Pseudoaneurysm occurs when the artery wall is damaged and the blood is contained by the surrounding tissues with the eventual formation of a fibrous sac communicating with the artery. We report a case of a 74-year-old man with inferior epigastric artery (IEA) pseudoaneurysm secondary to an 8-mm port placement during a robot-assisted laparoscopic radical cystectomy with ureteroileocutaneostomy. The pseudoaneurysm was initially diagnosed by contrast-enhanced ultrasound (CEUS); subsequently, a computed tomography (CT) scan and an angiography test were performed. The pseudoaneurysm was then treated successfully with embolization of the inferior epigastric artery. Awareness of this rare complication is of clinical importance to avoid excessive morbidity of affected individuals. [ABSTRACT FROM AUTHOR]

The load sensitivity of the relaxation phase was studied in rat papillary muscle, with isotonic afterloaded contractions and stretches applied after the peak of isometric twitches. The tension decay occurred earlier in isotonic than in isometric contractions. When a central region of the preparation was marked with small stainless steel pins, a lengthening of this region could be shown during relaxation of isometric (fixed end) contractions. This lengthening was earlier and faster in isotonic afterloaded contractions. Therefore the sensitivity of relaxation to load or length changes could be described in the context of the general mechanism of relaxation which takes into account the non uniform behaviour of the muscle and the internal movement during contractions. Interventions which decelerate the activation decay rate had different effects on the load dependence of relaxation. Caffeine addition and substitution of strontium for calcium abolished the load sensitivity while a temperature reduction had no influence on it. [ABSTRACT FROM AUTHOR]

Shortening deactivation is studied in isolated preparations of rat cardiac muscle by comparing tension redevelopment in partially fused isometric contractions with quick releases of different extent ('control' and 'test' displacements). The active shortening takes place during the ascending phase of the 1 contraction of the couple and the comparison of redeveloped tension is carried out at the peaks of both 1 and 2 contractions. The depressant effect of active shortening can be demonstrated at the 1 peak and also at the 2 one, after the restimulation of the muscle. This effect slowly decreases with time and disappears within approximately one second after active shortening took place. By keeping constant the control shortening and changing the test shortening, a clear dependence of depressant effect on the amount of displacement can be shown in the 1 and in the 2 contraction. The choice of a suitable muscle model consistent with the mechanical behaviour of Rat papillary muscle at rest or during isotonic or isometric contraction has been considered. Three different preload levels within the ascending limb of the Frank-Starling curve have been used in five papillary muscles. Series elastic (SE) and parallel elastic (PE) length-tension relationships have been evaluated according to five mechanical muscle models using data from systolic and diastolic quick-release manoeuvres. For each preload level and muscle model, the time course of the force-velocity relationship of the contractile element (CE) has been calculated using a PDP 8/L digital computer. Several mechanical and biological characteristics are considered which exclude four of the five selected muscle models. The Maxwell model with a damper seems to be the most suitable mechanical model for Rat papillary muscle. According to the selected muscle model it seems that Vmax is dependent on muscle length variation; the reliability of this parameter as an index of myocardial contractility is discussed. In order to observe an early change in protein synthesis in response to a rise in cardiac work load, incorporation of leucine and lysine into cardiac protein was studied in the rat heart-lung preparation. The following results were obtained. The incorporation of lysine into the trichloracetic acid precipitable fraction was less than that of leucine. The rate of leucine incorporation into left ventricular protein was higher (49.9 m μ/g min vs. 35.5 m μg/g min) at a high cardiac power level (31.2 g m/min) than at a low level (6.5 g m/min). A similar difference was observed in the right ventricle. Since the work level of the right ventricle is lower than that of the left, the incorporation per unit increase in cardiac power and work was larger in the right than in the left ventricle. However, the relative increase in incorporation expressed in percentage of that found at low power and work levels was of a similar magnitude between the two ventricles. The incorporation was unaffected by pretreatment with actinomycin (1 mg/kg) but was greatly inhibited by puromycin (10 mg/100 ml) or cycloheximide (25 mg/kg). It is proposed that this relationship between the amino acid incorporation and the levels of cardiac load may be the earliest signal in the development of cardiac hypertrophy. [ABSTRACT FROM AUTHOR]

When caffeine plus calcium is added to the perfusing medium, isometric relaxation of rat myocardium is no longer affected by length changes occurring during the twitch. Serum thyroid hormone concentrations and their metabolic fate are within the normal range limits in obese subjects. Also serum TSH concentrations and its response to TRH are normal, suggesting that tissue availability of thyroid hormones is normally preserved in these subjects. In contrast, during caloric restriction serum T3 concentrations decrease as a consequence of its reduced production rate from peripheral deiodination of T4. Opposite, serum rT3 concentrations markedly increase as a result of its decreased metabolic clearance rate. During caloric overfeeding serum T3 concentration increase whereas serum rT3 concentrations decrease. In this condition the production rate of T3 increases.
