Background The aim of this study was to determine whether the treatment with doxycycline before and after oocyte retrieval is as effective as salpingectomy in minimizing the detrimental effect of hydrosalpinx on the outcomes of IVF-ET. Methods A retrospective analysis was done for the outcomes of the IVF-ET cycles of patients with hydrosalpinx who underwent laparoscopic salpingectomy prior to IVF cycle (n = 260) or were treated with extended doxycycline treatment during the IVF cycle (n = 45). In doxycycline group, doxycycline (100 mg twice daily) was started 1 week before anticipated oocyte retrieval and was continued for 1 week after oocyte retrieval. In salpingectomy group, the mesosalpinx was coagulated as close as possible to the fallopian tube. Results The implantation, clinical pregnancy, ongoing pregnancy and live birth rates were significantly higher in the salpingectomy group (20.87% Vs. 9.91%, P value =0.007, 44.62% Vs. 20%, P value = 0.002, 39.62% Vs. 17.78%, P value = 0.005 and 37.31% Vs. 15.56%, P value = 0.005 respectively). Conclusion Salpingectomy is more effective than extended doxycycline treatment in improving the outcomes of IVF-ET in patients with hydrosalpinx undergoing IVF-ET. Further, larger well designed randomized controlled trials should be conducted to confirm the findings of this study.

We are conducting a prospective study trying to determine, in both sexes, the frequency of appearance of ectopic Leydig cells, their preferred location, their relationship with nerve structures and the possible causes of their appearance. We have studied 86 cases that were removed according to different clinical indications for pathological study: uterine leyomiomas (n = 12), ovarian cystadenoma (n = 4), endometrial hyperplasia (n = 8), endometrial carcinoma (n = 12), cervical carcinoma (n = 4), seminoma (n = 4), fallopian tube ligatures (n = 24), vasectomies (n = 8), nonspecific orchiepididymitis (n = 2), and unknown (n = 8). We have observed ectopic Leydig cells in 13/86 cases (15.11%), 9/72 in the female samples (12.50%) and 4/14 in male samples (28.57%). The most frequent location was the mesosalpinx (4 of 13: 30.76%). These high figures lead us to believe that the ectopia of Leydig cells is not really a pathologic entity, but a finding related to specific functions yet to be determined.

Female adnexal tumors of probable Wolffian origin (FATWO) are rare tumors, which are mostly localized in the broad ligament or the mesosalpinx. They show high intratumor and intertumor variability of histological patterns (e.g. solid, tubular, cribriform and cystic) with usually unremarkable cellular and nuclear morphology and a lower mitotic rate. In general, they behave in a benign fashion but there are rare cases with malignant transformation, so that careful examination and surveillance are necessary. Differential diagnoses include Sertoli-Leydig cell tumors, metastasized endometrioid carcinoma and the FATWO-like variant of the endometrioid carcinoma of the fallopian tubes. The FATWOs express pancytokeratin, CD10, vimentin, calretinin and inhibin A. Estrogen and progesterone receptors are expressed in a minority of cases, whereas epithelial membrane antigen (EMA) is not detectable.

Background Serum anti-Müllerian hormone (AMH) concentration has been used to assess ovarian reserve in patients with endometriosis, especially when endometrioma surgery is involved. Previously, we reported that decreased serum AMH levels after cystectomy for endometriomas can recover to preoperative levels in some cases. In this present study, we assessed the sequential changes in serum AMH levels before and after cystectomy in terms of the state of the mesosalpinx prior to surgery. Methods The retrospective cohort study recruited 53 patients from a series of prospective studies conducted from 2009 to 2015. All patients underwent laparoscopic cystectomy for endometriomas. If either mesosalpinx was involved in the endometrioma or adnexal adhesion before cystectomy, the case was defined as ‘involved mesosalpinx’ (n = 14). If both mesosalpinx remained anatomically correct, the case was classified as ‘intact mesosalpinx’ (n = 39). Blood samples were obtained from the patients 2 weeks before surgery, and at 1 month and 1 year after surgery to assess serum AMH levels. Results The serum AMH levels (the involved group vs. the intact group) were 1.92 vs. 0.98 (P = 0.552) preoperatively, 0.59 vs. 1.99 (P = 0.049) at 1 month postoperatively, and 0.48 vs. 2.37 ng/mL (P = 0.007) at 1 year postoperatively. The involved mesosalpinx group showed a further decrease in serum AMH levels at 1 year postoperatively, while serum AMH levels in the intact mesosalpinx group tended to recover. Conclusion These results suggest that pre-existing mesosalpinx disturbance, in combination with adhesiolysis, may be involved in the medium- and long-term decrease in ovarian reserve after endometrioma surgery. A disturbance in ovarian blood supply via the mesosalpinx may underlie this. Trial registration UMIN-CTR UMIN000019369 . Retrospectively registered October 15, 2015.

Reproductive organs from mature Small East African zebu (SEAZ) heifers and cows slaughtered at the Morogoro abattoir were collected twice a month and evaluated over a period of 12 months. Out of the 402 animals from which reproductive organs were taken, 54% were pregnant, 24% were actively cycling and 22% were non-cycling. Various gross abnormalities were observed in the reproductive organs of about 16% of the cattle, and the major reproductive abnormality in both total and the non-cycling animals was various degrees of fibrous adhesion between the ovary and the infundibulum and mesosalpinx. It is concluded that, contrary to common belief, a majority of the female SEAZ cattle that are slaughtered are fertile.

Objectives Here, we report a case of simultaneous osseous metaplasia nodules of the submucosa and mesosalpinx after a first trimester abortion. Case presentation A 36-year-old woman presented to the Women’s Hospital, School of Medicine, Zhejiang University with vaginal bleeding and infertility resulting from osseous metaplasia nodules of the submucosa and mesosalpinx after a first trimester abortion. Diagnostic and operative hysteroscopy and laparoscope procedures were performed. The osseous metaplasia nodules disappeared after hysteroscopy and laparoscope interventions; 2 weeks postoperatively, the patient underwent a transvaginal ultrasound examination and the abnormal ultrasound appearance had resolved. Conclusions Osseous metaplasia nodules in the submucosa and mesosalpinx can be a rare cause of vaginal bleeding and infertility. Autologous tissue, not persistent heterologous tissue, may be the true reason for metaplasia. Treatment is by ultrasound-guided curettage or by hysteroscopic and laparoscope removal.

A patient with peritoneal papillary serous carcinoma involving the colon and omentum is presented. Following palliative right-sided hemicolectomy and partial omentectomy, chemotherapy was given. The patient had a second look operation after 6 months when a pelvic clearance was done and histology revealed some residual tumor in one mesosalpinx. She was well and clinically free of recurrence after a further 6 months. Serum CA125 and CEA levels were measured and CA125 levels remained high throughout.

A 32-year-old white lady suffering from tubal infertility was referred to our institution in November 1992 because of low abdominal pain due to a heterotopic pregnancy (one intrauterine sac and the other in the right tube). The patient had undergone, 8 weeks before, her second successful attempt at in vitro fertilization and embryo transfer. We decided to perform a laparoscopic salpingectomy. At inspection, the presence of tubo-ovarian adhesions was noted, secondary to the previous tubal microsurgical procedure, that were lysed by means of monopolar electrocoagulation. Salpingectomy was performed by combined bipolar cauterization and blunt-scissor dissection of the mesosalpinx. No uterine contractions were noted after surgery and the patients were discharged the next day. The rest of the pregnancy was uneventful and the patient spontaneously delivered vaginally a healthy female newborn weighing 3,060 g (Apgar score = 9) on June 20, 1993, at 39 weeks of gestation. A review of the published literature on laparoscopic surgery in pregnancy is given.

One thousand reproductive organs from gilts were collected during the period March-August at a slaughterhouse in central Sweden. The organs were inspected in respect of congenital defects and certain reproductive physiological data. The results are recorded in Tables 1–6. Figs. 1–7 show some types of malformations. The total frequency of malformations was 22.1 %, of which 14 % cysts in mesosalpinx and 4.1 % partial duplication of the vagina. The remainder consisted of general developmental defects and of local defects in the tubular genital tract. Malformations with presumed effect on fertility were found in 4.5 % of all organs examined, of which 0.8 % presumably caused permanent sterility and 3.7 % lowered fertility (small litters). Hydrometra was found in 33 cases, 2 of which in sexually mature gilts. The left ovary contained more corpora lutea than the right, and the mean weight of the left ovary was greater than that of the right, both in sexually mature and prepuberal gilts. Of 202 sexually mature gilts examined 16.9 % had cystic corpora lutea in their ovaries as a rule 2–4 in each ovary.
