Background and Objectives: Camrelizumab plus rivoceranib showed significant clinical benefits in progression-free survival and overall survival compared to sorafenib in patients with unresectable hepatocellular carcinoma (HCC). This study aimed to assess its cost effectiveness from the perspective of Chinese health care system. Methods: A Markov state-transition model was developed based on the Phase 3 randomized CARES-310 clinical trial data. Health state utility values were obtained from the CARES-310 clinical trial, and direct medical costs were derived from the relevant literature and local charges. The measured outcomes included quality-adjusted life-years (QALYs) and the incremental cost-effectiveness ratio (ICER). Probabilistic and one-way sensitivity analyses were performed to assess the uncertainty of the model. Results: In the base-case analysis, the incremental effectiveness and cost of camrelizumab plus rivoceranib versus sorafenib were 0.41 QALYs and $13,684.84, respectively, resulting in an ICER of $33,619.98/QALY, lower than the willingness-to-pay threshold of China ($35,864.61/QALY). Subgroup analyses revealed that the ICERs of camrelizumab plus rivoceranib versus sorafenib were $35,920.01 and $29,717.98 in patients with ALBI grade 1 and grade 2, respectively. One-way sensitivity analyses indicated that the cost of camrelizumab, the proportion of patients receiving subsequent treatment in the camrelizumab plus rivoceranib group, and the cost of rivoceranib were the most significant factors in the base-case analysis. Probabilistic sensitivity analysis suggested that the probabilities of cost effectiveness of camrelizumab plus rivoceranib were 61.27%, 51.46%, and 82.78% for any grade, and ALBI grade 1 and grade 2, respectively. Conclusions: Camrelizumab plus rivoceranib was more cost effective than sorafenib as first-line therapy for unresectable HCC in the Chinese setting. [ABSTRACT FROM AUTHOR]

This study sought to evaluate the effectiveness of introducing digital software in music education to improve academic performance and solfeggio class attendance. To assess academic performance, a test consisting of 25 theoretical questions and 25 practical tasks was developed. The Bologna model was used as an assessment system. The groups were compared by calculating the Student's t-test in terms of the "Test Score" and "Missed Classes" parameters. The collected data revealed that the mean score for the experimental group was 87.50 out of 100, while in the control group, it was much lower, namely 65.37. Likewise, the control group was characterized by satisfactory (D) performance in mastering solfeggio, with the mean score ranging between 64 and 73, whereas experimental group participants had an average of 82–89. The statistical data processing results showed that the six-month use of the ChordIQ app contributed to more effective mastering of solfeggio by amateur musicians. At the same time, the difference between the groups turned out to be significant in both cases, so the research hypothesis was confirmed for both of them (p < 0.05). Overall, the results showed that ChordIQ is a useful mobile app teaching the user music in an exciting and interactive way. The findings collected can be used to update traditional solfeggio curricula, conduct scientific studies in the field of interactive music education, as well as create new dedicated software. Introduction: Brain metastases (BM) remains the most cumbersome disease burden in patients with lung cancer. This study aimed to investigate whether serum brain injury biomarkers can indicate BM, to further establish related diagnostic models, or to predict prognosis of BM. Materials and methods: This was a prospective study of patients diagnosed with lung cancer with BM (BM group), with lung cancer without BM (NBM group), and healthy participants (control group). Serum neurofilament light chain (NfL) and glial fibrillary acidic protein (GFAP) were detected at baseline. We identified and integrated the risk factors of BM to establish diagnostic models. Results: A total of 158 patients were included (n = 37, 57, and 64 in the BM, NBM, and control groups, respectively). Serum biomarker levels were significantly higher in the NBM group than in the control group. Higher serum NfL and GFAP concentrations were associated with BM (odds ratios, 3.06 and 1.79, respectively). NfL (area under curve [AUC] = 0.77, p < 0.001) and GFAP (AUC = 0.64, p = 0.02) had diagnostic value for BM. The final diagnostic model included NfL level, age, Karnofsky Performance Status. The model had an AUC value of 0.83 (95% confidence interval [CI] 0.75–0.92). High NfL concentration was correlated with poor overall survival of patients with BM (hazard ratio, 3.31; 95% CI 1.22–9.04; p = 0.019). Conclusion: Serum NfL and GFAP could be potential diagnostic biomarkers for BM in patients with lung cancer. We established a model that can provide individual diagnoses of BM. We develop a Bayesian local influence procedure for generalized failure time models with latent variables and multivariate censored data. We propose to use the penalized splines (P-splines) approach to formulate the unknown functions of the proposed models. We assess the effects of minor perturbations to individual observations, the prior distributions of parameters, and the sampling distribution on statistical inference through various perturbation schemes. The first-order local influence measure is used to quantify the degree of minor perturbations to different aspects of a statistical model with the use of Bayes factor as an objective function. Simulation studies show that the empirical performance of the Bayesian local influence procedure is satisfactory. An application to a study of renal disease for type 2 diabetes patients is presented. Objective: The aim of this study was to identify whether PET/CT-related metabolic parameters of the primary tumor could predict occult lymph node metastasis (OLM) in patients with T1-2N0M0 NSCLC staged by 18F-FDG PET/CT.Methods: 215 patients with clinical T1-2N0M0 (cT1-2N0M0) NSCLC who underwent both preoperative FDG PET/CT and surgical resection with the systematic lymph node dissection were included in the retrospective study. Heterogeneity factor (HF) was obtained by finding the derivative of the volume-threshold function from 40 to 80% of the maximum standardized uptake value (SUVmax). Univariate and multivariate stepwise logistic regression analyses were used to identify these PET parameters and clinicopathological variables associated with OLM.Results: Statistically significant differences were detected in sex, tumor site, SUVmax, mean SUV (SUVmean), metabolic tumor volume (MTV), total lesion glycolysis and HF between patients with adenocarcinoma (ADC) and squamous cell carcinoma (SQCC). OLM was detected in 36 (16.7%) of 215 patients (ADC, 27/152 = 17.8% vs. SQCC, 9/63 = 14.3%). In multivariate analysis, MTV (OR = 1.671, P = 0.044) in ADC and HF (OR = 8.799, P = 0.023) in SQCC were potent associated factors for the prediction of OLM. The optimal cutoff values of 5.12 cm3 for MTV in ADC, and 0.198 for HF in SQCC were determined using receiver operating characteristic curve analysis.Conclusions: In conclusion, MTV was an independent predictor of OLM in cT1-2N0M0 ADC patients, while HF might be the most powerful predictor for OLM in SQCC. We present the case of an old woman with ALK-rearranged stage IV lung adenocarcinoma who received crizotinib. She presented with severe dyspnea on the 34th day, and diffuse ground-glass opacifications in her chest. A diagnosis of crizotinib-induced ILD was confirmed. Corticosteroids were administered. However, the disease was still progressing rapidly. Therefore, as a monoclonal antibody against vascular endothelial growth factor, bevacizumab was administered in low doses (200 mg on days one and three). Her symptoms began to improve. Our clinical experience indicates that bevacizumab combined with corticosteroids might be a promising treatment in crizotinib-induced ILD patients. [ABSTRACT FROM AUTHOR]

Aims: Moso bamboo ( Phyllostachys pubescens) is a typical native invasive plant imposing serious threats on ecosystem processes and functions. A primary concern is alterations of litter and soil N mineralization in evergreen broadleaved forests coupled with bamboo population expansion. Methods: We conducted a field study to determine the litter production, quality, N resorption efficiency, and soil N mineralization rates in bamboo-dominated forest (BDF) and adjacent uninvaded evergreen broadleaved forest (EBF) in subtropical China. Results: The mean annual litter production for BDF was 5.82 Mg ha, 36.0 % lower than that for EBF (9.09 Mg ha). Litter N concentration was also lower, but C: N was higher after bamboo expansion, coupled with higher N resorption efficiency for Moso bamboo and lower litterfall, resulting in potential N return decreasing as much as 60.41 kg N ha yr to the soil. The soil N net nitrification and mineralization rates exhibited lower values in BDF than in EBF. In addition, annual soil N mineralization rate was positively correlated with litter production but negatively with C: N ratio of litter. Conclusions: Expansion of bamboo into neighboring EBF decreased litter production and quality, reduced soil N mineralization rate, and ultimately retarded N cycling. There is a possible association between myeloperoxidase (MPO) 463G>A polymorphism and risk of lung cancer, but previous studies report conflicting results. We performed a meta-analysis of available molecular epidemiologic studies to comprehensively assess the association between MPO 463G>A polymorphism and risk of lung cancer. A systemic literature search was performed in Pubmed, Embase, and Wanfang databases for molecular epidemiologic studies on the association MPO 463G>A polymorphism and risk of lung cancer on March 16, 2013. The pooled odds ratios (ORs) with their 95 % confidence interval (95 % CI) were calculated to assess the strength of the association. Twenty-six individual case–control studies with a total of 18,433 subjects (7,752 cases and 10,681 controls) were finally included into the meta-analysis. Overall, MPO 463G>A polymorphism was significantly associated with decreased risk of lung cancer under two main genetic comparison models (for A versus G, OR = 0.91, 95 % CI 0.83–0.99, P = 0.035; for AG/AA versus GG, OR = 0.90, 95 % CI 0.81–0.99, P = 0.029). Meta-analysis of studies with high quality also showed that MPO 463G>A polymorphism was significantly associated with decreased risk of lung cancer under two main genetic comparison models (for A versus G, OR = 0.91, 95 % CI 0.83–0.99, P = 0.035; for AG/AA versus GG, OR = 0.90, 95 % CI 0.80–0.99, P = 0.048). Subgroup analysis by ethnicity further showed that there was a significant association between MPO 463G>A polymorphism and decreased risk of lung cancer in Caucasians but not in Asians. Based on the solutions of the Green's function for a saturated porous medium obtained by the authors, and using transformation of axisymmetric coordinates, Sommerfeld integrals and superposition of the influence field on a free surface, the authors have obtained displacement solutions of a saturated porous medium subjected to a torsional force in a half-space. The relationship curves of the displacement solutions and various parameters (permeability, frequency, etc.) under action of a unit of torque are also given in this paper. The results are consistent with previous Reissner's solutions, where a two-phase medium decays to a single-phase medium. This research inquires into the nature and degree of advertising effects on firm intangible values. Based on marketing research on consumer based brand equity, this paper challenges the prevailing decaying assumption employed in the accounting/finance disciplines to model the advertising-firm value relationship. Meanwhile, using financial data and methods, we provide new measures for the marketing discipline to evaluate the effectiveness of advertising to create brand intangible. Results indicate that advertising effects on firm intangible assets are sustainable and accumulative and support the asset/investment-like characteristics of advertising expenditures. The research provides an empirical method to assess long-term advertising performance and suggests firms' varying effectiveness in creating brand equity through advertising. The Human Genome Project has provided abundant gene sequence information on human and important model organisms. The chicken is well positioned from an evolutionary standpoint to serve as a link between higher and lower organisms, particularly mammals, and amphibia and fish. In this study we used stringent criteria to select 565 triples of chicken, human, and mouse candidate orthologs. We analyze the sequences with respect to nucleotide and amino acid similarities. This analysis also allows measurement of evolutionary distances of different proteins. We found that chicken-human and chicken-mouse sequence identities are highly correlated; similarly for chicken-human and chicken-mouse evolutionary distances. With chicken as the out-group, we found that mouse has a higher substitution rate than human, supporting the generation-time effect hypothesis. We also described the transversion bias, which is the preference for some transversions than others in nucleotide substitutions. We demonstrated that there are statistically significant properties in the differences of orthologous sequences. The differential patterns, in combination with sequence similarity analysis, may lead to the identification of genes that are very divergent from the mammalian orthologs. [ABSTRACT FROM AUTHOR]

Mycobacterium tuberculosis readily adapts to survive a wide range of assaults by modifying its physiology and establishing a latent tuberculosis (TB) infection. Here we report a sophisticated mode of regulation by a tRNA-cleaving toxin that enlists highly selective ribosome stalling to recalibrate the transcriptome and remodel the proteome. This toxin, MazF-mt9, exclusively inactivates one isoacceptor tRNA, tRNALys43-UUU, through cleavage at a single site within its anticodon (UU↓U). Because wobble rules preclude compensation for loss of tRNALys43-UUU by the second M. tuberculosis lysine tRNA, tRNALys19-CUU, ribosome stalling occurs at in-frame cognate AAA Lys codons. Consequently, the transcripts harboring these stalled ribosomes are selectively cleaved by specific RNases, leading to their preferential deletion. This surgically altered transcriptome generates concomitant changes to the proteome, skewing synthesis of newly synthesized proteins away from those rich in AAA Lys codons toward those harboring few or no AAA codons. This toxin-mediated proteome reprogramming may work in tandem with other pathways to facilitate M. tuberculosis stress survival. MazF endoribonucleases are thought to arrest growth of Mycobacterium tuberculosis by global translation inhibition. Here, Barth et al. show that MazF-mt9 cleaves a specific tRNA, leading to ribosome stalling at AAA codons and thus selective mRNA degradation and changes in transcriptome and proteome.
