Purpose: The purpose of this study was to report the results of subacromial arthroscopic decompression (SAD) without removal of the calcific deposits in patients with calcifying tendinitis., Methods: All patients between 2016 and 2019 were included if they were aged between 18 and 60 years and had an isolated calcific deposit. The Constant-Murley score (CMS), Disabilities of the Arm, Shoulder and Hand (Quick DASH) score, the Shoulder Pain and Disability Index (SPADI), the simple shoulder test (SST), and the single assessment numeric evaluation (SANE) were used for assessment. Time to return to work was recorded., Results: 24 patients (13 pilots, 11 cabin crew) with a mean age of 47.1 ± 7.8 years were included. Quick Dash improved from 68.8 preoperative, to 8.4 at 3 months, and 0.1 at 24 months. CMS improved from 37.4 preoperative, to 83 at 3 months, and 94 at 24 months. SPADI improved from 73.8 preoperative, to 5.4 at 3 months, and 1 at 24 months. SST improved from 22.5 preoperative, to 94.2 at 3 months, and 100 at 24 months. SANE improved from 33.5 preoperative, to 78.7 at 3 months, and 95.6 at 24 months. MCID, SCB, and PASS reached values above 83% at 3 months and 100% at 6 months, with the exception of SANE which reached 29% at 3 months and plateaued to 96% at 6 months. The mean time to return to work was 7.1 ± 2.1 weeks. Pilots returned at a mean of 6.9 ± 1.8 weeks; cabin crew returned to work at a mean of 7.8 ± 2.5 weeks., Conclusions: The results of this study suggest excellent short- and mid-term clinical outcomes can be achieved in patients with calcific tendinitis undergoing arthroscopic debridement and subacromial decompression without removal of calcific deposits. In this patient population, early surgical intervention was a potentially viable alternative to nonoperative treatment, and allowed early return to work., (© 2022. Purpose: The common peroneal nerve (CPN) can be injured during fibular-based posterolateral reconstructions due to its close relationship to the neck of the fibula. Therefore, the purpose of this study was to observe the course of the CPN and its branches around the fibular head and neck and quantify the position in relation to relevant bony landmarks and observe the relation between tunnel drilling for posterolateral corner reconstruction and both the tunnel entry and exit at the proximal fibula and the CPN and its branches was observed., Methods: In 101 (mean age = 70.6 ± 16 years) embalmed cadaver knees, the relationship between bony landmarks (tibial tuberosity, styloid process of fibula (APR)) and the CPN and its branches were established and 8 (M1-M8) distances from these landmarks measured; mean, SD and 95% CI were recorded. In 21 of these knees, a fibula tunnel was drilled as in PLC reconstruction and the association of the CPN and its branches to the tunnel entry and exit were judged by two independent observers. Fisher's exact test of independence was used to determine significant differences between genders. Tunnel intersection was analysed in a binary yes/no fashion and was described in frequencies and percentages., Results: The mean distance from the APR to where the CPN reaches the fibula neck (M1) was 31.4 ± 8.9 mm (CI:29.8-33.0); from the apex of the styloid process (APR) to where the CPN passes posterior to the broadest point of the fibular head (M3) was 21.7 ± 12.6 mm (CI:19.4-24.0); from the apex of the APR to the most proximal point of the CPN/CPN first branch in the midline of the fibular head (M2) was 37.0 ± 6.7 mm (CI: 35.4-37.7). Out of the 21 randomly selected knees for drilling, the first branch of the CPN was damaged at the tunnel entry point in 7 (33%), and in 5 knees (24%), the CPN was damaged at the tunnel exit. In one knee, at both the tunnel entry and exit, the first branch of the CPN and the CPN were intersected, respectively., Conclusion: The results of this study strongly suggest that the CPN is at risk when drilling the fibula tunnel performing fibula-based posterolateral corner reconstructions. The total injury rate was 57% with a 33% incidence of injury to the first branch of the nerve at the tunnel entry and 24% to the CPN at the tunnel exit., Clinical Relevance: Due to the high incidence of injury, percutaneous placement of guide pins and tunnel drilling is not recommended. The nerve should be visualized and protected by either a traditional open approach or minimally invasive techniques. With a minimally invasive approach, the nerve should be identified at the fibula neck and then followed ante- and retrograde.

Introduction: The purpose of this study was to perform a meta-analysis comparing open and arthroscopic surgical techniques for distal clavicle resection., Methods: A systematic review of Medline, Embase, Scopus, and Google Scholar identified relevant publications in the English and German literature between 1997 and 2017. All included studies were levels I-IV, describing both treatments, with a minimum of 12 month follow-up, had at least one validated outcome score and documented patient recruitment, study design, demographic details, and surgical technique. Studies were excluded if they were only abstracts or conference proceedings, involved revision procedures, or the loss to follow-up exceeded 20%. Publication bias and risk of bias were assessed using the Cochrane Collaboration tools, and heterogeneity was assessed using the I 2 statistic., Results: Four studies (n = 319 patients) met the criteria for inclusion. The pooled estimate for clinical outcomes (Constant, ASES) demonstrated no significant differences (SMD 0.323, I 2  = 0%, p = 0.065) between open and arthroscopic resection, although the analysis favored open resection. The pooled estimate for clinical outcomes (SST) also demonstrated no significant differences (SMD 0.744, I 2  = 49.82%, p = 0.144) between open and arthroscopic resection, but the analysis again favored open resection. The pooled estimate for VAS assessment of pain demonstrated no differences (SMD 0.217, I 2  = 58.96%; p = 0.404) between open and arthroscopic resection., Conclusion: The results of this study suggest that similar functional and clinical outcomes can be achieved with either open or arthroscopic distal clavicle resection. The observed trend that open resection may have a more favorable outcome warrants further investigation., Level of Evidence: Level 3; systematic review and meta-analysis.

Purpose: The purpose of this study was to investigate the isokinetic, eccentric and isometric hamstring/quadriceps (HQ) ratios in patients before and after ACL reconstruction (ACLR) using bone-patellar tendon grafts and to establish the relationships between HQ ratio and knee function., Methods: Forty-four patients (mean age of 26.6 years) underwent isokinetic testing of quadriceps and hamstring muscles before and after ACLR and HQ ratios were calculated. Lysholm, IKDC and Cincinnati Scores were used to assess function. Isokinetic concentric and eccentric peak torque (Nm/kg) was measured at three different speeds: 60, 120, and 180°/s. Isometric strength was tested at 30° and 60° of knee flexion., Results: For the isometric tests, the HQ ratio between the involved and non-involved limb was not different for the ACLD knee (p = 0.28) at 30° knee flexion, but significant at the 60° flexion angle (p = 0.02) and for the ACLR knees at 30° and 60° (p = 0.02). For the isokinetic tests, the ratio between involved and non-involved limb was significant for ACL-deficient knees at both 60 (p = 0.039) and 120°/s (p = 0.05). There were significant differences between limbs for all speeds in ACLR knees (p = 0.0003-0.01). For the eccentric tests, the HQ ratio between the involved and non-involved limbs was not significant for both the ACLD (p = 0.19) and ACLR knees (p = 0.29) at the speed of 60°/s. At 120 and 180°/s, there were significant differences between limbs for both the ACLD (p = 0.02) and ACLR knees (p = 0.003). Linear regression did not reveal significant relationships between Cincinnati, Lysholm, and IKDC scores and HQ ratios in the ACLD (R 2  = 0.35, p = 0.58; R 2  = 0.34, p = 0.63; R 2  = 0.38, p = 0.49). In contrast, there were significant correlations between the Lysholm and IKDC scores and HQ ratios in the ACLR knees (R 2  = 0.84, p = 0.002; R 2  = 0.86, p = 0.001)., Conclusions: The findings of this study suggest that the HQ ratio in ACLD patients was not a predictor, but an indicator of patient-perceived knee function following ACLR., Level of Evidence: Level IV, case series.

Purpose: Knee function, whether anterior cruciate ligament (ACL)-deficient or ACL-reconstructed, is related to many conditions, and no single biomechanical variable can be used to definitively assess knee performance. The purpose of this study was to investigate the relationship between extension and flexion muscle strength and knee function in patients prior and following ACL reconstruction., Methods: 44 ACL-deficient patients with a mean age of 26.6 years were tested between 3 and 6 months following an acute injury and 2 years following ACL reconstruction. All reconstructed patients underwent surgical reconstruction within 6 months of ACL injury using bone-patellar tendon and interference screws. The Cincinnati knee rating system was used to assess knee function. Muscle strength was assessed with the Biodex™ Dynamometer. Isokinetic concentric and eccentric flexion and extension peak torque (Nm/kg) was tested at three different speeds: 60°/s, 120°/s and 180°/s. Isometric strength was tested in 30° and 60° of knee flexion. Both the involved and non-involved legs were tested to calculate symmetry indices., Results: The mean Cincinnati score in the ACL-deficient patient was 62.0 ± 14.5 (range 36-84) and increased to 89.3 ± 9.5 (range 61-100) in the ACL-reconstructed patient. Significant relationships between knee function and muscle strength in the ACL-deficient group were observed for knee symmetry indices (r = 0.38-0.50, p = 0.0001-0.05). In the ACL-reconstructed group significant relationships between knee functionality were observed for isometric and isokinetic peak torque of the involved limb (r = 0.46-0.71, p = 0.0001-0.007)., Conclusion: The findings of this study suggest that neither extension nor flexion peak torque were correlates of knee function in the ACL-deficient knee. However, leg symmetry indices were correlated to knee function. In the ACL-reconstructed knee, knee symmetry indices were not related to knee function but extension and flexion isokinetic concentric and isometric peak torque were.

Introduction: Conventional cutting guides in total knee arthroplasty can potentially cause unintentional deviation from the planned direction and depth of bone resection resulting in malaligned components. The purpose of this study was therefore to investigate the accuracy of bone cutting jigs for both the femur and tibia using imageless navigation., Material and Methods: A total of 125 patients with a mean age of 66.7 ± 9.9 years underwent primary total knee arthroplasty with a Stryker Triathlon™ fixed bearing posterior cruciate retaining implant using imageless navigation. Coronal and sagittal position of the secured cutting jig was recorded and bone resection was checked with a rectangular probe attached to a navigation tracker., Results: There were significant within group differences for the femoral sagittal cut (mean δ = 0.9° [31 %]; p = 0.00001), femoral depth medial compartment (mean δ = 0.5 mm [5 %]; p = 0.001), femoral depth lateral compartment (mean δ = 0.7 mm [7 %]; p = 0.00001), proximal tibial cut (mean δ = 0.3 mm [25 %]; p = 0.001), tibial depth medial compartment (mean δ = 0.6 mm [10 %]; p = 0.0001) and tibia depth lateral cut (mean δ = 0.4 mm [5 %]; p = 0.002). Deviation of more than 2° was observed for the distal cut in the sagittal plane in 17 % and in 9.6 % for the proximal tibial cut in the sagittal plane of all patients., Conclusion: The results of this study demonstrated significant differences between the dialed in cut and "actual" bone resection achieved for all planes for both the femur and tibia. The femur sagittal cut demonstrated a tendency for an extended cut and the tibia showed a tendency for varus.

Purpose: Fluctuating levels of endogenous estrogen are thought to have an adverse effect on lower limb biomechanics, given the observed higher rate of ACL injury at certain phases of the menstrual cycle. The purpose of this study was to investigate the effects of fluctuating endogenous estrogen levels during the menstrual cycle on acceleration transients at the proximal tibia in young physically active females., Methods: Eleven females aged 16-18 years participated in this study and were compared to a male control group. Female subjects were tested at each of the four phases of the menstrual cycle: menses, follicular, ovulation and luteal. On each test occasion, acceleration transients at the proximal tibia were measured while subjects performed an abrupt deceleration task (simulated netball landing)., Results: No significant differences were found between the different phases of the menstrual cycle for peak tibial acceleration (PTA; P = 0.57), and time to zero tibial acceleration (TZTA; P = 0.59). However, there was a significant difference for time to peak tibial acceleration (TPTA) between menstruation and follicular (P = 0.04), menstruation and ovulation (P = 0.001), menstruation and luteal phase (P = 0.002), and follicular phase and ovulation (P = 0.007). In the male control group, no significant between-test session differences were observed for PTA (P = 0.48), TZTA (P = 0.08) and TPTA (P = 0.29). While there were no significant between-group differences for PTA (P = 0.21) and TZTA (P = 0.48), significant between-group differences were observed for TPTA (P = 0.001)., Conclusion: The results of this project strongly suggest that serum estrogen fluctuations have an effect on tibial acceleration profiles in young female athletes during different phases of the menstrual cycle.

Introduction: Neer type II lateral clavicle fractures are inherently unstable and debate continues regarding optimal treatment. The purpose of this study was to review the outcome of Neer type II lateral clavicle fractures treated using a novel technique, which incorporates a superiorly placed distal radius locking plate combined with TightRope™ fixation to augment the coraco-clavicular ligaments., Methods: Between 2007 and 2010, a total of 31 Neer type II fractures were managed operatively at our institution. Clinical outcomes were assessed using the SPADI, DASH, Constant, and Taft scores. Subjects were evaluated at 6 weeks, 3, 6 months, and 1 year. For the latest follow-up (mean 38.7 months), all patients were telephoned by an independent research associate and interviewed to establish SPADI and DASH scores. Radiological union was evaluated using standard views including an antero-posterior view, an angled (Zanca) view, and a trans-scapular view., Results: Bony union was achieved at a mean follow-up of 6.3 ± 4.1 weeks. At the 1-year follow-up, the AC joint remained reduced radiologically in all cases. At 1 year, the mean Constant score was 95.6 ± 4.82, the DASH score was 3.45 ± 3.02, and the SPADI score was 2.58 + 1.94. At the latest follow-up, at a mean of 38.7 + 14.8 months, the DASH score was 0.6 ± 2.1 and the SPADI score was 0.6 ± 2., Conclusion: Surgical fixation of Neer type II lateral clavicle fractures using a 2.4-mm distal radius locking plate in combination with TightRope™ suture augmentation results in a predictable outcome, preserving excellent shoulder function while maintaining a low complication rate.

Introduction: Primary wound closure in the management of open tibial fractures has generally been discouraged. Several prior studies suggest that infections are not caused by the initial contamination, but are instead the result of organisms acquired in the hospital. Primary wound closure after adequate wound care and fracture stabilisation could therefore be considered a reasonable option., Materials and Methods: We analysed 95 patients with open tibial fractures (Gustilo-Anderson type 1 to 3A) treated with primary fracture stabilisation and either delayed wound closure (group I) or primary wound closure (group II), with a minimum follow-up of 12 months., Results: Group I included 46 patients with a mean age of 30.2 years (16-56), and a mean follow-up of 13.5 months (12-18). Group II included 49 patients with a mean age of 33.4 (18-69), and a mean follow up of 13.7 months (12-16). One infection developed in group I (2%), and two infections developed in group II (4%). This difference was not found to have any statistical significance., Conclusion: Our results support other recent reports that the infection rate is not increased following primary wound closure after thorough debridement of less severe open fractures. The length of stay following primary closure (group II) was significantly shorter, and that should result in substantially more cost effective care of these serious injuries. We conclude that primary wound closure is a safe option in properly selected cases. Prospective multi-centre studies are needed to further evaluate the safety and efficacy of this treatment alternative.

Immunoreactive parathyroid hormone (iPTH), 25(OH)D3, 24,25(OH)2D3, 1.25(OH)2D3, creatinin, phosphate and calcium were determined in 33 children suffering from chronic renal failure as well as in 7 healthy children serving as a control group. In 9 children with preterminal renal insufficiency and in 13 children who had to undergo hemodialysis iPTH and creatinin were closely related. In the hemodialysed children's group serum phosphate was significantly elevated whereas calcium did not different from the control group. Treatment with vitamin D3 showed in those children undergoing hemodialysis increased 25(OH)D3 in normal 1.25(OH)2D3 plasma levels. Nevertheless, in the children with preterminal renal insufficiency as well as the children under hemodialysis iPTH was significantly increased. Thus, it is suggested that a negative feed back regulation controls the synthesis of 1.25(OH)2D3. After kidney transplantation the serum levels of iPTH, the vitamin D3 metabolites and creatinin became normal. There was no correlation between iPTH and 25(OH)D3 or 1.25(OH)2D3, but the correlation with creatinin (P less than 0.05, r = 0.7) before and after transplantation was statistically significant. Thus, the alterations in vitamin D metabolism and the hyperparathyroidism in chronic renal osteodystrophy appear to be largely independent of each other.
