We describe coverage of maternal syphilis screening, syphilis positivity, coverage of treatment and their association with maternal HIV infection and antiretroviral treatment (ART) status among pregnant women attending South African antenatal clinics. The 2019 antenatal care sentinel survey was a cross-sectional survey conducted from 1 October to 15 November 2019 at 1589 sentinel sites in all nine provinces of the country and aimed to enrol 36,000 pregnant women ages 15–49 years regardless of HIV, ART or syphilis status. Data collection procedures included obtaining written informed consent, a brief interview, medical record review and blood specimen collection. Completed data collection forms and specimens were sent to designated regional laboratories for data capture and HIV serology testing. Data analysis determined four outcomes i) syphilis screening coverage ii) syphilis positivity iii) coverage of any treatment and iv) with Benzathine penicillin G (BPG). Multivariable logistic regression models with or without interaction between HIV infection and ART status with province were used to determine factors associated with syphilis positivity. Of the 41 598 women enrolled, 35 900 were included in the analysis for syphilis screening coverage. The weighted syphilis screening coverage was 96.4% [95% Confidence Interval (CI) 95.9–96.7%] nationally and was lowest among HIV positive women not on ART at 93.5% (95% CI 92.2–94.5%). Syphilis positivity was 2.6% (95% CI 2.4–2.9%) nationally. Among those who were syphilis positive, 91.9% (95% CI 89.8–93.7%) had documentation of syphilis treatment status, of whom 92.0% (95% CI 89.8–93.9%) were treated, with the majority treated with one or more doses of BPG [92.2% (95% CI 89.8–94.3%)]. HIV-positive women, not on ART [adjusted odd ratio (aOR) 2.24 (95% 1.71–2.93)] and those on ART [aOR 2.25 (95% CI 1.91–2.64)] were more likely to be syphilis positive compared to those who were HIV negative. The national syphilis screening coverage met the global screening target of 95%. Syphilis positivity was higher among HIV positive women compared to negative women. Introduction of rapid syphilis testing and ensuring a universal supply of appropriate treatment for syphilis will reduce the likelihood of mother-to-child transmission of syphilis. Introduction: Consumption of unhealthy foods in children contributes to high levels of childhood obesity globally. In developing countries there is paucity of empirical studies on the association. This study employed propensity-score methods to evaluate the effect of unhealthy foods on overweight among children in Malawi using observational data. Methods: Data on 4625 children aged 6 to 59 months from the 2015-16 Malawi Demographic and Health Survey (MDHS) were analyzed. A multivariable logistic regression model of unhealthy foods (yes or no) on purported confounders of childhood overweight was used to obtain a child's unhealthy food propensity score. The propensity scores were then used to form matched sets of healthy and unhealthy fed children. The association between unhealthy foods and childhood overweight was assessed using the conditional logistic regression model. Results: The prevalence of overweight (body mass index (BMI) z-score > 2 standard deviations) was estimated at 4.5% (3.8%, 5.3%). The proportion of children who consumed unhealthy foods was estimated at 14.6% (95% CI: 13.1%, 16.2%). Our propensity score matching achieved a balance in the distribution of the confounders between children in the healthy and unhealthy food groups. Children fed unhealthy foods were significantly more likely to be overweight than those fed healthy foods (OR = 2.5, 95% CI: (1.2, 5.2)). Conclusion: The findings suggest the adverse effects of unhealthy foods on childhood overweight in Malawi. Thus, efforts to reduce unhealthy food consumption among children should be implemented and supported to address the problem of childhood overweight in Malawi and the sub-Saharan African region. To describe the prevalence of unintended pregnancy and its association with HIV status among pregnant women in South Africa. A cross-sectional survey was conducted between October and mid-November 2019 among pregnant women aged 15–49 years in 1589 selected public antenatal care facilities. Pregnancy intention was assessed using two questions from the London Measure of Unplanned Pregnancy. Survey logistic regression examined factors associated with unintended pregnancy. Among 34,946 participants, 51.6% had an unintended pregnancy. On multivariable analysis, the odds of unintended pregnancy was higher among women who knew their HIV-positive status before pregnancy but initiated treatment after the first antenatal visit (adjusted odds ratio [aOR], 1.5 [95% confidence interval (CI):1.2–1.8]), women who initiated treatment before pregnancy (aOR, 1.3 [95% CI:1.2–1.3]), and women with a new HIV diagnosis during pregnancy (aOR, 1.2 [95% CI:1.1–1.3]) compared to HIV-negative women. Women who were single, in a non-cohabiting or a cohabiting relationship, and young women (15–24 years) had significantly higher risk of unintended pregnancy compared to married women and women aged 30–49 years, respectively. A comprehensive approach, including regular assessment of HIV clients' pregnancy intention, and adolescent and youth-friendly reproductive health services could help prevent unintended pregnancy. Land management decisions have extensively modified land use and land cover in the Zambezi Region. These decisions are influenced by land tenure classifications, legislation, and livelihoods. Land use and land cover change is an important indicator for quantifying the effectiveness of different land management strategies. However, there has been no evidence on whether protected or communal land tenure is more affected by land use and land cover changes in southern Africa and particularly Namibia. Our study attempted to fill this gap by analyzing the relationship between land use and land cover change and land tenure regimes stratified according to protected and communal area in the Zambezi Region. Multi-temporal Landsat TM and ETM+ imagery were used to determine the temporal dynamics of land use and land cover change from 1984 to 2010. The landscape showed distinctive modifications over the study period; broad trends include the increase in forest land after 1991. However, changes were not uniform across the study areas. Two landscape development stages were deduced: (1) 1984-1991 represented high deforestation and gradual increase in shrub land; (2) 1991-2000 and 2000-2010 represented lower deforestation and slower agropastoral expansion. The results further show clear patterns of the dynamics, magnitude, and direction of land use and land cover change by tenure regime. The study concluded that land tenure has a direct impact on land use and land cover, since it may restrict some activities carried out on the land in the Zambezi Region. [ABSTRACT FROM AUTHOR]

This paper examines the socio-economic drivers of land use and land cover change and assesses the impacts of such changes to rural livelihoods in the Zambezi region of northern Namibia. We carried out a longitudinal analysis of Landsat imagery of land use and land cover. The analysis revealed that the amount of land in the region covered by forest increased significantly in the period from 1991 to 2010 whilst crop/grass land decreased. Focus group meetings, key informant interviews and semi-structured interviews covering 424 households stratified by gender were used. The results show that natural resource uses are vitally important in the rural livelihoods. The drivers of land use and land cover change are agricultural expansion, population increase and illegal logging. Livelihood coping strategies include piecework, food aid, borrowing from relatives and wild food collection. By gender stratification, piecework contributed 37 and 63 % while agriculture contributed 29 and 71 % of the income of male- and female-headed households. Logistic regression analysis showed knowledge of regulations, age group and species availability significantly ( p < 0.05) influenced the choice of a household's livelihood coping strategy. The study concludes that the changes in coping strategies influenced by a variety of factors have led to the diminished use of natural resources. For policy purposes, this suggests that state interventions can play a significant role in promoting more sustainable natural resource usage. Childhood acute lymphoblastic leukaemia (ALL) and Type 1 diabetes (T1D) share some common epidemiological features, including rising incidence rates and links with an infectious aetiology. Previous work has shown a significant positive correlation in incidence between the two conditions both at the international and small-area level. The aim was to extend the methodology by including shared spatial and temporal trends using a more extensive dataset among individuals diagnosed with ALL and T1D in Yorkshire (UK) aged 0–14 years from 1978–2003. Cases with ALL and T1D were ascertained from 2 high quality population-based disease registers covering the Yorkshire region of the UK and linked to an electoral ward from the 1991 UK census. A Bayesian model was fitted where similarities and differences in risk profiles of the two diseases were captured by the shared and disease-specific components using a shared-component model, with space-time interactions. The extended model revealed a positive correlation of at least 0.70 between diseases across all time periods, and an increasing risk across time for both diseases, which was more evident for T1D. Furthermore, both diseases exhibited lower rates in the more urban county of West Yorkshire and higher rates in the more rural northern and eastern part of the region. A differential effect of T1D over ALL was found in the south-eastern part of the region, which had a more pronounced association with population mixing than with population density or deprivation. Our approach has demonstrated the utility in modelling temporally and spatially varying disease incidence patterns across small geographical areas. The findings suggest searching for environmental factors that exhibit similar geographical-temporal variation in prevalence may help in the development and testing of plausible aetiological hypotheses. Furthermore, identifying environmental exposures specific to the south-eastern part of the region, especially locally varying risk factors which may differentially affect the development of T1D and ALL, may also be fruitful.
