Purpose Thyroid dysfunction in patients with Klinefelter syndrome (KS) remains an unresolved issue. Although low free thyroxine (FT4) levels within the normal range and normal thyroid stimulating hormone (TSH) levels have been reported, there is currently no data on nodular thyroid disease in this population. This study aims to evaluate the results of thyroid ultrasound (US) examinations in KS patients compared with healthy controls. Methods A cohort of 122 KS and 85 age-matched healthy male controls underwent thyroid US screening and thyroid hormone analysis. According to US risk-stratification systems, nodules ≥1 cm were examined by fine needle aspiration (FNA). Results Thyroid US detected nodular thyroid disease in 31% of KS compared to 13% of controls. No statistical differences in the maximum diameter of the largest nodules and in moderate and highly suspicious nodules were found between patients and the control group. Six KS patients and two controls with nodules underwent FNA and were confirmed as cytologically benign. In line with published data, FT4 levels were found significantly near the lower limit of the normal range compared to controls, with no differences in TSH values between the two groups. Hashimoto’s thyroiditis was diagnosed in 9% of patients with KS. Conclusions We observed a significantly higher prevalence of nodular thyroid disease in KS compared to the control group. The increase in nodular thyroid disease is likely linked to low levels of FT4, inappropriate TSH secretion, and/or genetic instability.

Boron nitride nanotubes (BNNTs) are emerging nanomaterials with analogous structures and similarly impressive mechanical properties to carbon nanotubes (CNTs), but unique chemistry and complimentary multifunctional properties, including higher thermal stability, electrical insulation, optical transparency, neutron absorption capability, and piezoelectricity. Over the past decade, advances in synthesis have made BNNTs more broadly accessible to the nanomaterials and other research communities, removing a major barrier to their utilization and research. Therefore, the field is poised to grow rapidly and see the emergence of BNNT applications ranging from electronics to aerospace materials. A key challenge, that is being gradually overcome, is the development of manufacturing processes to make “neat” BNNT materials. This overview highlights the history and current status of the field, providing both an introduction to this Focus Issue—BNNTs: Synthesis to Applications—as well as a perspective on advances, challenges, and opportunities for this emerging material. The mode of delivery influences the genital image and perception, especially regarding the effects of delivery on sexual life and intercourse. The current literature has not adequately investigated the relationship between delivery and genital appearance. The aim of the study is to determine whether the mode of delivery changes the genital perception of the woman and, in doing so, influences their acceptance. The secondary aim is to analyze the impact of genital appearance on sexuality.A prospective survey regarding genital appearance and the impact of delivery mode on vulvar perception was conducted in patients 6 weeks after childbirth. We enrolled 365 women for evaluation, divided into three groups: spontaneous vaginal delivery (SVD 295 women 80.82%), operative vaginal delivery (OVD 36 women 9.86%) and cesarean section (CS 34 women 9.31%).There was a statistically significant difference in the frequency of vulva inspection and in the perception of genital modifications among the groups (p 0.001 and p 0.001, respectively). The perception of overall genital modifications was significantly correlated with the frequency of inspection (p = 0.004) and the delivery mode (p = 0.0002).Mode of delivery may influence the genital perception and appearance of genitalia without a decrease of sexual life and daily activity in childbirth.

Delayed gastric emptying (DGE) is a common complication in surgery, but incidence and relevance following multivisceral resection are unknown.Data from 100 consecutive patients treated for primary retroperitoneal sarcoma (RPS) were analyzed from our institutional prospectively maintained database from January 2019 to April 2020. DGE severity was graded according to the International Study Group of Pancreatic Surgery and classified as primary or secondary to other complications. The primary outcome was incidence and grade of clinically relevant DGE (grades B-C). Secondary outcomes were correlation with patient, tumor, and treatment characteristics, and non-DGE morbidity [Clavien-Dindo (CD) grade ≥ 3].Forty-two patients developed DGE and 28 had clinically relevant DGE. DGE was primary in 10 patients and secondary in 18 patients; the most common associated complications were: infections (11/18, 61.1%), pancreatic leak (7/18, 38.9%), bleeding (6/18, 33.3%), and bowel leak (6/18, 33.3%). DGE was related to longer length of hospital stay (P0.001), ICU admission (P = 0.004), ICU length of stay (P = 0.001), postoperative complications (CD [Formula: see text] 3 in 14/28 in DGE patients vs 11/72 in no-DGE; P = 0.04), and re-operation (P = 0.03). With multivariate analysis, the independent risk factors for DGE were patient comorbidities (OR 1.05; 95% CI 1.01-1.1; P = 0.04) and tumor size (OR 1.05; 95% CI 1.0-1.1; P = 0.02).Following multivisceral resection, DGE is a clinically relevant event that can be caused by an underlying complication. Prompt diagnosis and treatment of both DGE and any underlying complications led to full recovery in all cases.

The indigo blue dye is widely used in the textile industry, specifically in jeans dyeing, the effluents of which, rich in organic pollutants with recalcitrant characteristics, end up causing several environmental impacts, requiring efficient treatments. Several pieces of research have been conducted in search of effective treatment methods, among which is electrocoagulation. This treatment consists of an electrochemical process that generates its own coagulant by applying an electric current on metallic electrodes, bypassing the use of other chemical products. The purpose of this study was to evaluate the potential use of iron slag in the electrocoagulation of a synthetic effluent containing commercial indigo blue dye and the effluent from a textile factory. The quantified parameters were color, turbidity, pH, electrical conductivity, sludge generation, phenol removal, chemical oxygen demand (COD), and total organic carbon (TOC). The electrocoagulation treatment presented a good efficiency in removing the analyzed parameters, obtaining average removal in the synthetic effluent of 85% of color and 100% of phenol after 25 min of electrolysis. Le caratteristiche cliniche sono molto variabili tra i soggetti con KS, sebbene le piu comuni siano la grave compromissione della spermatogenesi e l’ipofunzione delle cellule di Leydig, con conseguente azoospermia e ipogonadismo ipergonadotropo. Inoltre, sono stati descritti diversi fenotipi neurocognitivi e psicosociali, cosi come una maggiore prevalenza di eventi avversi cardiovascolari, metabolici e correlati alla salute dell’osso che potrebbero spiegare l’incremento della mortalita osservato nella KS. Un’adeguata conoscenza e la corretta gestione clinico-diagnostica e terapeutica puo incidere positivamente sulla qualita di vita dei pazienti KS.

Background: The familial nonmedullary thyroid cancer (FNMTC) is suspected to be a Mendelian condition in up to 3–8% of thyroid cancers. The susceptibility chromosomal loci and genes of 95% of FNMTC cases remain to be characterized. The inheritance of FNMTC appears to be autosomal dominant with incomplete penetrance and variable expressivity. The finding of the causative gene of FNMTC and the identification of patients at risk that need genetic testing were our aim. Methods: We analyzed by whole-exome sequencing patients and non-affected relatives of five families with at least two family members affected by papillary thyroid cancer, selecting for new or extremely rare variants with predicted pathogenic value. Results: A family showed, in all three affected members, a new loss-of-function variant (frameshift deletion) in BROX gene at 1q41 that was absent from all internal and external databases. In a second family with three affected relatives, we found an additional new BROX variant. The smaller families presented no variants in BROX or in the other causative genes studied. Conclusions: BROX could be a new causative gene for FNMTC. Variants in BROX may result in the haploinsufficiency of a key gene involved in the morphogenesis of MVBs, in the endosomal sorting of cargo proteins, and in EGFR. Functional studies are needed to support this result. The thorough genomic analysis by NGS in all families with three or more affected members should become a routine approach to obtain a comprehensive genetic view and find confirmative second cases.

Canadian seniors who undergo hip and knee arthroplasty often experience significant postoperative pain, which could result in persistent opioid use. We aimed to document the impact of preoperative opioid use and other characteristics on postoperative opioid prescriptions in elderly patients following hip and knee replacement before widespread dissemination of opioid reduction strategies. We conducted a historical cohort study to evaluate postoperative opioid use in patients over 65 yr undergoing primary total hip and knee replacement over a ten-year period from 1 April 2006 to 31 March 2016, using linked de-identified Ontario administrative data. We determined the use of preoperative opioids and the duration of postoperative opioid prescriptions (short-term [1–90 days], prolonged [91–180 days], chronic [181–365 days], or undocumented). The study included 49,638 hip and 85,558 knee replacement patients. Eighteen percent of hip and 21% of knee replacement patients received an opioid prescription within 90 days before surgery. Postoperatively, 51% of patients filled opioid prescriptions for 1–90 days, while 24% of hip and 29% of knee replacement patients filled prescriptions between 6 and 12 months, with no impact of preoperative opioid use. Residence in long-term care was a significant predictor of chronic opioid use (hip: odds ratio [OR], 2.64; 95% confidence interval [CI], 1.93 to 3.59; knee: OR, 2.46; 95% CI, 1.75 to 3.45); other risk factors included female sex and increased comorbidities. Despite a main goal of joint arthroplasty being relief of pain, seniors commonly remained on postoperative opioids, even if not receiving opioids before surgery. Opioid reduction strategies need to be implemented at the surgical, primary physician, long-term care, and patient levels. These findings form a basis for future investigations following implementation of opioid reduction approaches.

We propose a dedicated, ultra-deep spectroscopic survey in the near infrared (NIR), that will target a mass-limited sample of galaxies during two of the most fundamental epochs of cosmic evolution: the formation of the first galaxies (at z $\gtrsim $ ≳ 6; cosmic dawn), and at the peak of galaxy formation activity (at redshift z$\sim $ ∼ 1–3; cosmic noon). By way of NIR observations (λ= 0.8–2μ m), it is possible to study the UV Lyman-α region in the former, and the optical rest-frame in the latter, allowing us to extract fundamental observables such as gas and stellar kinematics, chemical abundances, and ages, providing a unique legacy database covering these two crucial stages of cosmic evolution. The need to work in the NIR at extremely low flux levels makes a ground-based approach unfeasible due to atmospheric emission and absorption. Only with the largest facilities of the future (e.g. ELT) will be possible to observe a reduced set of targets, comprising at most of order thousands of galaxies. Likewise, from space, the small field of view of JWST and its use as a general purpose facility will yield a rather small set of high quality NIR spectra of distant galaxies (in the thousands, at best). Our project (codename ) aims to produce $\sim $ ∼ 1 million high quality spectra, with a high S/N in the continuum, where information about the underlying stellar populations is encoded. The main science drivers are: The connection between the star formation history and the mass assembly history. The role of AGN and supernova feedback in shaping the formation histories of galaxies, with a quantitative estimate of quenching timescales. The formation of the first galaxies. The source of reionization. Evolution of the metallicity-mass relation, including [α/Fe] and individual abundances. Precision cosmology through detailed studies of the “baryon physics” of galaxy formation, probing the power spectrum over scales k$\sim $ ∼ 1 Mpc− 1.

Little is known about how co-infections and genotype dynamics affect Mycoplasma hyopneumoniae infection in fattening pigs. This study was aimed at assessing the role of co-infections in M. hyopneumoniae outbreaks, their influence on the presence of M. hyopneumoniae genotypes and their impact on consequent lung lesions. Tracheobronchial swabs (TBS) from 300 finishers were collected from 10 farms at the onset of enzootic pneumonia outbreaks and 1 month later, sampling of 3 groups per farm: Group A showed clinical signs first, Group B was housed near Group A, and Group C was located in a different building. Pigs’ lungs were scored at the slaughterhouse. TBS were tested for the main pathogens involved in respiratory diseases, and samples positive for M. hyopneumoniae were genotyped by multiple-locus variable-number tandem repeat analysis (MLVA). Pigs in Group A showed the highest prevalence and load of M. hyopneumoniae. A positive association was detected between M. hyopneumoniae and Mycoplasma hyorhinis, whereas Actinobacillus pleuropneumoniae was more frequent when the M. hyopneumoniae load was higher. Nevertheless, co-infection had no effect on lung lesion scores. The presence of multiple MLVA types (mixed infections) increased in time only in pigs from Group C and was positively associated with porcine reproductive and respiratory syndrome virus infection. Lung lesions were more severe in pigs with at least one TBS positive for M. hyopneumoniae and in pigs with a history of mixed infections. The central role of M. hyopneumoniae and relevance of mixed infections suggest that increased biosecurity might be beneficial for lung lesion sequelae.

Boron nitride nanotubes (BNNTs) have attracted attention for their predicted extraordinary properties; yet, challenges in synthesis and processing have stifled progress on macroscopic materials. Recent advances have led to the production of highly pure BNNTs. Here we report that neat BNNTs dissolve in chlorosulfonic acid (CSA) and form birefringent liquid crystal domains at concentrations above 170 ppmw. These tactoidal domains merge into millimeter-sized regions upon light sonication in capillaries. Cryogenic electron microscopy directly shows nematic alignment of BNNTs in solution. BNNT liquid crystals can be processed into aligned films and extruded into neat BNNT fibers. This study of nematic liquid crystals of BNNTs demonstrates their ability to form macroscopic materials to be used in high-performance applications.

Respiratory disease trials are profoundly affected by non-pharmaceutical interventions (NPIs) against COVID-19 because they perturb existing regular patterns of all seasonal viral epidemics. To address trial design with such uncertainty, we developed an epidemiological model of respiratory tract infection (RTI) coupled to a mechanistic description of viral RTI episodes. We explored the impact of reduced viral transmission (mimicking NPIs) using a virtual population and in silico trials for the bacterial lysate OM-85 as prophylaxis for RTI. Ratio-based efficacy metrics are only impacted under strict lockdown whereas absolute benefit already is with intermediate NPIs (eg. mask-wearing). Consequently, despite NPI, trials may meet their relative efficacy endpoints (provided recruitment hurdles can be overcome) but are difficult to assess with respect to clinical relevance. These results advocate to report a variety of metrics for benefit assessment, to use adaptive trial design and adapted statistical analyses. They also question eligibility criteria misaligned with the actual disease burden.

A search for direct pair production of scalar partners of the top quark (top squarks or scalar third-generation up-type leptoquarks) in the all-hadronic tt ¯plus missing transverse momentum final state is presented. The analysis of 139 fb−1 of √s = 13 TeV proton–proton collision data collected using the ATLAS detector at the LHC yields no significant excess over the Standard Model background expectation. To interpret the results, a supersymmetric model is used where the top squark decays via t ˜ → t(∗) χ˜ 0 1 , with t(∗) denoting an on-shell (off-shell) top quark and χ˜ 0 1 the lightest neutralino. Three specific event selections are optimised for the following scenarios. In the scenario where mt ˜ > mt + mχ˜ 0 1 , top squark masses are excluded in the range 400–1250 GeV for χ˜ 0 1 masses below 200 GeV at 95% confidence level. In the situation where mt ˜ ∼ mt +mχ˜ 0 1 , top squark masses in the range 300–630 GeV are excluded, while in the case where mt ˜ < mW + mb + mχ˜ 0 1 (with mt ˜ − mχ˜ 0 1 ≥ 5 GeV), considered for the first time in an ATLAS all-hadronic search, top squark masses in the range 300–660 GeV are excluded. Bone mineral abnormalities (defined as Chronic Kidney Disease Mineral Bone Disorder; CKD-MBD) are prevalent and associated with a substantial risk burden and poor prognosis in CKD population. Several lines of evidence support the notion that a large proportion of patients receiving maintenance dialysis experience a suboptimal biochemical control of CKD-MBD. Although no study has ever demonstrated conclusively that CKD-MBD control is associated with improved survival, an expanding therapeutic armamentarium is available to correct bone mineral abnormalities. In this position paper of Lombardy Nephrologists, a summary of the state of art of CKD-MBD as well as a summary of the unmet clinical needs will be provided. Furthermore, this position paper will focus on the potential and drawbacks of a new injectable calcimimetic, etelcalcetide, a drug available in Italy since few months ago.
