Mit den in den letzten Jahren immer weiterentwickelten Operationsmethoden ist die Makulachirurgie ein zunehmend atraumatischer Eingriff fur das Auge geworden. Bei Patienten mit zusatzlicher Linsentrubung bietet sich ein einzeitiges Verfahren mit kombinierter Kataraktoperation an. Ziel dieser Studie ist es, die funktionellen Ergebnisse und Komplikationen nach elektiver Makulachirurgie mit und ohne gleichzeitige Phakoemulsifikation und Kunstlinsenimplantation zu erfassen. In die Studie wurden retrospektiv alle Patienten eingeschlossen, die im Zeitraum von 2010 bis 2016 mittels Pars-plana-Vitrektomie (ppV; 25 Gauge) an der Makula aufgrund einer epiretinalen Gliose, eines Makulaforamens oder einer vitreoretinalen Traktion operiert wurden und eine Nachbeobachtungszeit von mindestens 3 Monaten aufwiesen. Die Operationen wurden von einem Operateur durchgefuhrt. Anschliesend erfolgte eine Analyse der funktionellen Ergebnisse, moglicher Risikofaktoren sowie aufgetretener Komplikationen. Eingeschlossen wurden insgesamt 781 Augen, von denen 517 (66 %) kombiniert mit Phakoemulsifikation, Kunstlinsenimplantation und 25-Gauge-ppV, Membranektomie, ILM-Peeling und SF6-Gas oder Luft operiert wurden. Die mittlere Nachbeobachtungszeit lag bei 17 Monaten. Der mittlere logMar-Visus praoperativ betrug 0,59, postoperativ 0,4; 200 Patienten waren bereits pseudophak. Bei 64 phaken Augen erfolgte keine kombinierte Operation, 40 (62,5 %) davon benotigten aufgrund einer Cataracta complicata innerhalb von 13,6 Monaten eine Phakoemulsifikation mit Kunstlinsenimplantation, 18 sogar innerhalb von 6 Monaten. Bei den Komplikationen zeigten sich vergleichbare Ergebnisse zwischen der alleinigen ppV und der kombinierten Operation insbesondere hinsichtlich IOL-Dislokation oder Iriscapture. Insgesamt ist die elektive Makulachirurgie sowohl ohne, v. a. aber auch mit kombinierter Phakoemulsifikation und Kunstlinsenimplantation ein komplikationsarmes Operationsverfahren, sodass im Hinblick auf das Operationsmanagement und die postoperative Rehabilitation eine kombinierte Operation insbesondere in Zeiten einer Pandemie gegenuber einem zweizeitigem Vorgehen bevorzugt werden sollte.

Durch die vollstandige Integration der SD-OCT (Spectral-Domain-optische Koharenztomographie) in das Operationsmikroskop bietet die intraoperative optische Koharenztomographie (iOCT) die Moglichkeit einer gezielten hochauflosenden Bildgebung in Echtzeit. Der Operateur erhalt neben dem En-Face-Bild eine Live-Darstellung aller Netzhautschichten, des Glaskorpers sowie der Instrument-Gewebe-Interaktion, die fur intraoperative Entscheidungen genutzt werden kann. Im Vergleich zur herkommlichen Operationsmikroskopie ermoglicht die iOCT eine weitaus hohere Prazision vitreoretinalchirurgischer Masnahmen und ist essenzielle Voraussetzung zur Umsetzung Echtzeit-kontrollierter Operationstechniken im Mikrometerbereich.

To investigate whether visual acuity improvement achieved after surgical treatment for macular hole (MH) can be predicted by preoperative multifocal ERG (mfERG) central/peripheral amplitude ratio. Thirty patients with unilateral MH were included. Evaluations with comprehensive ophthalmological examination including best-corrected visual acuity (BCVA) were performed at baseline and 1, 3, 8, 24 and 48 weeks after surgery, while mfERG (Diagnosys LLC; 61 hexagons—30°) and spectral-domain optic coherence tomography (sOCT—Heidelberg Engineering) were performed at baseline and 2 months after surgery. mfERG results are shown by means of the ratio between the amplitudes’ average from rings 1 and 2 (central) and rings 4 and 5 (peripheral): the P1 ratio. mfERG data from 20 normally sighted age-matched subjects was used for comparison. A macular hole index (MH index) was defined as the quotient between hole height and base measured on OCT. Twenty-six patients finished the 48-week follow-up. Mean ± SE (logMAR) preoperative BCVA was 0.93 ± 0.22 and improved in 0.25 ± 0.07 at 48 weeks. mfERG P1 ratio was reduced at baseline and increased significantly after surgery. A significant correlation was observed between preoperative P1 ratio and BCVA gain at week 8 (r = −0.42; P = 0.033). There was no significant correlation between preoperative MH index and postoperative BCVA (P > 0.05). Retinal function assessed using the ratio between central and peripheral mfERG responses might be used as predictor of visual acuity outcome after macular surgery for MH.

Background The purpose of the current study is to report the anatomical and functional results of off-label human amniotic membrane graft as primary intervention to repair large to giant macular holes and in reoperations when wide internal limiting membrane peeling was unsuccessful. Methods Retrospective chart review was carried out in five different centers to identify all cases that had undergone off-label human amniotic membrane graft for the treatment of large or failed macular holes (MH). Data collected included age, gender, other concomitant diagnosis, symptoms duration, lens status, number of previous surgeries, macular hole measurements (minimum and base linear diameters), mean post-operative follow-up (months), and pre- and post-operative best corrected visual acuity (BCVA). Main outcome measures were anatomical MH closure rates and final BCVA (in logMAR). Nonparametric Wilcoxon rank-sum test was used because the data was not normally distributed, a P values Results Nineteen eyes of 19 patients were identified and included in the study. Mean age was 66.21 ± 14.96 years and predominantly females (84%). All eyes had successfully closed MH with a single intervention with no recurrences during a mean of 9 ± 3.87 months follow-up. The median BCVA in logMAR preoperative was 1.30 ± 0.44 (0.80–2.0), approximately 20/400 on Snellen chart and the median BCVA in logMAR postoperative was 1.0 ± 0.72 (0.4–3.0) approximately 20/200 (p Conclusion The use of human amniotic membrane graft seems to be a viable and effective alternative for the treatment of large and persistent macular holes. However, further larger prospective controlled studies are necessary to confirm our preliminary results of this new surgical technique.

Die hochauflosende optische Koharenztomographie (Spectral-Domain-OCT, SD-OCT) gilt als Goldstandard bezuglich Diagnose, Management und Verlaufskontrolle von Patienten mit vitreoretinalen Erkrankungen. Bei Patienten mit Glaskorperblutungen ist die praoperative Diagnostik zur Evaluation des intraokularen und retinalen Status limitiert, sodass die finale Therapieentscheidung lediglich wahrend der explorativen Vitrektomie getroffen werden kann. Wir berichten uber unsere Erfahrungen beim Einsatz der intraoperativen SD-OCT (iSD-OCT) wahrend explorativer Vitrektomie bei Patienten mit Glaskorperblutung. In Rahmen dieser Fallserie wurden 9 Patienten (11 Augen) mit Glaskorperblutungen untersucht. Praoperativ wurde der intraokulare Status mittels Sonographie erhoben. Im Anschluss erfolgte eine explorative 23G-Vitrektomie unter Visualisierung mit dem im Op.-Mikroskop (OPMI Lumera 700, Carl Zeiss Meditec, Jena, Deutschland) vollstandig integrierten iSD-OCT-Gerat (Rescan 700, Carl Zeiss Meditec, Jena, Deutschland). Die iSD-OCT ermoglichte bei allen Patienten zusatzlich zum intraoperativen En-Face-Mikroskopbild eine intraoperative Evaluation praretinaler, intraretinaler und subretinaler Strukturen in Echtzeit. Das operative Vorgehen – insbesondere die Notwendigkeit eines Peelings epiretinaler Membranen (ERM) bzw. der Membrana limitans interna (ILM), einer intravitrealen Medikamenteneingabe und die Wahl der geeigneten Tamponade – wurde anhand der iSD-OCT-Befunde festgelegt bzw. optimiert. Die iSD-OCT ist als zusatzliches diagnostisches Mittel bei Patienten mit Glaskorperblutungen sehr hilfreich. Es ermoglicht zusatzlich zum En-Face-Mikroskopbild eine intraoperative Echtzeitdiagnostik und erleichtert die Entscheidung der operativen Strategie.

Spectral domain optical coherence tomography (SD-OCT) has become a standard diagnostic tool in the surgical management of vitreomacular interface disorders. The high-resolution cross-sectional information obtained from SD-OCT is a perfect complement to vitreoretinal surgery. It provides detailed intraoperative anatomical views that are not possible with a microscope.To investigate the value of intraoperative real-time OCT with respect to improvement of surgical techniques in the management of vitreomacular disorders.A review of the current literature was conducted and an analysis of own systematically evaluated data was included to provide a comprehensive overview of potential applications for the clinical use of intraoperative real-time OCT in macular surgery.Intraoperative real-time OCT can provide detailed visualization of epiretinal membranes and help to identify whether complete membrane removal has been achieved following surgery. In addition, it can provide qualitative and quantitative information that has previously not been available and assist in surgical decision-making. Intraoperative real-time OCT allows membrane peeling to be performed in selected cases without using retinal dyes, whereas it is not ideal for accurately guiding the surgeon while performing maneuvers.Intraoperative real-time OCT provides high-resolution visualization of the effects of surgical maneuvers on the microarchitecture of the retina and surrounding tissues and will fill a gap in the understanding of the pathophysiology and prognostic factors of vitreomacular disorders; however, with currently available systems, accurate intraoperative real-time guidance of surgical maneuvers is hindered by several limiting factors.

Pars plana vitrectomy represents an important treatment tool in eyes with diabetic macular edema (DME). Vitrectomy primarily promotes resolution of DME by relieving tangential and vitreomacular traction, but has additional benefits of removing depot of VEGF and pro-inflammatory cytokines, and improving macular oxygen delivery and perfusion. Therefore, vitrectomy has most significant benefit in tractional DME, and potential efficacy in non-tractional DME. The advent of modern vitrectomy with advanced visualization techniques has made macular surgery significantly more safe and improved treatment outcomes. The purpose of the article is to review major studies on the surgical treatment of DME and offer perspectives on strategies to maximize anatomic and functional outcomes of diabetic vitrectomy.

The aim of this work is to investigate the biocompatibility and staining properties of DSS: 3,3'-Di-(4-sulfobutyl)-1,1,1',1'-tetramethyl-di-1H-benz[e]indocarbocyanine (DSS).Dye concentrations of 0.5, 0.25, and 0.1% were evaluated (290 and 295 mOsm). Toxicity was assessed using a colorimetric test measuring the inhibition of ARPE 19 cell, human primary RPE cell, and human Müller cell proliferation. Exposure time was 30, 60, 120, and 300 s. Indocyanine green (ICG) (0.5, 0.25, and 0.1%) served as a control. Cells were also illuminated with plain white light (750 mW/cm(2)) for 10 min to assess phototoxic effects. Besides staining of porcine and human lens capsule, internal limiting membrane (ILM)-staining was assessed by applying 0.25 and 0.5% DSS over the macula in two human post-mortem eyes.DSS of 0.25 and 0.1% showed no toxic effect on primary RPE cells and MIO-M1cells, and 0.5, 0.25, and 0.1% for ARPE-19 cells. In MIO-M1cells, 0.5% dye showed a significant reduction of mitochondrial dehydrogenase activity only following an exposure time of 300 s. Following illumination, ICG showed a significantly more pronounced effect on cell viability in primary RPE cells and MIO-M1cells compared to DSS. The absorption maximum is found at 591 nm; the even more bathochromic fluorescence proceeds with a common Stokes' shift where maxima at 620 and 660 nm with a quantum yield of 32% were found. The fluorescence is sufficiently hypsochromic and the fluorescence quantum yield high enough for an easy visual detection. The contrast and staining properties at the ILM were excellent and allowed for a controlled removal of the ILM during surgery. No penetration into deeper retinal layers was noted.Our results indicate that this new cyanine dye DSS may represent an alternative for ILM staining due to its matched absorption concerning visibility and fluorescence qualities as well as its good biocompatibility.

To describe the incidence, clinical features, and evolution of paracentral retinal holes occurring after macular surgery.A retrospective non-randomized study of 909 patients operated on for either a macular hole (MH, n = 400 patients) or an epiretinal membrane (ERM, n = 509 patients) between 2004 and 2009. Six patients (0.6%) developed a paracentral macular hole after surgery. Their clinical, auto-fluorescence, and optical coherence tomography (OCT) characteristics as well as their visual outcomes were studied.The mean age of patients was 70 years. Paracentral holes occurred approximately 5 weeks after surgery (with a range of 2-12 weeks). All patients were asymptomatic. Five patients underwent ILM peeling during initial surgery. Paracentral retinal holes were located superiorly to the fovea in three cases and temporally in the other three cases. Mean pre-operative BCVA was 20/200 and mean post-operative BCVA was 20/40. The eye where the eccentric MHs were closest to the fovea (inferior to 1 optic disc area) had the poorest final visual acuity. Autofluorescence imaging showed a bright fluorescence in paramacular holes. On OCT images, they were shown to be flat full-thickness holes. No treatment was attempted. No rhegmatogenous complications or choroidal neovascularization occurred in any of the patients. Mean follow-up was 2 years.In summary, paracentral MHs are uncommon complications which can occur at the site where ILM peeling has been initiated or completed. Except for the closest holes to fovea, they have good visual prognosis and do not require any treatment underlining the importance of initiating the ILM peeling as far as possible from the fovea.

PROBLEMSTELLUNG: Ziel der Studie war es zu klaren, ob mittels hochauflosender optischer Koharenztomografie (OCT) eine zusatzliche Aussage betreffend anatomischer und funktioneller Prognose bei verschiedenen Makulapathologien getroffen werden kann. METHODE: In dieser retrospektiven Studie wurden alle makulachirurgischen Eingriffe, die im KH Hietzing von Juli 2008 bis Juli 2009 durchgefuhrt wurden, hinsichtlich postoperativem Visusanstieg und Verlauf im OCT analysiert. Visus und zentrale Netzhautdicke im OCT praoperativ und 1 Monat postoperativ, sowie der Visus bei der Kontrolle nach durchschnittlich 3 Monaten wurden verglichen. 45 Augen wurden entsprechend ihrer Pathologie unterteilt in: 19 Augen (42,2%) mit epiretinaler Fibrose (ERF), 8 (17,8%) mit vitreomakularer Traktion (VMT), 10 (22,2%) mit durchgreifendem Makulaloch (ML) und 8 (17,8%) mit lamellarem Makulaloch oder Pseudohole. ERGEBNISSE: Bei den insgesamt 45 PatientInnen kam es 1 Monat postoperativ zu einem signifikanten Visusanstieg von 0.27 ± 0,17 Snellen auf 0,42 ± 0.24 Snellen (p < 0,001) und zu einer signifikanten Abnahme der zentralen Netzhautdicke im OCT von 385,4 ± 94,6 µm auf 353,4 ± 66,5 µm (p = 0,015). Bei PatientInnen mit ERF, die bei der letzten Kontrolle eine durchgehende IS/OS-Junction im OCT aufwiesen, hatten sowohl praals auch postoperativ einen besseren Visus (0,41 ± 0,20 vs. 0,27 ± 0,11, p = 0,084 und 0,58 ± 0,22 vs. 0,38 ± 0,15, p = 0,053). Auch PatientInnen mit VMT wiesen einen tendenziell hoheren Visusgewinn auf (0,52 ± 0,22 vs. 0,36 ± 0,18, p = 0,310) sowie PatientInnen mit lamellaren ML und Pseudoholes (0,45 ± 0,07 vs. 0,66 ± 0,24, p = 0,137). PatientInnen mit durchgreifendem Makulaloch mit Lochschluss nach dem Ersteingriff (55,6%) hatten praoperativ einen kleineren Lochdurchmesser (537,7 ± 246,7 µm vs 984,3 ± 239,3 µm; p = 0,062). SCHLUSSFOLGERUNG: Mit Hilfe des hochauflosenden OCTs lasst sich der postoperative Verlauf nach Makulachirurgie objektiver und genauer kontrollieren, wodurch eine verlasslichere Aussage bezuglich der Visusprognose im Zusammenhang mit durchgehender IS/OS-Junction moglich wird.

The advantages of the use of optical contrast agents in surgery, such as vitreoretinal microsurgery, are presented. Requirements concerning these materials and the potentials and limitations of their application are discussed on the basis of their coloristic properties. Applications of fluorescent dyes are examined.

Subfoveale choroidale Neovaskularisationen (CNV) sind neben der geographischen Atrophie die haufigste Ursache fur einen irreversiblen Verlust der zentralen Sehscharfe bei altersabhangiger Makuladegeneration (AMD) Mit der intravitrealen Injektion antiangiogenetischer Substanzen steht heute fur viele Feuchte AMD-Formen eine palliative pharmakologische Therapie zur Verfugung. Komplexere exudative Befunde, und insbesondere der uberwiegende Teil der Patienten mit trockenen Veranderungen profitieren nicht durch letztere Therapieform. Durch Weiterentwicklung vitreoretinaler chirurgischer Techniken konnte das Spektrum interventioneller Strategien bei der AMD erganzt werden. Dieser Artikel beleuchtet derzeitige und zukunftige Moglichkeiten chirurgischer Therapien bei der AMD.

Microplasmin (Thrombo-Genics Ltd., Dublin, Irland) ist ein rekombinanter Wirkstoff, bestehend aus der katalytischen Domane von humanem Plasmin. Wir untersuchten post mortem et in vivo, ob Microplasmin eine hintere Glaskorperabhebung induzieren kann, und ob mit morphologischen oder funktionellen Netzhautveranderungen zu rechnen ist. In humanen Spenderaugen und in vivo bei Kaninchen, Minipig, Katze und Primaten wurde Microplasmin intravitreal appliziert und der Grad der vitreoretinalen Separation standardisiert mittels Rasterelektronenmikroskopie bestimmt. Zusatzlich wurden die Techniken der Transmissionselektronenmikroskopie, der konfokalen Mikroskopie und der Elektroretinographie (ERG) angewandt. Microplasmin kann post mortem et in vivo eine vollstandige und dosisabhangige hintere Glaskorperabhebung induzieren. Morphologische oder immunzytochemische Netzhautveranderungen finden sich nicht. Dauerhafte ERG-Veranderungen sind nicht nachzuweisen. Microplasmin trennt den Glaskorper von der Netzhaut ohne morphologische oder funktionelle Netzhautveranderungen zu induzieren. Mit diesen praklinischen Ergebnissen fuhren wir derzeit die ersten klinischen Studien zur Microplasmin-assistierten Vitrektomie durch. Microplasmin (ThromboGenics Ltd., Dublin, Ireland) is a recombinant molecule consisting of the catalytic domain of human plasmin. We investigated the cleaving effect of microplasmin at the vitreoretinal interface in post mortem eyes and in vivo. Microplasmin was injected into the vitreous of post mortem eyes and in different animal models. The vitreoretinal interface was investigated by scanning and transmission electron microscopy and by laser scanning microscopy. In vivo, electroretinography was also performed. Microplasmin separates the vitreous from the retina in a dose-dependent fashion. The retinal architecture including the ultrastructure of the inner and outer retina is well preserved. There are no alterations of retinal morphology and immunocytochemistry between treated eyes and control eyes. Microplasmin cleaves the vitreoretinal junction and helps to completely separate the vitreous from the retina. Several clinical trials are currently underway.

Das Spektrum der Reoperationen nach elektiver Makulachirurgie wurde an einem groseren Patientenkollektiv retrospektiv untersucht. Dabei wurden alle Zweiteingriffe auser der Kataraktoperation berucksichtigt. Zwischen Juli 1995 und Juni 2001 wurden 353 Augen aufgrund eines Macular pucker (n=218) oder eines Makulaforamens (n=135) einer pars-plana-Vitrektomie (PPV) mit hinterer Glaskorper(GK)-Abhebung, membrane peeling und SF6/Luft-Instillation unterzogen. Vier verschiedene Operateure waren an diesen Eingriffen beteiligt. Bei allen Patienten wurde 3–4 Wochen vor dieser Erst-PPV eine zirkulare periphere Netzhautkryopexie durchgefuhrt. Die postoperativen Kontrollen erfolgten mit einem Follow-up von durchschnittlich 20,9 Monaten. Untersucht wurden die Anzahl der erforderlichen Revitrektomien sowie die postoperative Amotiorate. Bei unserem Patientenkollektiv musste in 33 Fallen (9,3%) eine erneute PPV wegen eines unzureichenden postoperativen Makulabefundes durchgefuhrt werden, davon 17/218 (7,8%) nach Macular pucker wegen Rezidivmembranen und 16/135 (11,8%) nach Makulaforamen wegen eines mangelnden Lochverschlusses. In 7/353 (2,0%) trat postoperativ eine rhegmatogene Amotio retinae auf, in 2/353 (0,6%) eine postoperative Endophthalmitis. Bei 9,3% unserer Patienten wurde eine Re-PPV aufgrund eines unbefriedigenden Makulabefundes notwendig. Die postoperative Amotiorate ist mit 2,0% deutlich niedriger als in den meisten anderen Studien und weist auf einen moglichen positiven Effekt der praoperativen zirkularen Kryoretinopexie zur Senkung dieses Risikos hin.

The traditional Gass classification of macular holes can now be supplemented by additional and more detailed morphologic information obtained with optical coherence tomography (OCT). The perifoveal vitreous detachment and subsequent anterior foveal traction is considered to be the primary pathomechanism of macular hole formation. In cases of persistent traction on the fovea it may lead to foveal dehiscence. A possible explanation for intraretinal cyst formation may be secondary vitreous body fluid accumulation within the retina. A classification of macular holes based on additional information from OCT images is possible. Thus, OCT is a valuable tool for differential diagnosis of a pseudo macular hole versus macular hole, precise stage classification, therapy decision making process, outcome control after macular surgery, and prognosis prediction.

The hardcover book “Vital Dyes in Vitreoretinal Surgery— Chromovitrectomy”, edited by C. H.Meyer is a multiauthored book encompassing 164 pages divided into 12 chapters. The book is volume No 42 in the book series “Developments in Ophthalmology”, edited by W. Behrens-Baumann. The book begins with a tribute to Prof. Peter Kroll, to whom editor Carsten Meyer dedicates this book for his lifetime achievements. Prof Kroll was head of the department of ophthalmology at the university of Marburg, Germany, a reknowned retina specialist and former teacher of the editor. The sequence of the book chapters appears logical. The first three chapters are focusing on the vitreous, followed by chapters on ICG use in peeling procedures, and closing with chapters on new dyes, recommendations for safe clinical application of new dyes and latest developments in this field. The first chapters about the vitreous include wellillustrated embryologic and histological descriptions, and both in vitro and in vivo visualization techniques using OCT-SLO combination techniques, presented by Jerry Sebag. Eduardo Rodrigues and co-workers present in their chapter an overview of the evolution of vital dyes in vitreoretinal surgery. Intraoperative images of vitreous staining with triamcinolone, autologous blood or preoperative intravenous fluorescein application are shown by Jorg Schmidt and co-workers. The next chapters describe biostaining with indocyanine green (ICG). Here a very detailed review is presented by Salvatore Grisanti and coworkers, including 192 literature quotes, on controversies and scientific data regarding ICG use in macular surgery, ranging from aspects like ICGconcentration, -dye persistence, -osmolarity, -sodium content to illumination effects. In the subsequent chapter, Arnd Gandorfer and coworkers focus on ICG toxicity, and Gregor Wollensak describes biomechanical considerations of ICG use. The following chapters report clinical experience and biochemical properties with the alternative stains Trypan Blue, Patent Blue V and Brilliant Blue G. Michel Farah and co-workers summarize the current knowledge of Trypan Blue, and Stefan Mennel and coworkers present data on the Trityl dyes Patent Blue V and Brilliant Blue G on the RPE and blood–retina barrier, with promising results particularly for Brilliant blue G. Hiroshi Enaida and Iatsuro Ishibashi present their detailed animal model evaluations and first clinical results with Brilliant Blue G in humans. The final three chapters focus on new developments in dyes and novel applications. Timothy Jackson provides here a sound overview on different vital stains and their potential target structures. He describes interesting techniques to identify clinical invisible retinal holes using vital stains either in the vitreous cavity or in the subretinal space. The next chapter addresses safety aspects of introducing new dyes for vitreoretial surgery. Christos Haritoglou and coworkers define here safety parameters that should be analyzed before dyes are being tested in humans. They range from light absorption properties of dyes, cell culture models to the selection of appropriate animal models. A. W. A.
