Purpose: Sexual and gender minority (SGM) populations experience cancer treatment and survival disparities; however, inconsistent sexual orientation and gender identity (SOGI) data collection within clinical settings and the cancer surveillance system precludes population-based research toward health equity for this population. This qualitative study examined how hospital and central registry abstractors receive and interact with SOGI information and the challenges that they face in doing so. Methods: We conducted semi-structured interviews with 18 abstractors at five Surveillance, Epidemiology, and End Results (SEER) registries, as well as seven abstractors from commission on cancer (CoC)-accredited hospital programs in Iowa. Interviews were transcribed, cleaned, and coded using a combination of a priori and emergent codes. These codes were then used to conduct a descriptive analysis and to identify domains across the interviews. Results: Interviews revealed that abstractors had difficulty locating SOGI information in the medical record: this information was largely never recorded, and when included, was inconsistently/not uniformly located in the medical record. On occasion, abstractors reported situational recording of SOGI information when relevant to the patient's cancer diagnosis. Abstractors further noticed that, where reported, the source of SOGI information (i.e., patient, physician) is largely unknown. Conclusion: Efforts are needed to ensure standardized implementation of the collection of SOGI variables within the clinical setting, such that this information can be collected by the central cancer registry system to support population-based equity research addressing LGBTQ + disparities. The periodic trend to cetacean mass stranding events in the Australian island state of Tasmania remains unexplained. This article introduces the hypothesis that domoic acid poisoning may be a causative agent in these events. The hypothesis arises from the previously evidenced role of aeolian dust as a vector of iron input to the Southern Ocean; the role of iron enrichment in Pseudo- nitzschia bloom proliferation and domoic acid production; and importantly, the characteristic toxicosis of domoic acid poisoning in mammalian subjects leading to spatial navigation deficits. As a pre-requisite for quantitative evaluation, the plausibility of this hypothesis was considered through correlation analyses between historical monthly stranding event numbers, mean monthly chlorophyll concentration and average monthly atmospheric dust loading. Correlation of these variables, which under the domoic acid stranding scenario would be linked, revealed strong agreement (r = 0.80-0.87). Background/objectives: Sugar intake may be causally associated with chronic disease risk, either directly or by contributing to obesity. However, evidence from observational studies is mixed, in part due to the error and bias inherent in self-reported measures of sugar intake. Objective biomarkers may clarify the relationship between sugar intake and chronic disease risk. We have recently validated a biomarker of sugar intake in an Alaska Native (Yup'ik) study population that incorporates red blood cell carbon and nitrogen isotope ratios in a predictive model. This study tested associations of isotopic estimates of sugar intake with body mass index (BMI), waist circumference (WC) and a broad array of other physiological and biochemical measures of chronic disease risk in Yup'ik people.Subjects/methods: In a cross-sectional sample of 1076 Yup'ik people, multiple linear regression was used to examine associations of sugar intake with BMI, WC and other chronic disease risk factors.Results: Isotopic estimates of sugar intake were not associated with BMI (P=0.50) or WC (P=0.85). They were positively associated with blood pressure, triglycerides (TG) and leptin, and are inversely associated with total-, high-density lipoprotein- and low-density lipoprotein-cholesterol and adiponectin.Conclusions: Isotopic estimates of sugar intake were not associated with obesity, but were adversely associated with other chronic disease risk factors in this Yup'ik study population. This first use of stable isotope markers of sugar intake may influence recommendations for sugar intake by Yup'ik people; however, longitudinal studies are required to understand associations with chronic disease incidence. Clock gene anomalies have been suggested as causative factors in autism. We screened eleven clock/clock-related genes in a predominantly high-functioning Autism Genetic Resource Exchange sample of strictly diagnosed autistic disorder progeny and their parents (110 trios) for association of clock gene variants with autistic disorder. We found significant association (P<0.05) for two single-nucleotide polymorphisms in per1 and two in npas2. Analysis of all possible combinations of two-marker haplotypes for each gene showed that in npas2 40 out of the 136 possible two-marker combinations were significant at the P<0.05 level, with the best result between markers rs1811399 and rs2117714, P=0.001. Haplotype analysis within per1 gave a single significant result: a global P=0.027 for the markers rs2253820–rs885747. No two-marker haplotype was significant in any of the other genes, despite the large number of tests performed. Our findings support the hypothesis that these epistatic clock genes may be involved in the etiology of autistic disorder. Problems in sleep, memory and timing are all characteristics of autistic disorder and aspects of sleep, memory and timing are each clock-gene-regulated in other species. We identify how our findings may be relevant to theories of autism that focus on the amygdala, cerebellum, memory and temporal deficits. BACKGROUND: Argon plasma coagulation (APC) has been used to trim uncovered Elgiloy® stents, but the extent of thermal damage and bile duct injury is not known. The goal of this study was to evaluate the safety and efficacy of APC for this application. METHODS: Eight Elgiloy® (covered and uncovered) and four nitinol stents were deployed in the bile duct at ERCP in 12 anesthetized pigs. In nine pigs, the excess distal ends were trimmed in vivo using short bursts of APC. Three pigs served as controls. Bile ducts and stent specimens were then harvested for gross and histological examinations by a single-blinded pathologist. RESULTS: APC effectively trimmed all the stents. Seven APC-treated bile ducts and three controls showed epithelial distortion consistent with pressure injury from stent expansion. Two APC-treated bile ducts showed mild thermal injury. The damage was superficial, extending to a maximum depth of 0.1 mm with rare foci involving subepithelial connective tissue. CONCLUSION: APC at indicated settings effectively cuts covered and uncovered Elgiloy® as well as nitinol stents, but can cause biliary epithelial injury secondary to conduction of heat and electrical energy. The stability of diamorphine (0.02 mg/ml as the hydrochloride) in 250 ml bupivacaine hydrochloride (0.15% wt/vol infusion) was studied by high pressure liquid chromatography at temperatures in the range 7 to 45°C. Diamorphine hydrochloride was degraded by approximately 0.13% per day at 7°C. No bupivacaine hydrochloride degradation was detectable during the study. The storage life of the combination at 7°C, based on the lower 95% confidence limit of the time to 5% diamorphine hydrochloride degradation, was 14 days. The stability at 25°C was adequate to allow transport and administration over 24 h at ambient temperature. Stability was also maintained for at least 24 h at 32 and 45°C. Infusion of the mixture with an ambulatory infusion pump which uses a standard polyvinyl chloride infusion bag is therefore possible. A study of its compatibility with different infusion pump medication reservoirs was not undertaken. The drugs were also stable on frozen storage at −18°C for up to 6 months. [ABSTRACT FROM AUTHOR]

Ten patients with histologically documented peritoneal mesothelioma were treated with intraperitoneal cisplatin 200 mg/m2, sodium thiosulfate rescue and etoposide 65-290 mg/m2 every 4 weeks for a maximum of six cycles. All had epithelial or mixed epithelial-fibrous histology. Toxicity was tolerable, with 50% sustaining grade 3 or 4 granulocytopenia. There was one episode of neutropenic fever. Grade 2 peripheral neuropathy occurred in one patient, grade 1 in five patients. Complete remission occurred in one of five patients with measurable disease. Median survival for patients whose tumors were surgically debulked to < 2 cm residua prior to treatment was 22 months, while it was 5 months for those with measurable, surgically inaccessible disease (P = 0.0731 by Cox regression proportional hazard model). These data suggest that patients who present with resectable disease may benefit from an aggressive adjuvant approach. This possibility warrants prospective testing in a randomized clinical trial. Hematogenous metastasis to the psoas muscle is rare, and the resulting clinical symptoms may mimic psoas abscess or hemorrhage. When the clinical history is not specific, CT is important in documenting the presence of a psoas mass and providing biopsy guidance for histologic diagnosis. A 23-year-old woman presented with acute appendicitis. At laparoscopy the appendix appeared to be strangulating itself. The purpose of this study was to identify the specific aspects of social engagement that distinguish infants with autism from infants of similar age and developmental level who do not have autism. Ten parents of preschoolers with autism and 10 parents of matched children without autism were given a semistructured interview, the Detection of Autism by Infant Sociability Interview (DAISI), which elicits reports on whether 19 aspects of social engagement characteristic of typically developing infants were present at some time during the child's first 24 months. The reports of infants with autism differed from those of the control group on 16 items. Findings suggest that infants with autism have marked limitation in both person-to-person and person-person-object social engagement, in keeping with the theory that autism involves impairments in primary as well as secondary intersubjectivity (Hobson, 1993a). Quantitative ground-water tracing of conduit-dominated karst aquifers allows for reliable and practical interpretation of karst ground-water flow. Insights into the hydraulic geometry of the karst aquifer may be acquired that otherwise could not be obtained by such conventional methods as potentiometric-surface mapping and aquifer testing. Contamination of karst aquifers requires that a comprehensive tracer budget be performed so that karst conduit hydraulic-flow and geometric parameters be obtained. Acquisition of these parameters is necessary for estimating contaminant fate-and-transport. A FORTRAN computer program for estimating total tracer recovery from tracer breakthrough curves is proposed as a standard method. Estimated hydraulic-flow parameters include mean residence time, mean flow velocity, longitudinal dispersivity, Peclet number, Reynolds number, and Fronde number. Estimated geometric parameters include karst conduit sinuous distance, conduit volume, cross-sectional area, diameter, and hydraulic depth. These parameters may be used to (1) develop structural models of the aquifer, (2) improve aquifer resource management, (3) improve ground-water monitoring systems design, (4) improve aquifer remediation, and (5) assess contaminant fate-and-transport. A companion paper demonstrates the use of these hydraulic-flow and geometric parameters in a surface-water model for estimating contaminant fate-and-transport in a karst conduit. Two ground-water tracing studies demonstrate the utilityof this program for reliable estimation of necessary karst conduit hydraulic-flow and geometric parameters. [ABSTRACT FROM AUTHOR]
