Treatment of brackish water from pathogenic microbes is crucial for sustainable aquaculture production and preventing the spread of infectious diseases. However, the treatment of brackish water is still challenging due to the high salinity and the high antimicrobial resistance. Here, we exploit a facile and effective approach to synthesize silica gel embedded with silver nanoparticles (7–48 nm) for broad-spectrum antimicrobial activity. The incorporation of silver nanoparticles into silica gel (AgNPs@SG) is confirmed by flame atomic absorption spectrometry, X-ray diffraction, N2 physisorption, and transmission electron microscopy. The AgNPs@SG material exhibits wide-spectrum antimicrobial activity against the studied microorganisms (Pseudomonas aeruginosa, Escherichia coli, and Candida albicans) due to preventing the aggregation of silver nanoparticles and their effective contact with the microorganisms. Most importantly, the applicability of the synthesized AgNPs@SG for the microbial treatment of brackish water is investigated on different water samples collected from Manzala Lake. Remarkably, the amount of viable bacteria in the brackish water decreases by about 93% using AgNPs@SG material that not only combats antibiotic-resistant strains but also works under harsh conditions such as multiple-source contamination, high eutrophic state, and salinity.

To evaluate arterial spin labeling (ASL) and diffusion-weighted imaging (DWI) in discrimination of benign from malignant paranasal sinus (PNS) tumors.A prospective study was done upon 42 cases of PNS masses that underwent magnetic resonance ASL and DWI of the head. Tumor blood flow (TBF) and apparent diffusion coefficient (ADC) of the masses were calculated by two observers. The pathological diagnosis was malignant (n = 28) and benign (n = 14) cases.For both observers, the malignant PNS masses had significantly higher TBF (P 0.001, 0.001) and lower ADC (P 0.001, 0.001) than in benign masses. The ROC curve analysis of TBF, The threshed TBF was (121.45, 122.68 mL/100 g/min) used for differentiation between benign and malignant PNS masses, revealed sensitivity (92.9%, 89.3%), specificity (85.7%, 85.7%), accuracy (90.5%, 88.1%) and the AUC was 0.87 and 0.86 by both observers. the ROC curve analysis of ADC, The threshold ADC (1.215, 1.205 X10Combined using of TBF and ADC have a role in differentiation malignant from benign PNS masses.

Purpose To assess the role of diffusion tensor imaging in assessing liver and splenic parenchymal infiltration in Gaucher’s disease (G.D.) type I and III before and after therapy. Methods A prospective study was conducted upon 28 consecutive patients with G.D. type I and III and 28 age and sex-matched controls. They underwent an MRI and DTI of the liver and spleen. Mean diffusivity (M.D.) and fractional anisotropy (F.A.) values of the liver and spleen were evaluated before and after treatment and compared with control. Results There was a statistically significant difference in the M.D. value of the liver and spleen between untreated patients and controls and between control and treated patients and in the M.D. value of the liver and spleen between untreated and treated patients. There is a statistically significant difference in the F.A. value of the liver and spleen between untreated patients and controls and in the F.A. value of the liver and spleen between untreated and treated patients. Hemoglobin level was positively correlated with the M.D. value of the spleen. Clinical score was negatively correlated with M.D. value of the spleen and was positively correlated with F.A. values of the liver and F.A. values of the spleen. Spleen volume was negatively correlated with M.D. values of the spleen. Conclusion Significant difference in M.D. and F.A. values of liver and splenic parenchyma in p atients with type I and III G.D. and controls, and between untreated and treated patients. The M.D. and F.A. values were well correlated with some biomarkers of disease activity.

Bone marrow embolism (BME) is likely a consequence of fractures in which pulmonary vessels are the most affected. However, some cases of BME were reported in the absence of trauma. Thus, a traumatic injury might not be necessary for developing BME. This study discusses BME cases in patients without signs of fractures or blunt trauma. The discussion addresses various possible mechanisms for the appearance of BME. Options include cancer in which bone marrow metastasis is a suggestive cause. Another proposal is the chemical theory where bone marrow fats are released via lipoprotein lipase in a pro-inflammatory state, resulting in vascular/pulmonary obstruction. Other cases discussed in this study are hypovolemic shock and drug-abuse related BME. All autopsy cases with BME were included regardless of the cause of death for a period of 2 years. Autopsies involved complete dissection with the macroscopic evaluation of the affected organs, including the heart, lungs, and brain. Tissues were also prepared for microscopic examination. Of the 11 cases, eight showed non-traumatic BME (72%). These findings conflict with theories in the literature that BME most commonly occurs after fractures or trauma. One of the eight cases exhibited mucinous carcinoma; one is presented with hepatocellular carcinoma; and two cases showed severe congestion. Lastly, one case was found to be associated with each of the following conditions: liposuction, drug abuse, pulmonary hypertension, and heart failure. Each case suggests a different pathophysiology for developing BME, yet the exact mechanisms are not fully understood. Further study of non-traumatic associated BME is recommended.

Background Neuropsychiatric manifestations of lupus (NPSLE) are considered one of the major and most devastating lupus manifestations. The aim of this study was to assess the neuropsychiatric manifestations in systemic lupus erythematosus (SLE) patients and estimate the effectiveness of brain magnetic resonance spectroscopy (MRS) and anti-ribosomal P antibody test in early detection of NPSLE. This cross-sectional study was carried out on 50 SLE patients. Demographic, clinical, and therapeutic data were assessed. All patients were subjected to thorough rheumatological and neuropsychiatric evaluation. Serologic tests included antinuclear antibodies, anti-double-stranded DNA, and anti-ribosomal P protein antibodies. Radiologic evaluation included brain MRS. Results The mean age was 26.9 ± 98.9 years; the median disease duration was 18 (0–108) months. Headache was the most common neurological symptom (40%). Depression was not detected in 29 patients (58%), mild in 15 patients (30%), and moderate in 6 patients (12%). Anti-ribosomal P antibody titer was significantly elevated in patients with active in comparison with those with inactive lupus disease (p = 0.026). Brain MRS showed a statistically significant reduction in N-acetylaspartate creatine ratio (NAA/Cr) among patients with active lupus disease (p = 0.015) with a statistically significant increase in choline creatine ratio (Cho/Cr) among patients with inactive lupus disease (p = 0.049). There was a statistically significant negative correlation between the level of NAA/Cr and anti-ribosomal P antibody titer among patients with active lupus disease (p < 0.001). Conclusions Headache is the most common neurological manifestation among SLE patients. Anti-ribosomal P antibody titer is elevated in active SLE patients. The changes of NAA/Cr and Cho/Cr in brain MRS can be of help to differentiate between the active and inactive SLE.

Background The pro-inflammatory cytokine, interleukin 16 (IL-16), has been shown to be secreted in low levels in knee osteoarthritis (KOA). The aim of the study was to examine the relationship between IL-16 polymorphisms and the risk of KOA in the Egyptian population, as well as the clinical and radiographic severity of KOA. Results IL16 rs11556218 thymidine triphosphate (T) T G (guanosine triphosphate), GG, TG + GG genotypes, and G allele (odd ratio (OR) = 0.315; 95% confidence interval (CI) = 0.191–0.518; P P = 0.014; OR = 0.323; 95% CI = 0.202–0.519, P P P = 0.008; OR = 0.309, 95% CI = 0.105–0.916, P = 0.034; OR = 0.485, 95% CI = 0.304–0.775, P = 0.002; OR = 0.537, 95% CI = 0.365–0.791, P = 0.001 respectively); and rs4072111 CT, TT, CT + TT genotypes, and T allele (OR = 0.537, 95% CI = 0.323–0.893, P = 0.017, OR = 0.316, 95% CI = 0.096–0.843, P = 0.049, OR = 0.502, 95% CI = 0.309–0.816, P = 0.005; OR = 0.534, 95% CI = 0.353–0.809, P = 0.004 respectively) were associated with a decreased KOA risk, and they were significantly associated with decreased the Western Ontario and McMaster Universities Arthritis Index (WOMAC) and the Kellgren-Lawrence (K/L) scores. Neither IL-16 serum levels nor IL-16 polymorphisms were associated with the susceptibility to KOA. Low KOA risk was associated with the haplotypes GTC and TCT. Conclusion There was no correlation between serum IL-16 levels and KOA susceptibility or IL-16 polymorphisms. GTC and TCT haplotypes were associated with low KOA risk. The variant alleles rs11556218GG, TG + GG; rs4778889 CC, TC + CC; and rs4072111 TT, CT + TT were associated with a reduced risk of KOA.

Heavy metal (HM) pollution is a worldwide environmental issue. Given the urgent need to develop more powerful approaches for effective phytoremediation of HMs, isolation of novel endophytic strains from hyperaccumulator plants having potent HM tolerance is the main objective in this research. Moreover, the recovered strains were characterized and subjected to radiation mutagenesis to enhance their tolerance to HMs. Among 105 isolates, Alternaria alternata AUMC14431 was identified as the most effective Cd+2 tolerant strain having high recorded tolerance index (TI) (76.24%); in addition, the recorded minimum inhibitory concentration (MIC) was 300 ppm. Meanwhile, Chaetomium globosum AUMC14432 was identified as the most effective Pb+2 and Ni+2 tolerant strain having high recorded TI (97.46 and 93.34%, respectively); in addition, the evaluated MICs were 250 and 200 ppm, respectively. UV and gamma irradiation of the tested strains enhanced their Cd+2 and Pb+2 tolerance significantly (P ≤ 0.05). Meanwhile, irradiation had a negative impact on Ni+2 tolerance of C. globosum. The mutation incidence at the molecular level arising from exposure to irradiation was investigated. Genomic DNA of both the wild and mutated endophytic strains were isolated followed by random amplified polymorphic DNA (RAPD-PCR) analysis, using two short primers. A remarkable difference in DNA gel pattern between the wild type and mutated strains was observed. In conclusion, the novel isolated and irradiated endophytic strains, A. alternata S5 and C. globosum El26, having high efficiency in Cd+2 and Pb+2 tolerance, respectively, are considered to be prospective and powerful bioremediation candidates for potential application in microbially assisted phytoremediation.

Background Outcome of HCV treatment with direct antiviral agents in malignant patients is questionable. The aim is to assess the safety and efficacy of DAAs in treatment of chronic HCV patients who received chemotherapy for malignancies. Materials Retrospective cohort study of 83 patients with HCV post chemotherapy receiving DAAs treatment compared to a matched group of 88 chronic HCV patients without cancer. Demographic, laboratory and abdominal ultrasound data, and SVR were taken for all patients. Results Patients’ data revealed mean age (52 years) and BMI (29). A total of 52% of HCV patients were females, and 83.6% were treatment naïve. Patients with cancer had higher FIB4 values and more cirrhosis (20.5% vs. 13.6%) with no statistical significance. Total bilirubin and HbA1C levels were significantly higher in HCV patients without cancer. All patients in either groups received SOF-based DAAs except 2 cases received PAR/OMP/RBV. SVR rate was very high and comparable between the two groups (100% and 97.7% in post chemotherapy and control groups) with no statistical difference. Mortality was represented in 23% in patients post chemotherapy with FIB4 score considered the only predictor for mortality. Conclusion DAAs have excellent efficacy in patients post chemotherapy. Further studies should be conducted for their concomitant use with chemotherapy.

To evaluate the diagnostic value of diffusion tensor imaging (DTI) of renal cortex in assessment of lupus nephritis (LN) and prediction of its pathological subtypes. Prospective study was performed upon 39 female patients with pathologically proven LN and 16 sex- and age-matched healthy controls. Patients and controls underwent DTI of kidney. Mean diffusivity (MD) and fractional anisotropy (FA) of renal cortex were calculated by two radiologists. LN patients were pathologically classified into either non-proliferative (n = 15) or proliferative (n = 24). Mean MD of renal cortex in LN was significantly lower (p = 0.001) than that of controls with cut-off (2.16 and 2.2 X10−3mm2/s), area under curve (AUC) of (0.92, 0.94) and accuracy of (91%, 89%) for both observers. Mean FA of renal cortex in LN was significantly higher (p = 0.001) than that of controls with cut-off (0.20, 0.21), AUC of (0.86, 0.82) and accuracy of (86%, 84%) for both observers. Renal cortex MD and FA in non-proliferative LN were significantly different (p = 0.001) from that of proliferative LN for both observers. There was excellent inter-observer agreement of MD and FA (ICC = 0.96 and 0.81). MD and FA of renal cortex may help to assess renal affection in LN patients and predict its pathological subtypes.

Failure in earth dams mainly occurs due to seepage to the unprotected downstream face of the dam. Therefore, many precautions are taken into consideration to protect dams from this phenomenon. Construction horizontal, inclined, trapezoidal or pipe filters one of the dam protection methods. Cut off also can be constructed to minimize seepage discharge directed to the downstream face of the dam. Seepage through an earth dam with internal cut off is experimentally studied in the laboratory of Irrigation Engineering and Hydraulics Department, Faculty of Engineering, Alexandria University, Egypt on a Hele-Shaw model. Effect each of the relative penetration depth (d/D) and the relative distance of cut off (X/Dw) on both the relative drop in the total head upstream (∆H/H) and relative length of filter (Lf/H) is studied. The pervious layer thickness beneath the dam is also studied to investigate its effect on the filter length and total seepage discharge. The optimum position of the cut off also is obtained from analysis of the experimental results. Experimental results are drawn in dimensionless form.

To assess diffusion tensor imaging (DTI) of spleen in prediction and grading of esophageal varices (OV) in cirrhotic children. This prospective study was conducted upon 30 children with cirrhotic children with OV and 10 age-gender matched controls that underwent DTI of abdomen. Mean diffusivity (MD) and fractional anisotropy (FA) of spleen were calculated and matched with the grading of OV at endoscopy and laboratory biomarkers of portal hypertension. Mean ADC of spleen in patient was significantly different (p = 0.001) from that of controls by both reviewers respectively. The cutoff ADC measurement of the spleen used for prediction of OV was ≥ 0.75 and ≥ 0.76 × 10−3mm2/s with AUC was 0.993 and 0.997 for both reviewers respectively. The FA of the spleen in patient was different (p = 0.01) from of controls of both reviewers respectively. Cutoff FA of spleen used for prediction of OV was ≤ 0.35 and ≤ 0.36 for both observers respectively. ADC and FA of spleen was correlated with platelets count (r = − 0.713, 0.392; p = 0.001, 0.012) and prothrombin time (r = 0.518, − 0.380; p = 0.001, 0.016). DTI metrics of spleen can predict and grade OV and correlated with laboratory biomarkers of portal hypertension.

Background Early diagnosis and appropriate management of shock aimed at prevention of prolonged hypoperfusion has shown to decrease morbidity and mortality in patients with undifferentiated shock. However, there is often a challenge in emergency department (ED) – where diagnosis is mainly based on clinical signs and standard monitoring parameters. Early use of point of care ultrasound could reduce the diagnostic time and improve diagnostic accuracy. Purpose The aim of this study is to investigate the accuracy of echocardiography - ultrasound protocol to identify the cause of shock in ED. Method The study was conducted on 150 shocked patients admitted to emergency department of Alexandria Main University Hospital from December 2018 to December 2020. The study was conducted to reach initial impression about shock etiology which was then compared to final diagnosis to determine accuracy, agreement, sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV). Results One hundred forty patients were included in the study (10 patients were excluded). The protocol was 100% accurate for diagnosing cases with obstructive and mixed obstructive distributive shock. It showed excellent rule- out characteristics for cardiogenic shock (sensitivity and NPV = 100%). There was almost perfect agreement between provisional and final shock type for mixed distributive cardiogenic shock (kappa 0.915). Echo- US protocol had lowest agreement and PPV for patients with hypovolemic shock Kappa 0.48 and 35% respectively. Conclusion The Echo- US protocol showed a high accuracy in identifying shock etiology in ED and is likely a promising diagnostic tool in emergency care.

Chemical investigation of the methanolic extract of endophytic Aspergillus niger SB4, isolated from the marine alga Laurencia obtuse, afforded the pentacyclic polyketide, RF-3192C (1), the dimeric coumarin orlandin (2), fonsecin B (3), TMC-256A1 (4), cyclo-(Leu-Ala) (5), and cerebroside A (6).The chemical structure of RF-3192C (1) is assigned herein for the first time using 1D/2D NMR and HRESI-MS. Additionally, the revision of the NMR assignments of orlandin (2) was reported herein as well. Investigation of the antimicrobial activities of isolated compounds revealed the high activity of RF-3192C (1) against Pseudomonas aeruginosa and Bacillus subtilis, and moderate activity against yeast. Moreover, an in vitro cytotoxic activity against liver (HEPG2), cervical (HELA), lung (A549), prostate (PC3), and breast (MCF7) cancer cell lines of the isolated compounds was evaluated. The isolation and taxonomical characterization of the producing fungus was reported as well.

To differentiate pediatric solid malignant from the benign parotid tumors with diffusion-weighted MR imaging (DWI). A retrospective study comprising 38 children with parotid tumors (21 boys and 17 girls aged from 2 months to 17 years) was conducted using (DWI) of the parotid gland. Apparent diffusion coefficient (ADC) maps were generated. The ADC value of the parotid tumors was calculated. The mean ADC value of malignant parotid tumors (1.08 ± 0.1, 1.04 ± 0.1 × 10−3mm2/s) was significantly lower [P = 0.001] than that of benign lesions (1.69 ± 0.2, 1.72 ± 0.3 × 10−3mm2/s). A threshold of ADC of 1.40, 1.33 × 10−3mm2/s was used for differentiating malignant parotid tumors from benign lesions and led to the best results of the area under the curve of 0.940, 0.929, accuracy of 86, 89%, sensitivity of 94, 94%, specificity of 80, 85%, negative predictive value of 94.1, 94.4%, and positive predictive value of 81, 85%. There was insignificant difference in ADC values of malignant lesions (P = 0.23, 0.30) as well as within benign lesions (P = 0.25, 0.08). DWI is an innovative anticipating imaging technique that can be used in the differentiation of pediatric solid malignant parotid tumors from benign lesions.

Aiming to develop specific diagnostic tool for invasive fungal infection, a radiopharmaceutical based on anidulafungin was investigated. A two-step approach was used to label the drug with technetium-99m to retain its biological activity. 99mTc-tricarbonyl-Anidulafugin complex was obtained in high radiochemical yield (~98%). The complex was radio-chemically characterized and physico-chemically evaluated to assess its suitability as a radiopharmaceutical. Thereafter, it was evaluated in vitro and in vivo using different models; sterile inflammation, bacterial and fungal infection in mice and showed specific accumulation [C. albicans model (~6.5%), A. fumigatus model (~5.2%)]. All results supported its usefulness as specific tracer for fungal infection.

To describe the MR and CT imaging features of organized hematomas [OH]. A retrospective analysis of MR imaging and CT of eight patients [6 male, 2 females; age range is 6–86 years] with pathological proven OH was performed. Routine contrast MR and CT scan were done for all patients, and diffusion-weighted imaging and dynamic contrast CT were done for only five patients. Imaging analysis was done for the signal intensity, the enhancement pattern at MR imaging, the ADC value at DWI, bone remodeling at CT, and the neo-vascularization at dynamic contrast CT. OH revealed T2 hypo-intense of the matrix in all patients with complete [n = 3] and incomplete [n = 5] marginal hypo-intensity rim. The lesions revealed a frond-like pattern of contrast enhancement of the central part of the lesion that associated with a peripheral rim of contrast enhancement. OH revealed unrestricted diffusion with high ADC value [1.7–2.5 × 10–3 cm2/s] in 5 patients. CT scan showed geographic bone remodeling and thinning of the nasal turbinates, and the sinus wall and contrast CT showed neo-vascularization with frond-like branching arteries of the central part of the lesions in five patients. We conclude that MR and CT imaging findings can help in the diagnosis of OH.

To evaluate the role of susceptibility-weighted imaging (SWI) in patients with idiopathic intracranial hypertension (IIH). A prospective study was done on 55 patients with IIH who underwent SWI of the brain. The images were evaluated by two independent readers for cerebral microbleeds (CMBs) and the interobserver agreement between both readers was calculated. The graphic rating scale (GRS) for headache was calculated. CMBs were found in 16 (29%) of patients with IIH for both readers with excellent interobserver agreement (K = 0.8, p = 0.001). There was excellent interobserver agreement for location (K = 0.8, p = 0.001) and side of CMBs (K = 0.8, p = 0.001). There was good interobserver agreement for size of CMBs (K = 0.75, p = 0.002) and number (K = 0.6, p = 0.006). The mean GRS for headache in patients with CMBs (5.61 ± 1.3) was significantly higher (p = 0.02) than that of patients without CMBs (4.9 ± 0.8). We concluded that SWI can detect CMBs in patients with IIH especially in patients with higher GRS for headache.

Background Acute-on-chronic liver failure (ACLF) is a distinct syndrome associated with high short-term mortality. Early identification of patients at high risk is essential to determine emergency for transplantation and decide and prioritize the need for intensive care unit (ICU). We aimed to evaluate the performance of the different prognostic scores in the prediction of in-hospital mortality in patients with ACLF. A total of 249 patients with ACLF were included and followed till discharge from the hospital. Univariate and Cox regression analyses were used to assess the performance of liver-specific (Child-Pugh and MELD) and ACLF prognostic scores (CLIF-C OF, CLIF-SOFA, CLIF-C AD, CLIF-C ACLF) in the prediction of in-hospital mortality. Results Patients were mostly males (71.1%) with a mean age of 53.9 ± 12.8 years. The etiology of pre-existing liver disease was HCV in 57.8%. Sepsis was the most common precipitating factor (49.8%) and the mortality rate was 74.3%. In univariate analysis, all scores were significantly higher in the deceased group (POR 3.25, 95% CI 1.03–10.25, POR 1.04, 95% CI 1.02–1.06, P=0.044). Conclusions All the studied scores could predict in-hospital mortality of patients with ACLF. However, CLIF-C ACLF and Child-Pugh class performed better as they could significantly and independently predict mortality.

Background Cardiogenic shock (CS) is associated with significant morbidity and mortality. The impact of beta-blocker (BB) use on patients who develop CS remains unknown. We sought to evaluate the clinical outcomes and hemodynamic response profiles in patients treated with BB in the 24 h prior to the development of CS. Methods Patients with CS enrolled in the DObutamine compaREd to MIlrinone trial were analyzed. The primary outcome was a composite of all-cause mortality, resuscitated cardiac arrest, need for cardiac transplant or mechanical circulatory support, non-fatal myocardial infarction, transient ischemic attack or stroke, or initiation of renal replacement therapy. Secondary outcomes included the individual components of the primary composite and hemodynamic response profiles derived from pulmonary artery catheters. Results Among 192 participants, 93 patients (48%) had received BB therapy. The primary outcome occurred in 47 patients (51%) in the BB group and in 52 (53%) in the no BB group (RR 0.96; 95% CI 0.73–1.27; P = 0.78) throughout the in-hospital period. There were fewer early deaths in the BB group (RR 0.41; 95% CI 0.18–0.95; P = 0.03). There were no differences in other individual components of the primary outcome or in hemodynamic response between the two groups throughout the remainder of the hospitalization. Conclusions BB therapy in the 24 h preceding the development of CS did not negatively influence clinical outcomes or hemodynamic parameters. On the contrary, BB use was associated with fewer deaths in the early resuscitation period, suggesting a paradoxically protective effect in patients with CS. The current investigation was carried out to assess the efficacy of dietary garlic (GP) and/or chitosan (CH) powders to protect European sea bass, Dicentrarchus labrax, against the adverse effects of zearalenone (ZEN) toxicity. Hence, ZEN (0.725 g/kg diet) was incorporated in a fish diet alone or with 30 g GP and/or 10 g CH/kg diet and the treatments were assigned as a control diet (T1), a control diet + ZEN (T2), a control diet + ZEN + GP (T3), a control diet + ZEN + CH (T4), and a control diet + ZEN + GP + CH (T5). Fish (30.7 ± 0.6 g) fed one of the tested diets up to apparent satiation thrice a day for 4 weeks. After the feeding trial, fish were intraperitoneally injected with pathogenic bacteria Vibrio alginolyticus for 14 days during which fish mortality was observed. The dietary ZEN inhibited fish growth and feed utilization; meanwhile, co-supplementation of GP and/or CH restored the reduced fish performance as a result of reducing the ZEN toxicity. The ZEN toxicity induced macrocytic hypochromic anemia in European sea bass; however, significant reductions were observed in values of red blood cells (RBCs), hemoglobin (Hb), hematocrit (Ht), and mean corpuscular hemoglobin concentration (MCHC) together with significant elevations in mean corpuscular volume (MCV) and mean corpuscular hemoglobin (MCH) values in comparison with the control fish. Likewise, lymphopenia, monocytosis, leucopenia, eosinophilia, neutrophilia, and thrombocytopenia were the characteristic features of ZEN-toxicated fish with drastic reduction in white blood cells (WBCs) counts. Significant reductions in total protein, albumin, globulin, and lysozyme were also observed in fish serum due to ZEN toxicity. The co-supplementation of GP and/or CH to ZEN-fed fish elevated significantly values of RBCs, Hb, MCHC, and WBCs count with decreasing and increasing of its constituents. Significant rises in total protein, albumin, globulin, and lysozyme were observed with co-supplementation of GP and/or CH to ZEN-toxicated fish to be near these fed on the control diet. The ZEN toxicity also suppressed the fish immunity and subsequently elevated fish susceptibility to V. alginolyticus infection causing highest fish mortality as compared with other treatments. The co-supplementation of GP or CH to fish significantly enhanced their immunity causing a high relative percent of survival (RSP) of fish after bacterial infection (18.8 or 43.8%, respectively); meanwhile, fish fed both of GP and CH showed better RSP (68.8%). On the other hand, the co-supplementation of GP or CH reduced the ZEN residue (1.18 and 0.87 μg/kg dry weight) in fish muscles. These results show that CH was more effective than GP in enhancing fish immunity and protection against ZEN toxicity, whereas the co-supplementation of both GP and CH was most effective in protecting fish against ZEN toxicity resulting in lowest ZEN residue (0.53 μg/kg dry weight) in fish muscles.

Background Quality of life (QOL) is a multidimensional concept that is used to describe the individuals’ perceptions, satisfaction, and evaluation of different areas of their own lives, such as physical health and functioning, psychological and emotional well-being, social roles, and relationships. Few studies have addressed the effect of rheumatoid disease on the QOL in the Egyptian community. Results Functional disability based on health assessment questionnaire disability index was significantly increased with the increase in disease activity based on disease activity score-28, where all patients on remission and 56% of those with low disease activity had mild functional disability. Approximately 89% of patients with low disease activity had mild anxiety based on Hamilton anxiety scale, and the majority (75%) of those patients with moderate disease activity and 83.3% of patients on remission had mild anxiety. The majority (85.7%) of those with high disease activity were depressed based on Zung self-rating depression score. Disease activity score-28 score had a positive significant correlation with health assessment questionnaire disability index (P=0.01, r=0.40), Hamilton anxiety scale-A (P=0.01, r=0.46), Zung score (P=0.01, r=0.46), and rheumatoid arthritis QOL (r=0.70, P=0.00), and negative correlation with female sexual index (r=−0.80, P=0.00), all domains of SF-36, and all five domains of WHOQOL. Conclusion This study confirms that rheumatoid arthritis causes impairment of all aspects of QOL (limitation of physical function, physical disability, and pain), mental health disorders (anxiety and depression), and social, environmental, and also sexual dysfunction.

The first calcium complex from nature, Coumamarin (1), 7-hydroxy-3-methoxy-2-oxo-2H-chromene-6-carboxylate Ca(II) complex, was isolated from Aspergillus sydowii ASTI, together with diorcinol (2), violaceol I (3), hydroxysydonic acid (4), cyclo (Trp-Phe), kojic acid, ergosterol, and uracil. The producing strain was isolated from marine water sample collected from Tiran Island, Red Sea, Egypt. Structure 1 was assigned by intensive 1D, 2D NMR, HR-ESIMS, and X-ray crystallography as well. Coumamarin is potentially active against certain tested bacteria and yeasts, while showing no cytotoxic activity against human cervix carcinoma cell line (KB-3-1). Taxonomically, the fungus was identified by phylogenetic analysis of its 18S rRNA gene sequence.

The primary goal of this manuscript is to develop a computer assisted diagnostic (CAD) system to assess pulmonary function and risk of mortality in patients with coronavirus disease 2019 (COVID-19). The CAD system processes chest X-ray data and provides accurate, objective imaging markers to assist in the determination of patients with a higher risk of death and thus are more likely to require mechanical ventilation and/or more intensive clinical care.To obtain an accurate stochastic model that has the ability to detect the severity of lung infection, we develop a second-order Markov-Gibbs random field (MGRF) invariant under rigid transformation (translation or rotation of the image) as well as scale (i.e., pixel size). The parameters of the MGRF model are learned automatically, given a training set of X-ray images with affected lung regions labeled. An X-ray input to the system undergoes pre-processing to correct for non-uniformity of illumination and to delimit the boundary of the lung, using either a fully-automated segmentation routine or manual delineation provided by the radiologist, prior to the diagnosis. The steps of the proposed methodology are: (i) estimate the Gibbs energy at several different radii to describe the inhomogeneity in lung infection; (ii) compute the cumulative distribution function (CDF) as a new representation to describe the local inhomogeneity in the infected region of lung; and (iii) input the CDFs to a new neural network-based fusion system to determine whether the severity of lung infection is low or high. This approach is tested on 200 clinical X-rays from 200 COVID-19 positive patients, 100 of whom died and 100 who recovered using multiple training/testing processes including leave-one-subject-out (LOSO), tenfold, fourfold, and twofold cross-validation tests. The Gibbs energy for lung pathology was estimated at three concentric rings of increasing radii. The accuracy and Dice similarity coefficient (DSC) of the system steadily improved as the radius increased. The overall CAD system combined the estimated Gibbs energy information from all radii and achieved a sensitivity, specificity, accuracy, and DSC of 100%, 97% ± 3%, 98% ± 2%, and 98% ± 2%, respectively, by twofold cross validation. Alternative classification algorithms, including support vector machine, random forest, naive Bayes classifier, K-nearest neighbors, and decision trees all produced inferior results compared to the proposed neural network used in this CAD system. The experiments demonstrate the feasibility of the proposed system as a novel tool to objectively assess disease severity and predict mortality in COVID-19 patients. The proposed tool can assist physicians to determine which patients might require more intensive clinical care, such a mechanical respiratory support.
