Not all stents are suitable for children. For instance, premounted stents can be used in infants and small children but cannot dilate with age to accommodate adult-sized pulmonary arteries. Conversely, the Pul-Stent adapts to somatic growth. Thus, our hospital implemented the Pul-Stent in pediatric patients with branch pulmonary artery stenosis. This study summarizes our initial experience with Pul-Stents in this patient population, including the efficacy and safety. We implanted 37 Pul-Stents in 35 patients between August 2014 and June 2015. The patients’ mean age and weight at stent implantation were 6.7 ± 3.0 years and 20.9 ± 8.7 kg, respectively. Bench testing revealed that axial shortening of the Pul-Stent was minimal with further dilation, and the radial strength did not change. The stents were successfully deployed in all cases, except two with minor malpositioning. Primarily, 8–12 mm mounting balloons were used for the initial implantation, and a long sheath (8–10 F) was used for delivery. After stent implantation, the minimal lumen diameter in the stenosed segment increased by 50% in 97% (34/35) of patients. Furthermore, the pressure gradient across the stenosed segment decreased by 50% in 77% (23/30) of biventricular patients. One stent fracture and one stent restenosis were noted during the follow-up visits (mean follow-up time: 4.6 ± 1.7 years). Eighteen patients (51%) underwent repeat catheterization; ten had successful redilation. No aneurysms or stent fractures were observed. Our initial results indicate that the Pul-Stent is safe and effective in pediatric patients and can be further dilated over time to accommodate somatic growth. Moreover, the Pul-Stent has good compliance and adequate radial strength to treat pulmonary artery stenosis effectively.

Single degradation systems based on dielectric barrier discharge plasma (DBDP) or persulfate (PS) oxidation cannot achieve the desired goals (high degradation efficiency, high mineralization rate and low product toxicity) of degrading atrazine (ATZ) in river sediment. In this study, DBDP was combined with a PS oxidation system (DBDP/PS synergistic system) to degrade ATZ in river sediment. A Box–Behnken design (BBD) design including five factors (discharge voltage, air flow, initial concentration, oxidizer dose and activator dose) and three levels (-1, 0 and 1) was established to test a mathematical model by response surface methodology (RSM). The results confirmed that the degradation efficiency of ATZ in river sediment was 96.5% in the DBDP/PS synergistic system after 10 min of degradation. The experimental total organic carbon (TOC) removal efficiency results proved that 85.3% of ATZ is mineralized into CO2, H2O and NH4+, which effectively reduces the possible biological toxicity of the intermediate products. Active species (sulfate (SO4•−), hydroxy (•OH) and superoxide (•O2−) radicals) were found to exert positive effects in the DBDP/PS synergistic system and illustrated the degradation mechanism of ATZ. The ATZ degradation pathway, composed of 7 main intermediates, was clarified by fourier transform infrared spectroscopy (FTIR) and gas chromatography-mass spectrometry (GC-MS). This study indicates that the DBDP/PS synergistic system is a highly efficient, environmentally friendly, novel method for the remediation of river sediment containing ATZ pollution.

With the recent revolution in the green economy, agricultural solid waste resource utilization has become an important project. A small-scale laboratory orthogonal experiment was set up to investigate the effects of C/N ratio, initial moisture content and fill ratio (v cassava residue: v gravel) on the maturity of cassava residue compost by adding Bacillus subtilis and Azotobacter chroococcum. The highest temperature in the thermophilic phase of the low C/N ratio treatment is significantly lower than the medium and high C/N ratios. The C/N ratio and moisture content have a significant impact on the results of cassava residue composting, while the filling ratio only has a significant impact on the pH value and phosphorus content. Based on comprehensive analysis, the recommended process parameters for pure cassava residue composting are a C/N ratio of 25, an initial moisture content of 60%, and a filling ratio of 5. Under these conditions, the high-temperature conditions can be reached and maintained quickly, the organic matter has been degraded by 36.1%, the pH value has dropped to 7.36, the E4/E6 ratio is 1.61, the conductivity value has dropped to 2.52 mS/cm, and the final germination index increased to 88%. The thermogravimetry, scanning electron microscope, and energy spectrum analysis also showed that the cassava residue was effectively biodegraded. Cassava residue composting with this process parameter has great reference significance for the actual production and application of agriculture.

The pathological mechanism of intervertebral disc degeneration (IDD) is an unanswered question that we are committed to exploring. A20 is an anti-inflammatory protein of nucleus pulposus (NP) cells and plays a protective role in intervertebral disc degeneration.This study aims to investigate the molecular mechanism by which A20 attenuates disc degeneration.The proteins of interest were measured by immunoblotting, immunofluorescence, ELISA assay, and immunohistochemical technique to conduct related experiments. Immunofluorescence assays and mitochondrial membrane potential (JC-1) were used to assess mitophagy and mitochondrial fitness, respectively.Here, we demonstrated that A20 promoted mitophagy, attenuated pyroptosis, and inhibited the degradation of the extracellular matrix, consequently significantly ameliorating disc degeneration. Mechanistically, A20 reduces pyroptosis and further suppresses cellular mTOR activity. On the one hand, A20-induced mTOR inhibition triggers BNIP3-mediated mitophagy to ensure mitochondrial fitness under LPS stimulation, as a result of mitigating mitochondrial dysfunction induced by LPS. On the other hand, A20-induced mTOR inhibition reduces the loss of mitochondrial membrane potential and the generation of Mitochondrial ROS.The study revealed that A20 promotes BNIP3-mediated mitophagy by suppressing mTOR pathway activation against LPS-induced pyroptosis.

Biomarkers are crucial biological indicators in medical diagnostics and therapy. However, the process of biomarker discovery and validation is hindered by a lack of standardized protocols for analytical studies, storage and sample collection. Wearable chemical sensors provide a real-time, non-invasive alternative to typical laboratory blood analysis, and are an effective tool for exploring novel biomarkers in alternative body fluids, such as sweat, saliva, tears and interstitial fluid. These devices may enable remote at-home personalized health monitoring and substantially reduce the healthcare costs. This Review introduces criteria, strategies and technologies involved in biomarker discovery using wearable chemical sensors. Electrochemical and optical detection techniques are discussed, along with the materials and system-level considerations for wearable chemical sensors. Lastly, this Review describes how the large sets of temporal data collected by wearable sensors, coupled with modern data analysis approaches, would open the door for discovering new biomarkers towards precision medicine.

Our primary objective was to document COVID-19 induced changes to perinatal care across the USA and examine the implication of these changes for maternal mental health. We performed an observational cross-sectional study with convenience sampling using direct patient reports from 1918 postpartum and 3868 pregnant individuals collected between April 2020 and December 2020 from 10 states across the USA. We leverage a subgroup of these participants who gave birth prior to March 2020 to estimate the pre-pandemic prevalence of specific birthing practices as a comparison. Our primary analyses describe the prevalence and timing of perinatal care changes, compare perinatal care changes depending on when and where individuals gave birth, and assess the linkage between perinatal care alterations and maternal anxiety and depressive symptoms. Seventy-eight percent of pregnant participants and 63% of postpartum participants reported at least one change to their perinatal care between March and August 2020. However, the prevalence and nature of specific perinatal care changes occurred unevenly over time and across geographic locations. The separation of infants and mothers immediately after birth and the cancelation of prenatal visits were associated with worsened depression and anxiety symptoms in mothers after controlling for sociodemographic factors, mental health history, number of pregnancy complications, and general stress about the COVID-19 pandemic. Our analyses reveal widespread changes to perinatal care across the US that fluctuated depending on where and when individuals gave birth. Disruptions to perinatal care may also exacerbate mental health concerns, so focused treatments that can mitigate the negative psychiatric sequelae of interrupted care are warranted.

Phosphate (P) removal is significant for water pollution control. In this paper, a novel penicillin biochar modified with zirconium (ZMBC) was synthesized and used to adsorb P in water. The results showed that ZMBC had a porous structure and magnetic properties, and the zirconium (Zr) was mainly present in the form of an amorphous oxide. P adsorption displayed strong pH dependence. The Freundlich model described the adsorption process well, and the saturated adsorption capacity was 27.97 mg/g (25 ℃, pH = 7). The adsorption kinetics were consistent with the pseudo-second-order model, and the adsorption rates were jointly controlled by the surface adsorption stage and intraparticle diffusion stage. The most common commercial oils in the Chinese market are two petrol types with octane levels of 93 and 97. To determine the source spectrum of air pollutant emissions, we herein investigated the specific emission sizes of the total suspended particles (TSP), total carbon (TC), elemental carbon (EC), organic carbon (OC), and polycyclic aromatic hydrocarbons (PAHs) from a petrol engine fueled with 93# and 97# petrol in 2016 (based on Chinese national IV gasoline standard). We found that while 93# emitted a higher TSP content, 97# emitted greater TC, EC, OC, and PAHs. The highest carbon contents were found in the 0.25 µm and 0.44-1.0 µm size fractions for the 93# and 97# petrol, respectively. OC content showed a significant positive correlation with EC, and EC2 (at 740 ℃) was the main carbon fragment in both petrol exhausts. The highest PAH content occurred in the 0.25-0.44 µm size-bin, differing from the results for TC, EC, and OC, and medium molecular weight (4 rings) PAHs were the primary component in the emissions. These results indicate that fuel composition and octane sensitivity have a prominent effect on the size distributions of TC (including EC, OC, and PAHs). Thus, more studies on the carbon content at specific emission sizes in petrol exhaust should be conducted to clarify the main factors impacting these variations.

Since the main canal of the Middle Route of South-to-North Water Diversion Project was put into operation, the species, quantity and biomass of algae and shellfish have responded quickly to the environment, and a single dominant species has appeared in the community, demonstrating the remarkably abnormal proliferation property of algae and shellfish. In order to evaluate the safety risk of abnormal proliferation of algae and shellfish in the Middle Route and realize dynamic analysis of all kinds of influencing factors, a comprehensive risk evaluation system of algae and shellfish in the Middle Route of South-to-North Water Diversion Project based on comprehensive weighting and Four-Element Connection Number (MCEN) was constructed by integrating the Analytic Hierarchy Process (AHP) and the weight assignment theory of CRITIC. The system consists of 21 evaluation indexes selected from risk factors and carriers. Taking Henan section in the Middle Route as an example, comprehensive risk evaluation system of algae and shellfish in the Middle Route of South-to-North Water Diversion Project was applied to calculate the partial connection number of each order and obtain the risk development trend of each indicator. The results showed that algae and shellfish in the Middle Route were in a very safe state at the safety risk level of level Ⅰ. Finally, reasonable measures to reduce the risks facing algae and shellfish in the Middle Route of South-to-North Water Diversion Project are given, which effectively make up the deficiency of existing evaluation methods.

This study aimed to explore whether sex is influences tinnitus severity and whether the risk factors for tinnitus severity are the same in tinnitus patients of different sexes.This was a retrospective study of data from 1427 patients complaining of tinnitus in a local hospital otolaryngology clinic from November 2019 to January 2022. All patients were interviewed and assessed by otoscopy, pure-tone audiometry, tinnitus handicap inventory (THI), visual analogue scale (VAS), and tinnitus refinement test.THI values were higher in females than in males (P = 0.00). Types of tinnitus sounds (OR 0.667, P = 0.000) and degree of hearing loss (OR 1.318, P = 0.000) were risk factors for tinnitus severity in males. Types of tinnitus sounds (OR 0.789, P = 0.005), sensation level (OR 1.023, P = 0.037), tinnitus types (OR 1.163, P = 0.041), tinnitus location (OR 1.198, P = 0.026), and the degree of hearing loss (OR 1.303, P = 0.000) were risk factors for tinnitus severity in females. Sex was an influencing factor for tinnitus severity. There were different risk factors for the tinnitus severity in different sexes.The risk factors for tinnitus severity differed according to sex in tinnitus patients, and the risk factors for tinnitus severity were greater in women than in men. These findings add to the literature on sex differences in tinnitus and suggest that medical and psychological screening of affected individuals and customized tinnitus treatment for each individual with tinnitus are needed.ChiCTR2200057958, 2022/3/24 (retrospectively registered trials).

Software knowledge community contains a large scale of software knowledge entity information, complex structure and rich semantic correlations. It is significant to recognize and extract software knowledge entity from software knowledge community, as it has great impact on entity-centric tasks such as software knowledge graph construction, software document generation and expert recommendation. Since the texts of the software knowledge community are unstructured by user-generated texts, it is difficult to apply the traditional entity extraction method in the domain of the software knowledge community due to the problems of entity variation, entity sparsity, entity ambiguity, out-of-vocabulary (OOV) words and the lack of annotated data sets. This paper proposes a novel software knowledge entity extraction model, named AttenSy-SNER, which integrates syntactic features and semantic augmentation information, to extract fine-grained software knowledge entities from unstructured user-generated content. The input representation layer utilizes Bidirectional Encoder Representations from Transformers (BERT) model to extract the feature representation of the input sequence. The contextual coding layer leverages the Bidirectional Long Short-Term Memory (BiLSTM) network and Graph Convolutional Network (GCN) for contextual information and syntactic dependency information, and a semantic augmentation strategy based on attention mechanism is introduced to enrich the semantic feature representation of sequences as well. The tag decoding layer leverages Conditional Random Fields (CRF) to solve the dependency between the output tags and obtain the global optimal label sequence. The results of model comparison experiments show that the proposed model has better performance than the benchmark model in software engineering domain.

Background The triglyceride-glucose (TyG) index has been recognized as being an alternative cardiometabolic biomarker for insulin resistance associated with the development and prognosis of cardiovascular disease (CVD). However, the prospective relationship between baseline and long-term trajectories of the TyG index and carotid atherosclerosis (CAS) progression has yet to be investigated. Methods This longitudinal prospective cohort study included 10,380 adults with multiple general health checks at Peking University Third Hospital from January 2011 to December 2020. The TyG index was calculated as ln (fasting triglyceride [mg/dL] × fasting glucose [mg/dL]/2). The latent class trajectory modeling method was used to analyze the TyG index trajectories over the follow-up. Based on univariate and multivariate Cox proportional hazards analyses, hazard ratios (HRs) and 95% confidence intervals (CIs) were calculated for the baseline and trajectory of the TyG index. Results During a median follow-up period of 757 days, 1813 participants developed CAS progression. Each 1-standard deviation (SD) increase in the TyG index was associated with a 7% higher risk of CAS progression after adjusting for traditional CVD risk factors (HR = 1.067, 95% CI 1.006–1.132). Similar results were observed when the TyG index was expressed as quartiles. According to different trajectory patterns, participants were categorized into low-stable, moderate-stable, and high-increasing groups. After multivariate adjustment, the moderate-stable group had a 1.139-fold (95% CI 1.021–1.272) risk of CAS progression. The high-increasing trajectory of the TyG index tended to be associated with CAS progression (HR = 1.206, 95% CI 0.961–1.513). Conclusions Participants with higher baseline and moderate-stable trajectory of the TyG index were associated with CAS progression. Long-term trajectories of the TyG index can help to identify individuals at a higher risk of CAS progression who deserve specific preventive and therapeutic approaches.

User preference information plays an important role in knowledge graph-based recommender systems, which is reflected in users having different preferences for each entity–relation pair in the knowledge graph. Existing approaches have not modeled this fine-grained user preference feature well, as affecting the performance of recommender systems. In this paper, we propose a deep knowledge preference-aware reinforcement learning network (KPRLN) for the recommendation, which builds paths between user’s historical interaction items in the knowledge graph, learns the preference features of each user–entity–relation and generates the weighted knowledge graph with fine-grained preference features. First, we proposed a hierarchical propagation path construction method to address the problems of the pendant entity and long path exploration in the knowledge graph. The method expands outward to form clusters centered on items and uses them to represent the starting and target states in reinforcement learning. With the iteration of clusters, we can better learn the pendant entity preference and explore farther paths. Besides, we design an attention graph convolutional network, which focuses on more influential entity–relation pairs, to aggregate user and item higher order representations that contain fine-grained preference features. Finally, extensive experiments on two real-world datasets demonstrate that our method outperforms other state-of-the-art baselines.

Purpose To explore whether glycated albumin (GA) or fasting plasma glucose (FPG), both routinely monitored during patients’ hospital stay, can be used to predict post-transplantation diabetes mellitus (PTDM). Methods All kidney transplantation recipients (KTRs) from January 2017 to December 2018 were followed-up for 1 year. PTDM was diagnosed from day 45 post-operation to 1 year. When the completeness was above 80%, FPG or GA data on the day was selected, analyzed, and presented as range parameters and standard deviation (SD) and compared between PTDM and non-PTDM groups in fluctuation and stable periods. The predictive cut-off values were determined via receiver operating characteristic (ROC) analysis. The PTDM combined predictive mode, formed by the independent risk factors derived from logistic regression analyses, was compared with each independent risk factor with the independent ROC curve test. Results Among 536 KTRs, 38 patients developed PTDM up to 1 year post-operatively. Neuropsychiatric disorders are multifactorial disorders with diverse aetiological factors. Identifying treatment targets is challenging because the diseases are resulting from heterogeneous biological, genetic, and environmental factors. Nevertheless, the increasing understanding of G protein-coupled receptor (GPCR) opens a new possibility in drug discovery. Harnessing our knowledge of molecular mechanisms and structural information of GPCRs will be advantageous for developing effective drugs. This review provides an overview of the role of GPCRs in various neurodegenerative and psychiatric diseases. Besides, we highlight the emerging opportunities of novel GPCR targets and address recent progress in GPCR drug development.

Acute lung injury or acute respiratory distress syndrome (ALI/ARDS) is a common complication after aneurysmal subarachnoid hemorrhage (aSAH), and is associated with worse neurologic outcomes and longer hospitalization. However, the effect of ALI/ARDS in SAH has not been well elucidated. The purpose of this study was to determine the incidence of ALI/ARDS in a cohort of patients with SAH and to determine the risk factors for ALI/ARDS and their impact on patient prognosis. We performed a retrospective analysis of 167 consecutive patients with aSAH enrolled. ALI/ARDS patients were rigorously adjudicated using North American-European Consensus Conference definition. Regression analyses were used to test the risk factors for ALI/ARDS in patients with SAH. A total of 167 patients fulfilled the inclusion criteria, and 27% patients (45 of 167) developed ALI. Among all 45 ALI patients, 33 (20%, 33 of 167) patients met criteria for ARDS. On multivariate analysis, elderly patients, lower glasgow coma scale (GCS), higher Hunt-Hess grade, higher simplified acute physiology score (SAPS) II score, pre-existing pneumonia, gastric aspiration, hypoxemia, and tachypnea were the strongest risk factor for ALI/ARDS. Patients with ALI/ARDS showed worse clinical outcomes measured at 30 days. Development of ALI/ARDS was associated with a statistically significant increasing the odds of tracheostomy and hospital complications, and increasing duration of mechanical ventilation, intensive care unit (ICU) length and hospitalization stay. Development of ALI/ARDS is a severe complication of SAH and is associated with a poor clinical outcome, and further studies should focus on both prevention and management strategies specific to SAH-associated ALI/ARDS.
