Study design:Single-centre, prospective (comparative cohort) clinical study, with ethical approval and patient consent.Objective:Confirmation of vertebral fracture union can pose significant challenges for clinicians in the management of spinal cord injury and in the decisions around patient mobilization. Bony union is usually assessed with computed tomography (CT). This study hypothesizes that magnetic resonance imaging (MRI) can identify vertebral bone union.Setting:A major spinal injuries unit in the United Kingdom.Method:Patients underwent CT and MRI at 12 weeks post-injury, if conservatively managed, or 12 weeks post-fixation. With CT as the gold standard, the MRI scans were reviewed blind to the CT result and indicators for fracture healing were compared.Results:A total of 35 patients with 55 fractures were imaged. Comparison of CT and MRI showed sensitivity of 88%, specificity of 100% and positive predictive value of 100% for fracture union imaged with MRI using CT as the gold standard.Conclusion:MRI correlates well with CT in identifying vertebral fracture union and non-union. We suggest that where imaging is indicated in the assessment of vertebral body fracture healing MRI can be used routinely with CT reserved for problematic or inconclusive cases. The purpose of our study was to compare various aspects of radiology training schemes in ten countries. A questionnaire was sent to senior residents in the UK, USA, Canada, Australia, New Zealand, Italy, Egypt, India, Malaysia and Greece. The questions concerned length of training, required pre-training experience, the organization of the training scheme, teaching, resources, stages at which residents can independently perform and report examinations, fellowships, and progression to jobs. A wide variety of training, ranging from highly scheduled programs with detailed aims and objectives, to self-learning occurs across the world. Examinations and assessments are also variable. There are lessons to be learned from varying practices; more exchanges of ideas should be encouraged. In view of the "internationalization" of radiology services and the variation in training styles an international qualification for quality assurance purposes may be desirable. Treatment options for hepatocellular carcinoma have evolved over recent years. Interventional radiologists and surgeons can offer curative treatments for early stage tumours, and locoregional therapies can be provided resulting in longer survival times. Early diagnosis with screening ultrasound is the key. CT and MRI are used to characterize lesions and determine the extent of tumour burden. Imaging techniques are discussed in this article as the correct imaging protocols are essential to optimise successful detection and characterisation. After treatment it is important to establish regular imaging follow up with CT or MRI as local residual disease can be easily treated, and recurrence elsewhere in the liver is common.

With the increasing use of abdominal cross-sectional imaging, incidental adrenal masses are being detected more often. The important clinical question is whether these lesions are benign adenomas or malignant primary or secondary masses. Benign adrenal masses such as lipid-rich adenomas, myelolipomas, adrenal cysts and adrenal haemorrhage have pathognomonic cross-sectional imaging appearances. However, there remains a significant overlap between imaging features of some lipid-poor adenomas and malignant lesions. The nature of incidentally detected adrenal masses can be determined with a high degree of accuracy using computed tomography (CT) and magnetic resonance imaging (MRI) alone. Positron emission tomography (PET) is also increasingly used in clinical practice in characterizing incidentally detected lesions. We review the performance of the established and new techniques in CT, MRI and PET that can be used to distinguish benign adenomas and malignant lesions of the adrenal gland.

Improvements in imaging technology and the expanding use of imaging have led to a rapid increase in the discovery of incidental renal lesions. These can present both the radiologist and the referring clinician with diagnostic dilemmas. This article addresses the most frequently encountered lesions and provides a framework for the diagnostic and management pathways for both solid and cystic lesions.
