Objectives: The aim of this study was to evaluate clinical and radiological findings and the role of periapical infection and antecedent dental treatment of infected focus teeth in odontogenic maxillofacial abscesses requiring hospital care. Materials and methods: In this retrospective cohort study, we evaluated medical records and panoramic radiographs during the hospital stay of patients ( n = 60) admitted due to odontogenic maxillofacial infection originating from periapical periodontitis. Results: Twenty-three (38 %) patients had received endodontic treatment and ten (17 %) other acute dental treatment. Twenty-seven (45 %) had not visited the dentist in the near past. Median age of the patients was 45 (range 20-88) years and 60 % were males. Unfinished root canal treatment (RCT) was the major risk factor for hospitalisation in 16 (27 %) of the 60 cases ( p = .0065). Completed RCT was the source only in 7 (12 %) of the 60 cases. Two of these RCTs were adequate and five inadequate. Conclusions: The initiation of inadequate or incomplete primary RCT of acute periapical periodontitis appears to open a risk window for locally invasive spread of infection with local abscess formation and systemic symptoms. Thereafter, the quality of the completed RCT appears to have minor impact. However, a considerable proportion of the patients had not received any dental treatment confirming the importance of good dental health. Thus, thorough canal debridement during the first session is essential for minimising the risk for spread of infection in addition to incision and drainage of the abscess. If this cannot be achieved, tooth extraction should be considered. The genus Phlomis is one of the largest genera of subfamily Lamioideae (Lamiaceae) with more than 100 recognised species distributed in Asia, southern Europe and northern Africa. Morphological studies have suggested that the species can be split into two morphologically distinct groups that have previously been recognised as either sections or separate genera. The current study represents the first molecular phylogenetic study of the genus in which taxa representative of the entire genus are included. In contrast to considerable morphological and ecological variation among taxa, sequence variation in the trnL intron, the trnL - F intergenic spacer, and the rps16 intron was low, resulting in sparse phylogenetic resolution, particularly among the South West Asian and Mediterranean taxa. However, the results support a split of the genus into two separate groups that are recognised here as the genera Phlomoides and Phlomis in order to decrease taxonomic complexity. The close relationship between Phlomis s.l. and Eremostachys, pointed out by several authors, is confirmed, and in a noteworthy new finding, the small Asian genus Notochaete, is more closely related to the Phlomis s.l. lineage than previously assumed. The two monotypic genera Pseuderemostachys and Lamiophlomis as well as Notochaete hamosa are transferred to Phlomoides and three new combinations are made. Character optimisation analyses based on geographic distributions of all taxa show that the groups obtained by molecular phylogenetic analysis correlate well with biogeography. CD40, a tumor necrosis factor receptor family member, is an emerging target for cancer therapy being best appreciated as an important regulator of the anti-tumor immune response. In this study, we report the development of a replication-defective recombinant adenovirus (RAd) vector expressing human CD40 ligand (RAd-hCD40L) and show that sustained engagement of the CD40 pathway in malignant cells results in direct anti-proliferative and pro-apoptotic effects. Thus, transduction of CD40-positive bladder, cervical and ovarian carcinoma cell lines with RAd-hCD40L potently inhibits their proliferation in vitro, whereas CD40-negative lines remain unresponsive. RAd-hCD40L is also found to be superior to recombinant CD40L in inducing carcinoma cell death and in amplifying the cytotoxic effects of the chemotherapeutic agents 5-fluorouracil, cis-platin and mitomycin C. Soluble CD40L is produced by RAd-hCD40L transduced carcinoma cells but unlike other soluble tumor necrosis factor family ligands, it does not interfere with the death-promoting activity of its membrane-bound form. In a mouse xenograft tumor model bearing a human bladder carcinoma, intratumoral delivery of RAd-hCD40L suppresses cancer growth. These findings highlight the potential of exploiting the CD40 pathway in carcinomas using CD40L gene transfer alone or in combination with other modalities for cancer therapy. Our results have also broader implications in understanding the multifaceted anti-tumor activities of the CD40 pathway in carcinomas, which thus offer an attractive option for future clinical application. In cranio-maxillofacial surgery, bone transplantation is needed for treatment of bony defects. An autograft, allograft or biomaterial can be used. Autogenous bone grafts are considered to be the best materials available, but there are some disadvantages in their use including donorsite morbidity, need for a second operative site and limited graft supply. A search for new bone-graft materials therefore remains necessary. We prepared a mixture of tricalcium phosphate (TCP), which is a resorbable, non-toxic, osteoconductive ceramic material and ε-caprolactone-lactide copolymer P(ε-CL/DL-LA), a resorbable polymer, and placed it in the dermis and in mandibular bone defects in 13 rabbits. Follow-up times were two, three, seven, eight, 12, 15 and 18 weeks, tissue reactions were assessed, histologically and immunohistochemically. Times of resorption of the material from tissues were reported. We found that the mixture caused a mild inflammatory reaction when placed in bone and severe inflammation when placed in dermis. No highly fluorescent layer of tenascin or fibronectin was found surrounding the implant area. The mixture was excellent to handle and very easy to place into bone defects. A description is given of the occurrence of upper and lower lip cancers in Finland in the period 1953-1974 with the aim of reaching conclusions on the possible differences in the etiological factors involved. Substantial differences with respect to various epidemiological parameters, such as incidence rates, trends in the incidence with time, male/female ratios of the incidence rates, age-incidence curves, urban/rural ratios of the incidence rates, and geographical distribution, were demonstrated between upper and lower lip cancers. On the other hand, similarities in the occurrence of upper lip cancer and non-melanomatous skin cancer of the head and neck could be observed. It was concluded that in terms of etiology upper lip cancer could be regarded as a separate entity and different from 'true' lip cancer of the lower lip. Two patients with seronegative HLA-B27-connected spondylarthropathy, associated in one case with psoriasis and in the other with Reiter's disease, who had atlanto-axial subluxation (AAS) are reported. The AAS caused muscle weakness and intractable occipito-cervical pain. Both patients were treated operatively with a Gallie type C-C fusion. The symptoms subsided following surgery and the patients remained free of cervical spine discomfort. The report also indicates that the rarer variants of the HLA-B27 associated chronic seronegative spondylarthropathies may cause AAS complicated by neurological symptoms and that the disorder is amenable to surgical intervention. Extracellular matrix (ECM) proteins are known to play a role in inflammatory and hyperplastic processes. Our aim in the present study was to study the distribution of tenascin (Tn), cellular fibronectins (cFn) and myofibroblasts around biodegradable poly-L/D-lactide (PLA) implants with monoclonal antibodies (MAb). Ethylene-oxide and gamma-irradiation sterilized PLA plate-type implants were inserted into the dorsal subcutaneous tissue of ten adult rabbits. Follow-up times were 4, 12, 16, 36 and 48 wk. Only some inflammatory cells were observed. In electron microscopy, a close coherence between the implant and the stromal tissue was seen. Immunoreactivity for Tn, cFn and α-actin was detected as a distinct layer bordering the implant, regardless of the sterilization method for the first 36 wk. From week 36 onwards, Tn immunoreactivity was downregulated while cFn immunoreactivity still persisted. A moderate upregulation for myofibroblasts was seen on the week 48 specimens, when hydrolysation of PLA implant had started. The persistent content of myofibroblasts, Tn and cFn suggests a prolonged wound healing produced by PLA implants. The absence of Tn at the week 48 specimens suggests that cFn, rather than Tn may be needed for α-actin-mediated contraction by myofibroblasts. [ABSTRACT FROM AUTHOR]

In previous studies poly (ortho ester) (POE) has shown promise as a resorbable device, a hemostatic sealant and as a carrier for drugs in bone surgery. The aim of this study was to evaluate the tissue reactions of solid poly (ortho ester) implanted into both tibiae of 17 rabbits. One half of the rods were sterilized by gamma radiation and the other half by ethylene oxide. The follow-up times were from 1 week to 21 weeks, after which the animals were killed and the bony specimens examined histologically. The connective tissue samples were examined immunohistochemically in order to study the occurrences of two extracellular matrix glycoproteins, tenascin and fibronectin. The results showed that solid poly (ortho ester)s induce a moderate inflammatory reaction for 9 weeks. Tenascin and fibronectin were present in samples from 1 week up to 4 weeks. It was also found that gamma sterilized POE was resorbed at week 7 and ethylene oxide sterilized POE at week 13. [ABSTRACT FROM AUTHOR]

This report describes the case of a 35-year-old man with juvenile rheumatoid arthritis. The patient experienced swallowing difficulties that were initially thought to be a result of the disease affecting the cervical spine. A maxillofacial examination, however, showed a total loss of the mandibular condyles, short rami and bodies, and a retrusion of the jaw. This was considered to be the main cause of the dysphagia, and not a glossopharyngeal nerve compression. The surgical-orthodontic management of these difficult cases is discussed. Interstitial collagenase present in human jaw cyst extract and purified human fibroblast-type collagenase (MMP-1) were both efficiently inhibited in vitro by clodronate, an osteoactive, antiresorptive bisphosphonate. The IC50 of clodronate to inhibit MMP-1 is 150 microM. These findings suggest an extended and hitherto undescribed properties for clodronate/biphosphonates in prevention and treatment of tissue degradation in both bone and soft tissue destructive diseases.
