Defoliation caused by strong tropical cyclones can modify the partitioning of incident solar radiation between the sensible, latent, and substrate heat fluxes. While previous work has shown hurricane defoliation to warm near-surface air temperature along its track, this study more directly contextualizes the warming to human heat stress and exposure via the heat index (HI). For this case study, the spatial extent and temporal persistence of defoliation produced by Hurricane Laura (2020) in southwestern Louisiana was characterized using the normalized difference vegetation index (NDVI). The defoliated land surface was then assimilated into the Weather Research and Forecasting (WRF) model version 4.2 and compared to a control, normal-foliage simulation for the 30 days following landfall. Over southwest Louisiana, the largest HI increase occurred at 0600 UTC (1:00 AM LT) with an average increase of + 0.25 °C, and the exposure time to HI ≥ 30 °C increased by 8.1% after accounting for the defoliated landscape. Meanwhile, Cameron, Louisiana, the site of Laura's landfall where defoliation was most severe, cumulatively experienced an extra 33 h of HI values exceeding 26 °C, while mean HI increased by 1.2 °C at 0300 UTC. Additional WRF experiments were performed with altered "landfall" years of 2017 and 2018 to determine the sensitivity of defoliation-driven HI changes to the ambient synoptic conditions. While synoptic conditions modulated the magnitude of increase, HIs nonetheless experienced statistically significant increases in both hypothetical "landfall" years. Background: Membranous nephropathy is an uncommon cause of nephrotic syndrome in pediatrics. Methods: We reviewed our kidney biopsy records for patients ≤ 20 years of age with membranous nephropathy without evidence of systemic lupus erythematosus within 6 months of biopsy (January 1995–September 2020). Staining for PLA2R, NELL1, THSD7A, SEMA3B, EXT2 (3 biopsies), and IgG-subclass were performed. Results: Sixteen children (≤ 12 years) and 25 adolescents (13–20 years) were identified. Four children and 15 adolescents showed autoantigen positivity: PLA2R+/SEMA3B- (13), SEMA3B+/PLA2R+ (2), SEMA3B+/PLA2R− (1), NELL1 (1), EXT2+ (2), and THSD7A (0). Co-morbidities associated with PLA2R positivity included IPEX syndrome, active hepatitis B, Von Hippel Lindau syndrome, solitary kidney, type 1 diabetes, hyperuricemia, pregnancy (1), obesity (3), type II diabetes, H. pylori, viral prodrome, and nephrolithiasis. The SEMA3B+/PLA2R− adolescent was pregnant, the NELL1+ adolescent was obese, and the two EXT2+ adolescents eventually met the clinical criteria for lupus (4, 9 years post-biopsy). Co-morbidities among the remaining 24 patients included remote hepatitis B (2), Down's syndrome, lysinuric protein intolerance, recurrent UTIs, hypothyroidism, pregnancy (3), and obesity (2). Follow-up data was available for 12 children and 16 adolescents. Of the 12 children, 6 achieved complete remission, 4 achieved partial remission, and 2 had no response to treatment (1 transplant). Of the 16 adolescents, 4 achieved complete remission, 4 achieved partial remission, and 8 had no response to treatment (3 transplants). A child with "full-house" immunofluorescence staining achieved spontaneous disease remission. Conclusion: Our non-lupus membranous nephropathy cohort represents one of the largest pediatric studies to date. Background: Proliferative glomerulonephritis with monoclonal IgG deposits (PGNMID) is a glomerular disease defined by non-organized glomerular deposits of heavy and light chain–restricted immunoglobulin and is rarely reported in children. Methods: We characterized a series of nine pediatric patients from two academic centers with biopsy-proven PGNMID and additionally describe two patients with monotypic IgG in the setting of IgM deposition. Results: Each patient presented with hematuria and/or proteinuria; however, only five had elevated serum creatinine. Prodromal or concurrent infection was identified in six patients, low C3 in five, and alternate complement pathway gene variants in two. No monoclonal serum proteins were identified in five tested patients. Seven patients had monotypic deposits composed of IgG3-λ, two showed IgG3-κ, and one each IgG1 and IgG3 with lambda dominance in the setting of IgM deposition. The glomerular pattern was predominantly mesangial proliferative or membranoproliferative glomerulonephritis (MPGN). Treatment and outcomes were variable; four patients have recent PGNMID diagnoses and therefore minimal follow up, one had relatively stable kidney function for over a decade, and six experienced kidney failure, with four receiving transplants. Recurrent deposits of the same isotype were identified in five of six transplanted kidneys, corresponding to three of four transplanted patients. One of these patients developed PGNMID recurrences in three separate kidney allografts over a 20-year disease course. Conclusions: Our study emphasizes the need for upfront IgG subclass investigation in pediatric mesangial or MPGN with IgG deposition and monotypic or biased light-chain staining. Background and Objective: Abaloparatide, an anabolic osteoporosis treatment administered by subcutaneous (SC) injection, increases bone mineral density (BMD) and reduces fracture risk in postmenopausal women with osteoporosis. The abaloparatide-solid Microstructured Transdermal System [abaloparatide-sMTS (Kindeva, St Paul, MN, USA)], which delivers abaloparatide intradermally, is in development to provide an alternative method for abaloparatide delivery. The objective of this study was to evaluate the ability of subjects to self-administer abaloparatide-sMTS, based on pharmacokinetic and pharmacodynamic markers. Methods: In this single-arm, open-label, Phase 1b study, 22 healthy postmenopausal women aged 50–85 years with low BMD were trained to self-administer abaloparatide-sMTS 300 μg once daily to the thigh for 5 min for 29 days. The primary endpoint was systemic exposure to abaloparatide. Secondary endpoints included percent change from baseline in serum procollagen type I N-terminal propeptide (s-PINP), patient experience, and safety. Results: All 22 subjects completed the study. At baseline, mean age was 65.2 years, mean total hip T-score was − 1.32, and mean lumbar spine T-score was − 1.98. On Day 1, the median time to reach maximum concentration (Tmax) for abaloparatide-sMTS was 0.33 h and geometric mean (CV %) maximum concentration (Cmax) and area under the concentration-time curve from time 0 to the time of the last quantifiable concentration (AUC0–t) were 447 (38.0) pg/mL and 678 (45.3) pg·h/mL, respectively; the pharmacokinetic profile was similar on Days 15 and 29. Median percentage change in s-PINP was 45.4% and 64.4% at Days 15 and 29, respectively. The most common adverse events (AEs) were application site erythema, pain, and swelling, which were mostly of mild or moderate severity. No AEs led to study drug withdrawal and no serious AEs were reported. The success rate for self-administration at first application was 99.7%, and subject acceptability was high (~ 4.5 on a 5-point Likert Scale). Conclusions: Subjects successfully self-administered abaloparatide-sMTS, which provided a consistent pharmacokinetic profile over 29 days and produced s-PINP increases from baseline similar to that observed in the pivotal trial with abaloparatide-SC. Treatment options for osteoporosis include drugs that prevent bone resorption and anabolic agents that build new bone. Bone anabolic agents, such as abaloparatide, have been shown to increase bone mineral density and reduce the risk of fracture in postmenopausal women with osteoporosis. Currently, all bone anabolic agents are delivered by subcutaneous injection. However, some patients do not like injectable treatments, which can negatively impact patients' adherence to prescribed medication. In this study, we describe a novel mode of administration, the abaloparatide-solid Microstructured Transdermal System (abaloparatide-sMTS), which is applied to the thigh for 5 min and delivers abaloparatide intradermally. The study showed that this new method delivered abaloparatide into the blood as effectively as subcutaneous injections and demonstrated signs of activity in the body. Study participants were satisfied with abaloparatide-sMTS and found it easy to use. The 2017 and 2018 Atlantic hurricane seasons poignantly illustrated the dangers tropical cyclones pose to US, Central American, and Caribbean coastlines. In particular, Hurricane Maria inflicted widespread damage, including catastrophic defoliation, to Puerto Rico, altering surface heat fluxes and possibly modifying precipitation patterns. This study assesses whether defoliation-driven changes to surface energy fluxes redistribute precipitation in the months following a powerful hurricane landfall. Remote sensing analyses of Maria-related vegetation reduction and recovery from Puerto Rico were adapted to the Georgia coastline. In this novel methodology, the resulting landscape evolution, characterized by an instantaneous vegetation reduction with a gradual recovery, was assimilated into the Weather Research and Forecasting model at a convection-allowing a 3-km grid spacing for the 1 June–1 August 2017 period. The experiment revealed that Maria-scale defoliation reduced precipitation by 14% during the month following landfall within a 50 × 50 km zone containing the hypothetical landfall location. A maximum deficit of 20.0% was reached 4 weeks after landfall. For June 2017, the modeled 14% deficit would have shifted the precipitation total from the 61st to the 47th percentile for years 1981–2016. Meanwhile, precipitation totals were unchanged on the domain scale. The near-landfall drying was also evident in three less-severe defoliation simulations, suggesting that systematic precipitation redistribution near the landfall location is possible following storms considerably weaker than Hurricane Maria. Analyses of the temperature and wind fields suggest that coastal kinematic flow is altered by the introduction of a thermally driven pressure gradient in the defoliated zone. [ABSTRACT FROM AUTHOR]

This study investigates differences in the word processing skills of students with and without reading difficulties who read Turkish, an entirely transparent orthography. Thirty-five students diagnosed as poor readers and 51 typically developing controls were tested across two experiments, one that assessed their ability to process identicalness of isolated real words as opposed to pseudowords and another that assessed their ability to judge semantic relatedness of two real words. Participants were from two education levels; half of them were 3rd-4th graders and half were 6th–7th graders. An integrative view of the findings points to an apparent failure of Turkish poor readers to develop a lexicalized reading route that mediates word recognition by means of permanent orthographic knowledge. Moreover, their ability to effectively process word letter sequences along a non-lexical grapheme-to-phoneme conversion-based reading route was found to be seriously restricted. Objectives: The International Continence Society defines nocturia as the need to void one or more times during the night, with each of the voids preceded and followed by sleep. The chronic sleep disturbance and sleep deprivation experienced by patients with nocturia affects quality of life, compromising both mental and physical well-being. This paper aims to characterise the burden of nocturia by comparing published data from patients with nocturia with data from patients with any of 12 other common chronic conditions, specifically focusing on its impact on work productivity and activity impairment, as measured by the instrument of the same name (WPAI).Methods: A systematic literature review of multiple data sources identified evaluable studies for inclusion in the analysis. Study eligibility criteria included use of the WPAI instrument in patients with one of a predefined list of chronic conditions. We assessed the quality of each included study using the Newcastle-Ottawa scale and extracted basic study information, work and activity impairment data. To assess how work and activity impairment from nocturia compares with impairment from other common chronic diseases, we conducted two data syntheses (pooled and unpooled).Results: The number of evaluable studies and the range of overall work productivity impairment reported, respectively, were as follows: nocturia (3; 14-39 %), overactive bladder (5; 11-41 %), irritable bowel syndrome/constipation (14; 21-51 %), gastroesophageal reflux disease (GERD) (13; 6-42 %), asthma/allergies (11; 6-40 %), chronic obstructive pulmonary disease (COPD) (7; 19-42 %), sleep problems (3; 12-37 %), arthritis (13; 21-69 %), pain (9; 29-64 %), depression (4; 15-43 %) and gout (2; 20-37 %).Conclusions: The overall work productivity impairment as a result of nocturia is substantial and was found to be similar to impairment observed as a result of several other more frequently researched common chronic diseases. Greater awareness of the burden of nocturia, a highly bothersome and prevalent condition, will help policy makers and healthcare decision makers provide appropriate management of nocturia. Spikes in the membrane potential of neurons comprise the currency of information processing in the brain. The ability of neurons to convert any information present across their multiple inputs into a significant modification to the pattern of their emitted spikes depends on the rate at which they emit spikes. If the mean rate is near the neuron's maximum, or if the rate is near zero, then changes in the inputs have minimal impact on the neuron's firing rate. Therefore, a neuron needs to control its mean rate. Protocols that either dramatically increase or decrease a neuron's firing rate lead to multiple compensatory changes that return the neuron's mean rate toward its prior value. In this primer, first as a summary of our previous work (Cannon and Miller in J Neurophysiol 116(5):2004-2022, 2016; Cannon and Miller in J Math Neurosci 7(1):1, 2017), we describe the advantages and disadvantages of having more than one such control mechanism responding to the neuron's firing rate. We suggest how problems of two, coexisting, potentially competing mechanisms can be overcome. Key requirements are: (1) the control be of a distribution of values, which the controlled variable achieves over a fast timescale compared to the timescale of the control system; (2) at least one of the control mechanisms be nonlinear; and (3) the two control systems are satisfied by a stable distribution or range of values that can be achieved by the variable. We show examples of functional control systems, including the previously studied integral feedback controller and new simulations of a "bang-bang" controller, that allow for compensation when inputs to the system change. Finally, we present new results describing how the underlying signal processing pathways would produce mechanisms of dual control, as opposed to a single mechanism with two outputs, and compare the responses of these systems to changes of input statistics. To investigate the potential of psychological capital as a resource for academic adjustment, 250 BA students were asked to complete two questionnaires, one assessing participants' psychological capacity, the other academic adjustment. Average grade-point scores were collected at two points in time as an additional measure of academic adjustment. Correlational as well as SEM analyses suggest that psychological capital is a positive resource with a central role in students' academic adjustment. Peatlands are important carbon reserves in terrestrial ecosystems. The microtopography of a peatland area has a strong influence on its carbon balance, determining carbon fluxes at a range of spatial scales. These patterned surfaces are very sensitive to changing climatic conditions. There are open research questions concerning the stability, behaviour and transformation of these microstructures, and the implications of these changes for the long-term accumulation of organic matter in peatlands. A simple two-dimensional peat microtopographical model was developed, which accounts for the effects of microtopographical variations and a dynamic water table on competitive interactions between peat-forming plants. In a case study of a subarctic mire in northern Sweden, we examined the consequences of such interactions on peat accumulation patterns and the transformation of microtopographical structure. The simulations demonstrate plausible interactions between peatland growth, water table position and microtopography, consistent with many observational studies, including an observed peat age profile from the study area. Our model also suggests that peatlands could exhibit alternative compositional and structural dynamics depending on the initial topographical and climatic conditions, and plant characteristics. Our model approach represents a step towards improved representation of peatland vegetation dynamics and net carbon balance in Earth system models, allowing their potentially important implications for regional and global carbon balances and biogeochemical and biophysical feedbacks to the atmosphere to be explored and quantified. Introduction: Despite the availability of multiple effective therapies, discontinuation/switching of treatment is common for many patients with rheumatoid arthritis (RA). This study was designed to examine initiation of biologic disease-modifying anti-rheumatic drugs (bDMARDs) within the Consortium of Rheumatology Researchers of North America (Corrona) RA Registry, and characterize reasons for discontinuation. Methods: Inclusion criteria were: Corrona-registered adults (≥18 years) with RA (2002-2011); age of RA onset: ≥16 years; ≥6 months' follow-up after initiation of first/subsequent bDMARD. Patients receiving both tumor necrosis factor antagonists and non-TNF antagonists were included. Treatment discontinuation was defined as first report of stopping initial therapy or initiation of new bDMARD at/between visits, using a follow-up physician questionnaire. Results: Overall, 6209 patients met inclusion criteria and 80.7% received TNF antagonists. Median time to discontinuation/change of therapy was 25.1 months (26.5 months with TNF antagonists vs. 20.5 months with non-TNF antagonists; log-rank p < 0.0001); 82.2, 67.3, and 51.1% of patients remained on therapy at 6, 12, and 24 months, respectively. Reasons for discontinuation were captured for 49.2% of patients, including: loss of efficacy (35.8%); physician preference (27.8%); safety (20.1%); patient preference (17.9%); and no access to treatment (9.0%). Baseline factors with greatest correlation to discontinuation were modified Health Assessment Questionnaire scores, patient-reported anxiety/depression, initiation of bDMARD treatment in 2007-2010 versus 2002-2003, and Clinical Disease Activity Index scores. Conclusions: Almost one-third of patients in the US discontinue currently available bDMARD therapies for RA by 12 months and almost half by 24 months, most commonly due to loss of efficacy. Homeostatic processes that provide negative feedback to regulate neuronal firing rates are essential for normal brain function. Indeed, multiple parameters of individual neurons, including the scale of afferent synapse strengths and the densities of specific ion channels, have been observed to change on homeostatic time scales to oppose the effects of chronic changes in synaptic input. This raises the question of whether these processes are controlled by a single slow feedback variable or multiple slow variables. A single homeostatic process providing negative feedback to a neuron's firing rate naturally maintains a stable homeostatic equilibrium with a characteristic mean firing rate; but the conditions under which multiple slow feedbacks produce a stable homeostatic equilibrium have not yet been explored. Here we study a highly general model of homeostatic firing rate control in which two slow variables provide negative feedback to drive a firing rate toward two different target rates. Using dynamical systems techniques, we show that such a control system can be used to stably maintain a neuron's characteristic firing rate mean and variance in the face of perturbations, and we derive conditions under which this happens. We also derive expressions that clarify the relationship between the homeostatic firing rate targets and the resulting stable firing rate mean and variance. We provide specific examples of neuronal systems that can be effectively regulated by dual homeostasis. Type-A γ-aminobutyric acid receptors (GABAARs) are the principal mediators of rapid inhibitory synaptic transmission in the human brain. A decline in GABAAR signalling triggers hyperactive neurological disorders such as insomnia, anxiety and epilepsy. Here we present the first three-dimensional structure of a GABAAR, the human β3 homopentamer, at 3 Å resolution. This structure reveals architectural elements unique to eukaryotic Cys-loop receptors, explains the mechanistic consequences of multiple human disease mutations and shows an unexpected structural role for a conserved N-linked glycan. The receptor was crystallized bound to a previously unknown agonist, benzamidine, opening a new avenue for the rational design of GABAAR modulators. The channel region forms a closed gate at the base of the pore, representative of a desensitized state. These results offer new insights into the signalling mechanisms of pentameric ligand-gated ion channels and enhance current understanding of GABAergic neurotransmission. [ABSTRACT FROM AUTHOR]

This study examines the links between inequality in the time allocations of partners in couple families and the satisfaction they have with the way they spend their time. It uses time diary and self-reported satisfaction data from the 2006 Australian Time Use Survey. It shows that inequality in the time allocations of partners in couple families affects the satisfaction with the current allocation of time in families without children, but not in families with children. Congenitally corrected transposition of the great arteries (ccTGA) is a rare malformation with diverse morphology. We assessed features of fetuses with ccTGA and evaluated neonatal and pediatric outcomes. This was a retrospective review of fetuses with ccTGA at Birmingham Women's and Children's Hospital born from 2005 to 2019. Of thirty-six fetuses identified, six had unavailable prenatal data, one was postnatally diagnosed with isomerism and 29 fetuses were evaluated. ccTGA without associated cardiac lesions was found in 28% (8/29), ccTGA with significant VSD in 31% (9/29), ccTGA with pulmonary obstruction in 24% (7/29) and ccTGA with complex anomalies in 17% (5/29). Tricuspid regurgitation (TR) was observed in 17% (5/29) and heart block (HB) in 10% (3/29) prenatally. Six, that is 21% underwent genetic testing of which one was abnormal. Five extra-cardiac anomalies were reported prenatally and postnatally. Pregnancy was discontinued in five, of which two had moderate TR. There were thirty-one liveborn. Coarctation of the aorta was found in five postnatally but not suspected prenatally. In one, pulmonary stenosis was underestimated; otherwise, prenatal morphology was confirmed. Cardiac interventions were performed in 77% (24/31) liveborn with 39% (12/31) undergoing neonatal intervention. Overall, 6/31 liveborn died including all three with prenatal heart block and one with TR. Estimated survival for all liveborn at 1, 5 and 10 years was 87% (95% CI 76–100%), 83% (95% CI 72–98%) and 80% (95% CI 66–96%) respectively. Accurate prenatal diagnosis of ccTGA is critical for counseling. Early outcomes are favorable with 77% of liveborn undergoing surgery. This study aimed to determine the factors underlying the reading comprehension (RC) failure of prelingually deaf Turkish readers. Participants were 77 individuals with prelingual deafness and a hearing control of 78 normally developing hearing individuals selected from three distinct levels of education (3rd-4th graders = elementary; 6th-7th graders = middle; 9th-10th graders = high). We applied an experimental paradigm manipulating the semantic plausibility and syntactic complexity of sentences to compare RC at the sentence level. In line with findings reported for prelingually deaf individuals reading in other orthographies, the deaf participants tested in the present study manifested rather alarmingly impoverished RC skills in comparison to their hearing counterparts. The findings suggest that this reading failure is rooted in an apparent deficiency in the ability to recruit syntactic and prior knowledge to make sense of what they read. Platinated triplex-forming oligonucleotides (TFOs) consisting of 2′-methoxythymidine and 2′-methoxy-5-methylcytidine and an N-7 platinated deoxyguanosine (G) at the 5′-(G-TFO), 3′-(TFO-G), or 3′- and 5′-(G-TFO-G) ends of the TFO form mono-(G-TFO and TFO-G) and interstrand (G-TFO-G) cross-links with target DNA as a result of reaction of the G with guanines adjacent to the homopurine TFO binding site in the target. The extent of cross-linking is greatest when the G is located on the 3′ end of the TFO and the target guanine is on the same strand as the TFO binding site. Multiple, contiguous deoxyguanosines in the TFO binding site or a cytosine adjacent to the G of the TFO significantly reduce cross-linking. DNA reporter plasmids in which platinated TFOs were cross-linked at a site in the transcribed region between a CMV promoter and a luciferase reporter gene were transfected into Chinese hamster ovary cells, and luciferase expression was compared with that for the corresponding non-cross-linked plasmid. Luciferase expression was inhibited 95 % when TFO-G was bound and cross-linked to the transcribed strand, demonstrating that the cross-linked TFO was able to block transcription elongation. Further inhibition (99 %) was observed in nucleotide excision repair (NER) deficient cells, suggesting that NER may repair this lesion. The 3′-G group of TFO-G protects the TFO from degradation by exonucleases found in mammalian serum. Taken together, these results suggest that platinated TFOs of the type TFO-G may find applications as agents for suppressing DNA transcription and consequently inhibiting gene expression in mammalian cells. This study was designed to clarify the nature of the mental representations underlying the processing of letters. A total of 96 Hebrew readers randomly recruited from three levels of education were asked to make rapid same/different judgments for Hebrew letter dyads with monosyllabic and bisyllabic names. The results obtained from the performance of participants under perceptual and conceptual processing conditions suggest that Hebrew readers access nominal letter representations in order to mediate letter processing in tasks that cannot be resolved on the basis of a sheer perceptual analysis of the letters' visual properties. The finding that the retrieval of nominal letter representations was evident for participants who differed rather markedly in their letter-processing speeds highlights the central role of letter names in the processing of isolated letters. Low bone mineral density (BMD) is a strong risk factor for low trauma fractures in the postmenopausal population without known chronic kidney disease (CKD). In stage 1-3 CKD, low BMD can also be used to predict fracture risk with the gradient of risk similar to patients without CKD even though patients with stage 3 CKD have an approximate doubling of risk compared with age-matched patients without CKD. This greater risk of fracture in stage 3 CKD is not calculated in the current FRAX model. In stage 4-5 CKD, BMD by dual-energy x-ray absorptiometry (DXA) is a poor predictor of fracture risk probably related to the severe derangements in bone metabolism in severe CKD, which alter bone quality and strength not measured by DXA. Serial BMD by DXA, however, may be useful in all stages of CKD to monitor for potential loss of BMD or effect of pharmacological agents to improve BMD. Newer radiological technologies, particularly high-resolution peripheral quantitative computerized tomography (HRpQcT) of the radius and tibia show promise to define the microstructural changes in bone that explain the greater risk of fracture observed in patients with CKD versus patients without CKD. BMD by DXA may still be of value across the spectrum of CKD, but physicians should realize its limitations and understand the greater risk of fracture in patients in all stages of CKD as compared to age-matched and BMD-matched patients without CKD. The Arctic land area has warmed by >1 °C in the last 30 years and there is evidence that this has led to increased productivity and stature of tundra vegetation and reduced albedo, effecting a positive (amplifying) feedback to climate warming. We applied an individual-based dynamic vegetation model over the Arctic forced by observed climate and atmospheric CO for 1980-2006. Averaged over the study area, the model simulated increases in primary production and leaf area index, and an increasing representation of shrubs and trees in vegetation. The main underlying mechanism was a warming-driven increase in growing season length, enhancing the production of shrubs and trees to the detriment of shaded ground-level vegetation. The simulated vegetation changes were estimated to correspond to a 1.75 % decline in snow-season albedo. Implications for modelling future climate impacts on Arctic ecosystems and for the incorporation of biogeophysical feedback mechanisms in Arctic system models are discussed. [ABSTRACT FROM AUTHOR]

A review of research investigating prelingually deaf individuals' word reading skills, phonemic awareness, and reading comprehension challenges the widely held conclusion that the impoverished readings skills of such individuals are the result of poor phonological processing abilities. Compelling findings show that-notwithstanding often remarkably poor phonemic awareness of the prelingually deaf-such readers succeed in developing word-reading strategies that sustain written word recognition at comparable levels to their hearing counterparts. Moreover, evidence suggests no direct causal relationship between their sensitivity to the phonological properties of words and their ability to comprehend connected text. These findings further indicate that such readers may gain from the development of orthographic knowledge in conjunction with syntactic awareness and metacognitive skills but not necessarily from the sheer fostering of their phonemic/phonological awareness. This study investigates the importance of vowel diacritics for the retention of Hebrew word lists, with word lists being manipulated along the dimension of word frequency and syllabic length. Eighty university students participated in the study. Half of the participants (40) were tested with the word lists presented in fully-pointed (voweled) Hebrew while the other half (40) were given the word lists in unpointed Hebrew (with vowel diacritics removed). Analyses of each group's recall rates and recall-order accuracy indicate that, overall, the presence of vowel diacritics had no facilitating effect on the participants' quantitative and qualitative Short Term Memory (STM) performance. This was found to be true whether word frequency (high vs. medium to low) or syllabic length (mono vs. trisyllabic) were computed as within-subject factors. Attempts to adequately interpret the absence of a pointing effect on STM performance based upon Orthographic Depth Hypothesis (ODH) and Grain Size Theory (GST) proved unfruitful. This study seeks to provide new insight into the phonemic, orthographic, and syntactic awareness of individuals with prelingual deafness and the way those contribute to reading. Two tests were used: one designed for the assessment of phonemic/orthographic awareness (PO/OA) and another examining reading comprehension (RC) in contexts where prior knowledge was either helpful or not. Participants were 83 prelingually deaf individuals (DIs): 21 primary school, 36 high school, and 26 university students. The control group consisted of 85 hearing individuals (HIs) from parallel education levels (29 primary school, 29 high school, 27 university). Contrary to predictions made by current reading theories, findings imply that the failure of DIs to develop sensitivity to the phonological properties of words may not underlie their reading difficulties. Rather, this weakness seems to reflect a processing deficit at the supra-lexical (sentence) level where the final meaning of single words is elaborated by its integration based upon syntactic (structural) knowledge. The relationship between bisphosphonate-induced bone mineral density (BMD) gains and antifracture efficacy remains to be fully elucidated. Data from two antifracture studies were analyzed. Postmenopausal osteoporotic women received oral (2.5 mg daily, 20 mg intermittent) or intravenous (0.5 mg, 1 mg quarterly) ibandronate. Outcome measures included moving averages plots and logistic regression analyses of the relationship between BMD change and vertebral fracture rate. In moving averages plots, ibandronate-induced BMD gains were consistently associated with decreased fracture rates. In the oral study, total-hip BMD increases at years 2 and 3 and lumbar spine BMD increases at year 3 were associated with 3-year vertebral fracture rate (relative risk reduction [RRR] at year 3 for 1% change from baseline: hip, 7.9% [95% CI 2.1-13.5%, P = 0.0084]; lumbar spine, 4.7% [-0.1% to 9.3%, P = 0.0565]). In the intravenous study, total-hip BMD increases at years 1, 2, and 3 and lumbar spine BMD increases at years 2 and 3 were significantly associated with vertebral fracture rate (RRR at year 3 for 1% change from baseline: hip, 11.6% [7.0-16.0%, P < 0.0001]; lumbar spine, 6.9% [2.9-10.6%, P = 0.0008]). In a pooled analysis, changes in total-hip and lumbar spine BMD were associated with 3-year vertebral fracture risk reduction and explained a substantial proportion of the antifracture effect (23-37% at 2 and 3 years). Climate reconstructions from data sensitive to past climates provide estimates of what these climates were like. Comparing these reconstructions with simulations from climate models allows to validate the models used for future climate prediction. It has been shown that for fossil pollen data, gaining estimates by inverting a vegetation model allows inclusion of past changes in carbon dioxide values. As a new generation of dynamic vegetation model is available we have developed an inversion method for one model, LPJ-GUESS. When this novel method is used with high-resolution sediment it allows us to bypass the classic assumptions of (1) climate and pollen independence between samples and (2) equilibrium between the vegetation, represented as pollen, and climate. Our dynamic inversion method is based on a statistical model to describe the links among climate, simulated vegetation and pollen samples. The inversion is realised thanks to a particle filter algorithm. We perform a validation on 30 modern European sites and then apply the method to the sediment core of Meerfelder Maar (Germany), which covers the Holocene at a temporal resolution of approximately one sample per 30 years. We demonstrate that reconstructed temperatures are constrained. The reconstructed precipitation is less well constrained, due to the dimension considered (one precipitation by season), and the low sensitivity of LPJ-GUESS to precipitation changes. Using experimental, observational and interview-assessment methods, we examined relations among mother–child discussion skills and suggested strategies for coping with postdivorce interparental conflict in a conflict task, children’s memory for those strategies in a later recall interview, and children’s self-reported use of coping strategies in response to parental conflict at home. Participants included 50, 9–12 year-old children (50% female, 11% Mexican-American, 81% Euro-American, 8% other) and their mothers. Results indicated that the frequency of suggested coping strategies in the conflict task significantly predicted children’s later recall of the same strategies, and recalled strategies significantly predicted children’s self-reported use of the same strategies at home. Mothers’ and children’s discussion skills were less robust predictors of coping, except for secondary control and disengagement coping at-home. The present study was designed to further clarify whether impoverished phonological skills of prelingually deaf readers undermine their ability to process written words. The word reading skills of forty students with prelingual deafness recruited from two levels of education (primary school, mid/high school) and a chronologically age-matched hearing control group—all of them reading Modern Standard Arabic (MSA) in a diglossic context—were examined with a research paradigm that required the categorization of written real words and paralleling pseudo-homophones. In general, findings suggest that despite severely impaired phonological decoding skills, prelingually deaf MSA readers eventually succeed to develop word reading skills that sustain word recognition with hearing-comparable efficiency. Linguistic representations underlying these skills seem to be of a nonphonological, most likely orthographic nature. Findings are discussed with reference to the validity of reading theories that assign a central role to phonology in the explanation of reading failure in individuals with prelingual deafness. These scores are linked to employed native and foreign-born men in the 2000 Census. Earnings increase with the respondent’s proficiency in English, with the English proficiency required for the occupation, and when those with high levels of proficiency work in jobs requiring English-language skills (interaction effect). Microtubules are indispensable for Golgi complex assembly and maintenance, which are integral parts of cytoplasm organization during interphase in mammalian cells. Here, we show that two discrete microtubule subsets drive two distinct, yet simultaneous, stages of Golgi assembly. In addition to the radial centrosomal microtubule array, which positions the Golgi in the centre of the cell, we have identified a role for microtubules that form at the Golgi membranes in a manner dependent on the microtubule regulators CLASPs. These Golgi-derived microtubules draw Golgi ministacks together in tangential fashion and are crucial for establishing continuity and proper morphology of the Golgi complex. We propose that specialized functions of these two microtubule arrays arise from their specific geometries. Further, we demonstrate that directional post-Golgi trafficking and cell migration depend on Golgi-associated CLASPs, suggesting that correct organization of the Golgi complex by microtubules is essential for cell polarization and motility. Triplex-forming oligonucleotides (TFOs) can bind specifically to polypurine sequences in double-stranded DNA. A single interruption of this polypurine tract can greatly destabilize triplex formation. The stability of triplexes can be significantly enhanced by covalently linking the TFO to its DNA target with reactive functional groups conjugated to the TFO. Covalently cross-linked TFOs are effective inhibitors of transcription of the target DNA sequence. We have designed a TFO with a platinum-modified base that can interact with and cross-link to a cytosine interruption in the polypurine tract of a target DNA duplex. The TFO contains an N4-(aminoalkyl)cytosine derivatized with cis-diamminediaquaplatinum(II) or trans-diamminediaquaplatinum(II). When bound to its target, the tethered platinum of the TFO can reach across the major groove and form an adduct with the guanine N7 of the interrupting C·G base pair. The optimal tether length is five methylene groups, and cross-linking is most efficient when the tether is modified with trans-diamminediaquaplatinum(II). Cross-linking requires that the TFO is bound to its designated DNA target. Addition of cyanide to the cross-linked TFO product reversed the cross-link, behavior that is consistent with the presence of a platinum–guanine adduct. The kinetics of the cross-linking reaction were studied and the half-life of the cross-linking reaction was approximately 3 h. Our results demonstrate that platinum-conjugated TFOs can be designed to cross-link with DNA targets that contain a single pyrimidine interruption. Modifications of this type may prove useful for expanding the DNA sequences that can be targeted by TFOs and increasing the stability of the resulting triplexes. [ABSTRACT FROM AUTHOR]

The aim of the study was to elucidate the nature and efficiency of the strategies that readers with phonological dyslexia use for temporary retention of written words in Working Memory (WM). Data was gathered through a paradigm whereby participants had to identify serially presented written (target) words from within larger word pools according to their presentation order, with word pools containing code-specific distracter (CSD) words and non-code-specific distracter (NCSD) words. Analyses focused on three aspects of performance: (1) false recognition of target words; (2) correct recognition of target words; and (3) retention of word presentation order. Participants were readers with diagnosed phonological dyslexia ( n = 20, mean grade level = 9.05 [0.89]) and a control group of regular readers ( n = 25, mean grade level = 9.00 [0.76]). Results provide direct evidence that the dyslexic readers and the regular readers used essentially different memory coding strategies for the temporary retention of written words, with the former predominantly relying on a visual strategy and the latter on a phonological strategy. Findings further pinpointed a notably impoverished ability of the dyslexic readers to retain word presentation order. For an intense pulsed light (IPL) device, knowledge of the spectral output is useful in order to provide effective treatment and target specific structures in the skin. It is also a requirement in order to perform a safety assessment. A novel spectral measurement system has been developed to detect the optical radiation output of intense pulsed light devices. The system has a time resolution of 450 mus and a wavelength resolution of 0.6 nm. This enabled us to observe spectral changes, both within a pulse and between pulses, in a pulse train. The output from a free discharge IPL source and four different treatment handpieces was measured. A shift in the spectral distribution between pulses, and within a pulse, was discovered. Cys-loop ligand-gated ion channels mediate rapid neurotransmission throughout the central nervous system. They possess agonist recognition sites and allosteric sites where modulators regulate ion channel function. Using strychnine-sensitive glycine receptors, we identified a scaffold of hydrophobic residues enabling allosteric communication between glycine-agonist binding loops A and D, and the Zn2+-inhibition site. Mutating these hydrophobic residues disrupted Zn2+ inhibition, generating novel Zn2+-activated receptors and spontaneous channel activity. Homology modeling and electrophysiology revealed that these phenomena are caused by disruption to three residues on the '−' loop face of the Zn2+-inhibition site, and to D84 and D86, on a neighboring β3 strand, forming a Zn2+-activation site. We provide a new view for the activation of a Cys-loop receptor where, following agonist binding, the hydrophobic core and interfacial loops reorganize in a concerted fashion to induce downstream gating. This study seeks to clarify the relation between the phonological skills of both dyslectic readers and prelingually deafened readers and their ability to conceptually process written stimuli. Data was gathered by means of a test designed for the assessment of orthographic and phonemic awareness, and a categorization paradigm designed for the examination of written word processing skills. Twenty individuals with diagnosed dyslexia (grade = 9.05), 11 individuals with prelingual deafness (grade = 8.18), and 25 normally-developing hearing readers ( n = 25, grade = 9.00) participated in the study. In general, findings indicate that the reading disorders manifested by dyslectic and deaf readers have different origins none of which is directly related to their phonological abilities. It is suggested that in prelingually deafened readers, but maybe also in dyslectic readers, teachers should foster the development of orthographic knowledge as the basis for proficient reading without making such development contingent on the processing of the phonology of written words. The objective of this study was to clarify the nature and efficiency of the word recognition skills of prelingually deafened Hebrew readers and the way these skills relate to their reading comprehension level. Relevant data were gathered by means of a research paradigm demanding the same/different categorization of phonologically or formationally manipulated word pairs and by a sentence comprehension test (SCT). Participants were prelingually deafened individuals ( n=22, mean grade=8.7) who attested to using sign language as their preferred means for communication, and a task-matched hearing control group ( n=40, mean grade=8.9). In general, findings suggest that both hearing readers and readers with prelingual deafness rely upon orthographic knowledge rather than upon their primary language for mediating the processing of written words in the working memory (WM). As predicted, in comparison to their hearing counterparts, the reading comprehension of the prelingually deafened participants, as a group, was significantly impaired. Comparative analyses of quantitative and qualitative aspects of the same/different categorizations of word pairs by the participant groups, and the way these correlated with performance on the SCT clearly indicated, however, that this weakness is not rooted in poor word recognition skills but probably reflects a lack of syntactic knowledge crucial for the adequate, post-lexical processing of recognized words. Finally, the findings convincingly show that, despite their profound hearing loss, some individuals who were prelingually deaf acquired rule-based knowledge for the adequate processing of written text. The experience in randomized placebo-controlled clinical trials may differ from that in community practice. The pivotal teriparatide [rhPTH(1–34)] studies were initiated when few therapeutic options for osteoporosis were available. The Direct Analysis of Non-Vertebral Fractures in Community Experience (DANCE) study is a prospective observational trial designed to examine the occurrence of nonvertebral fragility fractures in a large, diverse patient population treated with teriparatide. The occurrence of clinical vertebral fractures and back pain will also be examined, as will bone mineral density, bone mineral content, bone area, safety and tolerability. Subjects will be followed through a course of teriparatide therapy for up to 24 months and for an additional 24 months after cessation of treatment. Therefore, subjects may participate in this study for up to 48 months. DANCE will provide data on the effectiveness and tolerability of teriparatide therapy in clinical practice that will complement the results of published controlled clinical trials. The present experiment represents an attempt to learn from the comparison of dyslectic readers, prelingually deafened readers, and a control group of typically developing hearing readers, in four particular areas, about factors determining their reading skills.Twenty-three students with diagnosed dyslexia (mean grade level 9.7 [1.53]), 20 students with prelingual deafness (mean grade level 8.9 [1.33]), and a control of 41 typically developing hearing students (mean grade level 8.9 [1.43]) participated in the study.Four distinct areas were examined which included: (a) the rapid, repetitive execution of two simple, familiar motor actions; (b) the rapid, consecutive judgment of the identicalness of two digit stimuli; (c) the rapid, consecutive, perceptual processing of word pairs; and (d) the rapid, consecutive, conceptual processing of word pairs. Pharmacoeconomics is vitally important to drug manufacturers in terms of communicating to external decision-makers (payers, prescribers, patients) the value of their products, achieving regulatory and reimbursement approval and contributing to commercial success. Since development of new drugs is long, costly and risky, and decisions must be made how to allocate considerable research and development (R&D) resources, pharmacoeconomics also has an essential role informing internal decision-making (within a company) during drug development. The use of pharmacoeconomics in early development phases is likely to enhance the efficiency of R&D resource use and also provide a solid foundation for communicating product value to external decision-makers further downstream, increasing the likelihood of regulatory (reimbursement) approval and commercial success. This paper puts the case for use of pharmacoeconomic analyses earlier in the development process and outlines five techniques (clinical trial simulation [CTS], option pricing [OP], investment appraisal [IA], threshold analysis [TA] and value of information [VOI] analysis) that can provide useful input into the design of clinical development programmes, portfolio management and optimal pricing strategy. CTS can estimate efficacy and tolerability profiles before clinical data are available. OP can show the value of different clinical programme designs, sequencing of studies and stop decisions. IA can compare expected net present value (NPV) of different product profiles or study designs. TA can be used to understand development drug profile requirements given partial data. VOI can assist risk management by quantifying uncertainty and assessing the economic viability of gathering further information on the development drug. No amount of pharmacoeconomic data can make a bad drug good; what it can do is enhance the drug developers understanding of the characteristics of that drug. Decision-making, in light of this information, is likely to be better than that without it, whether it leads to faster termination of uneconomic projects or the allocation of more appropriate resources to attractive projects. Objective: To evaluate the efficacy and tolerability of alendronic acid 70mg once weekly for the treatment of male osteoporosis. Patients and methods: This randomised, double-blind, placebo-controlled, 12-month trial compared the effect of alendronic acid 70mg once weekly or placebo (randomised 2 : 1) on bone mineral density (BMD) in 167 men with spine or hip BMD at least 2 standard deviations (SD) below the mean for young normal white males or nontraumatic fracture. All patients received calcium and vitamin D (colecalciferol). We measured lumbar spine, hip and total body BMD, and biochemical markers of bone turnover. Fractures were collected as adverse events. Results: Alendronic acid 70mg once weekly produced significant BMD increases from baseline of 4.3% at the spine, 2.1% at the femoral neck, 2.4% at the trochanter, and 1.4% at the total body, which were all significantly greater than placebo (p < 0.05). The increase at the lumbar spine was significant relative to baseline and placebo after 6 months of treatment (p < 0.001). The treatment effect was consistent regardless of BMD, age, height, weight, body mass index (BMI) and hypogonadal status at baseline. Alendronic acid significantly decreased biochemical markers of bone turnover relative to baseline and placebo. Alendronic acid was generally well tolerated, with an incidence of gastrointestinal adverse events similar to placebo. We develop a model using human capital theory and an immigrant adjustment process to generate hypotheses on the acquisition of destination-language skills among immigrants. The model is tested for adult male immigrants in the 1991 Census of Canada. Use of English or French is greater, the younger the age at migration, the longer the duration of residence, the higher the educational attainment, the farther the country of origin from Canada, and the linguistically closer the mother tongue to English or French, and among those who are not refugees, those from a former British, French, or American colony, and those who live in an area where fewer people speak the respondent's mother tongue. This paper uses the data on males and females from the 1989 Legalized Population Survey (LPS), a sample of aliens granted amnesty under 1986 Immigration Reform and Control Act, to analyse English language proficiency and earnings. We use a model of English language proficiency that is based on economic incentives, exposure and efficiency variables that measure the costs and benefits of aquiring English language skills. Consistent with the model, in this sample of former illegal aliens, English language proficiency is greater for those with more schooling, who immigrated at a younger age, who have been in the United States longer, with a more continuous stay, and who have less access to other origin language speakers where they live. Major recurring themes of poststructuralist thought are articulated at the beginning of the 1960's in Lacan's commentaries on Sophocles' Antigone and Plato's Symposium. Derrida's reading of the Phaedrus as well as his citation of the Platonic tradition in his dispute with Foucault over the latter's Histoire de la folie further confirm the importance of the Hellenic tradition to poststructuralism's engagement with occidental philosophy. Finally, Foucault's late turn to Stoicism and the philosophers of the Roman imperial period can be seen as a response to and criticism of the positions established by Lacan and Derrida. Human visual systems have evolved to extract ecologically relevant information from complex scenery. In some cases, the face in the crowd visual search task demonstrates an anger superiority effect, where anger is allocated preferential attention. Across three studies (N = 419), we tested whether facial hair guides attention in visual search and influences the speed of detecting angry and happy facial expressions in large arrays of faces. In Study 1, participants were faster to search through clean-shaven crowds and detect bearded targets than to search through bearded crowds and detect clean-shaven targets. In Study 2, targets were angry and happy faces presented in neutral backgrounds. Facial hair of the target faces was also manipulated. An anger superiority effect emerged that was augmented by the presence of facial hair, which was due to the slower detection of happiness on bearded faces. In Study 3, targets were happy and angry faces presented in either bearded or clean-shaven backgrounds. Facial hair of the background faces was also systematically manipulated. A significant anger superiority effect was revealed, although this was not moderated by the target's facial hair. Rather, the anger superiority effect was larger in clean-shaven than bearded face backgrounds. Together, results suggest that facial hair does influence detection of emotional expressions in visual search, however, rather than facilitating an anger superiority effect as a potential threat detection system, facial hair may reduce detection of happy faces within the face in the crowd paradigm. Recent research has suggested that individual differences in young children's capacity for empathy and altruism has heritable components and may be related to early temperament. These individual differences, like other human capacities such as intelligence, may be distributed normally. Thus, “high-empathy” children may exhibit cognitive, emotional, and behavioral precocity in empathic and altruistic responding, and begin expressing these capacities early in life. Twenty-seven 2- to 8-year-old ( M = 53 months), primarily Euro-American high-empathy children (10 male) were identified by childcare center staff. Ethnographically oriented interviews of their mothers provided detailed accounts of the children's empathic and altruistic responding in everyday interactions at home and school. As a group, the children were described as emotionally positive, especially sociable individuals whose empathic and altruistic responses were highly spontaneous, occurred more frequently, and differed from peers in depth, intensity, and persistence. Parallels were drawn between these capacities and temperamental dispositions for self-regulation and emotional reactivity. Neural populations with strong excitatory recurrent connections can support bistable states in their mean firing rates. Multiple fixed points in a network of such bistable units can be used to model memory retrieval and pattern separation. The stability of fixed points may change on a slower timescale than that of the dynamics due to short-term synaptic depression, leading to transitions between quasi-stable point attractor states in a sequence that depends on the history of stimuli. To better understand these behaviors, we study a minimal model, which characterizes multiple fixed points and transitions between them in response to stimuli with diverse time- and amplitude-dependencies. The interplay between the fast dynamics of firing rate and synaptic responses and the slower timescale of synaptic depression makes the neural activity sensitive to the amplitude and duration of square-pulse stimuli in a nontrivial, history-dependent manner. Weak cross-couplings further deform the basins of attraction for different fixed points into intricate shapes. We find that while short-term synaptic depression can reduce the total number of stable fixed points in a network, it tends to strongly increase the number of fixed points visited upon repetitions of fixed stimuli. Our analysis provides a natural explanation for the system's rich responses to stimuli of different durations and amplitudes while demonstrating the encoding capability of bistable neural populations for dynamical features of incoming stimuli. Neural circuits with multiple discrete attractor states could support a variety of cognitive tasks according to both empirical data and model simulations. We assess the conditions for such multistability in neural systems using a firing rate model framework, in which clusters of similarly responsive neurons are represented as single units, which interact with each other through independent random connections. We explore the range of conditions in which multistability arises via recurrent input from other units while individual units, typically with some degree of self-excitation, lack sufficient self-excitation to become bistable on their own. We find many cases of multistability—defined as the system possessing more than one stable fixed point—in which stable states arise via a network effect, allowing subsets of units to maintain each others' activity because their net input to each other when active is sufficiently positive. In terms of the strength of within-unit self-excitation and standard deviation of random cross-connections, the region of multistability depends on the response function of units. Indeed, multistability can arise with zero self-excitation, purely through zero-mean random cross-connections, if the response function rises supralinearly at low inputs from a value near zero at zero input. We simulate and analyze finite systems, showing that the probability of multistability can peak at intermediate system size, and connect with other literature analyzing similar systems in the infinite-size limit. We find regions of multistability with a bimodal distribution for the number of active units in a stable state. Finally, we find evidence for a log-normal distribution of sizes of attractor basins, which produces Zipf's Law when enumerating the proportion of trials within which random initial conditions lead to a particular stable state of the system.
