Lung ultrasound (LUS) has rapidly emerged in COVID-19 diagnosis and for the follow-up during the acute phase. LUS is not yet used routinely in lung damage follow-up after COVID-19 infection. We investigated the correlation between LUS score, and clinical and laboratory parameters of severity of SARS-COV-2 damage during hospitalization and at follow-up visit. Observational retrospective study including all the patients discharged from the COVID-19 wards, who attended the post-COVID outpatient clinic of the IRCCS Policlinico San Matteo in April-June 2020. 115 patients were enrolled. Follow-up visits with LUS score measurements were at a median of 38 days (IQR 28-48) after discharge. LUS scores were associated with the length of hospitalization (p 0.001), patients' age (p = 0.036), use of non-invasive ventilation (CPAP p 0.001 or HFNC p = 0.018), administration of corticosteroids therapy (p = 0.030), and laboratory parameters during the acute phase (WBC p 0.001, LDH p 0.001, CRP p 0.001, D-dimer p = 0.008, IL-6 p = 0.045), and inversely correlated with lymphocyte count (p = 0.007). We found correlation between LUS score and both LDH (p = 0.001) and the antibody anti-SARS-CoV-2 titers (p value = 0.008). Most of these finding were confirmed by dichothomizing the LUS score (≤ 9 or 9 points). We found a significantly higher LUS score at the follow-up in the patients with persistent dyspnea (7.00, IQR 3.00-11.00) when compared to eupnoeic patients (3.00, IQR 0-7.00 p 0.001). LUS score at follow-up visit correlates with more severe lung disease. These findings support the hypothesis that ultrasound could be a valid tool in the follow-up medium-term COVID-19 lung damage.

Este artículo contiene 19 páginas, 4 figuras, 2 tablas., The extent and impacts of biological invasions on biodiversity are largely shaped by an array of socio-economic and environmental factors, which exhibit high variation among countries. Yet, a global analysis of how these factors vary across countries is currently lacking. Here, we investigate how five broad, country-specific socio-economic and environmental indices (Governance, Trade, Environmental Performance, Lifestyle and Education, Innovation) explain country-level (1) established alien species (EAS) richness of eight taxonomic groups, and (2) proactive or reactive capacity to prevent and manage biological invasions and their impacts. These indices underpin many aspects of the invasion process, including the introduction, establishment, spread and management of alien species. They are also general enough to enable a global comparison across countries, and are therefore essential for defining future scenarios for biological invasions. Models including Trade, Governance, Lifestyle and Education, or a combination of these, best explained EAS richness across taxonomic groups and national proactive or reactive capacity. Historical (1996 or averaged over 1996–2015) levels of Governance and Trade better explained both EAS richness and the capacity of countries to manage invasions than more recent (2015) levels, revealing a historical legacy with important implications for the future of biological invasions. Using Governance and Trade to define a two-dimensional socio-economic space in which the position of a country captures its capacity to address issues of biological invasions, we identified four main clusters of countries in 2015. Most countries had an increase in Trade over the past 25 years, but trajectories were more geographically heterogeneous for Governance. Declines in levels of Governance are concerning as they may be responsible for larger levels of invasions in the future. Based on the available literature, women living with HIV (WLWH) seem to show greater cognitive and emotional disadvantages than men living with HIV (MLWH). Our aim was to compare the cognitive performance of MLWH and WLWH in an Italian cohort of People Living With HIV (PLWH) and to analyse factors potentially contributing to sex differences in cognitive function. We ran a retrospective, cross-sectional analysis of a monocentric dataset of PLWH who were administered a standardized neuropsychological test battery (SNB) during routine clinical care. We enrolled 161 Italian PLWH who are on combined antiretroviral therapy (cART): 114 (70.8%) MLWH and 47 (29.2%) WLWH.Global cognitive performance (composite z score) (GCP) was significantly higher in MLWH than WLWH [mean 0.19 (SD 0.85) vs − 0.13 (SD 0.96); p = 0.039]. Moreover, WLWH obtained significantly higher scores on the Zung Depression Scale than MLWH [mean 41.8 (SD 10.9) vs 36.7 (SD 9.2); p = 0.003]. However, there was no statistically significant direct effect between male sex and better GCP (p = 0.692) in the context of a mediation model. On the contrary, the associations between male sex and better GCP were mediated by higher level of education (a*b = + 0.15, Bootstrap CI95 = 0.05 and 0.27) and a lower Zung depression score (a*b = + 0.10, Bootstrap CI95 = 0.02 and 0.21).In conclusion, the global cognitive performance of WLWH is lower than that of MLWH. However, other demographic and clinical factors besides sex might help explain differences in their neurocognitive functions and make it possible for us to monitor them and identify those patients most in need.

The Anthropocene is characterized by unparalleled human impact on other species, potentially ushering in the sixth mass extinction. Yet mitigation efforts remain hampered by limited information on the spatial patterns and intensity of the threats driving global biodiversity loss. Here we use expert-derived information from the International Union for Conservation of Nature Red List on threats to 23,271 species, representing all terrestrial amphibians, birds and mammals, to generate global maps of the six major threats to these groups: agriculture, hunting and trapping, logging, pollution, invasive species, and climate change. Our results show that agriculture and logging are pervasive in the tropics and that hunting and trapping is the most geographically widespread threat to mammals and birds. Additionally, current representations of human pressure underestimate the overall pressure on biodiversity, due to the exclusion of threats such as hunting and climate change. Alarmingly, this is particularly the case in areas of the highest biodiversity importance., Red List information is used to generate global maps of the likelihood of impacts on terrestrial vertebrates exerted by agriculture, hunting and trapping, logging, pollution, invasive species and climate change.

Many interesting applications of hyperbolic systems of equations are stiff, and require the time step to satisfy restrictive stability conditions. One way to avoid small time steps is to use implicit time integration. Implicit integration is quite straightforward for first-order schemes. High order schemes instead also need to control spurious oscillations, which requires limiting in space and time also in the linear case. We propose a framework to simplify considerably the application of high order non-oscillatory schemes through the introduction of a low order implicit predictor, which is used both to set up the nonlinear weights of a standard high order space reconstruction, and to achieve limiting in time. In this preliminary work, we concentrate on the case of a third-order scheme, based on diagonally implicit Runge Kutta ($$\mathsf {DIRK}$$ DIRK ) integration in time and central weighted essentially non-oscillatory ($$\mathsf {CWENO}$$ CWENO ) reconstruction in space. The numerical tests involve linear and nonlinear scalar conservation laws.

Many ST-segment elevation acute coronary syndrome (STEACS) patients fail to activate the Emergency Medical System (EMS), with possible dramatic consequences. Prior studies focusing on barriers to EMS activation included patients with any acute coronary syndrome (ACS) without representation of southern European populations. We aimed to investigate the barriers to EMS call for patients diagnosed for STEACS in Italy. A prospective, single-center, survey administered to all patients treated with primary percutaneous coronary intervention for STEACS in a tertiary hospital in northern Italy from 01/06/2018 to 31/05/2020. The questionnaire was filled out by 293 patients. Of these, 191 (65.2%) activated the EMS after symptoms onset. The main reasons for failing to contact EMS were the perception that the symptoms were unrelated to an important health problem (45.5%) and that a private vehicle is faster than EMS to reach the hospital (34.7%). Patients who called a private doctor after symptoms onset did not call EMS more frequently than those who did not and 30% of the patients who did not call the EMS would still act in the same way if a new episode occurred. Previous history of cardiovascular disease was the only predictor of EMS call. Information campaigns are urgently needed to increase EMS activation in case of suspected STEACS and should be primary focused on patients without cardiovascular history, on the misperception that a private vehicle is faster than EMS activation, and on the fact that cardiac arrest occurs early and may be prevented by EMS activation. Many types of brachioplasty techniques have been described in the literature, and the main focus has been scar aesthetics, reproducibility and safety. Little attention has been given to other two aspects of the procedure: overall aesthetic with a focus on the torso-brachial angle and on the lymphatic distress related to the procedure. In this paper, we described a novel technique of brachioplasty called lipobrachiopexy, a lymph-sparing procedure which includes tendon suspension suture to improve cosmetics . Over 18 months, 22 consecutive patients underwent bilateral lipobrachioplasty with circumferential liposuction sparing brachial artery perforators, J-scar dermolipectomy and superficial fascia suspension to the pectoralis major tendon. Aesthetic outcomes, lymphatic function, sensory function and patient’s satisfaction were evaluated at 1-year follow-up. The correction of the bat wing deformity and the shape of the transition of the upper arm to the chest was evaluated by quantifying the torso-brachial angle using Photoshop. Lymphatic function was analysed pre-operatively at 1, 6 and 12 months after surgery by indocyanine green lymphography (PDE, Hamamatsu Photonics, Japan). An average of 447.5 cc (range, 350–550 cc) of fat was aspirated for each side. No major complications were experienced. Patients’ and surgeons’ satisfaction was high to very high in all cases. The lymphatic function was found preserved, with the same physiological linear patterns and tracer progression pre-operatively and 1 year after surgery. The torso-brachial angles showed significant improvements (86.7 ± 14.7–100.7 ± 10.2 right side and 85.4 ± 16.3–101.5 ± 9.9 left side). Lipobrachiopexy is a safe and effective technique that adds to the recent trends in brachioplasty, a reestablishment of the brachial fascial systems and addressing the anatomical etiological factor of the bat wing deformity. It has become evident that term weighting has a significant effect on relevant document retrieval for which various methods are proposed. However, the main question that arises is which weighting method is the best? In this paper, it is shown that proper aggregation of weights generated by carefully selected basic weighting methods improves retrieval of the relevant documents with respect to the user’s needs. Toward this aim, it is shown that even using simple central tendency measures such as average, median or mid-range over an appropriate subset of basic weighting methods provides term weight that not only outperforms using each basic weighting method but also results in more effective weights in comparison with recently proposed complicated weighting methods. Based on exploiting the proposed method on various datasets, we have studied the effects of normalization of the basic weights, normalization of the vector lengths, the use of different components in the term frequency factor, etc. Results reveal the criteria for selecting an appropriate subset of basic weighting methods that would be fed to the aggregator in order to achieve higher retrieval precision.

Observed variations in the ground electrical conductivity (σb*) measurements obtained with EMI instruments depend on several soil proxies that infuence crop growth and development such as salinity (σe), contents of water (θw), clay (wc) and organic matter (wom) and bulk density (ρb). However, the relative contributions of all these to σb* are unknown. This knowledge is needed to improve the planning and interpretation of σb* data for precision agriculture applications. Recently, a semi-empirical model has been developed to relate σb* measurements taken with an EM38 device to σe, θw, wc, wom and ρb and also soil temperature (t). In this work this model was subjected to a global sensitivity analysis (GSA) based on the soil data obtained during two surveys carried out, one in summer and the other in autumn, in an ample irrigated area in SE Spain. On the basis of the multiple linear regression meta-models developed for the σb* measurements, these were found to linearly respond to the soil properties (R2 between 0.92 and 0.96) and thus, the GSA could be based on their standardised regression coefcients. According to these, the soil characteristics explain the following percentages of variance in σb* (PV): 30–34 (σe), 8–20 (θw), 32–47 (wc), 0.6–2.6 (wom), 5.7–7.5 (ρb) and 0.3–0.4 (t) with changes from the summer to the autumn season of − 4, − 12 and+15 in the PV explained by the most infuential properties, respectively, σe, θw and wc. The results of the GSA will help the planning and interpretation of σb* measurements for improving crop management.

The interaction between proteins is a fundamental event for cellular life that is generally mediated by specialized protein domains or modules. PDZ domains are the largest class of protein–protein interaction modules, involved in several cellular pathways such as signal transduction, cell–cell junctions, cell polarity and adhesion, and protein trafficking. Because of that, dysregulation of PDZ domain function often causes the onset of pathologies, thus making this family of domains an interesting pharmaceutical target. In this review article we provide an overview of the structural and functional features of PDZ domains and their involvement in the cellular and molecular pathways at the basis of different human pathologies. We also discuss some of the strategies that have been developed with the final goal to hijack or inhibit the interaction of PDZ domains with their ligands. Because of the generally low binding selectivity of PDZ domain and the scarce efficiency of small molecules in inhibiting PDZ binding, this task resulted particularly difficult to pursue and still demands increasing experimental efforts in order to become completely feasible and successful in vivo.

Engineering cyber-physical systems inhabiting contemporary urban spatial environments demands software engineering facilities to support design and operation. Tools and approaches in civil engineering and architectural informatics produce artifacts that are geometrical or geographical representations describing physical spaces. The models we consider conform to the CityGML standard; although relying on international standards and accessible in machine-readable formats, such physical space descriptions often lack semantic information that can be used to support analyses. In our context, analysis as commonly understood in software engineering refers to reasoning on properties of an abstracted model—in this case a city design. We support model-based development, firstly by providing a way to derive analyzable models from CityGML descriptions, and secondly, we ensure that changes performed are propagated correctly. Essentially, a digital twin of a city is kept synchronized, in both directions, with the information from the actual city. Specifically, our formal programming technique and accompanying technical framework assure that relevant information added, or changes applied to the domain (resp. analyzable) model are reflected back in the analyzable (resp. domain) model automatically and coherently. The technique developed is rooted in the theory of bidirectional transformations, which guarantees that synchronization between models is consistent and well behaved. Produced models can bootstrap graph-theoretic, spatial or dynamic analyses. We demonstrate that bidirectional transformations can be achieved in practice on real city models.

Prognostication after an out-of-hospital cardiac arrest (OHCA) remains a challenge. The peripheral-derived perfusion index (PI) is a simple and non-invasive way to assess perfusion. We sought to assess whether the PI was able to discriminate the prognosis of patients resuscitated from an OHCA. All the reports generated by the manual monitor/defibrillator (Corpuls 3 by GS Elektromedizinische Gerate G. Stemple GmbH, Germany) used for all the OHCAs who achieved ROSC treated by our Emergency Medical Service from January 2015 to December 2018 were reviewed. The mean PI value of each minute after ROSC was automatically provided by the device and the mean value of 30 min of monitoring (MPI30) was calculated. Pre-hospital data were collected according to the Utstein 2014 recommendations. Among 1,909 resuscitation attempts, ROSC was achieved in 346 and it was possible to calculate an MPI30 in 164. MPI30 was higher in the patients who survived at 30 days [1.6 (95% CI 1.2–2.1) vs 1 (95% CI 0.8–1.3), p = 0.0017]. At the multivariable Cox regression model, after correction for shockable rhythm, witnessed status, bystander CPR, age, and blood pressure, MPI30 was found to be an independent predictor of both 30-day mortality [RR 0.83 (95% CI 0.69–0.99), p = 0.036] and 30-day mortality or poor neurologic outcome [RR 0.85 (95% CI 0.72–0.99), p = 0.04]. Overall 30-day survival with good neurologic outcome was significantly different in the three tertiles [T1: 0.1–0.8; T2: 0.9–1.8 and T3: 1.82–7.8, log-rank p = 0.007]. The post-ROSC peripheral perfusion index was found to be an independent predictor of 30-day mortality or poor neurologic outcome. It could help prognostication in OHCA patients.

The increasing demand for further aesthetic outcome improvement in implant-based breast reconstruction after nipple-sparing mastectomy (NSM) leads to major novelties, including endoscopic and robotic NSM with the aim to limit scar visibility. In this paper, we report our experience with a novel and simplified surgical approach—single-axillary-incision non-endoscopic NSM and node surgery followed by an endoscopic DTI breast reconstruction—by focusing on reconstructive indications, technical refinements and aesthetic outcomes. Between June 2016 and October 2019, 14 women underwent this novel technique using definitive anatomical silicone gel filled, totaling 20 breasts. Reconstructive evaluation methods were clinically and photography-based assessment. Breast-Q has been used to quantify patient satisfaction. Feasibility and safety data are also provided. The average follow-up time was 11 months (range 3–42 months). No local–regional recurrence occurred during follow-up. Median operation time was 340 min; mean hospital stay was 4.1 days. The average mastectomy specimen weight was on average 139 g (ranging from 98 to 182 g). The average implant volume for the reconstructed side was 306 cc (ranging from 165 to 550 cc). Patients’ satisfaction was high to very high. Authors’ experience suggests that non-endoscopic transaxillary NSM, node surgery and endoscopic direct-to-implant breast reconstruction is a valid, oncological safe, aesthetically sound scarless option in breast cancer patients with small to moderate breast size. It should represent the incision of choice in patient with previous breast surgery with scars that may compromise flap/NAC vascularity using traditional NSM incisions.
