6. Clinical course and seroprevalence of COVID-19 in children with rheumatic diseases—cross-sectional study from a reference centre in Spain

Background Patients with rheumatic diseases have been considered at risk of COVID-19. A significant percentage of infections in children are asymptomatic or mild and can go unnoticed. This study aims to describe the seroprevalence of SARS-CoV-2 in a cohort of children with rheumatic diseases and assess possible risk factors. Methods: A cross-sectional study was performed in a pediatric rheumatology unit from a reference hospital in Madrid, from September 2020 to February 2021. Serology of SARS-CoV-2 was performed at the same time as their routine laboratory tests, and a specific questionnaire was completed by parents. Demographics, treatment and disease activity from laboratory-confirmed COVID-19 patients were compared to the data of patients without laboratory-confirmed COVID-19.Findings A total of 105 children were included. SARS-CoV-2 infection was demonstrated in 27 patients (25.7%): 6 PCR and 21 positive IgG serology. The mean age was 11.8 years , and the majority of the patients were females (72.4%). Most of the children were diagnosed with juvenile idiopathic arthritis (JIA) (70.3%; 19/27), followed by PFAPA (11.1%; 3/27). Immunosuppresive treatment was given in 88% of cases (24/27). Overall, 44.4% (12/27) of infected patients were asymptomatic. Three patients required hospital admission because of COVID-19, however none of them required oxygen supplementation. A total of 66.7% (18/27) of patients did not require any treatment or medical assistance. The seroprevalence in our cohort was 20% in contrast to the 7.7% population seroprevalence observed during the same study period in Spanish children. SARS-CoV-2 confirmed children with positive IgG or PCR were less frequently in remission (52% vs 72%; p 0.014). Moderate disease activity and treatment with oral corticosteroids were associated with higher risk for COVID-19 (OR 5.05; CI 95%: 1.56 - 16.3 and OR 4.2; CI 95%: 1.26 - 13.9 respectively). Conclusions In a cohort of pediatric patients with rheumatic disease and immunosuppressive therapy, moderate disease activity and oral corticosteroids were associated with COVID-19 positive patients. Seroprevalence was significantly higher compared to the same age healthy population. The clinical manifestations were mild and there were no severe infections among the patients.

During incremental exercise, two thresholds may be identified from standard gas exchange and ventilatory measurements. The first signifies the onset of blood lactate accumulation (the lactate threshold, LT) and the second the onset of metabolic acidosis (the respiratory compensation point, RCP). The ability to explain why these thresholds occur and how they are identified, non-invasively, from pulmonary gas exchange and ventilatory variables is fundamental to the field of exercise physiology and requisite to the understanding of core concepts including exercise intensity, assessment, prescription, and performance. This review is intended as a unique and comprehensive theoretical and practical resource for instructors, clinicians, researchers, lab technicians, and students at both undergraduate and graduate levels to facilitate the teaching, comprehension, and proper non-invasive identification of exercise thresholds. Specific objectives are to: (1) explain the underlying physiology that produces the LT and RCP; (2) introduce the classic non-invasive measurements by which these thresholds are identified by connecting variable profiles to underlying physiological behaviour; (3) discuss common issues that can obscure threshold detection and strategies to identify and mitigate these challenges; and (4) introduce an online resource to facilitate learning and standard practices. Specific examples of exercise gas exchange and ventilatory data are provided throughout to illustrate these concepts and a novel online application tool designed specifically to identify the estimated LT (θLT) and RCP is introduced. This application is a unique platform for learners to practice skills on real exercise data and for anyone to analyze incremental exercise data for the purpose of identifying θLT and RCP.

MUSCAT is a new large-format 1.1 mm continuum camera for local and extra-galactic astronomy currently being installed at the 50 m Large Millimetre Telescope Alfonso Serrano, LMT. The focal plane is populated with 1458 feedhorn coupled Aluminium LEKIDs, cooled to 130 mk, and read out with 6 frequency division multiplexed RF readout chains, each with a 500 MHz bandwidth. The nominal mux ratio is $$\sim$$ ∼ 250 resonators per chain with an average spacing of 2 MHz between resonators. In each RF chain, the multiplexed waveform generation/decomposition is performed with the BLAST-TNG firmware on a ROACH2 FPGA board with a MUSIC DAC/ADC. The quadrature modulated IF signals are mixed to and from the LEKID readout band on a custom card that houses IQ mixers, a programmable synthesizer, and additional programmable input and output gain control and filtering. Within the cryostat, the RF signals are attenuated prior to reaching the focal plane array and then amplified at 4K. A novel software system controls all the programmable hardware and handles instructions from the LMT instrument control system to record and store observation data, as well as performing automated resonator retuning and providing results of quick look analysis to the telescope operators. In this paper, we present the detailed design and in-lab performance of the cryogenic and room temperature electronics, and the software control system.

Hypofractionated whole breast irradiation (hWBI) and intraoperative radiotherapy (IORT) could be associated in breast cancer patients showing high-risk factors of local recurrence after breast conserving therapy (BSC). The aim of this trial was to evaluate, for the first time, the toxicity and cosmesis of hWBI after photon-IORT in high-risk patients treated by adjuvant chemotherapy. Thirty-one high-risk localized breast cancer patients treated by BCS, IORT (20 Gy), and adjuvant chemotherapy were included from February 2019 to August 2020 in this prospective trial, of hWBI (40.5 Gy/2.67 Gy/15 fractions). Acute and late toxicity (CTCAEv5.0) and cosmesis (Harvard scale), were assessed after treatment. All patients completed their treatment and were evaluable after treatment. No patients showed severe (G3) acute/late toxicity. Excellent/good cosmesis at least 6 months after completing the treatment, was present in 93.6% of the patients. hWBI in high-risk localized breast cancer patients treated by chemotherapy seems to have similar toxicity and cosmetic results than conventionally fractionated WBI in combination to photon-IORT after BCS.

This study evaluated (i) the relationship between oxygen uptake ( $$\dot{\text{V}}$$ O2) kinetics and maximal $$\dot{\text{V}}$$ O2 ( $$\dot{\text{V}}$$ O2max) within groups differing in fitness status, and (ii) the adjustment of $$\dot{\text{V}}$$ O2 kinetics compared to that of central [cardiac output (Q), heart rate (HR)] and peripheral (deoxyhemoglobin over $$\dot{\text{V}}$$ O2 ratio ([HHb]/ $$\dot{\text{V}}$$ O2)] O2 delivery, during step-transitions to moderate-intensity exercise. Thirty-six young healthy male participants (18 untrained; 18 trained) performed a ramp-incremental test to exhaustion and 3 step-transitions to moderate-intensity exercise. Q and HR kinetics were measured in 18 participants (9 untrained; 9 trained). No significant correlation between τ $$\dot{\text{V}}$$ O2 and $$\dot{\text{V}}$$ O2max was found in trained participants (r = 0.29; p > 0.05) whereas a significant negative correlation was found in untrained (r = − 0.58; p 0.05). τQ demonstrated a significant strong positive correlation with τHR in trained (r = 0.76; p 0.05). A significant overshoot in the [HHb]/ $$\dot{\text{V}}$$ O2 ratio was found in the untrained groups (p 0.05) The results indicated that when comparing participants of different fitness status (i) there is a point at which greater VO2max values are not accompanied by faster $$\dot{\text{V}}$$ O2 kinetics; (ii) central delivery of O2 does not seem to limit the kinetics of $$\dot{\text{V}}$$ O2; and (iii) O2 delivery within the active tissues might contribute to the slower $$\dot{\text{V}}$$ O2 kinetics response in untrained participants.

Amblyopia is the leading cause of vision impairment in children, and its early detection can avoid irreversible consequences for a child's visual ability. In this prospective study, to detect amblyopia risk factors, we examined patients aged 18 to 30 months in primary care settings. Patients were referred to an ophthalmologist for confirmation. The main aims were to detect amblyogenic risk factors and to assess the usefulness of a photoscreener in such settings. Out of 453 patients, 42 (9.3%) presented visual alterations according to the photoscreener, with astigmatism being the most common. The instrument had good sensitivity (89%) and specificity (91%), with a positive predictive value of 76% and a negative predictive value of 96%. Overall, 38% of the patients required follow-up, and 47% needed glasses. The automated screening device allowed these children to be diagnosed at an early stage.Conclusions: The use of a photoscreener to screen 2-year-old children in primary care settings was helpful and accurate. What is Known: • Early detection of amblyopic risk factors is important to avoid vision defects. However, it is very difficult to measure visual acuity using visual charts in children younger than 4 years old. What is New: • Instrument-based screening in children aged 18 to 30 months allows excellent detection of early amblyopia risk factors in primary care settings.

Visual exploration paradigms involving object arrays have been used to examine salience of social stimuli such as faces in ASD. Recent work suggests performance on these paradigms may associate with clinical features of ASD. We evaluate metrics from a visual exploration paradigm in 4-to-11-year-old children with ASD (n = 23; 18 males) and typical development (TD; n = 23; 13 males). Presented with arrays containing faces and nonsocial stimuli, children with ASD looked less at (p = 0.002) and showed fewer fixations to (p = 0.022) faces than TD children, and spent less time looking at each object on average (p = 0.004). Attention to the screen and faces correlated positively with social and cognitive skills in the ASD group (ps .05). This work furthers our understanding of objective measures of visual exploration in ASD and its potential for quantifying features of ASD.

We assessed different aspects of tactile perception in young children (3–6 years) with autism. Autistic and neurotypical children completed vibrotactile tasks assessing reaction time, amplitude discrimination (sequential and simultaneous) and temporal discrimination (temporal order judgment and duration discrimination). Autistic children had elevated and more variable reaction times, suggesting slower perceptual-motor processing speed and/or greater distractibility. Children with autism also showed higher amplitude discrimination and temporal order judgement thresholds compared to neurotypical children. Tactile perceptual metrics did not associate with social or tactile sensitivities measured by parent-reports. Altered tactile behavioral responses appear in early childhood, can be quantified but appear dissociated from sensitivity. This implies these measures are complementary, but not necessarily related, phenomena of atypical tactile perception in autism.

10-Alkylthiocolchicines have been obtained and characterized by spectroscopic methods and their biological activities as: cytotoxic, anti-inflammatory and analgesic activities have been tested. Cytotoxic activity against SKOV-3 ovarian cell line for 10-alkylthiocolchicine analogues was reported and tested compounds showed to be more active than commonly used doxorubicin. Some of tested C-10 alkylthiolated colchicines have been found to exhibit cytotoxicity at levels comparable to that of the natural product—colchicine. 10-Methylthiocolchicine has IC50 = 8 nM and 10-ethylthiocolchicine has IC50 = 47 nM in comparison to colchicine IC50 = 37 nM. Moreover for 10-alkylthioderivatives apoptosis test, cyclin B1 and cell cycle tests were performed. 10-n-Butylthiocolchicine was tested for anti-inflammatory and analgesic activities it showed to produce analgesic rather than anti-inflammatory effect.

The ability to orient and navigate in spatial surroundings is a cognitive process that undergoes a prolonged maturation with progression of skills, strategies and proficiency over much of childhood. In the present study, we used functional Magnetic Resonance Imaging (fMRI) to investigate the neurological mechanisms underlying the ability to orient in a virtual interior environment in children aged 10 to 12 years of age, a developmental stage in which children start using effective spatial orientation strategies in large-scale surroundings. We found that, in comparison to young adults, children were not as proficient at the spatial orientation task, and revealed increased neural activity in areas of the brain associated with visuospatial processing and navigation (left cuneus and mid occipital area, left inferior parietal region and precuneus, right inferior parietal cortex, right precentral gyrus, cerebellar vermis and bilateral medial cerebellar lobes). When functional connectivity analyses of resting state fMRI data were performed, using seed areas that were associated with performance, increased connectivity was seen in the adults from the right hippocampal/parahippocampal gyrus to the contralateral caudate, the insular cortex, and the posterior supramarginal gyrus; children had increased connectivity from the right paracentral lobule to the right superior frontal gyrus as compared to adults. These findings support the hypothesis that, as children are maturing in their navigation abilities, they are refining and increasing the proficiency of visuospatial skills with a complimentary increase in connectivity of longer-range distributed networks allowing for flexible use of efficient and effective spatial orientation strategies.

The aim of this study was to examine whether differences in vascular responsiveness associated with training status would be more prominent in the trained limb (leg) than in the untrained limb (arm) microvasculature. Thirteen untrained (26 ± 5 year) and twelve trained (29 ± 4 year) healthy men were submitted to a vascular occlusion test (VOT) (2 min baseline, 5 min occlusion, and 8 min re-oxygenation). The oxygen saturation signal (StO2) was assessed using near-infrared spectroscopy (NIRS) throughout the VOT. Altered patterns of sensory responsiveness are a frequently reported feature of Autism Spectrum Disorder (ASD). Younger siblings of individuals with ASD are at a greatly elevated risk of a future diagnosis of ASD, but little is known about the neural basis of sensory responsiveness patterns in this population. Younger siblings (n = 20) of children diagnosed with ASD participated in resting electroencephalography (EEG) at an age of 18 months. Data on toddlers’ sensory responsiveness were obtained using the Sensory Experiences Questionnaire. Correlations were present between hyporesponsiveness and patterns of oscillatory power, functional connectivity, and signal complexity. Our findings suggest that neural signal features hold promise for facilitating early identification and targeted remediation in young children at risk for ASD.

Autism Spectrum Disorder (ASD) is characterized by early attentional differences that often precede the hallmark symptoms of social communication impairments. Development of novel measures of attentional behaviors may lead to earlier identification of children at risk for ASD. In this work, we first introduce a behavioral measure, Relative Average Look Duration (RALD), indicating attentional preference to different stimuli, such as social versus nonsocial stimuli; and then study its association with neurophysiological activity. We show that (1) ASD and typically developing (TD) children differ in both (absolute) Average Look Duration (ALD) and RALD to stimuli during an EEG experiment, with the most pronounced differences in looking at social stimuli; and (2) associations between looking behaviors and neurophysiological activity, as measured by EEG, are different for children with ASD versus TD. Even when ASD children show attentional engagement to social content, our results suggest that their underlying brain activity is different than TD children. This study therefore introduces a new measure of social/nonsocial attentional preference in ASD and demonstrates the value of incorporating attentional variables measured simultaneously with EEG into the analysis pipeline.

In theory, a slow oxygen uptake ( $$\dot VO_2$$ ) kinetics leads to a greater accumulation of anaerobic by-products, which can, in turn, induce more neuromuscular fatigue. However, the existence of this relationship has never been tested. After two sessions to measure peak $$\dot VO_2$$ , peak power output (POpeak), and $$\dot VO_2$$ kinetics responses in the unfatigued state (τ $$\dot VO_2$$ MOD), 10 healthy young adults performed a 6-min cycling bout at 80% POpeak (INT6-min). $$\dot VO_2$$ kinetics responses were also measured during INT6-min. Neuromuscular fatigue was measured isometrically pre- and post-INT6-min (immediately post- and 15-s post-INT6-min) with an innovative cycle ergometer. Efficacy and safety data for combining bevacizumab, gemcitabine, and paclitaxel for locally advanced/metastatic breast cancer are limited.AVALUZ trial evaluates the combination of bevacizumab 10 mg/kg, gemcitabine 2,000 mg/m(2) plus paclitaxel 150 mg/m(2), on days 1 and 15 of each 28-day course in previously untreated HER-2 negative patients.Median progression-free survival (PES): 12.3 months. The overall response and clinical benefit rate (CR + PR + SD) were 72 % (95 % CI 60.9-82.0 %) and 89 % (95 % CI 80.3-95.3 %), respectively. Median overall survival: 27.4 mo. Baseline circulating tumor cell (CTCs) ≥2 versus CTCs 2 was associated with lower PFS, p = 0.046. Overall response was significantly greater in patients with intense angiotensin type 1 receptor (AGTR1) expression (99 vs. 60 % [p = 0.021]). The most frequent grade 3/4 adverse events were: neutropenia (10 %); febrile neutropenia (1 %); sensory neuropathy (13 %); and asthenia (6 %). Grade 3 adverse events of interest with bevacizumab included bleeding (1 %) and hypertension (4 %). One patient developed cardiac ischemia (1 %).Adding bevacizumab to chemotherapy appeared feasible and well tolerated, producing toxicity comparable to other effective combined first-line regimens. Baseline circulating endothelial cells and AGTR1 expression are predictive of PFS and response.

This study examined the impact of eccentric exercise-induced muscle damage on the rate of adjustment in muscle deoxygenation and pulmonary O2 uptake ( $$\dot{V}{\text{O}}_{{2{\text{p}}}}$$ ) kinetics during moderate exercise. Fourteen males (25 ± 3 year; mean ± SD) completed three step transitions to 90 % θL before (Pre), 24 h (Post24) and 48 h after (Post48) eccentric exercise (100 eccentric leg-press repetitions with a load corresponding to 110 % of the participant’s concentric 1RM). Participants were separated into two groups: phase II $$\dot{V}{\text{O}}_{{2{\text{p}}}}$$ time constant (τ $$\dot{V}{\text{O}}_{{2{\text{p}}}}$$ ) ≤ 25 s (fast group; n = 7) or τ $$\dot{V}{\text{O}}_{{2{\text{p}}}}$$ > 25 s (slow group; n = 7). $$\dot{V}{\text{O}}_{{2{\text{p}}}}$$ and [HHb] responses were modeled as a mono-exponential.
