Using survey data from three nationally representative surveys in 2004, 2009, and 2014 among senior high school students in Sweden, this study investigates trends in adolescents' lifetime prevalence of pornography use, frequency of pornography use, and type of pornography used over time. While almost all boys and a considerable proportion of girls used pornography across the three waves, the lifetime prevalence of pornography use decreased overall for both girls and boys. The share of boys who use pornography frequently increased over the three survey cycles; those who reported using pornography daily increased from 11% in 2004 to 24% in 2014. In contrast, there was no change in girls who reported using pornography daily, while the proportion who never used pornography increased from 40% in 2004 to 51% in 2014. Adolescents appear to use a narrower range of different pornography types over the survey cycles. Multiple logistic regression models were generated to investigate factors associated with pornography use over the 10-year period. The results suggest that rule-breaking behavior, having higher economic status and higher academic achievement were related to boy's pornography use, while rule-breaking behavior, early sexual debut and victimization were associated with girls' pornography use.

Prior research has described women's experiences with exercise-induced orgasm (EIO). However, little is known about men's experiences with EIO, the population prevalence of EIO, or the association of EIO with other kinds of orgasm. Using U.S. probability survey data, the objectives of the present research were to: (1) describe the lifetime prevalence of exercise-induced orgasm (EIO) and sleep orgasm; (2) assess respondents' age at first experience of EIO as well as the type of exercise connected with their first EIO; (3) examine associations between lifetime EIO experience and orgasm at respondents' most recent partnered sexual event; and (4) examine associations between lifetime EIO experience and sleep orgasms. Data were from the 2014 National Survey of Sexual Health and Behavior (1012 men and 1083 women, ages 14 years and older). About 9% of respondents reported having ever experienced exercise-induced orgasm. More men than women reported having experienced orgasm during sleep at least once in their lifetime (66.3% men, 41.8% women). The mean age for women's first EIO was significantly older than men (22.8 years women, 16.8 years men). Respondents described a wide range of exercises as associated with their first EIO (i.e., climbing ropes, abdominal exercise, yoga). Lifetime EIO experience was associated with lifetime sleep orgasms but not with event-level orgasm during partnered sex. Implications related to understanding orgasm and recommendations for clinicians and sex educators are discussed.

In this population-based survey, we have evaluated the lifetime prevalence of tic disorders and related sociodemographic factors and comorbidities of them. The data obtained from face-to-face interviews with 29,885 children and adolescents aged 6-18 years. We used the multistage cluster sampling method and a cross-sectional design. Tic disorders and other psychiatric disorders were assessed using the Persian version of the Kiddie Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version (K-SADS-PL). The lifetime prevalence of tic disorders was 1.5% (95% CI (1.3-1.7%)). The highest prevalence was in the age range of 15-18 years old with 1.9% (95% CI (1.6-2.3%)).57.7% (95% CI 51.8-63.3%) of patients with tic disorders had comorbidity with other psychiatric disorders. Increasing the prevalence of tic disorder with increasing age among Iranian children and adolescents, emphasizes the need to pay more attention to use of pharmacological and non-pharmacological treatments and increase education to families in this regard.

Background Work-related musculoskeletal disorder (WRMD) is a rising concern for surgeons, particularly those involved in minimally invasive surgery (MIS). Severe WRMD can adversely affect surgeon’s health and foreshorten their careers if not appropriately managed. Aims The aim of this study was to assess the prevalence of WRMD among Irish otolaryngologists and determine their knowledge of the best ergonomic principles. Methods A national survey was distributed electronically to all otolaryngology consultants and non-consultant hospital doctors (NCHDs) in Ireland. The survey assessed respondents’ age, grade, history of WRMD, and treatments sought for WRMD and knowledge of best ergonomic practice. Results Forty-nine of one hundred and two respondents completed the survey. The lifetime prevalence of WRMD among this cohort was 75.5%. Pain was the most commonly experienced symptom at 71.4%. The neck was the most frequently affected location (59.2%). Treatment for WRMD was sought by 36.7% of participants. The majority of respondents (73.5%) were unaware of recommendations made in the field of surgical ergonomics, while 85.7% were interested in learning ergonomic principles. Conclusion This study demonstrates a high prevalence of WRMD amongst otolaryngologists working in Ireland and demonstrates a need for ergonomic training for surgeons.

Auditory sensory over-responsivity (aSOR) is a frequently reported sensory feature of autism spectrum disorders (ASD); however, there is little consensus regarding its prevalence and severity. This cross-sectional study uses secondary data from the Autism Diagnostic Interview-Revised (ADI-R; Item 72: undue sensitivity to noise) housed in the US National Institute of Mental Health Data Archives to identify prevalence and severity of aSOR. Of the 4104 subjects with ASD ages 2–54 (M = 9, SD = 5.8) who responded to item 72, 60.1% (n = 1876) had aSOR currently (i.e., point prevalence) and 71.1% (n = 2221) reported having aSOR ever (i.e., lifetime prevalence). aSOR prevalence and severity were affected by age, but there were no associations with sex.

In the fifth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) separation anxiety disorder has been included in the chapter on anxiety disorders, thereby removing the age of onset restriction that previously required first onset during childhood or adolescence. Separation anxiety disorder has a lifetime prevalence of 4.8% and onset often occurs after the age of 18 years. Despite the high prevalence, separation anxiety disorder is often underdiagnosed and subsequently remains untreated. This narrative review summarizes the etiology, clinical features, diagnostic criteria as well as important differential diagnostic aspects, common comorbidity profiles and treatment implications of separation anxiety disorder. Furthermore, relevant implications for everyday practice and future perspectives for treatment and research are discussed.

Background Existing methods to estimate lifetime exposure to occupational carcinogenic agents could be improved. Objective We propose a new method to estimate the lifetime prevalence of exposure to occupational carcinogens using the example of painters and workers of the rubber industry in France. Methods From census, we calculated the proportion of painters and rubber industry workers using predefined occupational codes related to each occupation by sex and 10-year age group in 1982, 1990, 1999, 2007, and 2013. Using a beta-regression model, we obtained the yearly prevalence of exposure by 10-year age group over the period 1967-2007. We estimated the age- and sex-specific lifetime prevalence of exposure of the population in 2017 over 1967-2007, summing up the estimated prevalence of exposure for years 1967, 1977, 1987, 1997, and 2007 combined with a sex- and age-specific turnover factor. Corresponding population-attributable fractions were estimated for lung and bladder cancers in 2017. Results In 2017, we estimated that 5.6 and 0.2% of men in France had ever worked as a painter or in the rubber industry, respectively, during their working time. The lifetime prevalence of ever having worked as a painter or in the rubber industry was much lower in women: 1.8% and 0.1%, respectively. We estimated that 950 lung cancer and 40 bladder cancer cases were attributable to these occupations in 2017. Significance Based on accurate data and taking into account evolution of specific jobs over time, the proposed method provides good estimates of lifetime prevalence of exposure to occupational carcinogens. It could be applied in any other country with similar data.

Background Research on the prevalence of sexual violence in sport mostly focuses on incidents of sexual violence that happen in the context of sport, but few studies are available that systematically target the lifetime prevalence of sexual violence in the athletes (as persons), combining experiences in sport and outside the sport system. The aim of the current study was thus to compare sexual violence experiences of elite athletes in Germany in sport with those outside sport. Methods The study was carried out in cooperation with the German Olympic and Paralympic Committee who contacted their registered elite athletes. In total, 1529 elite athletes took part in an online survey and answered questions regarding their sexual violence experiences first in the context of sport and afterwards outside the sport context. Results Results show that 54.2% of the athletes had experienced some form of sexual violence during their lifetime, 20.6% even a severe incident of sexual violence. Incidents happened more often outside sport than in sport, but 48% of the victims were affected in both areas of life, indicating a high overlap of victimization experiences. Conclusion One recommendation from our results is therefore to make even stronger efforts to protect young athletes from becoming a victim of sexual violence. In case a sport club has a person of trust in the club, it can be an important place for athletes with sexual violence experiences (no matter whether experienced inside or outside sport), so that they are being heard and helped.

Relapse is a condition of getting back to illness after the recovery from illness. It results in stigma, high cost of treatments, and a decline in functionality both for patients and their families. It is common in mentally ill patients therefore this study attempted to assess lifetime prevalence and factors associated with relapse in a patient with mental illness. Institutional based cross-sectional study design was employed and simple random sampling techniques were used to select 178 study participants. Data were collected through face to face interviews by using a single item question to measure relapse. Data were entered by using Epi-data 3.1 software and exported to the statistical package for social science 22.0 software for analysis. Bivariate logistic regression analysis was conducted to see the association between dependent and independent variables and to identify significant variables associated with the outcome variable at P < 0.05 multivariable logistic regression analysis was done. One hundred seventy-eight mentally ill patients have participated in the study that yields a response rate of 100%. From all 125 (70.2%) of them had relapsed. Non adherance (AOR = 6.35, 95% CI, 2.524–15.852, P < 0.001), high disablity score (AOR = 3.728, 95% CI, 1.434–9.687, P = 0.007) and having single admission history (AOR = 0.196, 95% CI, 0.050–0.761 P = 0.019) were factors which indepedently associated with relapse of mentally ill patients. This study found more than two-thirds of patients had relapsed. Adherence to medication, functional disability, and having single admission were variables that predict the relapse of mentally ill patients.

Non-suicidal self-injury (NSSI) among adolescents is a major health concern globally. The epidemiology and determinant of NSSI have not been studied extensively in Iran. Thus, this longitudinal study sought to investigate the prevalence, incidence and contributing factors of NSSI among adolescents in Iran. This study was conducted at high school students aged 14-18 years old in the metropolitan city of Tabriz, located in north western Iran. Data were collected from 6229 10th-grade students at two time-points, with 12 months interval in October and November 2017 and 2018. A self-administered questionnaire was used to collect information on demographic, NSSI and cigarette and hookah smoking from the study participants. The results showed that lifetime prevalence of NSSI among students was 6.2% (95% confidence interval [CI]: 5.7 to 6.9) at the first assessment. Scratching and hair pulling were the most and least common methods of self-injury, respectively. Over the one year, 206 students (3.6% CI 95%: 3.1-4.1) reported the onset of NSSI. Tobacco smoking and previous year average grade were related to the incidence of NSSI. Although gender was significantly associated with the prevalence of NSSI, the relationship between gender and incidence of NSSI was not significant. Although the prevalence of NSSI was low among adolescents, the incidence rate was considerable. Further studies are required to understand the risk factors of NSSI in Iran.

Scarce data exist on the use of complementary and alternative medicine (CAM) by asylum seekers in Switzerland and their perception of discrimination. A cross-sectional study assessed the prevalence and type of CAM used by asylum seekers in one region of Switzerland and evaluated their self-perceived discrimination. Among the 61 asylum seekers who participated, lifetime prevalence of CAM use was 46%, with 28% reporting its use during the last year. Herbal medicine was the most frequently used CAM. Self-perceived discrimination was reported by 36% of asylum seekers, mainly related to their national origins. CAM users had a tendency to report more discrimination than non-users (44% vs. 30%). CAM use is prevalent among asylum seekers. Considering the importance of herbal medicine use and that only half of the respondents disclosed CAM use to their physician, clinicians should ask about it, notably because of potential risks of herb-drug interaction.

Poly-victimization can occur during childhood resulting in adverse events on health and well–being. The aim of this study is to determine the prevalence of past year as well as lifetime prevalence of poly-victimization among adolescents in Saudi Arabia (SA). A cross-sectional study was conducted in the five main provinces of SA. Boys and girls (n = 16,010) from public and private secondary high schools were selected to participate in the study. Nearly three-quarters (72%) experienced poly-victimizations during their lives and two third (66%) during the past year. Participants living with extended family, and living with a biologic and step-parent were 1.3 and 1.9 times respectively more likely to be poly-victimized during their lives and were 1.2 and 1.7 times respectively during the past year. Public policy should be established that encourages social workers and child protection services to screen for poly-victimization in cases of suspected adolescent maltreatment.

This study examined the lifetime prevalence of juvenile victimization, its demographic and socio-economic correlates, and predictors, using a random sample of 7–12th grade classes in three different-sized cities in Portugal. The sample consisted of 4,048 students (mean age = 15.27, standard deviation = 2.08). The overall prevalence of victimization, the diversity of its forms, and the prevalence of minor and serious victimization were calculated and used as broad measures. Findings show that half of the participants reported having suffered at least one victimization experience. Bivariate analyses revealed that specific demographic variables (age and city size) and socio-economic variables (relative family and personal affluence, number of caregivers, parents’ and personal immigrant status and father employment status) were associated with all broad measures of lifetime victimization. Multivariate analyses showed that being older (16-21 years), living in a medium or large city, having low relative family and personal affluence, and immigrant status were found to be associated with a heightened risk of lifetime victimization. Itch is the most frequent symptom of the skin, but its epidemiology including frequency has not been researched until a few years ago. Data is limited due to, e.g., the undulatory pattern of this symptom and the wide clinical spectrum such as itch being associated with a skin disease and itch on normal-looking skin. In addition, itch is an interdisciplinary symptom that is not paid attention to the same way in all clinical disciplines. This diversity may explain the incompleteness of epidemiological data of itch. With a recurrent symptom, it is important to consider different prevalence estimates (point-, 12-month, and lifetime prevalence) and to have a clear definition of itch. Recent population-based research demonstrates that more people suffer from chronic itch than previously assumed. The chapter summarizes new findings concerning the epidemiology of itch in the general population and in selected patient populations.

Ten years after the establishment of health monitoring units (GME) in Bavaria, temporal trends and urban-rural differences in parent-reported respiratory and allergic diseases as well as environmental tobacco smoke (ETS) exposure in preschoolers were analyzed in an explorative manner. Furthermore, associations between diseases and ETS exposure were studied. Parent questionnaires were used as part of the school entrance examination in two cross sectional studies (S1:2004/2005, n (1) = 6350;S2:2012/2013, n (2) = 5052). Temporal trends and urban-rural-differences were tested by X-2 tests. Associations between diseases and exposures were studied using multiple logistic regression analysis. The lifetime prevalence of atopic dermatitis declined from S1-S2 from 12.4 to 11.1 %, whereas those for hay fever, asthma, bronchitis and pseudocroup remained stable. In S1 and S2, bronchitis was less often reported in cities. The other diseases showed no urban-rural differences. The prevalence of children's ETS exposure at home declined from S1-S2 from 14.3 to 7.2 % and was generally higher in cities than in rural regions. There was no positive association between diseases and children's ETS exposure at home. In S2 an association was found between asthma and current parental smoking (OR = 1.60;95 % CI = (1.10-2.32)). The GME provide important data for regional distribution of respiratory and allergic diseases and ETS exposure of preschoolers in Bavaria. The results of the study are important for further development of questionnaires, which will be used in future GME.

Objective Worldwide, work-related musculoskeletal disorders (WMSDs) are a common cause of morbidity affecting occupational individuals such as health-care professionals. However, nothing is known about WMSDs in hospital nurses in Zimbabwe. This study was conducted to provide cross-sectional evidence of the 12-month prevalence, consequences and factors associated with WMSDs among 208 nurses at Parirenyatwa Group of Hospitals (PGH). Results The response rate for the study was 55.7%. The median age for the participants was 32.0 years (interquartile range = 29–36 years). The lifetime prevalence of WMSDs in nurses was 95.7% (n = 112). The first episodes were experienced in the first 5 years of working (n = 59, 52.7%). However, 82.1% (n = 96) nurses experienced WMSDs in the last 12 months. Low back pain was the most common WMSDs reported (n = 55, 67.9%). WMSDs were significantly associated with qualification attained, postgraduate ergonomic training and working experience. Overall, 87.5% (n = 84) nurses experienced at least one of the consequences of WMSDs. Cognisant of the limitations of the study, the present study found that WMSDs are a common occurrence among nurses at PGH. This creates a need for prompt hospital education programs aimed at raising awareness among nurses on the existence of WMSDs and the consequences at PGH. A genetic risk score (GRS) was developed for predicting fracture risk based on lifetime prevalence of femoral fractures in 924 consecutive autopsies of Japanese males. A total of 922 non-synonymous single nucleotide polymorphisms (SNPs) located in 62 osteoporosis susceptibility genes were genotyped and evaluated for their association with the prevalence of femoral fracture in autopsy cases. GRS values were calculated as the sum of risk allele counts (unweighted GRS) or the sum of weighted scores estimated from logistic regression coefficients (weighted GRS). Urinary tract infection is a common diagnosis made in both primary and secondary care, with a lifetime prevalence of 50 % in women. Although superficially simple to diagnose and manage, the diagnosis is often erroneous in older people. The majority of urinary tract infections are caused by enteric bacteria, specifically Escherichia coli and Enterobacteriaceae. In older people without symptoms directly attributable to the urinary tract, urinary tract infection (UTI) is unlikely yet often diagnosed. In the presence of urinary symptoms, urine dipstick is not a useful investigation and urine should be sent for culture to guide antibiotic treatment. This review summarises the current investigation and management of urinary tract infections in older men and women.

Atopic dermatitis is a chronic inflammatory skin condition including severe pruritus, xerosis, visible eczematous skin lesions that mainly begin early in life. Atopic dermatitis exerts a profound impact on the quality of life of patients and their families. The estimated lifetime prevalence of atopic dermatitis has increased 2~3 fold during over the past 30 years, especially in urban areas in industrialized countries, emphasizing the importance of life-style and environment in the pathogenesis of atopic diseases. While the interplay of individual genetic predisposition and environmental factors contribute to the development of atopic dermatitis, the recent increase in the prevalence of atopic dermatitis might be attributed to increased exposure to various environmental factors rather than alterations in human genome. In recent decades, there has been an increasing exposure to chemicals from a variety of sources. In this study, the effects of various environmental chemicals we face in everyday life - air pollutants, contact allergens and skin irritants, ingredients in cosmetics and personal care products, and food additives - on the prevalence and severity of atopic dermatitis are reviewed.

In this article, the results of the national survey of adult gambling behavior in North Cyprus (NC) in 2012 are presented. The aim of this study is to investigate the characteristics of adults' participation in gambling, and to determine the prevalence of 'problem and pathological gambling' in NC. The population of this study was formed from all the people living permanently in NC, speaking Turkish, and within the age group 18-65. Household interviews were conducted with 966 people. To obtain data, a 30 item questionnaire prepared by the researchers and a Turkish version of the Revised South Oaks Gambling Screen were used. Prevalence rates are compared with the results of the study conducted in 2007 using the same methodology and survey form. The lifetime prevalence of participating at least once in any of the 17 gambling activities investigated in the survey was 66.4 %. 3.5 % of the respondents scored as lifetime probable pathological gamblers and 9.2 % as probable problem gamblers. Risk factors for becoming probable problem and pathological gamblers include being male, being in the 19-28 age group, having a high education level, having a job and being born in Cyprus. This study shows that the prevalence of problem gambling is high in NC and increasing gradually. NC has socio-cultural features such as a history of colonization, socioeconomic problems and high unemployment, similar to other high prevalence gambling regions, which is suggestive of the importance of socio-cultural factors on gambling behavior.

Acne vulgaris is a common skin condition with 85 % lifetime prevalence. The prevalence and clinical characteristics of acne in pregnant women have not been studied intensively. The course of acne in pregnancy is highly variable, with many women experiencing exacerbation of their condition and some experiencing improvement. Topical medications are first line therapies for acne. In most cases of acne during pregnancy, whether it is presenting de novo or exacerbated by the pregnancy, both physicians and patients will prefer topical treatments, in order to avoid possible complications. However, safe systemic treatment options exist and should be considered for patients not responding to topical remedies. In this review, topical and systemic treatments appropriate for acne during pregnancy will be discussed, and their efficacy and safety will be appraised.

Few studies have examined the prevalence of problem and pathological gambling among clients of homeless service agencies. The objective of this study was to estimate the prevalence of problem gambling among these clients. We collected primary data on gambling using the NORC diagnostic screen for disorders. Using a modified time-location recruitment approach 264 clients of a community homeless service agency were screened for lifetime gambling problems. Descriptive statistics were produced using SPSSX. The prevalence of lifetime problem gambling was 10 % and that of pathological gambling was 25 % in this sample. The prevalence of lifetime problem and pathological gambling was alarmingly high relative to the general population lifetime prevalence. Better insight into interventions for gambling that might reduce risk of homelessness will help service agencies gauge the needs of their clients and to implement change to service delivery and screening practices.

Background In Zimbabwe, a recent increase in the volume of research on recurrent non-specific low back pain (NSLBP) has revealed that adolescents are commonly affected. This is alarming to health professionals and parents and calls for serious primary preventative strategies to be developed and implemented forthwith. Early identification initiatives should be prioritised in order to curtail the condition and its progression. In an attempt to be proactive in minimising the prevalence of recurrent NSLBP, this study was conducted to evaluate the content validity and test-retest reliability of a survey questionnaire with the aim of proffering a valid and reliable questionnaire which can be used in non-clinical settings to identify adolescents with recurrent NSLBP in Harare, Zimbabwe and determine the possible factors associated with the condition. Methods The study was conducted in two parts. The first part assessed content validity of the questionnaire using four experts derived from academia and clinical practice. The second part evaluated the reliability of the questionnaire among 125 high school-children aged between 13 and 19 years in a test-retest study. Results Twenty-six (26) out of thirty questions in the questionnaire had an Item Content Validity index of 1.00, demonstrating complete agreement among content experts. Overall, the Scale Content Validity Index for the questionnaire was 0.97. Item completion for the reliability study was satisfactory. The questionnaire items had kappa values ranging from 0.17 (slight agreement) to 1 (perfect agreement). High levels of reliability were found for the questions on school bag use (k=0.94), sports participation (k=0.97), and lifetime prevalence (k=0.89). Conclusion Excellent content validity and slight to perfect test-retest reliability was found for the Low Back Pain (LBP) questionnaire. These results are comparable to findings of other studies evaluating the psychometric properties of LBP questionnaires. Cognisant of the limitations of the study, the results of this study suggest that the LBP questionnaire could be used in local studies investigating LBP among adolescents although questions enquiring on functional limitations and sciatica may need further consideration. To estimate the prevalence and to describe clinical characteristics of tic disorders in 12–15 year old Warsaw schoolchildren. Children attending 24 randomly selected schools were screened by inquiring their parents and teachers. Children indicated as tic-positive by the screening procedure were investigated using semi-structured questionnaires and the Polish version of YGTSS scale. A validity study involved random selection and investigation of 130 non-indicated subjects. Out of 1,579 screened children, 104 met criteria for tic disorders, giving a lifetime prevalence of 9.9% (95% CI 7.1–12.6%) and a point prevalence of 6.7% (4.3–9.1%). Lifetime prevalence of ICD-10 tic disorders was 2.6% (95% CI 1.2–4.1%) for transient tic disorder (TTD); 3.7% (1.9–5.4%) for chronic tic disorder (CTD); 0.6% (0.2–0.9%) for Tourette disorder (TD); and 2.9% (1.2–4.6%) for non-specific tic disorder. Screening procedure had high sensitivity (92%) and low positive predictive value (18%). Tic disorders are common among Warsaw schoolchildren, have mild severity and form a continuum. The present study has confirmed numerous problems with studying neurobehavioral disorders in general population not referred to physicians, and stressed out the need to improve education on tic disorders in the general public.

Studies of the prevalence of pathological gambling (PG) in psychiatric and substance abusing patients suggest that the disorder is not rare. Most studies have been of substance abusers in treatment, and the rate of PG has been found to be several times higher than the rate found in community based epidemiological surveys. However, only one study has examined the prevalence of PG in a heterogeneous sample of patients, and this was a study of psychiatric inpatients. We are not aware of any prior study of the prevalence of PG in a psychiatric outpatient sample. In the present report from the Rhode Island Methods to Improve Diagnosis and Services (MIDAS) project we examined the current and lifetime prevalence of PG in 1,709 psychiatric outpatients interviewed with a semi-structured diagnostic interview that included a module to diagnose DSM-IV PG. Forty (2.3%) patients had a lifetime history of DSM-IV PG, all of whom had at least one other DSM-IV axis I disorder. Patients with PG had significantly more axis I disorders than patients without PG, and had significantly higher rates of bipolar disorder, social phobia, panic disorder with agoraphobia, alcohol use disorder, and other impulse control disorders. Possible reasons for the low prevalence of PG in our sample are discussed.

Urolithiasis is the third most common pathological disease afflicting the urinary tract, and usually occurs between the third and fourth decades of an individual's life. Epidemiological studies about this condition are lacking in our country. In 1998, we performed an epidemiological, cross-sectional study of the prevalence of urolithiasis in a sample of 1,086 subjects, which included men and women of all ages, selected from the general population of the city of Buenos Aires. The method used to gather basic information was an auto administered questionnaire about the present or past history of urolithiasis that was handled at the dwelling by a trained volunteer. We found a 3.96% lifetime prevalence of urolithiasis in the general population of Buenos Aires. The rate was slightly higher in men (4.35%) than in women (3.62%), with a male to female ratio of 1.19:1. No case of urolithiasis was found in subjects under the age of 20. In subjects over 19 years, the prevalence rate of the disease was 5.14%; 5.98% for men (CI 3.41-8.55%) and 4.49% for women (CI 2.61-6.37%). Prevalence increased with age, ranging from 2.75% in the 20-39 age group to 7.79% in thoseor=60 years. The life time prevalence rate of urolithiasis observed in Buenos Aires is similar to that reported in a few other studies performed among males and females in the general population of USA and Europe. Prevalence of urolithiasis increases with age both in men and in women.

Depression is a common disorder (lifetime prevalence nearly 20% in industrialized countries) that presents in two etiologically different forms (unipolar depression, bipolar disorder). Both forms are subject to a genetic influence, which is stronger in bipolar disorder. Unipolar depression is also strongly influenced by nongenetic environmental factors (e. g., traumas, socialization, critical life events). There is strong evidence for a gene-- environmental interaction in unipolar depression. The search for genes was recently very successful in bipolar disorder. There is currently no evidence for disposition genes with strong effects. The prediction of response to selective serotonin-reuptake inhibitors (SSRI) by genetic markers presents currently the best confirmed molecular-genetic findings for unipolar depression. Complicating factors are gene--environmental interaction, only small to modest effects of disposition genes, and putative epigenetic mechanisms.

While the overall differences were small between schizophrenics using (dual diagnosis) and those not using substances, dual-diagnosis patients in general reported more positive symptoms, especially more intense hallucinations. These differences were observed in patients with current (3-month) substance use on admission but not on discharge, possibly as a result of substance use. The most marked differencses were in previous suicide attempts and delinquency, which were more prevalent in dual-diagnosis schizophrenics. These findings indicate that patients with dual diagnosis are more disturbed than other schizophrenics. We discuss the implications for the self-medication hypothesis for substance use in schizophrenia and future research in this area are discussed.

The present study investigates whether highly self-preoccupied people (exhibiting a tendency to focus primarily on the self and to maintain self-focused attention) were more likely to experience major depressive episodes (MDEs) than those without such tendencies. One hundred nineteen young community residents, aged 18 to 21, took part in semistructured interviews, during which we investigated their past and present history of mental illness, including MDEs, as delineated by the Diagnostic and Statistical Manual of Mental Disorders (3rd ed. rev.; DSM-III-R). Self-preoccupation was measured by the Self-Preoccupation Scale (SPS). Of the 119 participants interviewed, the lowest and highest quarters in the SPS scores formed the low- and high-self-preoccupation (SP) groups. The lifetime prevalence of the DSM-III-R MDE was significantly greater among those high in SP than in the low SP group. Moreover, the high-SP group had significantly more depressive symptoms than the low-SP group. The contributory role of self-preoccupation to suicide ideation and the interpersonal aspects of self-preoccupation were discussed.

This article examines the effect of war events and pre-war depression on the prevalence of major depression during war. A total of 658 subjects aged 18-65 years were randomly selected from four Lebanese communities differentially exposed to the Lebanon Wars and were interviewed using the Diagnostic Interview Schedule (Arabic version). The individual levels of exposure to war events were assessed through a War Events Questionnaire. The lifetime prevalence of the DSM-III-R-defined major depression varied across the four communities from 16.3 to 41.9%; the final parameters predicting major depression since the onset of the wars were: depression before the wars and exposure to the wars. Both, individual levels of exposure to war and a history of pre-war depression, predict the development of depression during war.

The occurrence of depression in multiple sclerosis (MS) patients is very high, with a lifetime prevalence of up to 50%. This co-occurrence has important negative consequences for MS patients. Until now, questions about the appropriateness of classification criteria and the reliability of assessment instruments have not been completely answered. At this time, it seems worthwhile to try to clarify these points considering their relevance for clinical and therapeutic approaches to treat MS and particularly to treat depression in MS. The risk of underestimating depressive symptoms is noted and appropriate treatment is strongly recommended. More recent antidepressant drugs are preferable in treating depression in MS patients because they have a better profile of side effects. At present, combined pharmacological and psychological therapy seems to be the best approach. Recently, rTMS was also proposed for treating depression in MS patients.

Advances in psychiatric research have reestablished the importance of psychiatric community epidemiological studies. Psychiatric epidemiological surveys that used the same standardized diagnostic interview and classification system in different parts of the world were reviewed. The lifetime prevalence for any psychiatric morbidity ranged from 21 to 65%. The prevalence of psychiatric disorders was higher than that in the first- and second-generation community surveys. The relevance of these findings to clinical practice, public health, and the directions for future epidemiological research are discussed.

This paper reports on psychiatric case identification by the application of the Structured Clinical Interview for DSM-III-R (SCID) in a sample of 198 Greek migrants repatriated from western europe in a northwestern province of Greece. The current (1 month) prevalence of psychiatric morbidity, based on the total number of diagnosed cases, was found to be 43.4%. Lifetime prevalence of psychiatric disorders was found to be higher (49.4%). The majority of the sample were diagnosed as suffering from anxiety disorders and dysthymia. Psychiatric disorders were found to be more prevalent among middle-aged respondents. Duration of stay in the foreign country was a factor correlated with psychiatric morbidity. Of short-term migrants 54% were found to suffer from specific nosological entities, whereas 32% of long-term migrants were diagnosed as cases. The two objectives of this study, based on a sample of 1398 Swiss army conscripts born in 1966 who participated in a first study in 1985, were to measure the prevalence of low back pain (LBP) at age 26 years and its incidence between 19 and 26 years and to analyze the relationship between LBP and occupational, nonoccupational, or physical risk factors. The lifetime prevalence of LBP at age 26 was 69.1% and the incidence of LBP between 19 and 26, 44.7%. A history of LBP or a pathological physical examination result at age 19 did not predict the prevalence or the incidence at age 26. Recent epidemiological studies have established that the lifetime prevalence rate of depression is greater in women than in men. It was the purpose of the present study to investigate the possibility that the true prevalence of male depression is underestimated because males have learned through social rejection that it is inappropriate for them to openly express depressive feelings. Consistent with this notion that men only express depressive symptoms consonant with their traditional male sex role, a discriminant function analysis performed on the self-reported symptomatology of depressed patients revealed that men were more likely to report sex role appropriate symptoms such as work-related problems and somatic concerns. Since other self-presentational concerns may contribute to the sex difference in depression, it is suggested that future research directly examine the ways in which men experience and express symptoms of depression.

A population survey of rural, semi-rural and urban areas of Agra was carried out in order to determine the prevalence of severe mental disorders in an Indian community. The lifetime prevalence rate was 23,79 per 1000 and conditions such as schizophrenia and severe mental retardation occurred with approximately the same frequency as in other parts of the world. Severe hysterical states, mainly taking the form of “fits”, were common, particularly among women, among illiterate people in joint families and in castes which provided very protected conditions for females. High rates were also found among Punjabi refugees, who had had particularly difficult experiences during their migration.
