Objective: Diagnosing juvenile idiopathic arthritis (JIA) is challenging. Our study aimed to investigate the clinical significance of anti-α-1,4-D-polygalacturonic acid (PGA) antibodies in JIA, focusing on their role in diagnosis and assessing disease activity. Methods: In this prospective case–control study, we examined variations in serum levels of PGA-IgA and PGA-IgG among children with different types of JIA and healthy controls. Serum PGA-IgA and PGA-IgG levels were assessed concurrently in children with active and inactive JIA. Results: This study included 126 patients diagnosed with JIA, 13 neonates, and 76 healthy children. Serum PGA-IgA and PGA-IgG levels were assessed, which revealed significant differences in PGA-IgA levels between various JIA subtypes and controls. An analysis of PGA-IgA levels in various JIA states revealed a statistically significant difference. Receiver operating characteristic (ROC) analysis demonstrated the robust predictive capability of PGA-IgA, with an AUC of 0.879 (p < 0.001), along with a specificity of 0.842 and sensitivity of 0.848. Conclusion: Increased levels of anti-PGA antibodies, particularly PGA-IgA, were significantly associated with JIA. PGA-IgA may serve as a sensitive biomarker for disease activity in JIA and could potentially aid in the diagnosis of JIA. Socioeconomic status (SES) has been linked to mortality rates, with family income being a quantifiable marker of SES. However, the precise association between the family income-to-poverty ratio (PIR) and all-cause mortality in adults aged 40 and older remains unclear. A cross-sectional study was conducted using data from NHANES III, including 20,497 individuals. The PIR was used to assess financial status, and various demographic, lifestyle, and clinical factors were considered. Mortality data were collected from the NHANES III linked mortality file. The study revealed a non-linear association between PIR and all-cause mortality. The piecewise Cox proportional hazards regression model showed an inflection point at PIR 3.5. Below this threshold, the hazard ratio (HR) for all-cause mortality was 0.85 (95% CI 0.79–0.91), while above 3.5, the HR decreased to 0.66 (95% CI 0.57–0.76). Participants with lower income had a higher probability of all-cause mortality, with middle-income and high-income groups showing lower multivariate-adjusted HRs compared to the low-income group. This study provides evidence of a non-linear association between PIR and all-cause mortality in adults aged 40 and older, with an inflection point at PIR 3.5. Anionic defects of H− (H∙O) and oxygen vacancies with trapped electrons (V∙O) have been built in the lattice of a hydrothermally derived perovskite (Ba5Nb4O15, BNO) by CaH2 reduction in Ar flow at 600 °C for 12 h. The formed anionic defects in the CaH2-reduced BNO (CaBNO) have been found to improve drastically the ammonia synthesis activity of Ru/CaBNO by donating electrons from both H∙O and/or V∙O to Ru atoms. The stoichiometry of H∙O and V∙O has been determined by TPR and TPO method, respectively. The H∙O defects have been proved to react with N2 under ammonia synthesis conditions via TPD, XPS measurements and isotope (D2) reaction, which provides secondary pathways for ammonia synthesis. Kinetic studies revealed that oxygen vacancies accept spilledover H atoms from Ru particles to alleviate the H2 poisoning effect. Cs-promoter has been demonstrated to accelerate the formation of anionic defects in CaBNO under ammonia synthesis reactions. Background: The concept of acute kidney injury (AKI) substages has been recommended to better phenotype AKI and identify high-risk patient groups and therefore improve the diagnostic accuracy of AKI. However, there remains a gap between the recommendation and the clinical application. The study aimed to explore the incidence of AKI substages based on a sensitive AKI biomarker of urinary cystatin C (uCysC), and to determine whether AKI substages were relevant with respect to outcome in critically ill children. Results: The multicenter cohort study enrolled 793 children in pediatric intensive care unit (PICU) of four tertiary hospitals in China. Children were classified as non-AKI, sub-AKI and AKI substages A and B according to uCysC level at PICU admission. Sub-AKI was defined by admission uCysC level ≥ 1.26 mg/g uCr in children not meeting the KDIGO criteria of AKI. In children who fulfilled KDIGO criteria, those with uCysC < 1.26 was defined as AKI substage A, and with ≥ 1.26 defined as AKI substage B. The associations of AKI substages with 30-day PICU mortality were assessed. 15.6% (124/793) of patients met the definition of sub-AKI. Of 180 (22.7%) patients with AKI, 90 (50%) had uCysC-positive AKI substage B and were more likely to have classical AKI stage 3, compared to substage A. Compared to non-AKI, sub-AKI and AKI substages A and B were risk factors significantly associated with mortality, and the association of sub-AKI (adjusted hazard ratio HR = 2.42) and AKI substage B (adjusted HR = 2.83) with mortality remained significant after adjustment for confounders. Moreover, AKI substage B had increased risks of death as compared with sub-AKI (HR = 3.10) and AKI substage A (HR = 3.19). Conclusions: Sub-AKI defined/based on uCysC occurred in 20.2% of patients without AKI and was associated with a risk of death close to patients with AKI substage A. Urinary CysC-positive AKI substage B occurred in 50% of AKI patients and was more likely to have classical AKI stage 3 and was associated with the highest risk of mortality. [ABSTRACT FROM AUTHOR]

A nomogram for assessing the risk of IgA vasculitis nephritis (originally termed Henoch–Schönlein purpura nephritis, HSPN) in IgA vasculitis (originally termed Henoch–Schönlein purpura, HSP) pediatric patients can effectively improve early diagnosis and prognosis of IgA vasculitis nephritis. However, currently, no nomogram is available. 246 IgA vasculitis and 142 IgA vasculitis nephritis Asian pediatric patients confirmed by renal biopsy were enrolled. Univariate and multivariate logistic regressions were performed to identify the independent risk factors and construct a series of predictive models. The receiver operating characteristic curve, calibration plot, decision curve analysis, net reclassification index and integrated discrimination index were used to screen the best model. Stratification analysis was applied to optimize model's clinical utility. An external validation set was introduced to verify the predictive efficiency. The final predictive model was converted to nomogram for visual use. We identified age, duration of rash (Dor), D-dimer and IgG as independent risk factors and constructed four models as follows: AIDD (Age + IgG + Dor + D-dimer), AIDi (Age + IgG + D-dimer), AIDo (Age + IgG + Dor) and ADD (Age + Dor + D-dimer), which achieved the receiver operator characteristic curve (AUROC) of 0.931, 0.920, 0.856 and 0.907, respectively. Finally, AIDi model with an AUROC of 0.956 and 0.897 in internal and external validating sets was proposed as a novel predictive model. In stratification analysis by gender and histological grade, the AUROC of AIDi was 0.949 in female, 0.926 in male, 0.933 in mild histological grades and 0.939 in severe histological grades, respectively. The SLC7A11/GPX4 axis plays an important role in ferroptosis during cardiac ischemia/reperfusion injury (IRI). The present study was designed to evaluate the impact of dexmedetomidine (DEX) post-conditioning on cardiac IRI and to explore whether the effect was achieved by SLC7A11/GPX4 signaling pathway regulation. Rat myocardial IRI was established by occluding the left anterior descending artery for 30 min followed by 2-h reperfusion. The infarct area was detected by diphenyltetrazolium chloride (TTC) staining; the cardiac function was evaluated by echocardiography. The levels of lipid peroxide biomarkers were measured to estimate the injury caused by lipid peroxide. HE staining and Sirius staining were utilized to assess myocardial damage and fibrosis. The mitochondrial morphology was observed by electron micrography. Western blot and quantitative real-time polymerase chain reaction were employed to measure the relative molecular characteristics. Our results showed that DEX administration at the beginning of reperfusion attenuated IRI-induced myocardial injury, alleviated mitochondrial dysfunction, decreased the level of reactive oxygen species (ROS), alleviated mitochondrial dysfunction, inhibited the activation of SLC7A11/GPX4, and modulated the expression of ferroptosis-related proteins, including SLC7A11, glutathione peroxidase 4 (GPX4), ferritin heavy chain (FTH), and cyclooxygenase-2 (COX-2). Conversely, the ferroptosis activator erastin partly suppressed the DEX-mediated cardio protection. Altogether, these results reveal that DEX inhibits ferroptosis by enhancing the expression of SLC7A11 and GPX4, thereby preventing cardiac I/R injury. [ABSTRACT FROM AUTHOR]

Video-based human motion analysis is an important research direction in the field of computer vision. It detects moving objects from video sequences, extracts key parts of the human body, and obtains useful information for human movements. In this paper, the joints of the teeing arm are first color-coded. The tennis teeing video is collected by a high-speed camera. The coordinates of the tacking points in each frame are used instead of the knuckles to study the trajectory of the teeing arm. In the process of video processing, after constructing a dictionary for a series of noise maps, the sparse representation idea was used to reconstruct an interference-free service diagram, and a mixture of Gaussian background modeling was used to extract the foreground of the motion. After obtaining the motion foreground, the marker points are extracted through the color features, and binarization operations are performed on the marker points. Next, the outline of the marker points is searched, the outline is surrounded by the minimum circle, and the returned circle center coordinates are used as the joint point coordinates. Pancreatitis is uncommon in systemic lupus erythematosus (SLE) and is rarely reported in children, possibly being related to macrophage activation syndrome (MAS). The incidence of MAS in children with lupus pancreatitis is unknown, as is their prognosis. In this case-based review, we report a pediatric patient with SLE complicated with pancreatitis and MAS, and performed a literature review. We report an 11-year-old girl with SLE and MAS who developed pancreatitis on the second day of methylprednisolone pulse therapy (500 mg/day). We continued methylprednisolone pulse therapy, and performed three rounds of DNA-immunoadsorption and three rounds of hemoperfusion. A second course of methylprednisolone pulse therapy was initiated 9 days later. The patient received a monthly cyclophosphamide pulse therapy (10 mg/kg/day, 2 consecutive days every month) for 6 months, after which she was treated with mycophenolate mofetil 20 mg/kg/day. The condition of the patient gradually improved, her blood amylase and lipase decreased. She was in a stable condition during 13-month follow-up period. Review of the literature of pediatric patients with SLE and pancreatitis showed that there are 127 cases that have been reported in the past 30 years, 40 cases were excluded in our study because of inadequate information. Of the 87 patients included in our literature review, the mortality rate was 33.33%, and 52.86% of the patients with pancreatitis had MAS at the same time. Pancreatitis is uncommon in SLE, but must be suspected if a patient with SLE develops digestive symptoms. Patients with SLE with pancreatitis have a high incidence of MAS and high mortality rate; however, early recognition and effective treatment can relieve the disease symptoms. [ABSTRACT FROM AUTHOR]

The promotion and application of low-carbon energy has become a basic energy policy in China. In the taxi industry, the adoption of natural gas is a typical reflection of the use of low-carbon energy, however, the application of CNG taxis in practice is difficult because of the ignorance of consumer behaviour choices and the lack of consideration of the reforming and maintenance costs of CNG taxis in practical use during the implementation of this policy. In this paper, the construction of a multi-stage dynamic game model is proposed: this is used to analyse the complex dynamic relationships among consumers, the taxis, and the policy-making behaviours of government. By the means of backward induction, a conclusion is drawn such that government should determine the subsidy coefficientper unit of product as well as the carbon taxand the optimal reduction rate of carbon emissions confirmed by operators determination. Based on these, concerning the behavioural choices among consumers, taxis, and government, this paper draws five conclusions, which answer the problems concerning whether, or not, the government should levy a carbon tax and subsidise such operations as well as how much that financial levy and subsidy should be. Besides, this paper also makes a value judgment of the future format/mode of taxi operations. The study shows that, with the increasing subsidy, the competitive advantages of the CNG taxis are strengthened while the revenues of oil-fired taxis decrease constantly, which forces oil-fired taxis out of the market. At the same time, the service prices of both kinds of taxis are proportional to the money paid by consumers and the positive impacts of service prices on CNG taxis are greater than that on oil-fired taxis.The carbon tax standard set by government can play a proper role only if it gives consideration to the interests of both consumers and CNG taxis. Unlabelled: Fluid overload (FO) has been associated with an increased risk for adverse outcomes in critically ill patients. Information on the impact of FO on mortality in a general population of pediatric intensive care unit (PICU) is limited. We aimed to determine the association of early FO with the development of acute kidney injury (AKI) and mortality during PICU stay and evaluate whether early FO predicts mortality, even after adjustment for illness severity assessed by pediatric risk of mortality (PRISM) III. This prospective study enrolled 370 critically ill children. The early FO was calculated based on the first 24-h total of fluid intake and output after admission and defined as cumulative fluid accumulation ≥5% of admission body weight. Of the patients, 64 (17.3 %) developed early FO during the first 24 h after admission. The PICU mortality rate of the whole cohort was 18 of 370 (4.9%). The independent factors significantly associated with early FO were PRISM III, age, AKI, and blood bicarbonate level. The early FO was associated with AKI (odds ratio [OR] = 1.34, p < 0.001) and mortality (OR = 1.36, p < 0.001). The association of early FO with mortality remained significant after adjustment for potential confounders including AKI and illness severity. The area under the receiver operating characteristic curve (AUC) of early FO for predicting mortality was 0.78 (p < 0.001). This result, however, was not better than PRISM III (AUC = 0.85, p < 0.001).Conclusion: Early FO was associated with increased risk for AKI and mortality in critically ill children.What Is Known: Fluid overload is associated with an increased risk for adverse outcomes in specific clinical settings of pediatric population. Background: Urinary interleukin-18 (uIL-18) and cystatin C (uCysC) are biomarkers of acute kidney injury (AKI). We hypothesized that in non-AKI neonates, the level of uIL-18 and uCysC would be higher in those with sepsis compared to those without sepsis. The aims of this study were to determine the association between urinary biomarkers and sepsis in non-AKI critically ill neonates, and to evaluate whether uIL-18 and uCysC could serve as predictors of sepsis in this population. Methods: The study included 111 non-AKI critically ill neonates with acute clinical deterioration suggestive of sepsis: 26 with infection, 57 without infection, and 28 were assigned to the unclassified group. Urinary samples were collected and a full sepsis screen was performed at the time of enrollment. Results: The level of uIL-18, but not uCysC, was significantly elevated in non-AKI septic neonates. Urinary IL-18 was an independent factor associated with sepsis assessed by multivariate analysis, had odds ratio of 1.73 (95 % CI 1.15 to 2.58, p = 0.008), and achieved the area under the receiver operating characteristic curve of 0.74 for predicting the presence of sepsis in non-AKI critically ill neonates. Conclusions: Sepsis has an impact on the level of uIL-18, but not on the uCysC in non-AKI neonates, suggesting systemic infection might influence the diagnostic value of uIL-18 to detect AKI in the general population. Background: Urinary interleukin-18 and cystatin-C are suggested to be biomarkers for predicting acute kidney injury (AKI). The aims of this study are to examine whether the urinary concentrations of interleukin-18 and cystatin-C vary with gestational age and other factors in non-AKI control neonates, and to determine whether urinary interleukin-18 and cystatin-C can predict AKI development in non-septic critically ill neonates, independently of potential confounders. Methods: We enrolled 62 non-septic critically ill neonates. Urine was collected every 48-72 h during the first 10 days of life. Results: Urinary concentration of cystatin-C, but not interleukin-18, decreased with increasing gestational age and body weight, but not with increasing postnatal age in non-AKI control neonates. Both urinary interleukin-18 and cystatin-C were associated with AKI, even after controlling for gestational and postnatal age, birth weight, gender, Apgar score and the score for neonatal acute physiology in non-septic critically ill neonates. Urinary interleukin-18 and cystatin-C had odds ratios of 2.27 and 2.07, and achieved the area under-the-receiver-operating-characteristic curve of 0.72 and 0.92, respectively, for predicting AKI. Conclusions: The urinary concentration of cystatin-C, but not interleukin-18, may decrease with increasing renal maturity.
