Background: Immediate prepectoral implant-based breast reconstruction (IBBR) rates have increased in recent years owing to improved cosmetic and psychological benefits. However, there is a lack of studies regarding complications rates following adjuvant radiotherapy (RT) among patients undergoing immediate prepectoral IBBR. Methods: We conducted a retrospective monocentric analysis of a cohort of consecutively treated patients who underwent NSM following immediate prepectoral IBBR at our institution between March 2017 and November 2021. Patient demographics, quality of life, complication rates, and oncological safety were evaluated in the RT and non-RT groups. Data analysis was performed using IBM SPSS Version 24 (IMB Corp., Armonk, NY, USA). Results: A total of 98 patients were examined: 70 were assigned to have prepectoral IBBR without RT and 28 to the group who had prepectoral IBBR with RT. There was a statistically significant difference in overall capsular contracture rate between the RT and non-RT group (18% vs. 4.3%, p=0.04). The total implant loss in the cohort was 4% (10.7% vs. 1.4%, p=0.05). We obtained a high percentages of all BREAST-Q categories in both groups; however, satisfaction with the breast and sexual well-being was higher in the non-RT group. The three-year overall survivals were 97.4% in the RT group and 98.5% in the non-RT group. Conclusion: Our findings showed that patients in the RT group had a higher rate of capsular contracture and implant loss than those in the non-RT group. However, complication rates were within acceptable range and with accurate preoperative information patients have more benefits from immediate reconstruction showing excellent overall quality of life irrespectively of radiation. The IMmediate Postpartum Access to Cardiac Therapy (IMPACT) procedure is a multidisciplinary, collaborative, highly coordinated clinical service in which a planned delivery and intensive neonatal care are offered for conditions where there is a high likelihood of postnatal instability. This process includes prenatal consultation with the parent(s), involving each service engaged with the delivery, postnatal resuscitation, and procedural care. A Cesarean section delivery is planned in an operating room with immediate access to a multifunctional procedural suite where the neonate can undergo rapid cardiac evaluation and initiation of interventional treatments which can have a positive, life-saving impact. Introduction: Office-based flexible cystoscopy is a common outpatient procedure in daily urology practice. Sometimes, cystoscopy procedures are performed on the initial day or scheduled on the following days. We designed this study to compare immediate versus scheduled cystoscopy in terms of anxiety and pain. Methods: In this study, 160 patients were prospectively randomized to undergo office-based flexible cystoscopy by the same urologist between November 2017 and January 2018. Participants were grouped as scheduled for a cystoscopy on the third day of their application (group 1) and immediate cystoscopy on the same day of the application (group 2). A visual analog scale (VAS), State-Trait Anxiety Inventory (STAI) and Beck Anxiety Inventory (BAI) were completed by the patients. Results: Among men, immediate cystoscopy group experienced an increased state anxiety score compared to scheduled group (51.21 ± 8.108 vs 35.29 ± 10.553; p < 0.001). BAI scores were 16.51 ± 8.078 for group1 vs 31.92 ± 8.403 for group2 (p < 0.001). The mean VAS score was 3 ± 1.183 and 4.55 ± 1.155 in group1 and group2, respectively (p < 0.001). Among women, both the trait anxiety score and state anxiety score were found significantly low in scheduled group (mean trait anxiety scores 44.71 ± 6.051 and 49.3 ± 6.670, mean state anxiety scores were 33.71 ± 8.776 and 44.15 ± 7 in group1 and 2, respectively; p < 0.0001). BAI scores were also low in scheduled group (19.02 ± 7.786 vs 34.13 ± 8.367). Additionally, the mean VAS score was significantly high in immediate cystoscopy group compared to scheduled cystoscopy group (3.50 ± 0.784 vs 2.61 ± 0.919; p < 0.001). Objectives: The aim of this prospective study was to assess marginal bone level (MBL), buccal and palatal bone thickness reduction (BTR) around implants in delayed and immediate placement protocols, 6 months after loading. Materials and Methods: Twenty-four patients were assigned to two treatment groups, and treated with an immediate (Test) or delayed implant placement (Control). Baseline and 6 months post-loading periapical and Cone Beam Computed Tomography radiographs were used to measure mesial, distal, buccal, and palatal MBL change, and buccal and palatal BTR. Results: There was no statistically significant difference between groups, in MBL change, mesially (p = 0.4220), distally (p = 0.774), buccally (p = 0.221), and palatally (p = 0.195). There was more MBL change on the buccal side than on the palatal side in both groups, control (p = 0.012) and test (p = 0.005). Buccal bone thickness decreased significantly in both groups, and at all four levels (p < 0.05). Buccal BTR was higher in test implants at 0, 2, and 4 mm (p = 0.005, p = 0.0018, p = 0.006) on the buccal side, and at 0 mm (p = 0.006) on the palatal side. Implant stability increased in both groups (p < 0.001). A positive correlation was found between insertion torque and stability in the control group (p = 0.024). Conclusions: Within the study limitations, MBL changes occur around implants and are comparable between groups. More buccal than palatal MBL change occurs in both groups. Buccal BTR is higher in immediate cases. Implant stability augmented in both groups. Purpose: Various graft materials have been studied for filling peri-implant gap (PIG), but there was no similar randomized clinical trial to evaluate the effect of Platelet-rich fibrin or alloplast or their combination on vertical bone implant contact (BIC) around immediate implants and their stability over a period of 1 year. Methods: Immediate implants were placed in maxillary anterior region of 30 subjects (n = 10). Either alloplast (group I) or L-PRF (group II) or both (group III) were used to fill the PIG following randomization chart. Vertical BIC was measured on peri-apical radiographs which were taken immediately after placement, after 3, 6 and 12 months using Image J software. Periotest was used to measure the implant stability at the time of implant placement, at 3, 6 and 12 months after implantation. Results: The comparison of distance from implant shoulder to the first visible bone-to-implant contact (IS-BIC) in each group showed statistically significant bone formation on mesial and distal sides over a 1 year period (p < 0.05). There was no significant difference in IS-BIC distance among the three groups at 1 year (p > 0.05). Periotest values showed that there was significant improvement in implant stability in all groups in 1-year period. On intergroup comparison, the mean difference of periotest values was statistically non-significant among three groups (p > 0.05). Conclusions: All the graft materials were effective in promoting osseointegration when used as PIG filling materials alone or in combination around immediate implants in maxillary anterior region. There is a paucity of evidence of the impact of sorafenib on MCT and it is the preferred therapy used in India. We decided to do an audit of all patients of MCT who were referred to us for systemic therapy. The objective of this exercise was to identify the treatment pattern, outcomes, and adverse events with therapy in MCT. Baseline demographics (age, gender, ECOG PS, comorbidities, habits), tumor details (site of metastasis), previous treatment details, clinical features at metastasis (symptomatic or asymptomatic), the pattern of treatment, adverse events (CTCAE version 4.02), date of progression, date of death and status, and follow-up were extracted from the rare tumor database and electronic medical records. Out of 75 patients referred for therapy for MCT, 47 (62.7%) patients were considered for immediate tyrosine kinase inhibitors as they had symptomatic status and 28 (37.3%) patients were kept on observation due to the asymptomatic nature of the disease. Out of the 28 patients, 15 (53.6%, n = 28) patients were subsequently started on TKI while in 13 (46.4%, n = 28) patients observation was continued. In the overall cohort, the median PFS was 18.9 months (95% CI 11.9–29.9) and OS was 26.6 months (95% CI 14.4–39.0). Among variables tested, only female gender had an impact on PFS (hazard ratio = 0.364 95% CI 0.148–0.895; P = 0.028) and the absence of lung metastasis had a positive impact on OS (hazard ratio = 0.443 95% CI 0.207–0.95; P = 0.037). Most commonly used TKI was sorafenib (n = 61) and sunitinib in 1 patient. The most common adverse events with TKI were palmo-plantar dysesthesia (50, 80.6%) and oral mucositis (25, 40.2%). The strategy of treating symptomatic MCT and observing in asymptomatic MCT is associated with reasonable PFS and OS. Introduction: To determine which day automated refraction stabilised the first week after uneventful cataract surgery. Methods: This was a prospective, single-blinded, randomised cohort study of 148 eyes in 148 consecutive patients undergoing uneventful cataract surgery. Automated refraction was measured from days 2 to 7 after surgery and compared to automated refraction measured 6 weeks post-surgery. We compared measurements using a hierarchical mixed-effect model. Results: Our adjusted results did not show a statistically significant change in spherical equivalent between the first week after surgery and 6 weeks after surgery (p-values: day 2: 0.914, day 3: 0.922, day 4: 0.168, day 5: 0.211, day 6: 0.457, day 7: 0.621). We measured the spherical error as stable except for day 5, where a statistically significant change of 0.32 dioptre (p = 0.049) was detected. Similarly, the cylindrical error was also stable on all days of measurements except on day 6, where a significant change of 0.28 dioptre (p = 0.034) was detected. Conclusion: The adjusted spherical equivalent was stable on days 2–7 after uneventful cataract surgery in our study population. However, as we observed a statistically significant difference in spherical and cylindrical errors on days 5 and 6, respectively, we could not conclude that automated refraction stabilised the first week after uncomplicated cataract surgery. The aim of this paper was to demonstrate the treatment outcomes following immediate functional loading concept of short implants inserted for single tooth replacement in the posterior maxilla. The study was performed on 63 patients who received short (6 mm) implants for single tooth replacement in the posterior maxilla. Forty-eight patients underwent immediate functional concept, whereas 15 of the implants were loaded 3 months after insertion. The patients were evaluated for up to 5 years after prosthesis completion. The endpoints included the evaluation of implant survival rate, crown length, bone resorption, plaque accumulation (PI), bleeding on probing (BOP), periodontal probing depth (PPD) and assessment of oral health impact profile (OHIP). At the end of the follow-up period of 5 years, three implants (6.3%) from the immediate loading group have failed during the observation period. Bone loss was significantly lower in the delayed loading group compared to the immediately loaded implants. At the end of the second year, BOP values were higher in the immediately loaded group. Throughout the observation period, PI values in the group with immediate loading were higher. PPD increased consistently and during the first 3 years in the immediate loading group. As a conclusion, short implants inserted for single tooth replacement at the posterior maxilla presented with satisfactory clinical outcomes in both immediate and delayed loading concepts. However, immediately loaded implants presented with an increased bone loss and higher BOP values. Long-standing ischemic priapism leads to cavernosal smooth muscle necrosis and erectile dysfunction [1]. We report a 28-year-old gentleman who presented with 3-week duration of neglected idiopathic ischemic priapism. As his stretched penile length was 9 cm, he was taken up for immediate implantation of penile prosthesis. With minimal difficulty in corporal dilation due to scarring, a 3-piece inflatable penile prosthesis was placed. He developed fever with chills and generalized rash in the immediate postoperative period and it responded to change of antibiotics and antihistaminics. Purpose: To compare visual and anatomical recovery of immediate versus deferred intravitreal Bevacizumab for the treatment of macular oedema secondary to branch retinal vein occlusion (BRVO).Methods: In a pilot study, 40 treatment naïve patients of branch retinal vein occlusion with macular oedema and vision 6/12 or less presenting within one month of onset were randomised into 2 groups (20 each) to receive either immediate intravitreal Bevacizumab or deferred (after 3 months of observation). Outcome in terms of visual recovery and decrease in central macular thickness on the Optical Coherence Tomography (OCT) from pre-treatment level was analysed at 6 and 12 months from starting of treatment and compared between the two groups.Results: The mean visual gain in the two groups early and delayed intervention was 0.38 log MARs and 0.15 log MAR units, respectively, and the superior vision gain in the early intervention group was statistically significant (p < 0.001). The difference in visual improvement between the two groups persisted till 1 year of follow-up. The early intervention group required fewer injections (2.6 ± 71 vs. 3.5 ± 0.51), and rescue laser treatment (15 vs. 25%) as compared to deferred group. Both groups showed significant decrease in central macular thickness (328 and 289 µ, respectively) from baseline thickness, but the difference between the two groups was not statistically significant (p = 0.45).Conclusions: Both early as well as deferred injection of Bevacizumab in macular oedema due to BRVO resulted in reduction of macular oedema and visual gain but immediate injection were associated with significantly greater visual gain with lesser number of injections fewer rescue laser treatment. In patients undergoing breast reconstruction after partial and total mastectomy, selecting the appropriate timing as well as the best method of reconstruction are essential to optimize the outcome. At M.D. Anderson Cancer Center, the timing of oncoplastic repair after partial mastectomy defects and breast reconstruction after mastectomy tends to dictate the technique for reconstruction. In patients undergoing repair of a partial mastectomy defect, immediate or delayed repair before radiation therapy allows for the use of the remaining breast tissue to perform the repair. Delayed repair after radiation therapy is usually performed with autologous fat grafting or a flap. Immediate breast reconstruction after mastectomy is preferable for patients with a low risk of requiring postmastectomy radiation therapy (XRT) (stage I breast cancer, some stage II). In patients who are deemed preoperatively to be at an increased risk of requiring XRT (stage II breast cancer), delayed-immediate breast reconstruction may provide an additional option. Delayed-delayed reconstruction may be a consideration in patients known preoperatively to require XRT (stage III breast cancer), to allow for a skin-preserving delayed reconstruction after XRT. Newer techniques for breast reconstruction after mastectomy include one-stage implant, implant-based reconstruction plus acellular dermal matrix, autologous fat grafting after negative suction applied to chest wall, and perforator-based autologous tissue flaps. Often, the decision to perform a partial or total mastectomy depends upon reconstructive issues, not oncology-related considerations. We present a series of 91 cases caused by both wild and domestic animals over a period of 5 years from 2005-2009. The objective of the study was to compare the results of primary reconstruction with the traditional method of debridement and secondary healing. A total of 91 patients were studied from January 2005-December 2009. In 54 patients, the injuries were carefully documented, and immediate reconstruction was performed. Thirty-seven patients with contraindications to surgery or who refused to undergo surgery were allowed to heal by secondary intention and were studied as a control group. The patients and their relatives were asked to rate their result and outcome. Among the 54 patients who underwent primary reconstruction, 37 patients were males and 17 were females. Age ranged from 2 to 70 years. Amongst the wild animals, bear was the most common offending animal while dog was the most common street animal. Overall bear bites were the most common presentation to our facility. Four out of 54 cases (7.4%) with primary reconstruction developed infection of the operative site. Of the patients with immediate reconstruction, 57.4% thought that the result was excellent, 35.2% patients were satisfied with the result but wanted further refinement while 7.4% were unhappy and wanted immediate revision. Of the 37 patients who did not undergo primary reconstruction, 16.2% thought that the result was excellent, 45.9% were satisfied and 37.8% wanted immediate revision. Silicone implants are often used in immediate breast reconstruction. Complications associated with silicone-based implant reconstruction, such as capsular contracture, implant palpability, and visibility, are best avoided by placing the implant under a reliable soft-tissue cover such as the pectoralis major muscle. This muscle, however, is not always sufficient for complete coverage of the silicone implant. This is especially true for large implants. By including the fascia of the upper abdominal muscles in the reconstruction, this problem can be overcome. We describe our experience with one-stage breast reconstruction utilizing the fascia of the upper abdominal muscles to provide adequate soft-tissue coverage of the implant. This technique was used in the reconstructions of ten patients over 4 years (2005-2009). This method was selected by the operating surgeon at the time of surgery if the pectoralis major muscle was felt to be of inadequate size to provide adequate implant coverage. The pectoralis major muscle was released from its sternal and caudal attachments to the chest wall. The rectus abdominis fascia and external oblique fascia were elevated as a combined cephalic-based flap. This fascial flap was advanced cranially and sutured to the released pectoralis major muscle after insertion of the implant. The mean size of the silicone implant was 448.2 cc and mean follow-up was 19.7 months. All implants were adequately covered with soft tissue at the end of each case. Complications included one patient with a hematoma, one patient with skin necrosis at the suture line, and one patient with an implant infection necessitating removal. There are many ways to provide soft-tissue coverage of silicone breast implants in breast reconstruction. These include acellular dermis slings, polyglycolic mesh, deepithelialized skin, and muscle. The ideal soft-tissue cover would be supple, easily harvested, of minimal morbidity, of minimal cost, and preferably autologous. We feel that the technique described here has these qualities and allows for complete coverage of silicone implants. An additional benefit of this technique is that it helps to increase the definition of the inframammary sulcus. PurposeTo assess the outcome of patients who underwent instantaneous pars plana vitrectomy for dislocated lens fragments during cataract surgery.MethodsThe medical records of 29 patients who underwent immediate pars plana vitrectomy for dislocated lens fragments at the same sitting of phacoemulsification surgery were reviewed. Data including patients' demographics, preoperative and postoperative vision, risk factors for lens fragment dislocation, and intraoperative and postoperative complications were recorded.ResultsThe mean follow-up period of the patients was 12.3 months (range, 3–47 months). The median preoperative visual acuity was 6/90 (range, LP to 6/15). The median final postoperative visual acuity was 6/18 (range, NLP to 6/9) and the mean improvement in vision after the operation was 4.5 lines. The commonest risk factors for dislocation of lens fragment were late detection of posterior capsule tear, splitting of anterior capsulorrhexis, and hard nucleus. After excluding nine eyes with other pre-existing ocular comorbidities, 10 (50%) eyes had a final visual acuity of 6/12 or better. Complications after surgery included three (10%) cases of retinal detachment and one (3%) patient developed epiretinal membrane. None of the patients in the study developed secondary glaucoma or cystoid macular oedema following surgery.ConclusionImmediate vitrectomy in the same sitting of the cataract surgery is a surgical option in the management of dislocated intravitreal lens fragments when vitreoretinal support is available.
