Background and objectives: The modified Rankin Scale (mRS) is one of the most frequently used outcome measures in trials in patients with an aneurysmal subarachnoid hemorrhage (aSAH). The assessment method of the mRS is often not clearly described in trials, while the method used might influence the mRS score. The aim of this study is to evaluate the inter-method reliability of different assessment methods of the mRS. Methods: This is a prospective, randomized, multicenter study with follow-up at 6 weeks and 6 months. Patients aged ≥ 18 years with aSAH were randomized to either a structured interview or a self-assessment of the mRS. Patients were seen by a physician who assigned an mRS score, followed by either the structured interview or the self-assessment. Inter-method reliability was assessed with the quadratic weighted kappa score and percentage of agreement. Assessment of feasibility of the self-assessment was done by a feasibility questionnaire. Results: The quadratic weighted kappa was 0.60 between the assessment of the physician and structured interview and 0.56 between assessment of the physician and self-assessment. Percentage agreement was, respectively, 50.8 and 19.6%. The assessment of the mRS through a structured interview and by self-assessment resulted in systematically higher mRS scores than the mRS scored by the physician. Self-assessment of the mRS was proven feasible. Discussion: The mRS scores obtained with different assessment methods differ significantly. The agreement between the scores is low, although the reliability between the assessment methods is good. This should be considered when using the mRS in clinical trials. Analysis of food or feed samples by DNA-based methods first needs a sample preparation before DNA extraction. Due to the large variety of types of food and feed samples, methods must be adapted to the matrices encountered. Within an accreditation scheme, auditors frequently require a clear decision system enabling the technical staff to determine which method has to be used to perform the sample preparation and the DNA extraction. In this paper, we present an identification key that easily determines the type of matrix and the subsequent operations. The method presented is used in the framework of GMO analysis but could be extended to a wide variety of other DNA-based methods applied to food and feed products. [ABSTRACT FROM AUTHOR]

Spontaneous coronary artery dissection (SCAD) represents around 25% of cases of acute coronary syndromes (ACS) in women aged 40–65 years who have few or no traditional cardiovascular risk factors. It is assumed that the incidence is underestimated, as the angiographic appearance of SCAD may often mimic atherosclerosis. This review aims to examine SCAD by focusing on the associated predisposing factors and precipitating stressors in this heterogeneous patient population, as well as the best treatment approach and the prognosis. Progressive knowledge has improved our current understanding of SCAD, but more awareness among clinicians is necessary. The Picard spacecraft was successfully launched on 15 June 2010, into a Sun-synchronous orbit. The mission represents one of the European contributions to solar observations and Essential Climate Variables (ECVs) measurements. The payload is composed of a Solar Diameter Imager and Surface Mapper (SODISM) and two radiometers: SOlar VAriability Picard (SOVAP) and PREcision MOnitor Sensor (PREMOS). SOVAP, a dual side-by-side cavity radiometer, measures the total solar irradiance (TSI). It is the sixth of a series of differential absolute-radiometer-type instruments developed and operated in space by the Royal Meteorological Institute of Belgium. The measurements of SOVAP in the summer of 2010 yielded a TSI value of 1362.1 W m with an uncertainty of ± 2.4 W m ( k=1). During the periods of November 2010 and January 2013, the amplitude of the changes in TSI has been on the order of 0.18 %, corresponding to a range of about 2.4 W m. [ABSTRACT FROM AUTHOR]

In humans, ten Toll-like receptor (TLR) paralogues sense molecular components of microbes, initiating the production of cytokine mediators that create the inflammatory response. Using N-ethyl-N-nitrosourea, we induced a germline mutation called Lps2, which abolishes cytokine responses to double-stranded RNA and severely impairs responses to the endotoxin lipopolysaccharide (LPS), indicating that TLR3 and TLR4 might share a specific, proximal transducer. Here we identify the Lps2 mutation: a distal frameshift error in a Toll/interleukin-1 receptor/resistance (TIR) adaptor protein known as Trif or Ticam-1. TrifLps2 homozygotes are markedly resistant to the toxic effects of LPS, and are hypersusceptible to mouse cytomegalovirus, failing to produce type I interferons when infected. Compound homozygosity for mutations at Trif and MyD88 (a cytoplasmic TIR-domain-containing adaptor protein) loci ablates all responses to LPS, indicating that only two signalling pathways emanate from the LPS receptor. However, a Trif-independent cell population is detectable when TrifLps2 mutant macrophages are stimulated with LPS. This reveals that an alternative MyD88-dependent ‘adaptor X’ pathway is present in some, but not all, macrophages, and implies afferent immune specialization. [ABSTRACT FROM AUTHOR]

Procedures were developed for the speciation of trace amounts of aluminium present in percolating water of forest soil by online coupling of different chromatographic separation methods to an ICP-MS detection system. Inorganic and organic aluminium species were fractionated on a cation exchange column IONPAC CG12 (10-32). Phytotoxic polymeric aluminium hydroxides, as e.g. Al13 (AlO4Al12(OH)24(H2O)12 7+), were determined using pyrocatechol violet (PCV) as a species dependant complexing reagent prior to the cation exchange step. Size fractionation of the organic aluminium species was obtained by size exclusion chromatography using the columns Superdex-75-HR 10/30 and Superdex-Peptide-HR 10/30. Validation of the speciation procedures proved that online coupling HPLC to the element selective and sensitive ICP-MS detection system leads to low detection limits of 0.3–0.6 μg/L and high precision and reproducibility (1.2–3.5%) of the speciation procedures. High-pressure digestion and a closed-vessel microwave heated system, both employing a mixture of nitric acid and hydrogen peroxide as digesting agent, were tested for decomposing the certified samples of BCR 278 mussel tissue ( Mytilus edulis) and of BCR 422 cod muscle to determine arsenic by use of FI-HG-AAS. While the microwave system is insufficient to mineralize arsenic in marine samples (arsenic recoveries of 13±10% in BCR 278, 2±1% in BCR 422; n=4), high-pressure ashing at 300 °C results in recovery percentages of 56±15% (n=4) in mussel tissue (BCR 278) and of 25±10% (n=4) in cod muscle (BCR 422). A dry ashing procedure is given as a reference digestion, yielding complete recoveries of arsenic for both materials. The nitrite interference arising during measurement can be entirely overcome by using an amino sulfuric acid concentration of about 350 mmol/L in the solutions for measurement. The Neotropical species Pachycondyla marginata conducts well-organized predatory raids on the termite species Neocapritermes opacus and frequently emigrates to new nest sites. During both activities the ants employ chemical trail communication. The trail pheromone orginates from the pygidial gland. Among the substances identified in the pygidial gland secretions, only citronellal was effective as a trail pheromone. Isopulegol elicited an increase in locomotory activity in the ants and may function as a synergist recruitment signal. The major human erythrocyte membrane (MN-) sialoglycoprotein was purified from MM, MN, and NN cells using detergent gel and ion exchange chromatography. N-terminal analyses with dansyl-chloride revealed serine in preparations from MM and leucine in those from NN erythrocytes, whereas glycoprotein isolated from MN cells contained both the above amino acids. These data strongly suggest that the above residues may represent the structural difference between the M and N antigens. Evidence was also obtained that the Ss-glycoprotein, which is associated with 'N' activity, exhibits the same N-terminal amino acid (leucine) as the MN glycoprotein from MN cells. It is shown by discontinuous sodium dodecylsulfate (SDS) polyacrylamide gel electrophoresis of human red cell membranes, followed by periodic acid Schiff (PAS) staining and densitometry, that the band PAS-3 (monomeric Ss glycoprotein) exhibits a polymorphism with respect to its staining intensity. In membranes of the genotype SS the staining intensity of this band is about 1.5 times higher than in ss membranes. The experimental error of the method does not permit one to decide, whether membranes of blood type Ss exhibit an intermediate staining intensity or not. The SDS electrophoretic molecular weight of PAS-3 is the same for SS, Ss, or ss membranes. The ratio of PAS to coomassie blue staining intensities, which are a measure of sialic acid and protein content, respectively, are equal for PAS-3 in glycoprotein preparations from SS and ss erythrocytes. Psoriatic arthritis is an inflammatory arthropathy that ultimately can lead to joint destruction. In this study, we investigated the immunophenotype of the inflammatory cells and the expression of interleukin-8 (IL-8), which is the hallmark chemoattractant cytokine of psoriasis in synovial membranes from patients exhibiting active psoriatic synovitis ( n=9). The tissue samples were examined by immunohistochemistry, Western blot analysis and in situ hybridisation. The inflammatory infiltrate consisted predominantly of CD3+ T lymphocytes, with a higher proportion of CD4+ than CD8+ T lymphocytes in six cases. CD3+ T lymphocytes were focally distributed near small blood vessels and the enlarged synovial intima. CD1+ interdigitating reticulum cells were not detected. CD22+ B lymphocytes and plasma cells were found in small aggregates without KiM4+ follicular dendritic cells. KiM8+ macrophages were located in the synovial intima and were distributed in a diffuse pattern near the synovial lining cells. CD15+ neutrophil granulocytes were detected in four cases. They were preferentially located in the vicinity of blood vessels and the synovial intima. IL-8 was found at a high level in the synovial lining cells and to a lesser extent in cells located in the perivascular areas. Immunofluorescence double staining showed IL-8 to be expressed in KiM8+ multinucleated giant cells, KiM8+ macrophages and CD3+ T lymphocytes. IL-8 receptor A was demonstrated in the synovial lining and in macrophages and lymphocytes. IL-8 was detected by immunoblot analysis of the synovial tissue at 8.4 kD. Employing in situ hybridisation, IL-8 mRNA was strongly and preferentially expressed in the synovial intima, as well as in macrophages and lymphocytes. The immunophenotype of the psoriatic arthritis inflammatory cells shows great similarity to the inflammatory infiltrate found in the synovial tissue of patients with rheumatoid arthritis. The preferential expression of IL-8 and IL-8 mRNA in the enlarged synovial intima and in lymphocytes and macrophages suggests that IL-8 exerts its action through activated mononuclear cells and T lymphocytes. It seems to play a role in regulating leucocyte traffic into the enlarged synovial intima and may contribute to the aggressive synovitis of patients with psoriatic arthritis. [ABSTRACT FROM AUTHOR]

This study compares the rebreathing characteristics of the Bain modification of the Mapleson 'D' type of T-piece circuit with those of the Mera F system which is used with the standard 'circle' anaesthetic machine. Six healthy adults anaesthetized with halothane were studied breathing spontaneously. The volume of inspired carbon dioxide was measured on each breath as a measure of rebreathing. The tidal volume (Vt) frequency of respiration (f) and blood Pco were also noted. These measurements were made initially with either the BAIN or the Mera F system and then changed to the alternate circuit for further studies. All measurements were made with a fresh gas flow rate (FGF) of 100ml · kg· min which is recommended with the Bain system. The inspired volume of carbon dioxide (rebreathing) with the Bain system was significantly greater than when the mera F was used. Although the mean blood Pco was not significantly lower when the mera F was used, some patients who cannot adequately compensate for this inspired carbon dioxide volume did become hypercapneic (maximum 8kPa [60torr]). This hypercapnia could be reduced by using a mera F system. The mera F is a co-axial system that combines the convenience of the tube-in-a-tube structure with the beneficial effects of controlled rebreathing during controlled ventilation. In these advantages it is no different from the Bain system. The mera F however, has the advantage of being adaptable to the commonly used 'circle' anaesthetic machines for spontaneous respiration in adults. This eliminates the rebreathing of carbon dioxide at a fresh gas flow of 100ml · kg· min, which occurs in adults during spontaneous respiration. The only disadvantage of the mera F system that we used in adults was its length (90cm). However, from a functional viewpoint, it can be lengthened without altering the rebreathing characteristics of the system. Cette étude compare les caractéristiques sur le rebreathing de la modification par Bain du circuit Mapelson D avec le mera F utilisé avec le système circulaire sur l'appareil d'anesthésie. Six adultes en bonne santé anesthésiés à l'halothane ont été étudiés en respiration spontanée. L'importance du rebreathing a été evalue par la mesure du volume de gaz carbonique a chaque respiration. Le volume courant, la frequence respiratoire et la Paco ont aussi été notes. Les études ont été effectuees d'abord avec le Bain ou le systeme mera F et on est ensuite passe a l'autre circuit pour étude subsequente. Toutes ces mesures ont été faite avec un debit de gaz frais de 100ml · kg· min tel que recommande pour le systeme Bain. Le volume de gaz carbonique inspiré (rebreathing) avec le Bain a été plus eleve de façon significative qu'avec le Mera F. Bien que la Pco moyenne n'ait pas été plus basse de façon significative lorsqu'on employait le Mera F, certains patients qui ne pouvaient pas compenser adéquatement pour le volume de gaz carbonique inspiré sont devenus hypercapniques (maximum 8 kPa ou 60 torr). Cette hypercapnie n'est pas survenue lors de l'emploi du système Mera F. Le Mera F est un circuit co-axial qui combine les avantages du conduit d'alimentation cheminant à l'interieuret d'un rebreathing accessible pendant la ventilation contrôlée. Par ces caractéristiques il ne diffère pas du circuit Bain. Le systeme mera F a cependent cet avantage de pouvoir s'adapter aux systèmes circulaires communément employe sur nos appareils d'anesthésie en respiration spontanee chez les adultes. Ceci élimine la réinspiration du gaz carbonique qui survient chez les adultes en respiration spontanée même lorsque le debit d'alimentation de gaz frais est de 100 ml · kg. Single-strand conformational polymorphisms (SSCP) of the connexin32 gene were analyzed in 121 patients possibly affected by Charcot-Marie-Tooth (CMT) disease. The 121 patients were selected from 443 possible CMT/HNPP (hereditary neuropathy with liability to pressure palsies) patients based on genetic linkage to Xq13.1, absence of the 17p12 duplication and deletion, and absence of point mutations in PMP22 and P0. We found five new mutations at nucleotides 105 (C-T), 316 (C-G), 321 (C-T), 328 (T-C), and 657 (G-C), and three mutations at nucleotide 126 (C-T), 249 (G-A), and 477 (G-A) previously described in other unrelated families. The African stink ants ( Pachycondyla tarsata) lay recruitment trails with secretions from sternal glands. The glandular secretions consist of 10 compounds, 9 of which have been chemically identified.
