Purpose: Preterm children with cerebral palsy (CP) often have varying hand dysfunction, while the specific brain injury with periventricular leukomalacia (PVL) cannot quite explain its mechanism. We aimed to investigate glymphatic activity using diffusion tensor image analysis along the perivascular space (DTI-ALPS) method and evaluate its association with brain lesion burden and hand dysfunction in children with CP secondary to PVL., Methods: We retrospectively enrolled 18 children with bilateral spastic CP due to PVL and 29 age- and sex-matched typically developing controls. The Manual Ability Classification System (MACS) was used to assess severity of hand dysfunction in CP. A mediation model was performed to explore the relationship among the DTI-ALPS index, brain lesion burden, and the MACS level in children with CP., Results: There were significant differences in the DTI-ALPS index between children with CP and their typically developing peers. The DTI-ALPS index of the children with CP was lower than that of the controls (1.448 vs. 1.625, P = 0.003). The mediation analysis showed that the DTI-ALPS index fully mediated the relationship between brain lesion burden and the MACS level (c' = 0.061, P = 0.665), explaining 80% of the effect., Conclusion: This study provides new insights into the neural basis of hand dysfunction in children with CP, demonstrating an important role of glymphatic impairment in such patients. Background: Hip displacement in children with cerebral palsy (CP) is monitored by measuring migration percentage on anteroposterior pelvis radiographs. However, proper positioning for radiography in children with spasticity is difficult. The reliability and accuracy of migration percentage as a function of patient positioning is unknown., Objective: To determine the effects of patient positioning on migration percentage measurements in children with CP., Materials and Methods: We identified children with CP (≤18-year-old) with pelvis CT and anteroposterior pelvis radiograph obtained <6 months apart (10/2018-11/2021). Digitally reconstructed radiographs were generated from each pelvis CT, to simulate nine different patient positions: neutral; 10° and 20° lordosis and kyphosis; and 10° and 20° right rotation and left rotation. Two radiologists measured migration percentages from the simulated and real pelvis radiographs. We used Spearman's rho to assess inter-rater reliability, and Wilcoxon signed rank test to determine statistical significance., Results: We studied sixty-three children (male=41; median age=8 years; range=4-18 years). The two radiologists' migration percentage measurements were highly correlated with each other across all simulated and real radiographs (Spearman's rho=0.86-0.99, P<0.01). For both readers and hips, migration percentages measured from real radiographs were significantly different from those measured from neutral simulated radiographs (P<0.01), with median absolute difference=5-6 percentage points (PP) and interquartile range (IQR)=9-12 PP. When comparing migration percentage measurements from neutral simulated radiographs to those in kyphosis/lordosis and right/left rotations, median absolute differences were 2-4 PP (IQR=3-8 PP) and 4-15 PP (IQR=6-17 PP), respectively., Conclusion: Inter-rater reliability of measured migration percentages is high, but accuracy decreases with patient positional changes., (© 2023. Cerebral palsy (CP) is the most common motor disorder in childhood. Recent studies in children with CP have associated weakened sensorimotor performance with impairments in the major brain white-matter (WM) structure, corpus callosum (CC). However, the relationship between CC structure and lower extremity performance, specifically gait and balance, remains unknown. This study investigated the transcallosal WM structure and lower limb motor stability performance in adolescents aged 10-18 years with spastic hemiplegic (n = 18) or diplegic (n = 13) CP and in their age-matched controls (n = 34). The modern diffusion-weighted MRI analysis included the diffusivity properties of seven CC subparts and the transcallosal lower limb sensorimotor tract of the dominant hemisphere. Children with CP had comprehensive impairments in the cross-sectional area, fractional anisotropy, and mean diffusivity of the CC and sensorimotor tract. Additionally, the extent of WM alterations varied between hemiplegic and diplegic subgroups, which was seen especially in the fractional anisotropy values along the sensorimotor tract. The diffusion properties of transcallosal WM were further associated with static stability in all groups, and with dynamic stability in healthy controls. Our novel results clarify the mechanistic role of the corpus callosum in adolescents with and without CP offering valuable insight into the complex interplay between the brain's WM organization and motor performance. A better understanding of the brain basis of weakened stability performance could, in addition, improve the specificity of clinical diagnosis and targeted rehabilitation in CP., (© 2023. Children with cerebral palsy are at increased risk of hip dislocation. Detecting hip subluxation through radiographic hip screening is an essential component of hip surveillance and has been shown to prevent hip dislocations. Large-scale hip surveillance programs are being implemented nationwide, highlighting the importance of uniform technical and reporting standards., (© 2021. Intrathecal baclofen has long been known to be an efficacious treatment of spasticity in children with cerebral palsy. Test bolus administration is often necessary to ensure patients will benefit from this treatment. The introduction of a intrathecal catheter for test bolus administration can prove challenging in a subset of this population, particularly those who have received surgery with postoperative spinal fusion masses. We outline a novel technique of inserting a spinal catheter for test bolus administration in a patient with a postoperative fusion mass whereby a fenestration is created through the lamina using an osteotomy needle.

We retrospectively studied the anamnesis, in particular the etiology, the clinical picture, and computed tomography/magnetic resonance imaging/ultrasound data, in the dynamics of a heterogeneous group of 127 patients with neurological and psychiatric pathological conditions. We were interested in the reasons for the occurrence, the clinical value of various neuroimaging abnormalities in the white matter of the brain, including the periventricular zone, the communication of their occurrence with the possible exit of CSF outside of the limits of the ventricular system. In some of the patients investigations into the cerebral blood flow in dynamics using transcranial Doppler was studied. Also in this regard, indications for and the application of minimally invasive neurosurgery techniques for brain revascularization were investigated.

Background: Respiratory and cardiovascular diseases are the most common causes of death in children with cerebral palsy., Objective: To evaluate sonographic carotid intima-media thickness, an early marker of atherosclerosis, in children with cerebral palsy and in healthy controls., Materials and Methods: One hundred children with cerebral palsy (65 boys), mean age 6.2 (SD, 2.1) years, and 35 age-matched and sex-matched healthy controls were included. Common carotid artery intima-media thickness was measured sonographically. Differences between patients and controls were evaluated with an independent samples t-test., Results: Age, sex distribution and levels of serum lipids were comparable between patients and controls. Average, right and left carotid artery intima-media were thicker in patients compared with controls (mean ± SD, 0.61 ± 0.13 mm vs 0.40 ± 0.03 mm; 0.61 ± 0.14 mm vs 0.40 ± 0.03 mm; 0.61 ± 0.13 mm vs 0.40 ± 0.03 mm, respectively; all P < 0.001)., Conclusion: Carotid intima-media is sonographically thicker in children with cerebral palsy compared with healthy controls, which may express an increased risk of atherosclerotic diseases.

Background: In the nonambulatory cerebral palsy (CP) population with a prior history of fracture, the use of pamidronate is not always effective in preventing further fractures., Objective: To test the hypothesis that when fractures occur after cyclic pamidronate, they will be at the proximal or distal end of a pamidronate band., Materials and Methods: Retrospective review of our CP patient database revealed 53 children who had received one or more complete courses of pamidronate therapy (five cycles over 12 months). Medical records were screened to identify children who had sustained a fracture or fractures after completing treatment., Results: Of 53 patients treated with pamidronate, only 14 sustained fractures after treatment. Radiographs were available for 11 patients, showing 19 fractures. Sixty-three percent of these fractures were located at a junction with pamidronate bands but not within the bands., Conclusions: We propose stress risers as the mechanism for fractures that have occurred where bone mineral density abruptly changes as a result of cyclic administration of pamidronate. We show a theoretical example of how alternative dosing might reduce the ratio and therefore decrease the chance of formation of a stress riser.

The early onset of degenerative cervical lesions has been well described in patients suffering from athetoid or dystonic cerebral palsy. Myelopathy can occur and aggravate of their unstable neurological status. Diagnosis and treatment are delayed and disrupted by the abnormal movements. This retrospective study was implemented to evaluate the symptoms, the anatomical findings, and the surgical management of seven patients from 20 to 56 years old suffering from cervical myelopathy and athetoid or dystonic cerebral palsy. The mean delay in diagnosis was 15 months and the mean follow-up was 33 months. The initial symptoms were spasticity, limbs weakness, paresthesias and vesico-sphinteric dysfunction. In addition to abnormal movements, imaging demonstrated disc herniation, spinal stenosis and instability. All patients were managed surgically by performing simultaneous spinal cord decompression and fusion. Two patients benefited from preoperative botulinum toxin injections, which facilitated postoperative care and immobilization. Strict postoperative immobilization was achieved for 3 months by a Philadelphia collar or a cervico-thoracic orthosis. All patients improved functionally with a mean Japanese Orthopaedic Association score gain of 1.5 points, in spite of the permanent disabilities of the myelopathy. Complications occurred with wound infection, metal failure and relapse of cervical myelopathy at an adjacent level in one case each. All the previous authors advised against isolated laminectomy but no consensus emerged from the literature analysis. Spinal fusion is usually recommended but can be complicated by degenerative adjacent deterioration. Surgical management provides good outcomes but requires a long-term follow-up.

Background: Torsional deformity in the lower extremities is well known in children and adolescents with hemiplegic cerebral palsy, however, little or nothing is known about possible differences in torsional behavior of the upper extremities. Alterations of torsion in the lower extremities influence the gait, alterations in the rotation behavior of the upper extremities, for example after fractures, lead to limited movement of the arms or to an altered movement radius. The aim of this study was to investigate whether there is a difference in humeral torsion between affected and non-affected arms in children and adolescents with hemiplegic cerebral palsy and which factors play a decisive role., Patients and Methods: A total of 33 children and adolescents with hemiplegic cerebral palsy (mean age 9.7 years, range 3-16 years) answered a questionnaire and were examined clinically, photographically and with a new ultrasound method. Severity of cerebral palsy was classified into three levels. Ultrasound measurements of humeral torsion were compared with the results from a normal collective (n=149)., Results: In approximately half (n=16, 48.5%) of the 33 children investigated, humeral torsion was directed more internally, in 17 (51.5%) children humeral torsion was directed more externally compared to the healthy side. The difference in humeral torsion between the affected and non-affected sides was significant (p<0.05) in both groups. Differences in humeral torsion between the affected and non-affected sides in children with hemiplegic cerebral palsy were significantly increased (p<0.00001) compared to differences in humeral torsion between left and right arms in healthy children. A correlation with the level of severity of hemiplegic cerebral palsy was observed but there was no correlation with age. Children and adolescents with external torsional deformity had a smaller range of rotation, a larger muscle force deficit of the affected upper extremity and did less sport using the affected arm than children and adolescents with internal torsional deformity., Conclusion: Humeral torsion deformity in children and adolescents with hemiplegic cerebral palsy was proven to exist in addition to the already well known difference in torsion of the lower extremities. Analogous to increased activity of external or internal rotator muscles codetermined by the palsy, increased external or internal humeral torsion occurs. External torsion deformity was associated with a smaller range of motion and seemed to have a greater importance for routine daily activities (e.g. muscle force, sports ability) than internal torsion deformity.

Background: Transient periventricular echodensities (PVE) in preterm infants affect neurodevelopmental outcome., Objective: To correlate the duration and grading of PVE with neurodevelopmental outcome., Materials and Methods: A retrospective, single-centre cohort study of infants with PVE diagnosed by ultrasonography from 1995 to 2000 with blinded grading and recognition of duration of PVE., Results: A total of 72 infants (median gestational age 32 weeks, median birth weight 1,668 g) were diagnosed as having PVE. Minor neurological abnormalities were diagnosed in seven (10%), cerebral palsy in six (8%), developmental delay in seven (10%), and mild mental retardation in two (3%) of the infants. Differences regarding adverse outcome were not significant between infants with severity grade 1 compared to severity grade 2 (16% and 22%, respectively). There was a significant increase in adverse neurodevelopmental outcome with increasing duration of PVE. Of 33 infants with duration of PVE<7 days, 1 (3%) had an adverse neurodevelopmental outcome, compared to 6 (24%) of 25 infants with a duration of PVE of 7-14 days, and 6 (43%) of 14 infants with a duration of PVE>14 days (P<0.002, RR 7.920, 95% CI 1.017-61.661; P<0.001, RR 14.143, 95% CI 1.871-106.895, respectively)., Conclusion: The duration, but not the grading, of transient PVE was significantly correlated with neurodevelopmental outcome.

Background: A high prevalence of abnormal cerebral MRI findings has been reported in low-birth-weight children., Objective: To compare MRI findings in very-low-birth-weight (VLBW) and term small-for-gestational-age (SGA) children with controls in early adolescence., Materials and Methods: Cerebral MRI was used to examine 55 VLBW, 54 SGA and 66 controls at 15 years of age. The MR images were qualitatively assessed, and size of ventricles, white-matter and grey-matter abnormalities were reported., Results: The VLBW teenagers had a higher prevalence of various MRI abnormalities than SGA children and controls. Dilation of the ventricular system, especially of the occipital horns, was found in 82% of the VLBW group, in 19% of the SGA group and in 21% of controls. White-matter reduction was found in 53% of the VLBW, in 6% of the SGA and in 2% of controls. Corpus callosum thinning was found in 47% of the VLBW, in 2% of the SGA and in 6% of controls. Periventricular gliosis was found in 29% of the VLBW, in 4% of the SGA and in 8% of controls., Conclusions: Cerebral MRI pathology in white matter is a common finding in VLBW teenagers. The findings may indicate minor perinatal PVL with resulting loss of white-matter tissue and ventricular dilation.

The surgical and pharmacological treatment of cerebral palsy patients is, in many cases, complemented by orthopaedic appliances. New knowledge and materials have expanded the possibilities for orthotic treatment in the last years, but have also led to confusion on the correct technology to use in different cases. This paper presents an overview of the current orthotic methods in cerebral palsy patients. Initially, we present the different ideas and show the limitations of the treatment. Next, we consider the problem of spastic hip dislocation and the possibilities of positioning the patients. Beds and splints cut from foam allow safe positioning of severely disabled patients without the risk of pressure sores. This appliance may prevent spastic hip dislocation in the long-term. Results of a patient questionnaire are presented. The third section deals with experiences with full contact braces and differences in their construction compared to conventional corsets. Our own results from a patient questionnaire and clinical cases will be presented. The use of a full contact brace may have a positive influence on the development of the spastic scoliosis.

Background: Pediatric patients are being treated with bisphosphonates for low bone mineral density. Skeletal radiographic findings have been described with bisphosphonates given orally and intravenously., Objective: To determine and describe the radiographic findings of cyclic intravenous bisphosphonate therapy in the growing skeleton., Materials and Methods: Retrospective review of radiographs of 32 patients with osteogenesis imperfecta or cerebral palsy treated with intravenous bisphosphonates on a quarterly schedule., Results: Principal observations were metaphyseal bands and increased bone mineral density. The bands varied in spacing according to the age of the patient, rate of growth, and the location of the metaphysis. Fractures continued to be seen in patients with osteogenesis imperfecta., Conclusion: Cyclic bisphosphonate therapy results in distinctive radiographic findings in the growing skeleton.

Our biomechanical in vitro tests compared the stability of the MACS HMA (Aesculap, Tuttlingen) implants to three established systems. The MACS HMA is a modular system consisting of porous hollow titanium screws with an outer diameter of 12 mm for monocortical use. We report the preliminary results of MACS HMA used for correction of scoliosis. All other implant systems used are designed with bicortical screws; one is for application with two screws/two rods and one uses a pullout-resistant nut behind the opposite cortex to increase stability. Significantly increased motion in the craniocaudal direction was recognized for bicortical standard screws (20 +/- 17 microns) compared to MACS HMA (10 +/- 11 microns) or the dual screw system (12 +/- 6 microns). Two-tailed t-test showed significantly higher stability for the dual screw system (4.2 kN) and the system with pullout-resistant nut (4.0 kN) compared to all other systems (p < 0.025 or higher). Bicortical implants (2.1-3.2 kN) and MACS HMA (2.6 kN) did not reveal significant differences in pullout strength. All biomechanical tests and in vivo use demonstrated favorable performance of MACS HMA implants.

Background: Children with cerebral palsy (CP), often nonambulatory and/or on anticonvulsants, are at increased risk for fractures. Bone mineral density (BMD) measured by the conventional techniques of dual-energy X-ray absorptiometry (DXA) often cannot be reliably or easily measured in these patients., Objective: To find an alternative site to whole body, spine and hip that can be conveniently used to measure BMD in CP patients., Materials and Methods: Having observed that CP patients prefer to lie on their sides, we explored measuring BMD at the distal femur in the lateral projection. A total of 92 scans were performed without sedation in 34 children and adolescents with CP, aged 4-19 years. Four femoral shaft subregions were created: two trabecular and two cortical., Results: The coefficients of variation (CV %) were generally higher for opposite-side comparisons (n = 12 patients) than for same-side comparisons (n = 16 patients). For intra- and interobserver analyses, CV % were higher for cortical regions than for trabecular regions. Overall, the CV % were similar to those for hip and spine., Conclusion: This peripheral site in the femur should be considered as an alternative for patients with CP when whole-body, hip and spine DXA are not practical.

Little data exist on the oral management of food boluses in neurologically normal children or children with cerebral palsy (CP). Twenty children with spastic CP and 20 neurologically normal children (age range: 6.2-12.9 years) were monitored with ultrasound imaging of the oral cavity during liquid and solid bolus tasks. A lip-cup contact detector synchronized to ultrasound image output was used during liquid tasks. Data collected from recorded ultrasound images were used to assess durational aspects of the oral phase of swallowing in neurologically normal children and children with CP. Coordinated analysis of ultrasound images with lip-cup contact data allowed timing of intervals in the pre-oral and oral phases of swallowing during liquid feeding tasks. Children with CP required more time than neurologically normal children for collection, preparation, oral transit, and total oral swallow time for 5-ml liquid boluses. Total oral swallow time was longer for solid bolus tasks in children with CP. Oral transit time for solid boluses was significantly longer than for liquid boluses in neurologically normal children and children with CP.

Many children with cerebral palsy (CP) suffer from feeding disorders. Twenty children with spastic CP and 20 neurologically normal children (age range 6.2-12.9 years) were monitored with ultrasound imaging of the oral cavity synchronized with surface electromyographic (EMG) recordings of masseter and infrahyoid muscles and respiratory inductance plethysmograph (RIP) recordings during feeding tasks. A lip-cup contact detector signaled contact of the drinking cup on the lip during liquid tasks. Children with CP required more time than normals for collection and organization of 5 ml and 75 ml liquid boluses for swallowing. The ventilatory preparation phase, recovery to baseline resting ventilatory pattern after swallowing, and total time for task completion were longer in children with CP for 5-ml and 75-ml tasks. The interval from lip-cup contact until alteration of ventilation from baseline resting ventilatory pattern was longer for children with CP during 75-ml tasks but not for 5-ml tasks. The interval from completion of the task-related cookie swallow until initiation of the next swallow was longer in children with CP than in normal children. These data provide evidence that children with CP manage solid boluses more easily than liquid boluses and small liquid boluses more easily than large liquid boluses. This investigation statistically confirms empirically based recommendations that children with CP be allowed more time to complete feeding tasks and consume small volume drinks rather than large volume drinks.

Parameters of linear measurement were compared with actual brain volume to assess the significance of linear measurements as indices of atrophy in 31 neurologically normal children and 22 neurologically abnormal children. Brain volume was established by means of an image-analyzing system using contiguous CT scans. The parameters or indices estimated were: (1) the maximum transverse width of both hemispheres, (2) the maximum longitudinal length of both hemispheres, (3) the maximum frontal subarachnoid space, (4) the maximum width of the interhemispheric fissure, (5) the maximum width of the Sylvian fissure, (6) Evans' ratio, (7) the maximum width of the third ventricle, (8) the cella media index, (9) the maximum width of the fourth ventricle. In neurologically normal children, the maximum transverse width of both hemispheres, the maximum longitudinal length of both hemispheres, the maximum width of the interhemispheric fissure and the maximum width of the Sylvian fissure correlated significantly with the combined volume (CV) of both hemispheres and basal ganglia. In particular, the maximum transverse width of both hemispheres and the maximum longitudinal length of both hemispheres had a high correlation. In neurologically abnormal children the maximum transverse width of both hemispheres and the maximum width of the interhemispheric fissure were significantly correlated with the CV of both hemispheres and basal ganglia.

Classification of cerebral palsy according to the topographical distribution of clinical phenomena permits determination of a prognosis of the natural history of CP and the probability of hip problems to some extent. In 55 patients with CP, 101 muscle release operations were performed between 1971 and 1988. Preoperatively, the diagnosis was established by the neuropediatrician, function was evaluated according to the Rancho-los-amigos system, and the X-rays of the hip were assessed according to Reimers. For the postoperative evaluation patients were grouped according to neurologic diagnosis: hemiplegia (4), diplegia (19), total body involvement (31). Patients with hemiplegia had no functional or radiological changes as a result of the operation. In diplegia functional deterioration was seen in 4 cases (21%); in 3 cases (16%) this meant loss of the ability to walk. The migration percentage was improved from 48% to 39% on average. In 19 cerebral palsy patients with total body involvement surgery was considered to be indicated on the basis of a suspected dislocation of the hip. No functional changes occurred as a result of surgery. Hip dislocation was successfully prevented in 90% of the cases. The migration percentage was improved from 73% to 33%. In another 12 patients with total body involvement, adductor and iliopsoas release was performed to allow better hygiene and care and for pain relief. These goals were achieved; neither the Rancho-Los-Amigos function classification system nor X-rays were used to evaluate the results.

The incidence of scoliosis in cerebral palsy is related to the severity of the neurological involvement, being most prevalent in patients with spastic quadriplegia. Neuromuscular spinal deformity, when present, may progress after cessation of skeletal growth, and the success of orthotic treatment for scoliosis is unpredictable. Hip pathology is directly related to pelvic obliquity but has no causal relationship to the development of scoliosis. Adequate preoperative nutritional assessment is vital to reduce perioperative complications, and segmental spinal fixation is the instrumentation of choice. Anterior arthrodesis is indicated for rigid deformities and for those thoracolumbar and lumbar curves extending into the pelvis with pelvic obliquity and spinal decompensation.

Three children with clinical evidence of cerebral palsy (CP) and normal cerebral computed tomography (CT) scans were evaluated by magnetic resonance imaging (MRI) to identify CT-undetectable white matter lesions in the watershed zones of arterial territories. The two patients with spastic diplegia showed bilateral lesions either in the subcortical regions or in the occipital periventricular regions. The patient with congenital hemiplegia exhibited unilateral lesions in the periventricular region. We conclude that MRI is more informative than CT for the evaluation of patients with CP.

This study has analysed the results of CT scans of the brains in children with congenital hemiplegia. The material consists of 111 out of a total of 151 children with this cerebral palsy syndrome in a population-based series. We have classified the morphological findings in five groups. The groups are designed to reflect the phase of maturation of the brain when the insult happened. The groups are: 1. Maldevelopment, 2. Periventricular atrophy, 3. Cortical-/subcortical atrophy, 4. Miscellaneous, 5. Normal. In contrast to previous reports we found a high proportion (17%) with maldevelopment. However the dominating morphological pattern was periventricular atrophy, consistent with a hypoxic-ischemic insult to the immature brain, seen in 42%. Cortical and/or subcortical atrophy was found in 12%. Three children (3%) presented with morphological patterns not possible to classify. The group with no pathology according to CT was 26%.

Eighty-three children with spastic cerebral palsy (CP) were examined with cranial CT. In 56 cases the CT findings were abnormal. The most frequent abnormality was atrophy, present in 44 patients. The frequency of pathologic CT increased with severity of the CP. Patients with CP of postnatal aetiology more often had abnormal CT than patients with other known causes. Pathologic CT findings were seen more often in patients with seizures than in patients without. Infarctions and hemiatrophy were much more frequent in patients with hemiplegia than in patients with other types of spastic CP. A special kind of central atrophy, called isolated atrophy around in the cella media, is described. This condition was seen in 20% of cases, most often in hemi- and paraplegic patients. Early infarctions in the border areas between the vascular territories of the internal carotid and the posterior cerebral artery may be the reason for this kind of atrophy.

The CT findings in 120 cerebral palsied children are analysed. The 72.5% positive findings are correlated with the clinical types, as well as the aetiological basis for the cerebral palsy. The spastic type, 83.3% of the total number of children, had the highest positive findings. The yield was increased in children with seizures (91.3%) and those in the postnatal group (90%), as well as those with birth trauma and neonatal asphyxia (94%). The findings were those of atrophy in 30.8%, hydrocephalus, in 10%, infarct in 11.6%, porencephaly in 8.3% and others. The atropic changes and their patterns are explained. Treatable lesions, such as tumour, hydrocephalus, subdural haematoma, porencephaly and hygroma were identified in 22.5% of cases. It is concluded that CT scan is definitely efficacious in the management of cerebral palsied children.

Hypoxic-ischaemic brain lesions may be detected as low density (LD) areas by means of computerized tomography (CT), but the clinical significance of such LD areas has been controversial. Since timing might be a critical factor, we studied the temporal evolution of LD areas in 9 asphyxiated term babies who had had two or more CT, and compared the changes to the neurodevelopmental outcome. Scans were classified according to the elapsed time after asphyxia as early (day 1-7, n = 6), intermediate (week 2-4, n = 7; week 4-7, n = 3) and late CT (3 months or more, n = 7). In early scans, no, or only ill defined, LD areas were seen in the periventricular region. In intermediate CT's, LD-zones were further diminished in those babies who later were normal. Sharply accentuated LD areas, however, appeared in those who later suffered from neurodevelopmental disorders. These LD areas, probably representing hypoxic-ischaemic lesions, were located periventricularly, extending into the subcortical white matter and the cortex, and usually involved both hemispheres symmetrically. They began to disappear at 4 to 7 weeks in some regions, possibly because of glial proliferation. LD persisting more than 4-7 weeks tended to transform into cyst-like lesions, or marked atrophy. We conclude (1) that hypoxic-ischaemic lesions appear as zones of low density on CT scans performed after the first week and (2) that the extent of such lesions can best be assessed between 9 to 23 days after asphyxia.
