Purpose: There is growing interest in the measurement of plasma levels of arginine vasopressin (AVP) for the assessment of mild dehydration. However, the principles of biological variation have not been applied to the study of AVP and understanding biological variation of AVP may provide insights regarding measurement thresholds. The purpose of this investigation was to determine the biological variation of AVP in healthy euhydrated individuals to understand the potential for establishing both static and/or change thresholds of importance., Methods: We studied 29 healthy volunteers (24 men and 5 women) while controlling for hydration and pre-analytical factors. All subjects completed between 2-8 trials where biological variation was determined using widely published methods. We determined the intraindividual, interindividual, and analytical coefficients of variation (CV I , CV G , and CV A , respectively) and subsequently the index of individuality and heterogeneity (II and IH, respectively)., Results: AVP did not reach the IH threshold required to be considered useful in the dynamic assessment of physiological deviations from normal. AVP levels approached the II threshold required to be considered useful in the static assessment of physiological deviations from normal., Conclusions: This analysis demonstrates that AVP assessment is unlikely to yield useful information about hydration status.

Purpose: Resting measures of ventilation and gas exchange are impacted by a variety of physiological stressors, such as those resulting from a research intervention or an extreme environment. However, the biological variation of these parameters, an important statistical consideration for identifying a meaningful physiological change, has not been quantified., Methods: We performed a retrospective analysis of 21 studies completed by the U.S. Army Research Institute of Environmental Medicine (USARIEM) from 1985 to present, totaling 411 healthy volunteers. First, we determined the intraindividual, interindividual, and analytic coefficients of variation (CV I , CV G , and CV A , respectively) and subsequently the index of individuality and heterogeneity (II and IH, respectively). Second, when deemed appropriate via these outcomes, we defined the accompanying static and dynamic thresholds, beyond which a significant deviation from normal is indicated., Results: End-tidal partial pressure of oxygen (P ET O 2 ) and the respiratory exchange ratio (RER) approached the II threshold required to be considered useful in the static assessment of physiological deviations from normal. P ET O 2 and peripheral oxygen saturation (SpO 2 ) approached the IH threshold required to be considered useful in the dynamic assessment of physiological deviations from normal., Conclusions: This analysis identifies RER and P ET O 2 as parameters that might be most useful when aiming to identify a meaningful ventilatory change following a research intervention or stressor. Alternatively, other parameters of ventilation and gas exchange, such as P ET CO 2 and V E , may be less useful for observing an anticipated physiological change.

Introduction: When substantial solute losses accompany body water an isotonic hypovolemia (extracellular dehydration) results. The potential for using blood or urine to assess extracellular dehydration is generally poor, but saliva is not a simple ultra-filtrate of plasma and the autonomic regulation of salivary gland function suggests the possibility that saliva osmolality (Sosm) may afford detection of extracellular dehydration via the influence of volume-mediated factors., Purpose: This study aimed to evaluate the assessment of extracellular dehydration using Sosm. In addition, two common saliva collection methods and their effects on Sosm were compared., Methods: Blood, urine, and saliva samples were collected in 24 healthy volunteers during paired euhydration and dehydration trials. Furosemide administration and 12 h fluid restriction were used to produce extracellular dehydration. Expectoration and salivette collection methods were compared in a separate group of eight euhydrated volunteers. All comparisons were made using paired t-tests. The diagnostic potential of body fluids was additionally evaluated., Results: Dehydration (3.1 ± 0.5% loss of body mass) decreased PV (-0.49 ± 0.12 L; -15.12 ± 3.94% change), but Sosm changes were marginal (<10 mmol/kg) and weakly correlated with changes in absolute or relative PV losses. Overall diagnostic accuracy was poor (AUC = 0.77-0.78) for all body fluids evaluated. Strong agreement was observed between Sosm methods (Expectoration: 61 ± 10 mmol/kg, Salivette: 61 ± 8 mmol/kg, p > 0.05)., Conclusions: Extracelluar dehydration was not detectable using plasma, urine, or saliva measures. Salivette and expectoration sampling methods produced similar, consistent results for Sosm, suggesting no methodological influence on Sosm.

Equivocal findings have been reported in the few studies that examined the impact of ambient temperature (T a) and hypohydration on cognition and dynamic balance. The purpose of this study was to determine the impact of acute exposure to a range of ambient temperatures (T(a) 10-40 °C) in euhydration (EUH) and hypohydration (HYP) states on cognition, mood and dynamic balance. Thirty-two men (age 22 ± 4 years, height 1.80 ± 0.05 m, body mass 85.4 ± 10.8 kg) were grouped into four matched cohorts (n = 8), and tested in one of the four T(a) (10, 20, 30, 40 °C) when EUH and HYP (-4 % body mass via exercise-heat exposure). Cognition was assessed using psychomotor vigilance, 4-choice reaction time, matching to sample, and grammatical reasoning. Mood was evaluated by profile of mood states and dynamic postural balance was tested using a Biodex Balance System. Thermal sensation (TS), core (T core) and skin temperature (T(sk)) were obtained throughout testing. Volunteers lost -4.1 ± 0.4 % body mass during HYP. T sk and TS increased with increasing T(a), with no effect of hydration. Cognitive performance was not altered by HYP or thermal stress. Total mood disturbance (TMD), fatigue, confusion, anger, and depression increased during HYP at all T(a). Dynamic balance was unaffected by HYP, but 10 °C exposure impaired balance compared to all other T(a). Despite an increase in TMD during HYP, cognitive function was maintained in all testing environments, demonstrating cognitive resiliency in response to body fluid deficits. Dynamic postural stability at 10 °C appeared to be hampered by low-grade shivering, but was otherwise maintained during HYP and thermal stress.

The cardiovascular response to standing (sit-to-stand change in heart rate; SSΔHR) is commonly employed as a screening tool to detect hypohydration (body water deficit). No study has systematically evaluated SSΔHR cut points using different magnitudes or different types of controlled hypohydration. The objective of this study was to determine the diagnostic accuracy of the often proposed 20 b/min SSΔHR cut point using both hypertonic and isotonic models of hypohydration. Thirteen healthy young adults (8M, 5F) underwent three bouts of controlled hypohydration. The first bout used sweating to elicit large losses of body water (mass) (>3 % sweat). The second two bouts were matched to elicit 3 % body mass losses (3 % diuretic; 3 % sweat). A euhydration control trial (EUH) was paired with each hypohydration trial for a total of six trials. Heart rate was assessed after 3-min sitting and after 1-min standing during all trials. SSΔHR was compared among trials, and receiver operator characteristic curve analysis was used to determine diagnostic accuracy of the 20 b/min SSΔHR cut point. Volunteers lost 4.5 ± 1.1, 3.0 ± 0.6, and 3.2 ± 0.6 % body mass during >3 % sweat, 3 % diuretic, and 3 % sweat trials, respectively. SSΔHR (b/min) was 9 ± 8 (EUH), 20 ± 12 (>3 % sweat; P < 0.05 vs. EUH), 17 ± 7 (3 % diuretic; P < 0.05 vs. EUH), and 13 ± 11 (3 % sweat). The 20 beats/min cut point had high specificity (90 %) but low sensitivity (44 %) and overall diagnostic accuracy of 67 %. SSΔHR increased significantly in response to severe hypertonic hypohydration and moderate isotonic hypohydration, but not moderate hypertonic hypohydration. However, the 20 beats/min cut point afforded only marginal diagnostic accuracy.

Insect repellents (e.g. N,N-diethyl-m-toluamide or DEET) applied to the skin can potentially interfere with sweat production and evaporation, thus increasing physiological strain during exercise-heat stress. The purpose was to determine the impact of 33% DEET lotion on sweating responses, whole body thermoregulation and thermal sensation during walking exercise in the heat. Nine volunteers (2 females, 7 males; 22.1 ± 4.9 years; 176.4 ± 10.0 cm; 79.9 ± 12.9 kg) completed 5 days of heat acclimation (45°C, 20% rh; 545 watts; 100 min/day) and performed three trials: control (CON); DEET applied to forearm (DEET(LOC), 12 cm(2)); and DEET applied to ~13% body surface area (DEET(WB),). Trials consisted of 30 min walking (645 watts) in 40°C, 20% rh environment. Local sweat rate (SR), onset and skin wettedness were measured in DEET(LOC), and heart rate (HR), rectal temperature (T (re)), skin temperature (T (sk)), RPE, and thermal sensations (TS) were measured during DEET(WB). No differences (p > 0.05) were observed between DEET(LOC) versus CON, respectively, for steady state SR (1.89 ± 0.44 vs. 2.09 ± 0.84 mg/cm(2)/min), SR area under the curve (46.9 ± 11.7 vs. 55.0 ± 20.8 mg/cm(2)), sweating onset, or skin wettedness. There were no differences (p > 0.05) in HR, T (re), T (sk), Physiological Strain Index, RPE or TS between DEET(WB) versus CON. DEET did not impact measures of local forearm sweating and when applied according to military doctrine, did not adversely impact physiological responses during exercise-heat stress. DEET can be safely worn during military, occupational and recreational activities in hot, insect infested environments.

A methodological discrepancy exists in the hydration assessment literature regarding the establishment of euhydration, as some investigations utilize a pre-hydration technique, while others do not (overnight fluid/food fast). However, the degree that plasma osmolality (P (osm)) dilutes when using the pre-hydration method and how body mass/composition might influence the results is not known. Thirty subjects (22 M, 8 F; 20 ± 2 years (mean ± SD); 1.8 ± 0.1 m; 75.8 ± 13.5 kg) had P (osm) measured after an 8-h food and fluid fast (overnight fast) and 90 min after a 500-mL (4-9 mL/kg) water bolus (pre-hydration). From pre- to post-bolus, participants' P (osm) declined from 297 ± 3.5 to 295 ± 3.8 mmol/kg (p < 0.05; ∆ -1.7 ± 3.5 mmol/kg). One-third of the sample diluted to more than -3 mmol/kg. The effect of body mass on P (osm) dilution was investigated by comparing dilution in the ten lightest (62.8 ± 3.4 kg) and heaviest (92.0 ± 9.8 kg) participants; however, the change between the light (∆ -1.9 ± 3.8 mmol/kg) versus heavy groups (∆ -1.1 ± 3.0 mmol/kg) was not different (p > 0.05). The correlation between body mass or total body water and change in P (osm) was weak (p > 0.05), as was the correlation between relative fluid intake based on mass and change in P (osm) (p > 0.05). The two methodologies appear to produce similar P (osm) values when measured in most individuals. However, the potential for significant dilution (>3 mmol/kg) should be considered when choosing the pre-hydration methodology.

This study examined vertical jump performance using a force platform and weighted vest to determine why hypohydration (approximately 4% body mass) does not improve jump height. Measures of functional performance from a force platform were determined for 15 healthy and active males when euhydrated (EUH), hypohydrated (HYP) and hypohydrated while wearing a weighted vest (HYP(v)) adjusted to precisely match water mass losses. HYP produced a significant loss of body mass [-3.2 +/- 0.5 kg (-3.8 +/- 0.6%); P < 0.05], but body mass in HYP(v) was not different from EUH. There were no differences in absolute or relative peak force or power among trials. Jump height was not different between EUH (0.380 +/- 0.048 m) and HYP (0.384 +/- 0.050 m), but was 4% lower (P < 0.05) in HYP(v) (0.365 +/- 0.52 m) than EUH due to a lower jump velocity between HYP(v) and EUH only (P < 0.05). However, vertical ground reaction impulse (VGRI) was reduced in both HYP and HYP(v) (2-3%) compared with EUH (P < 0.05). In conclusion, this study demonstrates the failure to improve jump height when HYP can be explained by offsetting reductions in both VGRI and body mass.

The effects of elevating plasma osmolality (P (osm)) on thirst ratings was studied in eight dehydrated males during exposure to 4 degrees C. On two occasions, subjects were dehydrated (DH; 3-4% body mass) via 90 min exercise-heat exposure and overnight fluid restriction (day 1). On a third occasion, subjects were exposed to heat but were given fluid (EU). On day 2, subjects consumed NaCl (NaCl; 0.1 g NaCl kg(-1) body mass in 500 ml H(2)O; DH only) or Placebo (P; 500 ml H(2)O; DH and EU). Subjects stood for 30 min at 24 degrees C and for 45 min at 4 degrees C (75 min post-dose). P (osm) was elevated (P < 0.05) 30 and 75 min after NaCl administration in DH + NaCl versus DH + P and EU + P treatments. Thirst ratings remained elevated (P < 0.05) in the DH + NaCl treatment 30 min after dosing and 45 min at 4 degrees C versus DH + P and EU + P. Attenuation of thirst when dehydrated in the cold can be over-ridden by increasing P (osm).

This study determined whether a torso-vest forced ambient air body ventilation system (BVS) reduced physiological strain during exercise-heat stress. Seven heat-acclimated volunteers attempted nine, 2-h treadmill walks at 200 W m(-2) in three environments, -40 degrees C, 20% rh (HD), 35 degrees C, 75% rh (HW), and 30 degrees C, 50% rh, (WW) wearing the Army Combat Uniform, interceptor body armor (IBA) and Kevlar helmet. Three trials in each environment were BVS turned on (BVS(On)), BVS turned off (BVS(Off)), and no BVS (IBA). In HD, BVS(On) significantly lowered core temperature (T (re)), heart rate (HR), mean skin temperature (T (sk)), mean torso skin temperature (T (torso)), thermal sensation (TS), heat storage (S), and physiological strain index (PSI), versus BVS(Off) and IBA (P < 0.05). For HW (n = 6), analyses were possible only through 60 min. Exercise tolerance time (min) during HW was significantly longer for BVS(On) (116 +/- 10 min) versus BVS(Off) (95 +/- 22 min) and IBA (96 +/- 18 min) (P < 0.05). During HW, BVS(On) lowered HR at 60 min versus IBA, T (sk) from 30 to 60 min versus BVS(Off) and IBA, and PSI from 45 to 60 min versus BVS(Off) and at 60 min versus IBA (P < 0.05). BVS(On) changes in T (re) and HR were lower in HD and HW. During WW, BVS(On) significantly lowered HR, T (sk), and T (torso) versus BVS(Off) and IBA (P < 0.05) during late exercise. Sweating rates were significantly lower for BVS(On) versus BVS(Off) and IBA in both HD and WW (P < 0.05), but not HW. These results indicate that BVS(On) reduces physiological strain in all three environments by a similar amount; however, in hot-dry conditions the BVS(Off) increases physiological strain.

Protective vests worn by global security personnel, and weighted vests worn by athletes, may increase physiological strain due to added load, increased clothing insulation and vapor resistance. The impact of protective vest clothing properties on physiological strain, and the potential of a spacer garment to reduce physiological strain, was examined. Eleven men performed 3 trials of intermittent treadmill walking over 4 h in a hot, dry environment (35 degrees C, 30% rh). Volunteers wore the US Army battledress uniform (trial B), B + protective vest (trial P), and B + P + spacer garment (trial S). Biophysical clothing properties were determined and found similar to many law enforcement, industry, and sports ensembles. Physiological measurements included core (T (c)), mean skin (T (sk)) and chest (T (chest)) temperatures, heart rate (HR), and sweating rate (SR). The independent impact of clothing was determined by equating metabolic rate in all trials. In trial P, HR was +7 b/min higher after 1 h of exercise and +19 b/min by the fourth hour compared to B (P < 0.05). T (c) (+0.30 degrees C), T (sk) (+1.0 degrees C) and Physiological Strain Index were all higher in P than B (P < 0.05). S did not abate these effects except to reduce T (sk) (P > S) via a lower T (chest) (-0.40 degrees C) (P < 0.05). SR was higher (P < 0.05) in P and S versus B, but the magnitude of differences was small. A protective vest increases physiological strain independent of added load, while a spacer garment does not alter this outcome.

Sweat prediction equations are often used outside their boundaries to estimate fluid requirements and generate guidance. The limitations associated with these generalized predictions have not been characterized. The purposes of this study were to: (1) evaluate the accuracy of a widely used sweat prediction equation (SHAP) when widening it's boundaries to include cooler environments (2 h) and very prolonged exercise (8 h), (2) determine the independent impact of holding skin temperature constant (SHAP36), and (3) describe how adjustments for non-sweat losses (NSL) and clothing saturation dynamics affect prediction accuracy. Water balance was measured in 39 volunteers during 15 trials that included intermittent treadmill walking for 2 h (300-600 W, 15-30 degrees C; n=21) or 8 h (300-420 W, 20-40 degrees C; n=18). Equation accuracy was assessed by comparing actual and predicted sweating rates (211 observations) using least-squares regression. Mean and 95% confidence intervals for group differences were compared against a zone of indifference (+/-0.125 l/h). Sweating rate variance accounted for by SHAP and SHAP36 was always high (r2>0.70), while the standard error of the estimate was small and uniform around the line of best fit. SHAP errors were >0.125 l/h during 2 and 8 h of exercise. SHAP36 errors were <0.125 l/h for 2 h conditions but were higher at 8 h in three of the six warmest trials. Adjustments for NSL and clothing saturation dynamics help explain SHAP errors at 2 and 8 h, respectively. These results provide a basis for future development of accurate algorithms with broader utility.
