Anti-filtration recharge wells are commonly installed in river channels and irrigation canals on the Shandong Peninsula, China, and can be used as an important recharge facility for underground reservoirs. However, during recharge, as the running time increases, the recharge capacity gradually decreases. This study was undertaken to develop a new recharge well and mitigate this deficiency. A round empty core infiltration and anti-filtration recharge well and laboratory recharge test equipment were developed. Subsequently, 1:25 models of the recharge pond, a round empty core recharge wellhead, and an artificial recharge well were prepared. Using equal recharge levels and ambient groundwater levels, laboratory steady-flow recharge tests on the existing anti-filtration recharge well, the new round empty core infiltration and anti-filtration recharge well, and an artificial recharge well were carried out. Experimental data on the measured groundwater table and single-well recharge quantity were also collected and analyzed. The results showed that compared with the existing anti-filtration recharge well, the new round empty core infiltration and anti-filtration recharge well had stronger anti-deposition and anti-scouring properties, and the single-well recharge quantity increased by 403%. With an increase in the number of recharge tests, the single-well recharge quantity gradually decreased and tended to stabilize as a whole. [ABSTRACT FROM AUTHOR]

Background: Minimizing incisions has the potential to decrease hernia formation and wound complications following laparoscopic surgery. It is currently unknown if using the stoma site for specimen extraction affects outcomes. This study aims to evaluate the impact of stoma site extraction on postoperative complication rates in laparoscopic colorectal surgery.Methods: After IRB approval, a retrospective comparative review of 738 consecutive patients (405 M) who underwent laparoscopic colorectal surgery with ileostomy between January 2008 and December 2014 was performed. Patients who had a minimally invasive surgery that required an ileostomy were included. Patients were classified into two groups: stoma site extraction (SSE) or non-stoma site extraction (NSSE) and compared by body mass index (BMI), age, comorbidities, American Society of Anesthesiologists score, length of stay, estimated blood loss, parastomal complications, and hernia rate.Results: The parastomal hernia rate was 10.1% for the SSE group (n = 14) and 4.2% for the NSSE group (n = 25) (p = 0.007). The need for additional surgeries was 7/139 (5.0%) for the SSE group and 27/599 (4.5%) for the NSSE group (p = 0.79). There was no difference in the hernia rate after stoma closure in either group. There was no difference in single incision laparoscopic surgery versus conventional laparoscopy or robotic-assisted laparoscopy on stoma site complications in patients with SSE. SSE, transfusion, and BMI >30 were found to be independent factors associated with increased stoma site complications.Conclusion: SSE does increase stoma site complications. SSE should be used with caution, or in conjunction with other techniques to reduce hernias in patients requiring a permanent stoma or with an elevated BMI. The increase in stoma site complications does not translate into additional surgeries or postoperative sequelae following stoma reversal and is a reasonable option in patients requiring a temporary stoma. Objects: To explore the roles of growth factor receptor-bound protein 14 (GRB14) in colorectal cancer (CRC) and its correlation with clinicopathological characteristics and prognosis of CRC patients. Results: GRB14 was localized in the cytoplasm of CRC and benign glandular epithelium cells, showing higher levels in CRC tissues compared with normal colon samples (P < 0.001). High GRB14 was associated with a high pathological grade (P = 0.045), advanced clinical stage (P = 0.018), enhanced tumor invasion (P < 0.001) and lymph node metastasis (P = 0.028). The cancer genome atlas (TCGA) mRNA sequence data showed that GRB14 was upregulated in CRC at an advanced clinical stage (P = 0.011) with enhanced tumor invasion (P < 0.001) and lymph node metastasis (P = 0.014). Kaplan-Meier survival curves revealed that CRC patients with high GRB14 levels had a shorter survival compared with those showing low GRB14 expression (P = 0.007). High GRB14 expression was an independent prognostic factor for CRC patients (HR 2.847, 95 %CI 1.058-7.659; P = 0.038). Conclusions: GRB14 may be an important cancer promoter that enhances CRC progression. Upregulated GRB14 levels may predict a poor clinical outcome in CRC patients. [ABSTRACT FROM AUTHOR]

The fragile histidine triad (FHIT) gene, a candidate tumor suppressor gene located at 3p14.2, has been shown to be involved in the carcinogenesis of many human tissues, including digestive tract tissues. However, the expression and the role of the FHIT in the initiation and the development of the colorectal cancer (CRC) are poorly understood. We have shown that the FHIT gene exhibits significantly decreased expression in human CRC compared to colorectal adenoma and normal colorectal tissue by tissue microarray (TMA). The positive rate of FHIT gene expression in normal colorectal tissue, adenoma and adenocarcinoma were 93.75%, 68.75% and 46.25%, respectively. We show this decreased expression to be significantly correlated with the progression of colorectal carcinoma ( P<0.05) as well as with differentiation and lymph node metastasis ( P<0.05). We detected two somatic alterations in the FHIT gene in human CRC. The presence of this mutation correlated significantly with decreased FHIT expression in the human CRC. In our present study we tested the hypothesis that the decreased FHIT expression resulted in apoptosis inhibition associated with abnormal expression of apoptosis related proteins. To test this hypothesis we did a series of experiments. In the first test, we assessed apoptosis status using a standard TUNEL (terminal deoxynucleotidyl transferase-mediated dUTP nick-end-labeling) assay by comparing FHIT-positive CRC vs. FHIT-negative CRC. In the second experiment, the protein expression of the FHIT and other apoptosis related proteins (Bax, Bcl-2 and Survivin) were measured in human CRC by TMA. Our combined results demonstrate the mutation in the FHIT gene significantly reduced FHIT expression in human CRC. Both TUNEL and TMA experiments demonstrated significantly inhibited apoptosis by down-regulation of Bax and the up-regulation of Survivin and Bcl-2. Collectively, these studies identify the mechanism by which an important tumor suppressor gene, FHIT is inactivated specifically in human CRC contributing to our understanding of the mechanism of colorectal carcinogenesis.
