The integration of new technical possibilities into the overall concept is the determining factor in progress in surgery. The availability of the BiClamp and its specially designed electrical concept mean that we now have a new bipolar coagulation forceps at our disposal. Based on the principles of electrosurgery, we developed a new current modulation that has improved hemostatic properties. The new current modulation can be used for bipolar laparoscopic coagulation forceps, and also for new types of forceps for open surgery. The main area of use in open surgery is vaginal hysterectomy. Initial retrospective studies all showed that the newly designed forceps are a successful surgical instrument. The use of the BiClamp for vaginal hysterectomy leads to less blood loss and less postoperative pain, with similar results in these respects to laparoscopic surgery. In some studies, the duration of surgery was shorter than with conventional vaginal hysterectomy, and in others of similar length. Prospective, randomized controlled studies have, however, yet to be performed. We are of the opinion that confirmation of the results achieved so far will increase the spectrum of indications for this bipolar coagulation forceps, leading to greater patient benefit. Despite the advantages of the laparoscopic-assisted technique of creation of a neovagina, the method is still time-consuming and has weak points. The main complications are the slipping off of the traction device, the ripping off of the threads, the unstable direction of the tension, and further technical complications. Through the perfected mechanics of the traction device and the application instruments all the complications due to the old mechanical system can now be avoided. The first clinical results show that using the new modern equipment leads to a significant reduction of the operation time and of the time of tension but still having the same equally good functional and anatomical results without a higher complication rate. Through the new developed traction device and the perfected application instruments an operation set exists for the optimization of the laparoscopic-assisted creation of a neovagina and therefore for the therapy of a lot of cases of vaginal aplasia. Tension-free vaginal tape (TVT) is nowadays an established operation for the treatment of stress urinary incontinence. In this article, we will demonstrate our standardized TVT procedure and ways of possible postoperative correction. A tight vaginal tape can be individually expanded by a Prolene-interponate within days and also years after surgery. On the other hand, we developed a technique to shorten a loose vaginal tape. Moreover, we will discuss our approaches in the case of an eroded Prolene tape, local estrogen therapy, tape padding with vaginal tissue and blunt removal of the tape by a small vaginal incision.
