Background: The EQ-5D is a validated and widely used generic measure of health-related quality of life (HRQoL) in both healthy individuals and those with various medical conditions. The objective of this study was to test whether EQ-5D-5L is reliable and valid for use among school sample adolescents and those with major mental health disorders in Ethiopia., Methods: Participants were recruited from ten sub-districts comprising the Butajira Rural Health Programme (BRHP) and Butajira major mental health disorders center. Data were collected using an Amharic (Ethiopia) EQ-5D-5L self-complete-paper and the questionnaire was administered 10 days after the first completion for test-retest procedures. Two-way mixed-effects models absolute intraclass correlation coefficient (ICC) was used to test reliability of the instrument while Kruskal-Wallis rank test with pairwise comparison was used to assess the known group validity of the instrument., Results: There were 501 (201 school sample and 300 adolescents with major mental health disorders) participants recruited and 497 were included in the sample for analysis. The ICC was high (ICC > 0.7, p < 0.001) for all EQ-5D-5L dimensions, EQ-5D-5L utility index and EQ-VAS scores. The findings revealed that the Amharic EQ-5D-5L has significant known group validity as shown by the difference in scores among various disease group (depression, schizophrenia, and bipolar) and experience of chronic illness., Conclusions: The results shows that the Amharic EQ-5D-5L is reliable and valid instrument for the measurement of HRQoL among adolescent populations in Ethiopia., (© 2022. Purpose: Spectral-domain optical coherence tomographies (OCTs) from different companies do not give identical retinal thicknesses. The purpose of this study was to evaluate if differences in thickness when using a spectral domain Cirrus OCT or a Heidelberg Spectralis are due to hardware differences, or if they are caused by the segmentation algorithms., Methods: Thirty-seven healthy eyes were examined within the same session with a Cirrus OCT and a Spectralis OCT, the latter using averaged B-scans. Scans from similar positions and passing the fovea were analyzed by custom-made software. Thickness was analyzed at the fovea, the central 1-mm line and the 6-mm line., Results: When Cirrus and Spectralis scans were analyzed with the same software, the retinal thickness at the foveal center was 225.92 μm (SD 17.0) using the Cirrus and 228.70 μm (SD 18.4) using the Spectralis; the difference of 2.78 μm was not significant (p = 0.055). For the central 1 mm, the difference was 1.78 μm (p = 0.0414), and for all points out to 6 mm, the Spectralis retinal thickness was also significantly larger than the Cirrus thickness (p = 0.0052), though the mean difference was only 1.85 μm. Also for the RPE&#95;OScomplex, Spectralis measured a greater thickness than did Cirrus, with a mean of 3.32 μm (p < 0.0001) for all points., Conclusion: The retinal thicknesses from the Cirrus and from the Spectralis differed by 14 μm with the standard software of the instruments, and by less than 3 μm when analyzed with the same custom-made software, indicating that the major differences between the two SD-OCT systems are due to differences in their built-in software algorithms.

Background: Idiopathic intracranial hypertension (IIH) is a condition of raised intracranial pressure (ICP) in the absence of space-occupying lesions or other known etiology. It primarily affects young obese females, and potentially causes permanent visual loss due to papilledema and secondary optic atrophy. The aim of this study was to evaluate the diagnostic value of optical coherence tomography (OCT) as a marker for CSF opening pressure in patients with idiopathic intracranial hypertension (IIH)., Methods: We conducted a case-control study of 20 newly diagnosed, 21 long-term IIH patients, and 20 healthy controls. Investigations included measurement of peripapillary retinal nerve fiber layer thickness (RNFLT) and total retinal thickness (RT), automated visual field testing, and measurement of CSF opening pressure. An OCT elevation diagram was developed as a new diagnostic tool. The diagnostic ability of OCT as a marker of increased ICP (> 25 cmH(2)O) was investigated using multiple regression and receiver operating characteristic (ROC) curves., Results: OCT elevation diagrams showed that in 60 % of patients newly diagnosed with IIH and in 10 % of patients with long-term IIH, 50 % or more of the OCT scans (RT and RNFLT) were above normal. The percentage of abnormal OCT scans was significantly associated with increased ICP (p < 0.0001). Estimated areas under the ROC curves increased from 77.1 to 86.9 by including OCT in multiple regressions. Autoperimetry pattern standard deviation was significantly increased (p = 0.0005) and mean deviation was significantly decreased (p = 0.0005) in IIH patients as compared to healthy controls., Conclusions: Increased peripapillary retinal thickness measured by OCT is associated with increased ICP in newly diagnosed IIH patients. OCT may thus serve as a valuable supplement to subjective assessment of papilledema in patients suspected of having IIH. In long-term IIH patients who have previously been treated, OCT appears to be of limited value in predicting ICP.

Idiopathic intracranial hypertension (IIH) primarily affects young obese females, and potentially causes visual loss and severe headache. The aim of this experiment is to examine relapse rate and long-term outcome in IIH patients. The methods involved in this experiment include a prospective controlled study of 18 newly diagnosed IIH patients followed for a mean observation period of 21.1 (±8.0) months. Treatment regime included diuretics, dietary recommendations and check-up visits at a dietician. Baseline and follow-up included neurological examination, detailed headache history and comprehensive neuro-ophthalmological examination, including fundus photography, Humphrey visual fields, and measurement of the retinal thickness (RT) and retinal nerve fiber layers (RNFL) by optical coherence tomography (OCT). Relapse was defined as recurrence of either: (1) papilledema or (2) symptoms and demonstrated raised ICP. The result of this experiment is that relapse was found in 28%. Visual function improved from baseline to follow-up and was generally favorable. In patients without relapse of papilledema RT and RNFL were significantly thinner than in healthy controls (p = 0.003 and 0.02), although atrophy was clinically detectable in only one patient. Headache was still present in 67% of the patients at follow-up. Headache was heterogenic and unrelated to relapse. After an initial reduction, weight increased again in the relapse group compared to reduced weight in the non-relapse group (p = 0.013). Thus, the conclusions drawn are that headache was persistent, difficult to classify, and equally represented in relapse and non-relapse patients. Headache was thus a poor marker of active disease. Relapse rate was high and clinically undetectable optic disc atrophy was discovered in apparently well treated IIH patients.

The functional consequence of long-term retinal detachment in the porcine model is examined by multifocal electroretinography (mfERG). Retinal detachment (RD) in humans leaves permanent visual impairment, despite anatomical successful reattachment surgery. To improve treatment, adjuvant pharmaceutical therapy is needed, and can only be tested in a suitable animal model. The porcine model is promising and the mfERG is well validated in this model. RD was induced in 18 pigs by vitrectomy and healon injection of various concentrations. Preoperatively and 6 weeks postoperatively eight animals were examined by mfERG. The major component P1 was analyzed statistically. Indirect ophthalmoscopy and bilateral color fundus photography (FP) were performed. Selected animals underwent high-resolution optical coherence tomography (OCT). Examination by ophthalmoscopy and FP showed that the RDs remained detached for the 6 weeks of follow-up. The P1 amplitude of the mfERG did not differ significantly between the detached areas, the surrounding attached areas, and the healthy eye (p = 0.25). Similarly, P1 implicit time did not differ between the areas (p = 0.85). The lack of functional consequences of long-term RD makes the porcine model unsuitable for examining adjuvant pharmaceutical RD treatment. Future studies should focus on foveated primates.

Optical coherence tomography (OCT) is a noninvasive imaging technique that provides real-time two- and three-dimensional images of scattering samples with micrometer resolution. By mapping the local reflectivity, OCT visualizes the morphology of the sample. In addition, functional properties such as birefringence, motion, or the distributions of certain substances can be detected with high spatial resolution. Its main field of application is biomedical imaging and diagnostics. In ophthalmology, OCT is accepted as a clinical standard for diagnosing and monitoring the treatment of a number of retinal diseases, and OCT is becoming an important instrument for clinical cardiology. New applications are emerging in various medical fields, such as early-stage cancer detection, surgical guidance, and the early diagnosis of musculoskeletal diseases. OCT has also proven its value as a tool for developmental biology. The number of companies involved in manufacturing OCT systems has increased substantially during the last few years (especially due to its success in opthalmology), and this technology can be expected to continue to spread into various fields of application.

Background: Optical coherence tomography (OCT) enables quantification of retinal layer thicknesses. Studies evaluating the ability of OCT to identify and quantify papilledema are few. Therefore, the aim was to evaluate the applicability of peripapillary OCT evaluation in an unselected, consecutive series of patients with confirmed idiopathic intracranial hypertension (IIH)., Methods: A case-control study of 20 patients with newly diagnosed IIH and 20 healthy subjects. Subjects were evaluated by means of direct ophthalmoscopy, masked evaluations of fundus photography, peripapillary retinal nerve fiber layer thickness (RNFLT), and peripapillary average retinal thickness (PART) measurements (Stratus OCT-3, fast RNFL 3.4 protocol)., Results: OCT evaluation of PART revealed the highest number of patients (n = 20) with optic disc abnormalities in either eye (90%), compared with 85%, 80% and 70% for RNFLT measurements, direct ophthalmoscopy, and fundus photography respectively. There was a linear association between means and differences of OCT data (RNFLT, PART) with a slope of 0.54 (SE 0.025), p < 0.001., Conclusion: Peripapillary OCT is a promising objective examination modality for optic disc evaluation in IIH, and may improve the identification of subtle disc swellings. Underestimation of RNFLT for increasing severities of papilledemas is suspected, making PART a more reliable parameter compared to RNFLT. Future larger validation studies are needed.

Idiopathic intracranial hypertension (IIH) is a disorder of raised intracranial pressure (ICP) in the absence of identifiable pathology. The purpose of this study was to evaluate the clinical presentation and monitor a 3-month course using frequent optical coherence tomography (OCT) evaluations, visual field testings and lumbar opening pressure measurements. A longitudinal study of 17 patients with newly diagnosed IIH and 20 healthy overweight controls were included in the study. Peripapillary retinal nerve fiber layer thickness (RNFLT) and retinal thickness (RT) measurements (Stratus OCT-3, fast RNFL 3.4 protocol), and Humphrey visual field testing were evaluated at regular intervals. Repeat lumbar puncture was performed at final visit (n = 13). The diagnostic delay was 3 months and initial symptoms were headache (94%), visual blurring (82%) and pulsatile tinnitus (65%). Complete clinical remission was achieved in 65%, partial in 29% and unchanged symptoms in 6%. Total average RNFLT and RT decreased significantly during the follow-up period (p < 0.0001 and p < 0.0001, respectively). Changes in RNFLT and RT correlated with improvements in visual field mean deviation (MD) (RNFLT: p = 0.006; RT: p = 0.03) and pattern standard deviation (PSD) (RNFLT: p = 0.002; RT: p = 0.003). In patients with weight-loss >3.5% of BMI, ICP decreased significantly (p = 0.0003). In patients with weight-loss <3.5% of BMI, changes in ICP were insignificant (p = 0.6). OCT combined with visual field testing may be a valuable objective tool to monitor IIH patients and the short term IIH outcome is positive. Weight-loss is the main predictor of a favorable outcome with respect to CSF pressure.

Aim: To study the long-term visual result of photocoagulation of clinically significant macular edema (CSME) in diabetic patients in relation to baseline retinal vascular leakage assessed by vitreous fluorometry., Methods: The study enrolled 36 eyes in 27 diabetic patients with CSME at baseline, all subsequently treated by photocoagulation. The diagnosis of CSME and treatment procedure followed the ETDRS criteria. The leakage through the retinal vessels was calculated as the blood-retinal barrier permeability. Five years after initial photocoagulation the patients were reexamined., Results: The mean visual acuity decreased with 16 letters at follow-up, and in 15 eyes visual loss was substantial, with a decrease of 15 letters or more, corresponding to at least 3 lines. The baseline permeability was significantly higher in eyes with substantial visual loss than in eyes with more stable vision (14.2 nm/sec and 6.3 nm/sec respectively; p = 0.006), the corresponding odds ratio was 16.1 (95% CI: 1.8-146; p = 0.014). The level of retinopathy was significantly correlated to visual loss (Fisher's exact test < 0.02)., Conclusion: Visual acuity decreased at follow-up, and a substantial visual loss of 3 or more lines in CSME was associated with higher retinal vascular leakage at baseline with an odds ratio of 16.1. For this relatively small sample, neither diabetes duration, age or HbA(1c) reached statistical significance, indicating that blood-retinal barrier permability is a strong predictor of the visual outcome. The results support the investigation of triamcinolone or VEGF inhibition in patients with severe leakage, and may help to identify patients in need for intravitreal injection.

We assessed the cost-effectiveness and cost-utility of treating influenza with neuraminidase inhibitors (oseltamivir and zanamivir) from a health care payer's and societal perspective in the United Kingdom. A simulation model was developed to predict morbidity and mortality due to influenza and its specified complications, comparing neuraminidase inhibitors with usual care in an otherwise healthy adult population. Robustness of the results was tested by one-way and multiway as well as probabilistic sensitivity analyses. Treatment with either neuraminidase inhibitor results in reduced morbidity and faster return to normal activities. However, oseltamivir dominates zanamivir in cost-utility analysis due to its lower costs. Comparing oseltamivir with usual care, the costs are pound14.36 per day of normal activity gained and pound5,600 per quality-adjusted life-year gained from the healthcare payer perspective. Oseltamivir dominates usual care from the societal perspective. Treatment with oseltamivir is a cost-effective strategy for otherwise healthy adults in the UK from both the healthcare payer and societal perspective.

This study estimated the economic burden of illness of obesity and selected comorbidities in terms of health outcome and costs to society and healthcare payer in Germany. The proportions of selected diseases (comorbidities) attributable solely to obesity were estimated using odds ratios/relative risks and prevalences based on data from the literature. The top-down approach was employed to match healthcare spending with the number of patients suffering from obesity (BMI 30+) and the major comorbidities to evaluate overall direct and indirect costs. In Germany there are approximately 12.24 million obese adults, 2.06-3.76 million of whom suffer from the selected comorbidities. From a societal perspective the total costs for obesity and comorbidities are euro 2,701-5,682 million per year and the direct treatment costs alone account for euro 1,343-2,699 million, imposing a major burden to the healthcare system. In view of the magnitude of the economic burden of illness there is a need for both further research and action at the health policy level.

Background: The carbonic anhydrase inhibitor acetazolamide (AZM) reduces macular oedema in some patients with retinitis pigmentosa. To better understand the oedema-reducing effect of AZM, the effect of AZM on passive permeability and active transport of fluorescein across the blood-retina barrier was studied in patients with retinitis pigmentosa and varying degrees of macular oedema., Method: The selection of patients was based on an introductory examination including vitreous fluorometry for qualitative assessment of the vitreous. Macular oedema was graded by fluorescein angiographic leakage. The effect of AZM on the transport properties of the blood-retina barrier was determined by differential spectrofluorometry, in a randomised, double-masked, cross-over study, comprising 2 weeks' treatment with AZM (500 mg/day) and 2 weeks' treatment with placebo. The penetration ratio, defined as the ratio between vitreous concentration 3 mm in front of the retina and the plasma integral, was determined for fluorescein and its metabolite fluorescein glucuronide at 30-60 min and at 120 min after fluorescein injection. Passive permeability and unidirectional permeability in the direction vitreous to blood, due to outward active transport of fluorescein, were determined in those cases where the curves for vitreous concentration of fluorescein could be fitted to a mathematical model. Visual acuity was tested by use of ETDRS standard logarithmic charts., Results: Twenty-two patients volunteered to participate in the study. Signs of significant vitreous detachment/liquefaction caused the exclusion of ten patients after the introductory examination. Nine patients with approximately intact vitreous and varying degrees of oedema completed the cross-over study. AZM treatment was related to a decrease in the penetration ratio of 21% for fluorescein (P=0.01) and of 22% for fluorescein glucuronide (P=0.004). Passive permeability and unidirectional permeability were determined in seven patients. AZM caused a decrease of 27% in the passive permeability of fluorescein (from 1.1 x 10(1) nm/s, P=0.031), and a 95% increase in unidirectional permeability of fluorescein (from 1.2 x 10(2) nm/s, P=0.047). AZM led to a reduction in the grade of macular oedema as determined by fluorescein angiography in three out of seven patients. Only small improvements (< or =5 letters) in visual acuity were noted., Conclusion: The present study indicates that the oedema-reducing effect of AZM is due to decreased leakage and stimulated active transport across the blood-retina barrier.

We collected benthic bacterial production data measured by (3)H thymidine incorporation (TTI) (25 studies), frequency of dividing cells (FDC) (3 studies), dark-C02 assimilation (1 study) and (3)H-adenine uptake (2 studies) from the literature, which included 18 marine, 6 river, and 2 lake studies. In all of the studies that used the TTI method, (3)H-DNA was isolated and incubations were carried out at in situ temperatures. Most of the researchers also determined (3)H-DNA extraction efficiencies and isotope dilution, thus interpretable estimates of bacterial production were used in the analysis. In marine sediments, bacterial production rates were linked to bacterial biomass, bacterial abundance, sediment organic matter, temperature, and sediment chlorophyll a, with these variables explaining between 40% and 68% of the variation in production rates. Simple relationships between production and bacterial biomass or bacterial abundance, or between production and sediment organic matter, were improved by also including temperature in the analysis of marine sediments. Sediment organic matter explained an appreciable fraction (58%) of the observed production in freshwater sediments. Temperature was the most powerful predictor of the observed variability in specific growth rates (r (2) = 0.48 and r (2) = 0.58) in marine and freshwater sediments, respectively. Thus, bacterial production and specific growth rates are most closely linked to substrate supply and temperature in marine and freshwater sediments.
