Objectives: To explore the risk factors for relapse of hyperglycemia in obese patients with type II diabetes mellitus (T2DM) who received laparoscopic Roux-en-Y gastric bypass (LRYGB) surgery. Methods: A retrospective analysis was performed on all obese patients with T2DM who underwent a LRYGB during the period 2011–2013. Demographics, preoperative body mass index (BMI), preoperative glycated hemoglobin A1c (HbA1c), adherence to lifestyle intervention, preoperative medication of insulin, and the time interval between surgery and diagnosis of T2DM were investigated and compared. Results: A total of 24 patients were included in our study. The median age was 45.5 years, the median BMI was 29.9 kg/m2, and the median HbA1c was 7.9%. Out of 24 patients, 54.2% (13/24) experienced a relapse of hyperglycemia. The 1-year, 3-year, and 5-year relapse rates were 4.2%, 12.5%, and 50.0%, respectively. The preoperative HbA1c level, C-peptide (2 h) level, and C-peptide (3 h) level were identified as independent variables for the relapse of hyperglycemia (8.11 ± 0.48 vs 7.72 ± 0.37 kg/m2, p = 0.036; 4.35 ± 1.46 vs 7.13 ± 4.10 ng/ml, p = 0.032; 3.76 ± 0.61 vs 5.99 ± 3.39 ng/ml, p = 0.029). Lifestyle intervention could reduce the hyperglycemia relapse rate (66.7 vs 41.7%) after LRYGB surgery. Conclusions: The preoperative HbA1c level and C-peptide level at surgery have an important significance in predicting the relapse of hyperglycemia after LRYGB surgery; lifestyle intervention is crucial for these patients. Purpose: This study was designed to compare robot-assisted gastrectomy with laparoscopy-assisted gastrectomy in surgical performance and short-term clinical outcomes for gastric cancer and evaluate the safety and feasibility of robotic surgery. Methods: A retrospective database of patients who underwent robotic or laparoscopic gastrectomy for gastric cancer between March 2010 and May 2013 was examined. After screening, 514 patients who underwent gastrectomy for gastric cancer were enrolled in this study: 120 robotic and 394 laparoscopic surgery. Patient demographics, surgical performance, and short-term clinical outcomes were examined. Results: All operations were performed successfully. The clinicopathologic characteristics were similar between the two groups. Compared with the laparoscopic group, the robotic group had less intraoperative blood loss (118.3 ± 55.8 vs. 137.6 ± 61.6 ml, P < 0.001), more lymph nodes dissection (34.6 ± 10.9 vs. 32.7 ± 11.2, P = 0.013), and longer operation time (234.8 ± 42.4 vs. 221.3 ± 44.8 min, P = 0.003). The survival rates were 90.2 % at 1 year, 78.1 % at 2 years, and 67.8 % at 3 years in the RAG group compared with 87.3 % at 1 year, 77.1 % at 2 years, and 69.9 % at 3 years in the LAG group. The difference in overall survival rate between the two groups was not statistically significant ( P = 0.812). In view of lymph node involvement, the 3-year survival rates for patients with negative nodal metastasis were 84.4 % in the RAG group versus 82.6 % in the LAG group ( P = 0.972) and 57.5 % in the RAG group versus 60.3 % in the LADG group ( P = 0.653) for those with positive nodal metastasis. Conclusions: Comparing well with laparoscopic gastrectomy, robot-assisted gastrectomy is a feasible and safe surgical procedure with clear operation field, precise dissection, minimal trauma, and fast recovery. Longer follow-up time and randomized, clinical trials are needed to evaluate the clinical benefits and long-term oncological outcomes of this new technology. [ABSTRACT FROM AUTHOR]

Background: Laparoscopically assisted gastric surgery has become an option for the treatment of early gastric cancer. However, the feasibility and safety of laparoscopically assisted gastrectomy for advanced gastric cancer has rarely been studied. This study evaluated the short- and long-term outcomes of laparoscopically assisted distal gastrectomy (LADG) for advanced gastric cancer. Methods: The study retrospectively analyzed the clinical and follow-up data for 346 cases after LADG and for 313 cases after conventional open distal gastrectomy (ODG) used to treat advanced gastric cancer from January 2004 to June 2009 at the authors' hospital. The surgical safety, postoperative complications, survival rate, and recurrence and metastasis of cancer were compared between the LADG and ODG groups. Results: The average time for the LADG and ODG procedures did not differ significantly (211 ± 56 vs 204 ± 41 min), but bleeding during the operation and incision length in the LADG group were significantly less than in the ODG group. The proximal and distal margins of tumors were, respectively, 6.25 ± 2.04 and 5.68 ± 1.71 cm in the LADG group compared with 6.29 ± 2.11 and 5.62 ± 1.59 cm in the ODG group. Neither intergroup difference was significant. The number of lymph node dissections also was similar in the two groups: 33.2 ± 12.5 in the LADG group and 32.8 ± 15.6 in the ODG group. The incidence of postoperative complications in the LADG group (6.7%) was significantly lower than in the ODG group (13.1%). During the follow-up period of 6 to 72 months (average, 37 months), the survival rates were 87.2% at 1 year, 57.2% at 3 years, and 50.30% at 5 years in the LADG group compared with 87.1% at 1 year, 54.1% at 3 years, and 49.2% at 5 years in the ODG group (all similar between the groups). The differences in recurrence and metastasis between the two groups were not statistically significant. Conclusion: Laparoscopically assisted gastrectomy for advanced gastric cancer is safe and effective. In this study, it did not differ significantly from open surgery in terms of survival rate or recurrence after surgery based on long-term follow-up evaluation. It can achieve the same beneficial effects as open surgery, and it has the advantages of a small operation wound, less bleeding, good safety, rapid postoperative recovery, and fewer complications.
