Introduction: The incidence of recurrence of OKC varied from 2.5 to 62%. Studies have linked recurrence to treatment methods and also clinical and pathological features. The aim of this study was to evaluate the 5-year recurrence and the factors associated with recurrence in odontogenic keratocysts of the jaws. Methods: A retrospective review of records was done from the Institute's Medical Records Directory from 2010 to 2021. The following data were obtained of the lesion; age at presentation, gender, site, subsite, radiographic presentation (locularity), radiographic borders, presence or absence of satellite cysts, inflammatory infiltrate, and treatment rendered presence or absence of cortical perforation and soft-tissue extension and presence or absence of recurrence. Kaplan Meir estimator was used to evaluate recurrence rate and log rank test was used to compare the survival amongst groups. Cox regression analysis was used to evaluate the odds ratio to find out the possible factors influencing risk of recurrence. A p value of < 0.05 was considered statistically significant at 95% confidence interval. Results: In our study cohort, 27.2% of patients had recurrence. Posterior maxillary lesions, multilocular lesions, lesions with scalloped borders, presence of soft-tissue extension and cortical perforation, presence of satellite cysts and inflammatory infiltrate and enucleation with peripheral ostectomy were significantly associated with recurrence. However, soft-tissue extension, cortical perforation, multilocular lesions and presence of satellite cysts were independent risk factors. Gorlin–Goltz syndrome (GGS) is a rare hereditary disease characterized by multiple basal cell carcinomas, odontogenic keratocyst (OKCs) and musculoskeletal malformations. Pathogenesis of the syndrome is attributed to abnormalities in the long arm of chromosome 9 (q22.3–q31) and mutations in the human patched gene (PTCH1 gene). Here, we report a rare case of an incidental finding of GGS in an 18-year-old male patient presenting multiple OKCs, calcification of the falx cerebri, and bifid rib. [ABSTRACT FROM AUTHOR]

Background: Keratoameloblastoma is a poorly characterized and rarely reported odontogenic neoplasm that can exhibit overlapping histopathologic features with conventional ameloblastoma and keratocystic odontogenic tumor (KCOT), with an ambiguous relationship to the so-called solid KCOT. Methods: A peripheral maxillary tumor causing bone saucerization in a 54-year-old male is described and investigated with immunohistochemistry and Next-Generation Sequencing (NGS). Results: Microscopically, the tumor comprised of a predominantly plexiform proliferation of odontogenic epithelium with central keratinization and evidence of surface origin. Peripheral cells exhibited nuclear palisading with variable reverse polarization, while stellate reticulum-like areas were observed internally. A few follicles and a few foci in the lining of cystic spaces revealed increased cellularity with cells exhibiting small but conspicuous nucleoli, focal nuclear hyperchromatism, and a few mitoses mostly seen in the peripheral outer cell layer. Nuclear staining for ki-67 was increased in those areas when compared with the other cystic, follicular, and plexiform areas. These features were interpreted as cytologic atypia suggesting also the possibility of a malignant process. Immunohistochemically, the tumor was positive for CK19 and negative for BRAF VE1, calretinin, and CD56. Ber-Ep4 was only focally positive. By sequencing, an ARID1A c.6527_6538delAG frameshift mutation (VAF: 5.8%), classified as likely oncogenic, and an FBXW7 c.1627 A > G missense mutation (VAF: 8.0%), classified as a variant of uncertain significance, were detected. Two mutations, probably germline (VAF ~ 50%), were recorded for RNF43 and FBXW7. No pathogenic variants were identified in PTCH1, BRAF, NRAS, HRAS, KRAS, FGFR2, or SMO genes. Conclusion: The significance of an ARID1A variant in keratoameloblastoma is uncertain since this variant has not been reported in ameloblastoma or KCOT, to date. Alternatively, it may characterize malignant transformation in the present case since ARID1A mutations have been encountered in various cancers. The effective management of odontogenic keratocyst (OKC) remains a subject of interest and confusion in the oral and maxillofacial surgery literature. Currently, there is a lack of consensus regarding the most appropriate treatment for patients with OKC. Of the various treatment options available, no modality to date has been shown to demonstrate a zero or near-zero recurrence rates except wide resection with clear margins. With the prevailing dearth of evidence based surgical protocols for the management of patients with OKC in the literature, this study aims to present a surgical algorithm, based on meta-analysis results, that hopefully will be beneficial in enhancing treatment of patients with this condition. Also, new meta-analysis was done to compare between modified Carnoy's solution (MCS) and 5-fluorouracil (5-FU) in respect of recurrence rate of OKC. Using parameters like size, lesion type (primary or secondary), syndromic or solitary nature of the lesion, presence of cortical perforations, and locularity; we present a decision tree, to aid treatment planning and help attain the least chance of recurrence in the management of the OKC. There was very low-quality evidence indicating that application of 5-FU, after enucleation and peripheral ostectomy of OKCs, significantly lowered recurrence rate when compared to MCS (RR = 0.087, CI: 0.017 to 0.436, P value = 0.003). Background: Aggressive benign odontogenic neoplasms are a group of lesions that have the potential to grow to an enormous size resulting in bony deformities, locally aggressive behavior and have high recurrence rate. Ameloblastomas, keratocystic odontogenic tumors, etc., are included under this group of lesions. Treatment of these lesions is challenging owing to their particular characteristics including high recurrence rate, locally aggressive behavior, etc. Materials and Methods: A retrospective study was conducted in 40 patients who had undergone enucleation and open packing as treatment for benign aggressive neoplasms in the department of oral and maxillofacial surgery. Results: In the study, all the cases were followed up for a minimum of 5 years clinically and radiographically. Satisfactory bone healing was noted with no evidence of recurrence in all cases. Large benign odontogenic neoplasms of mandible are not rare in developing countries such as India. Treatment of choice depends not only on extent and size of the lesion but also the socio-economic status, resources and available armamentarium. Whenever resection of segment mandible is planned for these patients, goal of the treatment should not be to restore function of the mandible alone but also to give esthetic visual appearance of the face. The present study was performed to determine postoperative functional and esthetic outcome in such patients. A total of 18 patients (20–35 years) with benign odontogenic neoplasm of mandible were enrolled for the study. After surgical intervention i.e., resection and reconstruction of mandible with mandibular reconstruction plate, all the patients were evaluated post-operatively for functional and esthetic outcome at the end of 1st and 4th week. Functional outcome were assessed based on Quality of life questionnaire and esthetic outcome based on vancouver scar assessment scale, clinical and radiological assessment. The mean scores of all the functional outcomes was improved significantly like pain, drooling of saliva, eating solid and liquid and speech except deglutition. The mean score of scar was recorded as 4.67. Occlusion was achieved in 100% and lips competency in 89% of patients. It is advised to immediately reconstruct the mandible after segmental mandibulectomy which eventually helps to improve the quality of life post-operatively of patients being treated for benign odontogenic neoplasm of mandible [ABSTRACT FROM AUTHOR]

Purpose: The aim of this study was to conduct epidemiologic investigations on the pattern of the lesion and differences between treatment modalities in terms of recurrence by reviewing follow-up records to form a basis for planning patient follow-up visits. Materials and methods: In this retrospective, single-center cohort study, 266 patients diagnosed with odontogenic keratocyst between 1993 and 2013 were included. Medical records and radiographic images were analyzed for age distribution, occurrence site and size, treatment modalities, and recurrence. Results: The average age at first diagnosis was 33.1 years, and the male to female ratio was 1.33:1.00. The highest rate of incidence was in the third decade followed by the fourth, second, and fifth decades. The incidence in the maxilla was 34%, and 66% in the mandible. Mandibular ramus was most commonly involved. Lesions between 3 and 6 crowns were the most common, and the rate of recurrence increased with size. Enucleation after decompression had higher rate of recurrence (35.8%) than enucleation (27.1%), but there was no statistical significance. Conclusion: The recurrence of odontogenic kerotocyst (OKC) was significantly associated with large size, multilocular form, and surgical procedure. Purpose: This study was conducted in order to gain insight in the actual ratio of odontogenic keratocysts occurring in the tooth-bearing area as compared to the posterior region of the jaws in order to come up with reliable data to base upon a rational treatment policy. Methods: We searched MEDLINE, Web of Science, Scopus, and Cochrane databases for studies reporting on the location of mandibular and maxillary odontogenic keratocysts. All records were independently assessed and a meta-analysis was performed. Risk difference with a confidence interval of 95% of having the lesion in the posterior region versus the tooth-bearing area was the effect measure. P value for the summary effect of < 0.05 was considered statistically significant. Results: The 2615 records retrieved were reduced to 34 studies to be qualitatively/quantitatively assessed. The pooled values showed that the difference in the clinical risk of having keratocysts in the posterior region of the mandible and in the tooth-bearing area of the maxilla is 21 and 43%, respectively (P < 0.02 and P < 0.00001). The paper is a systematic review of using the pharmaceutical substance, Carnoy's solution, to reduce the number of recurrences of such tumors as keratocystic odontogenic tumor and ameloblastoma. These are benign yet aggressive jaw tumors with a high percentage of recurrences (up to 60–80%). Surgical treatment of the above-described lesions can be performed using: (1) marsupialization (recurrence rate up to 24.8%); (2) enucleation with or without adjunct therapy; therapy may include cryotherapy with liquid nitrogen (recurrence rate up to 23.1%), peripheral ostectomy (recurrence rate up to 20%), and bone cavity treatment with Carnoy's solution (recurrence rate up to 6.6%); (3) marginal/segmental resection of the jaw (recurrence rate up to 2.5%). Despite the low percentage of recurrences when using marginal/segmental resection, this method requires a longer and more expensive anesthetic and surgical intervention as well as longer postoperative and rehabilitation periods. With the use of Carnoy's solution, it became possible to reduce the risks of recurrence of keratocystic odontogenic tumors and ameloblastomas from 60–80 to 6.6‒11.5%. Worldwide today, not only Carnoy's solution recommended by Cutler and Zollinger in 1933 is applied but also its modifications that are not inferior to the original substance in terms of removal efficiency of the remaining cystic cells. The safety of the use of the solution near the vessels and nerves is confirmed by works of a number of authors. It is concluded that, despite the large number of surgical treatments of both keratocystic odontogenic tumors and ameloblastomas, enucleation with the use of Carnoy's solution is the optimal method of choice for these diseases. Objectives: Odontogenic keratocyst (OKC) presents considerable variation in aggressiveness and propensity for recurrence, yet hitherto, no explicit clinicopathological features have been determined to clearly demonstrate the potential for relapse. This retrospective study aims to investigate the prognostic relevance of various clinicopathological features as well as immunoexpression of COX-2, bcl-2, PCNA, and p53 in sporadic OKC.Materials and methods: Among 41 patients with OKC treated by enucleation, the frequency of recurrence for various clinicopathological features as well as immunoexpression for COX-2, bcl-2, PCNA, and p53 was evaluated.Results: The mean follow-up was 8.49 years, and recurrences were ascertained in 29.27% of cases. We found significant differences between recurrent and non-recurrent cysts in terms of multilocularity (P = 0.029), cortical perforation (P = 0.001), and lesion size (P < 0.001). Hazard risk for the recurrence was 3.362 (95% CI 1.066-10.598) for multilocular cysts, 7.801 (95% CI 2.1-28.985) for evidence of cortical perforation, and 1.004 (1.002-1.006) for 1 mm2 of lesion size on panoramic radiographs. We also found that immunoexpression of PCNA significantly correlates with the radiographic evidence of cortical perforation (P = 0.048) and that there is significant positive correlation between expression of COX-2 and bcl-2 (P = 0.001) as well as significant negative correlation between immunoexpression of COX-2 and age (P = 0.002). None of the other analyzed factors were associated with the recurrence.Conclusions: Larger size, multilocularity, and cortical perforation in sporadic OKC may be correlated with the relapse.Clinical relevance: Immunohistochemical analyses of COX-2, bcl-2, PCNA, and p53 lack prognostic utility in sporadic OKC. Gardner syndrome (GS) is caused by mutations in the APC and besides adenomatous colorectal polyps includes such manifestations as osteomas, epidermoid cysts (ECs) and occasionally multiple pilomatricomas. More than 50 % of ECs in patients with GS exhibit pilomatricoma-like ghost cell keratinization. The latter may be explained by the fact that the development of both GS and pilomatricoma is driven by activation of the Wnt/β-catenin signaling pathway. A 62-year-old, Caucasian male with history of GS presented with a unilocular, mixed radiopaque/radiolucent mandibular lesion causing divergence and external root resorption of involved teeth. Histopathologically, the lesion was composed of two cystic components, an orthokeratinized odontogenic cyst (OOC) and a smaller one with characteristics of keratocystic odontogenic tumor (KCOT) featuring, focally, ghost cells and an epithelial morule-like structure. Dystrophic calcifications essentially similar to those seen in pilomatricomas were observed in the fibrous connective tissue wall. The KCOT and OOC epithelia revealed strong and diffuse cytokeratin (AE1/AE3) and β-catenin immunoreactivity. CD10 positive immunostaining was seen in the keratin and superficial spinous cell layers in both OOC and KCOT. The intraepithelial and mural ghost cells showed a cytokeratin (+), β-catenin and CD10 (−) immunophenotype. The diagnosis of OOC with ghost cell calcifications in association with KCOT was rendered. The patient was lost to follow-up. Although a coincidental co-existence cannot be excluded, ghost cell calcifications mimicking pilomatricoma-like changes in an unusual odontogenic cyst combining OOC and KCOT features as seen in this patient with GS may be explained by the common molecular mechanisms underlying the pathogenesis of cutaneous pilomatricomas and GS. Nevoid basal cell carcinoma syndrome, or basal cell nevus syndrome (Gorlin syndrome), is a rare autosomal dominantly inherited disorder that is characterized by development of basal cell carcinomas from a young age. Other distinguishing clinical features are seen in a majority of patients, and include keratocystic odontogenic tumors (formerly odontogenic keratocysts) as well as dyskeratotic palmar and plantar pitting. A range of skeletal and other developmental abnormalities are also often seen. The disorder is caused by defects in hedgehog signaling which result in constitutive pathway activity and tumor cell proliferation. As sporadic basal cell carcinomas also commonly harbor hedgehog pathway aberrations, therapeutic agents targeting key signaling constituents have been developed and tested against advanced sporadically occurring tumors or syndromic disease, leading in 2013 to FDA approval of the first hedgehog pathway-targeted small molecule, vismodegib. Nevoid basal cell carcinoma syndrome, a rare autosomal dominant disorder, comprises of a number of abnormalities such as multiple nevoid basal cell carcinomas, skeletal abnormalities and multiple keratocystic odontogenic tumors. Diagnosis may be difficult because of the variability of expressivity and different ages of onset for different traits of this disorder. The dental clinician may be the first to encounter and identify this syndrome, when the multiple cysts like radiolucencies are discovered on panoramic view. The epithelial lining of odontogenic keratocysts exhibits either parakeratosis or orthokeratosis. In 2005, the WHO classified odontogenic keratocysts with parakeratosis as keratocystic odontogenic tumors (KCOT). Odontogenic keratocysts with orthokeratosis were not classified as odontogenic tumors, but instead referred to as orthokeratinized odontogenic cysts (OOC). To clarify the difference between these two lesions, we investigated their biological characteristics using immunohistochemical studies for cytokeratins (CK) in KCOT and OOC as well as in dentigerous cysts (DC), radicular cysts (RC) and dermoid cysts (DMC). We examined twenty-five cases of KCOT, fifteen cases each of OOC, DC and RC, and ten cases of DMC. We studied the immunohistochemical expression of CK10, 13, 17 and 19. To evaluate the immunohistochemical staining pattern, we divided the epithelial lining of the lesions into three layers (surface layer: su, spinous layer: sp, basal layer: ba). For CK10, most OOC and DMC specimens of su and sp were positive. For CK13 and 19, most KCOT, DC and RC specimens of su and sp were positive. For CK17, most KCOT specimens of su and sp were positive. The percentages of total CK expression of su and sp, and ba of CK19 differed significantly between the lesions ( P < 0.001). Objectives: It is known that the magnetic resonance (MR) imaging features of dentigerous cysts (DCs) are the same as those of other cysts. The cavities of DCs generally show low signal intensity (SI) on T1-weighted imaging (T1WI). However, in our experience, DCs often show high SI on T1WI, rendering them difficult to distinguish from keratocystic odontogenic tumors (KCOTs). In this study, we evaluated seven cases of DC and 23 cases of KCOT to assess whether MR imaging can provide additional information for the differential diagnosis of DC and KCOT. Methods: Two trials were evaluated. The first trial was an analysis of the cystic cavity SI on T1WI. To compare the SI between patients, we performed T1WI with contrast (T1c). The second trial analyzed the SI uniformity ( U) on T1WI, T2-weighted imaging (T2WI), and short inversion time inversion recovery (STIR) images of the cystic cavity, because it has been reported that an MR imaging feature of KCOT is heterogeneous SI. Statistical analyses were performed using the Mann-Whitney U test and a receiver operating characteristic (ROC) curve. Results: The T1c values of DCs were significantly higher than those for KCOTs. The U values on T1WI and STIR images were significantly higher for DCs than for KCOTs. The T1c cutoff values (0.66 and 0.84) and U cutoff values for T1WI (82.1 %) and STIR images (79.8 %) were determined from the ROC analyses. Background: Keratocystic odontogenic tumor (KCOT) is one of the most common odontogenic neoplasms. There are many kinds of treatment modalities which can be a conservative method, such as enucleation or an aggressive method, such as enucleation followed by cryosurgery and radical surgical tactics with bone resection. In the literature a relatively high recurrence rate reported for KCOT with a range of 5-70 % depending on the method of treatment. Case presentation: The aim of this case report is to present the prosthetic rehabilitation of an 18-year-old male patient who had previously been operated on for a KCOT. Enucleation was preferred due to the size of the lesion. Even though a recurrence was observed 1 year postoperatively, a less extensive procedure was sufficient for the treatment. The patient was successfully rehabilitated with an implant after a follow up period of 19 months following the second operation. Conclusion: Prosthetic rehabilitation of a single tooth implant after enucleation of KCOT is an optimal treatment option. No temporization was needed due to the patient preferences. After waiting for a 3 month period of healing phase, the gingiva former was fixed. After 1 week an impression was made and the crown was cemented. The restoration was clinically successful in the 3 month, 6 month and 1 year of follow-up. [ABSTRACT FROM AUTHOR]

Background: A keratocystic odontogenic tumor (KCOT) is a benign destructive recurrent odontogenic cystic neoplasm. The microRNAs (miRNAs) miR-15a and miR-16-1 function as negative regulators of the anti-apoptotic gene BCL2 at the post-transcriptional level. Notably, high Bcl-2 immunoexpression is found in the epithelial lining of KCOTs, while the loss of Bcl-2 immunopositive cells is observed in marsupialized cysts. The purpose of this study was to investigate whether the transcription of miR-15a and miR-16-1 is altered in KCOTs and whether it is associated with BCL2 gene expression in such lesions. Methods: Using qRT-PCR and immunohistochemical analyses, we examined miR-15a/16-1 and BCL2 gene expression in KCOTs. The impact of miR-15a/16-1 expression on BCL2 gene translation was investigated by in vitro studies using primary KCOT culture cells. Results: Using qRT-PCR, we observed miR-15a and/or miR-16-1 downregulation in the majority of the KCOT samples (24 of 28). We also observed higher BCL2 mRNA expression in 19 of 20 KCOT frozen samples and moderate to high Bcl-2 immunopositivity in the basal layer cells of 16 of 18 paraffin embedded KCOTs (median: 42.6 %). In vitro over-expression of miR-15a/16-1 in human KCOT-1 primary cell cultures resulted in a decrease in Bcl-2 protein expression. Furthermore, all five paired KCOTs collected before and after marsupialization treatment exhibited an increase in miR-15a after the procedure. Conclusions: Our results suggest that KCOT neoplastic cells exhibit an anti-apoptotic profile that may be related to lower miR-15a/16-1 expression. Additionally, we demonstrated that miRNA expression increases after marsupialization, implicating an etiological and therapeutic role of miRNAs in KCOT. [ABSTRACT FROM AUTHOR]

Objectives. We rarely find multiple cystic lesions in the jaw in panoramic radiographs. Most reports of multiple cystic lesions have described keratocystic odontogenic tumors (KOTs) in patients with basal cell nevus syndrome. In this study, we performed radiographic and clinical examinations of patients with multiple cystic jaw lesions found during their first visit to our department to determine the proportion of basal cell nevus syndrome among multiple cystic lesions. Methods. Patients who had a panoramic radiograph taken at Okayama University Hospital between 1993 and 2000 were examined. Twenty-three patients with multiple cystic jaw lesions on a first panoramic radiography examination were entered into this study. Results. Of the 23 patients, 12 (52.2%) were diagnosed with basal cell nevus syndrome and 11 (47.8%) were not. Of the 14 patients with multiple KOTs, 12 (85.7%) were diagnosed as having basal cell nevus syndrome. Of the nine remaining patients with multiple cystic jaw lesions, only seven had multiple dentigerous cysts and two had multiple simple bone cysts. Cleft palate was observed in four of the 12 patients (33.3%) with basal cell nevus syndrome. Conclusion. Diagnosing basal cell nevus syndrome based on multiple cystic jaw lesions carries a high risk of misdiagnosis. A strong possibility exists that patients with multiple jaw KOTs have basal cell nevus syndrome. Furthermore, cleft palate should be considered an important oral finding in basal cell nevus syndrome because its occurrence was relatively frequent in our study. [ABSTRACT FROM AUTHOR]

Objectives: This study was aimed to analyze the reducing pattern of decompression on mandibular odontogenic keratocyst and to determine the proper time for secondary enucleation.Materials and methods: Seventeen patients with OKC of the mandible were treated by decompression. Forty-five series of CT data were taken during decompression and measured by using InVivo software (Anatomage, San Jose, Calif) and were analyzed.Results: The expected relative volume during decompression is calculated using the following formula: V(t) = Vinitial × exp.(at + 1/2bt2) (t = duration after decompression (day)).
