Irregularity Index (LI) and modeled through a multilevel linear regression with the predictor variables: village, tooth wear (T1-T0), age (T0), tooth loss (T1-T0), changes in intercanine width (T1-T0) and changes in arch perimeter (T1-T0). Results: A slight increase was observed (< 1 mm) in anterior dental crowding and a decrease in arch perimeter < 1.5 mm, while tooth wear increased between 0.65 and 0.99 units. The contextual variable (village) had no significant association with LI. In the upper arch, tooth loss was the only variable that showed an inverse association with LI (β=-0.41, p < 0.05). In the lower arch, the increase in dental crowding was inversely associated with tooth wear (β=-1.30, p < 0.05) and changes in arch perimeter (β=-0.31, p < 0.05). The other variables did not show significant associations. Conclusion: After 13 years, dental crowding and tooth wear increased, while the arch dimensions tended to decrease. The changes in long-term dental crowding seem to have distinct etiological components for each dental arch. In the mandible, the changes in incisor alignment were associated with increased tooth wear and decreased dental arch dimensions. Whereas in the maxilla, only tooth loss caused alterations in tooth alignment. It is suggested that the effect of increased tooth wear on the etiology of tertiary crowding is of small magnitude and restricted to the lower dental arch. [ABSTRACT FROM AUTHOR]

A random effects meta-analysis was used to estimate the pooled prevalence of HIV infection within minority indigenous populations of the South-East Asia (SEAR) and Western Pacific Regions (WPR). Sub-group analyses were conducted, and the sources of heterogeneity explored through meta-regression. The majority of studies were undertaken in high HIV risk subpopulations. There was a paucity of data for many countries with data from China representing 70% of the comparative studies. Within minority indigenous populations the pooled prevalence of HIV infection was 13.7% (95% CI 8.9, 19) and 8.4% (95% CI 6.3, 10.7) among other populations. The prevalence differential between populations was significant in the WPR (adjusted odds ratio 1.1, 95% CI 1.0, 1.2). Across both regions, in contrast to other populations, minority indigenous did not experience any significant reduction in HIV prevalence over the years of data collection. Objective: This study aimed to examine how Indigenous patients and biomedical healthcare providers understand and experience the Indigenous wellness services at a hospital in the Northwest Territories., Methods: The qualitative study (May 2018-June 2022) was overseen by a regional Indigenous Community Advisory Committee. Guided by Two-Eyed Seeing and post-colonial theory, the study employed a community-engaged research design, and included two strategies for data generation: (1) interviews with Indigenous Elders, patient advocates, biomedical healthcare providers, policy makers, and hospital administrators (n = 41), and (2) iterative sharing circles with Indigenous Elders (n = 4). Data from the interviews and first sharing circle were transcribed, thematically analyzed, and presented to the sharing circle Elders for validation., Results: The study revealed three overarching and related themes: (1) Elders and patient advocates emphasized that while the Indigenous wellness services at the hospital play a pivotal role connecting patients with cultural supports, the hospital was still not effectively bringing Indigenous healing practices into hospital care; (2) participants identified that structural factors (i.e., policy and governance decisions) shaped patients' experiences with the wellness services; and (3) participants underscored that deeply rooted forces (i.e., racism, colonialism, and biomedical dominance) inhibit the integration of Indigenous healing practices., Conclusion: The findings extend understandings of hospital-based Indigenous wellness services by surfacing relationships between deeply rooted forces, organizational structures, and Indigenous patients' experiences. Summary: 'Indigenous peoples' across the globe suffer a disproportionate burden of mental illness. However, this burden is not fully explored in India despite having the second largest absolute concentration of indigenous peoples in the world. We did a secondary analysis of data from a cross-sectional survey in indigenous populations from the Nilgiri Biosphere Reserve in South India. Symptoms suggestive of psychological distress were reported by 39.9% participants. Being alone, tobacco use, hypertension, hypertension in family member, and violent conflict in household were independently associated with psychological distress. More epidemiological studies need to be conducted to map the burden and elaborate the relationships between mental health problems and socio-cultural factors in indigenous populations in India. While stigmatisation is universal, stigma research in low- and middle-income countries (LMIC) is limited. LMIC stigma research predominantly concerns health-related stigma, primarily regarding HIV/AIDS or mental illness from an adult perspective. While there are commonalities in stigmatisation, there are also contextual differences. The aim of this study in DR Congo (DRC), as a formative part in the development of a common stigma reduction intervention, was to gain insight into the commonalities and differences of stigma drivers (triggers of stigmatisation), facilitators (factors positively or negatively influencing stigmatisation), and manifestations (practices and experiences of stigmatisation) with regard to three populations: unmarried mothers, children formerly associated with armed forces and groups (CAAFAG), and an indigenous population. Group exercises, in which participants reacted to statements and substantiated their reactions, were held with the 'general population' (15 exercises, n = 70) and 'populations experiencing stigma' (10 exercises, n = 48). Data was transcribed and translated, and coded in Nvivo12. We conducted framework analysis. There were two drivers mentioned across the three populations: perceived danger was the most prominent driver, followed by perceived low value of the population experiencing stigma. There were five shared facilitators, with livelihood and personal benefit the most comparable across the populations. Connection to family or leaders received mixed reactions. If unmarried mothers and CAAFAG were perceived to have taken advice from the general population and changed their stereotyped behaviour this also featured as a facilitator. Stigma manifested itself for the three populations at family, community, leaders and services level, with participation restrictions, differential treatment, anticipated stigma and feelings of scapegoating. Stereotyping was common, with different stereotypes regarding the three populations. Although stigmatisation was persistent, positive interactions between the general population and populations experiencing stigma were shared as well. This study demonstrated utility of a health-related stigma and discrimination framework and a participatory exercise for understanding non-health related stigmatisation. Results are consistent with other studies regarding these populations in other contexts. This study identified commonalities between drivers, facilitators and manifestations—albeit with population-specific factors. Police victim and witness interviewing in a northern Canadian territory with a predominantly Indigenous population was examined across two studies. In study 1, an Internet survey about interview training, practices and cross-cultural issues was completed by serving police officers (N = 37). In study 2, transcripts of interviews with Indigenous adult victims and witnesses (N = 20) were coded for the presence of various interviewing practices (e.g. question types, interruptions, talking time). Survey results showed that most officers were untrained in scientific-interviewing protocols but were aware of the general practices that constitute a competent interview (e.g. building rapport, requesting a free narrative). Most respondents indicated that cultural differences impact their interviewing style. Results of the transcript analysis showed that officers violated the 80/20 talking rule in 90% of the interviews and unproductive question types (e.g. closed yes/no) were used often. All interviews contained a request for a full account, most interviews contained elements of active listening and few interruptions were observed. These findings are discussed with reference to how interviewing and cross-cultural communication training could help police organizations who serve Indigenous populations. This paper explores strategies that support Aboriginal parents' mastery of evidence-based early learning strategies, and their impact on young Aboriginal children's learning outcomes. The three-year study followed 32 parent-child dyads attending Families as First Teachers (FaFT) playgroups in two remote Northern Territory communities in 2015–2017. Trained FaFT staff provided parents with coaching in the use of Conversational Reading—an evidence-based shared reading strategy in first language—at FaFT. The study examined patterns of parent mastery across the three-year study period, the relationship between levels of parent-child participation at FaFT (program dosage) and parent mastery, and the impact of parent mastery of Conversational Reading on young Aboriginal children's language and learning outcomes. By including measures of parent-child participation and parent mastery of key program strategies at three time points, the study also provides a picture of the fidelity of program implementation across the study period. The findings indicate that parents' mastery of strategies (and thus the fidelity of program implementation) increased over time in line with the program dosage parents received. Higher levels of parent-child participation at FaFT and parent mastery of strategies at the end of the program were positively associated with children's language and learning outcomes. This study demonstrates that the provision of coaching at playgroup is an effective way to build parent capacity in the implementation of evidence-based early learning strategies, and that supporting parent mastery of teaching strategies has the potential to improve the learning outcomes of young children in remote Aboriginal communities. The life satisfaction of the indigenous population in Arctic Alaska is quite high, perhaps higher than that of the U.S. population in general. Is wage employment brought by modern economic growth responsible for their high life satisfaction? Probably not. Interestingly, we find that household wage income and job opportunities per working-age Native are negatively associated with their life satisfaction. In contrast, non-wage income, which does not involve the sacrifice of time that can be used for subsistence activities, is positively associated with life satisfaction. A household's involvement in these traditional activities is found to be positively associated with life satisfaction as well. The findings challenge the common preconception about the effects of modernization and point to the importance of the non-wage subsistence activities as a preferred substitute for wage employment to this indigenous population. A combination of Christian religious beliefs and indigenous spiritual beliefs is also positively associated with their life satisfaction. [ABSTRACT FROM AUTHOR]

The traditions and habits of indigenous communities in México include the use of wood and biomass burning to cook their food, which generates large amounts of smoke and therefore pollution inside the households. This smoke is composed of a complex mixture of polycyclic aromatic hydrocarbons (PAHs) which at high levels of exposure cause carcinogenic, genotoxic effects and some chronic pulmonary and cardiovascular diseases; however, few studies relate kidney health with exposure to PAHs. Thus, the aim of this study was the evaluation of 10 hydroxylated metabolites of PAHs (OH-PAHs), and their correlation with biomarkers of early kidney damage renal (cystatin-C (Cys-C)), osteopontin (OPN), retinol-binding protein-4 (RPB-4), and neutrophil gelatinase-associated lipocalin (NGAL) in the indigenous population of the Huasteca Potosina in Mexico. The results demonstrate the presence of the OH-PAHs and kidney damage biomarkers in 100% of the study population. The OH-PAHs were shown in the following order of frequency, 1-OH-PYR > 4-OH-PHE > 2-OH-NAP > 1-OH-NAP > 9-OH-FLU > 3-OH-FLU > 2-OH-FLU > 3-OH-PHE and with the following percentages of detection 97.6, 87.8, 78, 73.2, 68.3, 31.7, 14.6, and 12.2%, respectively. NGAL and RBP-4 were present in above 85% of the population, with mean concentrations of 78.5 ± 143.9 and 139.4 ± 131.7 ng/g creatinine, respectively, OPN (64%) with a mean concentration of 642.6 ± 723.3 ng/g g creatinine, and Cys-C with a mean concentration of 33.72 ± 44.96 ng/g creatinine. Correlations were found between 1-OH-NAP, 2-OH-NAP, 9-OH-FLU, and 4-OH-PHE and the four biomarkers of early kidney damage. 3-OH-FLU with OPN and 1-OH-PYR correlated significantly with NGAL, OPN, and RPB-4. Objective: To provide tribe- specific child mortality rates and health indicators from selected states in India. Methods: We used Census 2011 data and Coale Demney methodology to estimate the infant mortality rate (IMR), under-five mortality rate (U5MR) and expectation of life at birth (LEB) for 123 tribes of selected states of India. Results: The estimated IMR and U5MR were higher in scheduled tribe population compared to respective state's total population. The IMR varied from 124 in the Birhore tribe of Chhattisgarh and Jharkhand, and the Bharias of Madhya Pradesh to 48 per 1000 live births in the Gamit tribe of Maharashtra. Similarly, the U5MR varied from the highest (203) in the Birhore tribe of Chhattisgarh to the lowest (57/1000 live births) in the Gamit tribe. The LEB varied from 72 years in the Gamit tribe of Maharashtra to 51 years in the Birhore tribe of Chhattisgarh. The study reveals that tribes have gross variation in child mortality rates and there is pressing need to prioritize tribe-specific action plans to improve their health indicators. Oncorhynchus masou, including subspecies of Oncorhynchus masou masou (yamame) and Oncorhynchus masou ishikawae (amago), is one of the salmonid groups impacted by human activity such as dam construction and release of non-native salmonids. In this study, we investigated the genetic structure of O. masou populations in the Sakawa and Sagami Rivers, Japan, by sequencing the mitochondrial control region. We hoped to identify genetically the O. masou populations specific to and originally native to Kanagawa Prefecture, where the two subspecies are thought to be present. The populations found in the upstream tributaries, where there has been no human impact and no upstream migration of fishes, were assumed to be descendants of the local O. masou populations in both river systems, and the morphological features seen here were similar to amago and yamame. However, both populations were genetically related to amago. In addition, only six haplotypes were detected in 315 individuals collected from 20 localities in the two river systems. Furthermore, haplotype diversity and nucleotide diversity of these populations were low, and high Fst values were observed. These results suggest that the population size is restricted and genetic diversity is decreasing in the O. masou populations of the Sakawa and Sagami Rivers. [ABSTRACT FROM AUTHOR]

We analyzed published data on the levels of serum 25(OH)D in apparently healthy individuals from the Russian Far North. The total sample included 2061 subjects of various age and ethnic groups. The serum levels of 25(OH)D and its age dynamics in the northerners living in towns are similar to those of the inhabitants of the temperate climate zone of Russia. Data on the age-related changes in the vitamin D status of the indigenous Arctic people with the traditional lifestyle are scanty. Serum 25(OH)D concentrations in the indigenous and alien population of the high-latitude regions reflect the seasonal changes in the daylight duration. The level of vitamin D decreases in winter and reaches minimum in February, i.e., after the end of the polar night. We compared data on rural Russian and indigenous people and found ethnic differences in the 25(OH)D concentrations; however, the vitamin D status of various indigenous groups of the Russian North is poorly studied. We could not find publications that analyze the 25(OH)D concentrations along with the direct assessment of food consumption. The available data do not contradict the opinion that the traditional food products are beneficial. However, there are no studies to support this opinion. There are little data on the effect of foods from marine mammal catch, marine and freshwater fishery, and venison on the vitamin D status of the indigenous people of the Russian North. Our objectives were to (1) compare the risks for poor long-term mental health outcomes among indigenous women with and without a teenage pregnancy and (2) determine if community and cultural factors modify this risk. We conducted a secondary analysis of the 2012 Aboriginal Peoples Survey. Respondents were women aged 25 to 49 years who had given birth to at least one child. Teenage mothers (age at first birth 13 to 19 years; n = 1330) were compared to adult mothers (age at first birth 20 years or older; n = 2630). Mental health outcomes were psychological distress, mental health status, suicide ideation/attempt, and alcohol consumption. To address objective 1, we used binary logistic regression analyses before and after controlling for covariates. To address objective 2, we tested the significance of interaction terms between teenage pregnancy status and effect measure modifiers. In unadjusted analyses, teenage pregnancy was associated with increased risk for poor/fair mental health [odds ratio (OR) 1.77, 95% confidence interval (CI) 1.24-2.53] and suicide attempt/ideation (OR 1.95, 95% CI 1.07-3.54). However, the associations were not statistically significant after adjusting for demographic, socioeconomic, environmental, and health covariates. Teenage pregnancy was not associated with increased risk for high psychological distress or heavy alcohol consumption in unadjusted or adjusted analyses. The interaction term for involvement in cultural activities was statistically significant for poor/fair mental health; however, after stratification, ORs were non-significant. Among indigenous mothers, teenage pregnancy was less important than broader social and health circumstances in predicting long-term mental health. [ABSTRACT FROM AUTHOR]

We estimated the prevalence of self-reported health professional-diagnosed anxiety disorders among Canadian First Nations adults living off-reserve, and assessed the relationship between anxiety disorders and Indigenous determinants of health (Status Indian, residential school attendance, knowledge of Indigenous language, and participation in traditional activities) using the 2012 Aboriginal Peoples Survey. Multivariable logistic regression models were performed using bootstrap weights. The prevalence of anxiety disorders was 14.5% among off-reserve First Nations adults. There was an increased odds of anxiety disorders among those participating in traditional activities compared to their counterparts (aOR 1.46, 95% CI 1.12-1.90). No association was found between anxiety disorders and other Indigenous determinants of health. There is a high prevalence of self-reported anxiety among First Nations adults living off-reserve. However, further studies are warranted to identify and assess the role of Indigenous determinants of health for anxiety disorders and other prevalent mental health conditions in this population. This article addresses the ethics of betel nut use in Taiwan. It first presents scientific facts about the betel quid and its consumption and the generally accepted negative health consequences associated with its use: oral and esophageal cancer, coronary artery disease, metabolic diseases, and adverse effects in pregnancy. It then analyzes the cultural background and economic factors contributing to its popularity in Asia. The governmental and institutional attempts to curb betel nut cultivation, distribution, and sales are also described. Finally, the article analyzes the bioethical implications of this often-ignored subject from the perspectives of human dignity, the good of health, vulnerable groups, cultural diversity, informed consent, and ethical blind spots. The Lípez Altiplano is an arid and cold highland desert, where population density was always low and there was no urbanization. Pre-Hispanic, the regional distribution of the population was related to the environment: the majority was concentrated where quinoa agriculture was possible. Following the Spanish conquest, however, mining activity allowed an unprecedented demographic development in those highlands. In this study, I analyze the consequences of mining activities on demography and socio-economic activities in Lípez, before, during and after the apogee of its most important colonial mine, San Antonio del Nuevo Mundo. Similar to most northern developed jurisdictions, Northern Australia has perennially featured a youthful population composition due to a high Indigenous composition and a young non-Indigenous population for whom migration flows are concentrated in young ages. However, demographic pre-cursors are now in place that will bring about relatively rapid and proportionally substantial population ageing in the population of the north of Australia, as has occurred elsewhere. More seniors will bring many opportunities from the social, economic and other capital they provide. Seniors will balance population compositions towards a less male dominated and less youthful structure, helping to reduce high rates of turnover. Nevertheless, there will be challenges and governments for northern Australian jurisdictions will in coming decades face structural shifts in population profiles and the economy. To date ageing in the north has received little in the way of policy attention, but planning for the needs of seniors and their communities must be forthcoming. This paper reports on the causes and consequences of these circumstances and provides research insights from soon-to-be and existing seniors on their hopes and concerns around 'ageing in place' in the north. We then ponder what governments might do to maximise potential benefits in the face of this significant demographic and social change. While delayed ageing in the north affords governments at all levels with opportunities to learn from the experiences, policies and programs of other areas, concerted consultation with seniors is necessary to deliver optimal support and community benefit for those who wish to 'age in place' in the north. The occurrence of genetic erosion in local maize varieties in Mexico is intensely debated. Recent publications from Mexico show contradicting results about the loss of local varieties. Genetic erosion is a complex process, and well-documented examples of actual genetic erosion are not common in the literature. We worked in a region in which adoption of improved varieties was negligible, but other factors affecting maize agriculture were at play. The objectives of the study were to describe changes in maize diversity in the last 10 years and to associate them with socio-economic and environmental changes in a region in Mexico's Central Highlands. We used richness and abundance of local varieties and diversity indices of races as indicators of maize diversity changes over time. We analyzed statistics and based on interviews we evaluated maize diversity changes between 2005 and 2015. We interviewed 113 farmers on two occasions with intervals from 5 to 10 years. According to climate statistics, rain has declined and temperature has increased. We also found a decrease in the lake level during the past 35 years. The total population in the region has doubled since the 1960s. The indigenous population has not changed significantly. Number of people working in agriculture has decreased since the 1960s. Rain fed agriculture decreased 8.1 % from 1990 to 2007. In four villages studied, farmed land area had decreased between 1995 and 2015. This reduction varies between 22 and 39 % depending on the village. Maize planted area decreased from 9675 to 8115 ha from 2003 to 2014. In the same period, avocado plantations grew from 34 to 786 ha. In despite of these changes, we did not find significant changes in average landraces per farmer (2.13 ± 0.28 in 2015) nor per village (4.15 ± 1.26 in 2015). Significant changes in maize races were not found either (1.91 ± 0.26 per farmer, 2.85 ± 0.86 per village in 2015). First Nations and Métis, two of Canada's constitutionally recognized Indigenous groups, suffer from poorer overall health than non-Indigenous Canadians. Current smoking, a known predictor of chronic health conditions, is close to twice as prevalent among Indigenous youth as it is among non-Indigenous Canadian youth. However, little population-level research has examined the correlates of current smoking among this population. Guided by a health framework centered on Indigenous-specific determinants, we used data from the 2012 Aboriginal Peoples Survey to examine the correlates of current smoking among First Nations and Métis youth aged 15-17 years living outside of First Nations reserves. Using binary logistic regression, we investigated how culturally specific factors, namely knowledge of an Indigenous language, participation in traditional activities, and family members' attendance at residential schools, were correlated with current smoking. We also considered demographic, geographic, socioeconomic and health-related correlates. Overall, an estimated 20.6% of First Nations and Métis youth reported current smoking. We found no significant associations between culturally specific activities and current smoking in the multivariate analyses, although those who spoke an Indigenous language were more likely to smoke. Those who participated in sports more often were less likely to smoke, and respondents who reported heavy drinking and who were from families with lower income were more likely to smoke. Gender, body mass index, urban/rural geography and regional geography, and mother's highest level of education were not significantly correlated with smoking. The results of our study support prior research that has found a disturbingly high prevalence of current smoking among Indigenous youth, compared to their non-Indigenous counterparts. Our results highlight the importance of considering sports participation, co-occurring health-risk behaviours and socioeconomic factors when developing interventions aimed at reducing the prevalence of smoking among First Nations and Métis youth. Background: To describe the prevalence, type, and mental health correlates of substance use disorders in a large sample of incarcerated Indigenous Australians. Methods: An epidemiological survey of the mental health of Indigenous people in custody in the state of Queensland, Australia was conducted using culturally informed methods. The prevalence, type and mental health correlates of substance use disorders were determined using a diagnostic interview and questionnaire. Results: In a sample of 396 Indigenous people (331 males, 65 females) the prevalence of any substance use disorder was 66%. Alcohol dependence (males 47%, females 55%) was the most common type of substance use disorder, followed by cannabis dependence (males 20%, females 26%). Mental illness (anxiety, depression and psychotic disorder), and lifetime suicide thoughts and attempts, were significantly more likely among those with a substance use disorder. The majority of the sample reported intoxication with alcohol (70%) and/or other drugs (51%) at the time of arrest. Most individuals (87%) had not accessed alcohol and other drug services in the 12 months prior to custody. Conclusions: Substance dependence was common in this sample and was associated with other forms of mental health adversity, yet most individuals reported no access to health services prior to incarceration. Effectively responding to substance dependence for Indigenous Australians is a public health and criminal justice priority. Culturally capable alcohol and other drug treatment services in custody and in the community are critical, and should be co-located and coordinated with mental health services. Background: Gastric banding has been promoted as less suitable for indigenous persons or persons who live remotely as it requires in person follow-up for band adjustment and may have higher rates of reoperation. This study assessed being an indigenous Australian or living remotely (but not both) on outcomes following gastric banding. Methods: Data was prospectively recorded on all 559 patients who underwent gastric banding by one surgeon at one private hospital in Darwin, between February 1998 and August 2014. Results: Forty persons (7 %) were indigenous and 93 (17 %) were remotely living (only 7 were both). At the last assessment (follow-up 37 (SD 31) months), overall percentage of excess weight loss (EWL) was 53 % (30 %), the percentage of total weight loss (TWL) was 23 (13), and 389 (70 %) achieved >50 % EWL. Seventy-two percent (43/60) ceased all diabetic medications. Ninety-two (17 %) came to reoperation. There was little difference between the indigenous and non-indigenous metropolitan-living groups, or between the remote and metropolitan non-indigenous groups in %EWL, %TWL, the proportion who achieved more than 50 % EWL, the time to achieve the goal weight, or cessation of diabetes medication. Similarly, there was little difference in the time to band removal or replacement. No person was directly compromised at band removal/replacement by delay due to dwelling remotely. Conclusions: In these select persons who underwent gastric banding in the private sector only, outcomes of weight loss and revisional surgery were acceptable and comparable between indigenous and non-indigenous metropolitan-dwelling persons as well as between remote and metropolitan-dwelling non-indigenous persons. This study aimed to estimate the prevalence of musculoskeletal (MSK) disorders and rheumatic diseases in the Chontal and Mixtec indigenous communities in the state of Oaxaca, Mexico, using the Community-Oriented Program for the Control of Rheumatic Diseases (COPCORD) methodology. After cross-culturally validating the COPCORD questionnaire for these communities, we conducted a cross-sectional, analytical, community-based census study using a house-to-house method. Positive cases of MSK disorders were assessed by primary care physicians and rheumatologists. The study population included participants aged ≥18 years from the indigenous communities of San Antonio Huitepec and San Carlos Yautepec. A total of 1061 persons participated in the study. Mean age was 46.9 years (standard deviation 19.9; age range 18-97 years); 642 (60.5 %) were women; 483 participants (45.5; 42.4-48.5 %) had MSK pain in the previous 7 days. Diagnoses were back pain 170 (16.0 %; 95 % confidence interval [CI] 13.8-18.3); osteoarthritis 157 (14.7 %; 95 % CI 12.7-17.0); rheumatic regional pain syndrome 53 (4.9 %; 95 % CI 3.7-6.4); rheumatoid arthritis 4 (0.3 %; 95 % CI 0.1-0.9); dermatomyositis 1 (0.09 %; 95 % CI 0.0-0.5); ankylosing spondylitis 1 (0.09 %; 95 % CI 0.0-0.5); systemic lupus erythematosus 1 (0.09 %; 95 % CI 0.02-0.5); and gout 1 (0.09 %; 95 % CI 0.0-0.5). 53.2 % had not received medical treatment for their disease. The prevalence of MSK disorders in indigenous communities in the Mixtec and Chontal regions is very high. The most common rheumatic diseases found were back pain and osteoarthritis. The prevalence and distribution of soil and water samples contaminated with enteroparasites of humans and animals with zoonotic potential (EHAZP) in Apucaraninha Indigenous Land (AIL), southern Brazil, was evaluated. An environmental survey was conducted to evaluate the presence of parasitic forms in peridomiciliary soil and associated variables. Soil samples were collected from 40/293 domiciles (10 domiciles per season), from November 2010 to June 2011, and evaluated by modified methods of Faust et al. and Lutz. Analyses of water from seven consumption sites were also performed. The overall prevalence of soil samples contaminated by EHAZP was 23.8 %. The most prevalent parasitic forms were cyst of Entamoeba spp. and eggs of Ascaris spp. The highest prevalence of contaminated soil samples was observed in winter (31%). The probability map obtained with geostatistical analyses showed an average of 47 % soil contamination at a distance of approximately 140 m. The parasitological analysis of water did not detect Giardia spp. or Cryptosporidium spp. and showed that all collection points were within the standards of the Brazilian law. However, the microbiological analysis showed the presence of Escherichia coli in 6/7 sampled points. Despite the low level of contamination by EHAZP in peridomiciliar soil and the absence of pathogenic protozoa in water, the AIL soil and water (due to the presence of fecal coliforms) are potential sources of infection for the population, indicating the need for improvements in sanitation and water treatment, in addition periodic treatment of the population with antiparasitic. Both girls and boys display an activation of adaptive-compensatory response relative to Caucasian newcomers, which manifests itself as a statistically significant increase in nonenzymatic components of the antioxidant defense system. In 2007-2008 the Council of Australian Governments agreed to six ambitious targets for addressing longstanding disparities between Indigenous and other Australians in health, education and employment outcomes. The 'National Integrated Strategy for Closing the Gap on Indigenous Disadvantage' (colloquially 'Closing the Gap') includes the goal of eliminating life expectancy gaps within a generation. This policy says nothing about what changes in the demographic structure of the population might be expected should there be even partial success towards this ambitious target. Information is required to analyse age and sex shifts for their effects on service demand and provision. In this study we apply cohort component projections modelling to the Northern Territory, the Australian jurisdiction with the largest Indigenous component in its population and the largest life expectancy gaps, to assess the demographic effects of closing the gap within a generation. Three scenarios are modelled: (1) No changes to Indigenous life expectancies from those estimated in 2010; (2) Complete success in closing the gap within a generation; and (3) A continuation of current forecasts about how Indigenous life expectancies will change into the future. Although closing the gap would only produce a small increase in the size of the next generation's Indigenous population, over and above that projected with lower life expectancy scenarios, it would result in substantial changes in age compositions, with associated shifts in various demographic indices. During the first randomized intervention trial (RESPIRE: Randomized Exposure Study of Pollution Indoors and Respiratory Effects) in air pollution epidemiology, we pioneered application of passive carbon monoxide (CO) diffusion tubes to measure long-term personal exposures to woodsmoke. Here we report on the protocols and validations of the method, trends in personal exposure for mothers and their young children, and the efficacy of the introduced improved chimney stove in reducing personal exposures and kitchen concentrations. Passive diffusion tubes originally developed for industrial hygiene applications were deployed on a quarterly basis to measure 48-hour integrated personal carbon monoxide exposures among 515 children 0-18 months of age and 532 mothers aged 15-55 years and area samples in a subsample of 77 kitchens, in households randomized into control and intervention groups. Instrument comparisons among types of passive diffusion tubes and against a continuous electrochemical CO monitor indicated that tubes responded nonlinearly to CO, and regression calibration was used to reduce this bias. Before stove introduction, the baseline arithmetic (geometric) mean 48-h child (n=270), mother (n=529) and kitchen (n=65) levels were, respectively, 3.4 (2.8), 3.4 (2.8) and 10.2 (8.4) p.p.m. The between-group analysis of the 3355 post-baseline measurements found CO levels to be significantly lower among the intervention group during the trial period: kitchen levels: −90%; mothers: −61%; and children: −52% in geometric means. No significant deterioration in stove effect was observed over the 18 months of surveillance. The reliability of these findings is strengthened by the large sample size made feasible by these unobtrusive and inexpensive tubes, measurement error reduction through instrument calibration, and a randomized, longitudinal study design. These results from the first randomized trial of improved household energy technology in a developing country and demonstrate that a simple chimney stove can substantially reduce chronic exposures to harmful indoor air pollutants among women and infants. Dual-record system methods are commonly used as a basis for population estimation. A basic assumption is that the units sampled are drawn only from the population to be estimated. This assumption cannot be met for remote Indigenous communities in Australia. A new variant of dual-record population estimation is presented, which relies on the availability of specific additional information to relax the assumption of perfect frame specification. This variant is applied to two remote Indigenous communities in the Northern Territory of Australia, using locally available data sources. This paper describes the emergence of a formal partnership between Manitoba First Nations and researchers in the Department of Community Health Sciences at the University of Manitoba. This partnership reflects two decades of a working relationship in Manitoba involving university researchers and First Nations communities, as well as new and innovative approaches to developing organizations, training initiatives and projects that strengthen First Nations principles of governance. The emerging trust that has developed between the Manitoba First Nations and the University has made it possible to extend this partnership into building further research capacity and evidence-based decision-making among First Nations. Discussions between the Assembly of Manitoba Chiefs and the Northern Health Research Unit resulted in the development of a Manitoba First Nations Centre for Aboriginal Health Research. Its mission is to initiate, coordinate and support research activities designed to assist First Nations and Aboriginal communities and organizations in their efforts to promote healing, wellness and improved health services in their communities. Much of the health research described in this journal was facilitated through this partnership, which demonstrated the value of partnerships and new funding opportunities to better address the health information needs of First Nations communities, particularly at a time when Aboriginal communities were highly skeptical of the value of academic research. Objectives: The objectives of this study were to: (1) report tuberculosis (TB) program performance for northern First Nations in the province of Manitoba; (2) present methods for TB program performance measurement using routinely collected surveillance data; and (3) advance dialogue on performance measurement of Canadian TB programs., Methods: Data on a retrospective cohort of people diagnosed with TB in Manitoba between January 1, 2008 and December 31, 2010, and their contacts, were extracted from the Manitoba TB Registry. Performance measures based on US-CDC were analyzed. Adjusted probability ratios (aPR) and 95% confidence intervals (CIs) were reported with comparisons between on-/off-reserve First Nations, adjusted for age, sex, and treatment history., Results: A cohort of n = 149 people diagnosed with TB and n = 3560 contacts were identified. Comparisons off-/on-reserve: Treatment completion (aPR = 1.03; 95% CI 0.995-1.07); early detection (aPR = 0.87; 95% CI 0.57-1.33); HIV testing and reporting (aPR = 0.42; 95% CI 0.21-0.83); pediatric TB (age < 15 years) (aPR = 1.20; 95% CI 0.47-3.06); retreatment for TB (aPR = 0.93; 95% CI 0.89-0.97); contact elicitation (aPR = 0.94; 95% CI 0.84-1.05); contact assessment (aPR = 0.69; 95% CI 0.50-0.94). Pediatric (ages < 15 years) TB incidence in northern Manitoba was 37.1 per 100,000/year., Conclusion: TB program performance varies depending on residence in a reserve or non-reserve community. Action is urgently needed to address TB program performance in terms of contact investigation and HIV testing/reporting for First Nations off-reserve and to address high rates of pediatric TB in northern Manitoba. First Nations collaboration and models of care should be considered both on- and off-reserve to improve TB program performance.

Background: Urban Indigenous populations in Canada are steadily growing and represent diverse and culturally vibrant communities. Disparities between Indigenous and non-Indigenous peoples' experiences of the social determinants of health are a growing concern. Under the guidance of the West End Aboriginal Advisory Council (WEAAC), Parkdale Queen West Community Health Centre (PQWCHC) launched the Niiwin Wendaanimak Four Winds Wellness Program that seeks to enhance health and community services for homeless and at-risk Indigenous populations in Toronto., Objectives: A process evaluation was carried out to (1) assess the collaborative service delivery model; (2) identify service gaps and issues for homeless and at-risk Indigenous populations; and (3) develop recommendations for how non-Indigenous organizations can provide culturally responsive services for Indigenous populations., Methods: In consultation with the WEAAC, a thematic analysis of qualitative data collected from 2 focus groups with community members who access the Niiwin Wendaanimak program and 17 key informant interviews with staff and peers was conducted., Results: The Niiwin Wendaanimak program bridges teachings of inclusivity and the practice of harm reduction to create a non-judgemental space where community members' dignity and autonomy is respected. Strengths of the program include Indigenous leadership and access to activities that promote wellness and community building., Conclusions: As a non-Indigenous service provider, PQWCHC is meeting the needs of homeless and at-risk Indigenous populations in Toronto. Program strengths, system gaps, and challenges including policy recommendations were identified.

Objectives: Meaningful social engagement is important to reduce risk for social isolation and loneliness. First Nations Elders are a unique group and little knowledge currently exists of their preferred forms of social interaction. The objective of this study was to describe the types of programs Nak'azdli Elders desire, identify barriers to participation, and improve creation of programs that address Elders' needs and interests., Methods: This project was co-created by the Nak'azdli Health Centre and Elders, located in Northern British Columbia, with support from academic partners when and where asked. An advisory committee selected participants perceived as able to complete the survey and available for interviewing. Participants were interviewed orally in English or Carrier in their homes or at a drop-in centre, by a well-respected Nak'azdli Elder. The Elder entered participant responses (including self-reported health, awareness and utilization for existing programs, and preferences for new programs) into a paper-based survey. Descriptive and content analysis were conducted., Results: Nak'azdli Elders (N = 38) were interested in wisdom sharing, social programs, and health-related activities. Elders wanted to be actively engaged in programs/activity selection, helping organize programs, knowledge sharing, skills, and stories. Barriers to participation included lack of transportation, personal health concerns, scheduling conflicts, and lack of knowledge about programs/activities., Conclusion: Nak'azdli Elders were interested in culturally relevant programs involving sharing cultural knowledge, teachings, and/or language with younger generations. Elders wanted to be engaged in all stages of activities, including planning, participation, and evaluation. Future programs should prioritize community collaboration and co-creation with Elders.

Objective: To examine the correlates of current smoking among Métis aged 18 years and older, with a particular focus on culturally-specific factors. Cultural factors included spirituality, knowledge of an Aboriginal language, membership in a Métis organization and attendance at Métis cultural events. Demographic, geographic, socio-economic and health-related variables were also considered., Methods: Data from 6,610 adult Métis aged 18 years and older who responded to the 2006 Aboriginal Peoples Survey and Métis supplement were used to examine the correlates of current smoking using sequential binary logistic regression modelling., Results: Overall, 39.9% of adult Métis respondents in the sample were current smokers. Adult Métis who reported a high level of spirituality were less likely to be current smokers. Those who spoke an Aboriginal language, or lived in a house where an Aboriginal language was spoken, were more likely to be current smokers. Being a member of a Métis organization and attending cultural events were not independently associated with current smoking. Métis with higher household income, greater education, higher self-perceived health, and greater physical activity participation were less likely to be current smokers, whereas those who reported heavy alcohol consumption were more likely to be current smokers., Conclusions: The results of this study suggest that interventions aimed at reducing smoking among adult Métis might be more successful if they include some connection to spirituality. It is also evident that co-occurring risk behaviours, in addition to demographic and socio-economic factors, are important considerations when developing interventions to reduce smoking among this population.

Objectives: Physical activity (PA) can help youth achieve balance among physical, mental, emotional and spiritual dimensions of health. The objective was to identify individual, family and community factors associated with PA among First Nations (FN) youth residing in on-reserve and northern FN communities., Methods: Participants were 4,837 youth (12-17 years of age) responding to the 2008/10 First Nations Regional Health Survey. Through in-person interviews, youth responded to questions about moderate-to-vigorous physical activity (MVPA), participation in traditional physical activities, and individuallevel, family and community factors., Results: When averaged across all days of the year, 65% of FN youth accumulated at least 60 min/day of MVPA and 48% of youth participated in at least one traditional FN PA in the previous year. Being male, having a lower number of chronic conditions, living in balance physically, living with at least one biological parent, having more relatives help youth understand their culture, having more community challenges and having more leisure/recreation facilities were independently associated with an increased likelihood of accumulating ≥ 60 min of MVPA. Younger age, being male, knowledge and use of FN language, living in balance spiritually, living with at least one biological parent, having more relatives help youth understand their culture, living in a community of ≤ 300 people, and perceiving the natural environment and community health programs as strengths were independently associated with participation in traditional FN physical activities., Conclusion: There are several correlates of PA from diverse ecological levels among FN youth.

Objectives: Physical activity has numerous mental, emotional, spiritual and physical benefits. The factors influencing physical activity among First Nations children have not been well studied. The objective was to examine the associations between several intrapersonal, family and community factors and physical activity among First Nations school-aged children residing in First Nations communities., Methods: Participants consisted of 3,184 children (6-11 years old) from the 2008/10 First Nations Regional Health Survey, a representative sample of First Nations persons who reside in on-reserve and northern First Nations communities. The survey addresses a holistic range of health issues. Primary caregivers completed interviews to assess each child's moderate-to-vigorous physical activity (MVPA), participation in traditional physical activities, six intrapersonal factors, four family factors and two community factors., Results: Based on primary caregiver reports, 72% of children accumulated a daily average of ≥60 minutes of MVPA and 54% participated in at least one traditional First Nations physical activity in the past year. Older age, having more people in the household, and having more relatives help the child understand their culture were independently associated with accumulating ≥60 minutes of MVPA. School attendance, use of First Nations language, having parents with a high school education, smaller community size, and having more community members help the child understand their culture were independently associated with participation in traditional First Nations physical activities., Conclusion: Among First Nations children, there are several correlates of physical activity from diverse ecological levels.

Objectives: Motor vehicle crashes (MVCs) are a leading cause of death for Canadian Aboriginal peoples; developing effective interventions should be a public health priority. While intervention research has been conducted outside of Canada, few formal program evaluations have been conducted in Canada. We reviewed Canadian and non-Canadian Indigenous road safety initiatives to inform future program development in Canada., Methods: A systematic review of the published and grey literature examining MVC intervention programs in Indigenous communities was performed. Studies published after 1980 reporting pre-post comparisons of MVC interventions in Indigenous communities were included in the review. These studies were assessed using a modified Participatory Action Research quality assessment tool. Haddon's Matrix of injury epidemiology and prevention was used to categorize crash-related risk factors targeted in the MVC interventions., Synthesis: A total of 11 studies met inclusion criteria, including 1 Canadian study and 10 non-Canadian studies. Successful intervention components included focus groups, training community members, educational activities, distribution of safety devices, collaboration with local law officials to enhance enforcement, driver-licensing courses, and incentive programs. Potential barriers to successful implementation and evaluation involved lack of incorporation of cultural and contextual factors, enforcement factors, and methodological limitations., Conclusion: Several effective strategies to reduce MVCs can be adapted and implemented at the community and national levels. Future directions might include using multiple intervention components and incorporating a collaborative, culturally and contextually appropriate approach, while promoting evaluation initiatives and widespread dissemination of findings.

Objective: To explore Inuit family understandings of sexual health and relationships in order to inform responsive public health interventions that are designed to meet the needs of Nunavummiut., Method: A qualitative indigenous knowledge approach was used for this study with a focus on Inuit epistemology and methodology, as described in the Piliriqatigiinniq Community Health Research Partnership Model. Interviews were conducted with 20 parents in three Nunavut communities in 2011. An immersion and crystallization analytical approach was used to analyze the data and to identify groupings or themes in the data. The stories shared by parents are honoured, keeping their words intact as often as possible in the presentation of results., Results: Parents in this study largely discussed sexual health in the context of historical community events related to settlement and/or residential schools. Residential schools and forced settlement into communities were linked to trauma, family separation, hardship and grief. These experiences were prominent in participants' understandings of sexual health and perceptions of sexual health behaviours among youth in the community., Conclusion: This study highlights the complexity of the landscape of sexual health in Nunavut and the need for public health approaches that are inclusive of Inuit family perspectives on sexual health. Greater understanding of historical and community context can contribute to the development of pertinent, evidence-based public health interventions that will meet the needs of the population.

Objectives: To examine the socio-economic, behavioural and Métis-specific factors that predict obesity among Métis children aged 6 to 14 years. Socio-economic factors included household structure and income, parental education and food insecurity. Cultural factors included knowledge of an Aboriginal language, participation in cultural activities, time spent with Elders and parental residential school attendance., Methods: The 2006 Aboriginal Peoples Survey, Children and Youth component collected data about Métis children, including child height and weight, reported by the person most knowledgeable about the child (PMK). Multivariate binary logistic regression was used to predict obesity, defined using IOTF BMI cut-offs. After testing for interactions, models were stratified by age (6-10, 11-14) and gender., Results: An estimated 18.5% of Métis boys and 14.4% of girls were obese. The effects of socio-economic factors and region varied across age and gender groups, although living in a lone-parent family and rural residence had consistent effects. Many effects of cultural variables were unexpected. Although PMK residential schooling was positively associated with obesity generally, the effects were negative among older girls. As expected, children participating in frequent physical activity generally had lower risk, independent of other factors., Conclusions: Although socio-economic factors are related to risk of obesity among Métis children, the effects may not be the same across age groups and for boys and girls. There is some evidence of independent effects of Métis-specific cultural factors, including parental residential schooling, on the risk of child obesity, but further investigation and better data are needed to understand these relationships.
