In this paper, we present the results of a scoping review in which we examined the scientific literature (2010–2021) on faith-based organizations (FBOs) working within the field of poverty alleviation, focusing on the way studies define and use the term FBO. Fifty-two relevant studies were identified and included. Our research shows that the term FBOs is primarily used in American studies. Moreover, there is no broad consensus on the exact definition or meaning of the term nor on its scope. Because of this lack of consensus and the inherent shortcomings of the term, we suggest to replace the term FBO by the term "religion-based solidarity initiatives" (RSIs), We define RSIs as: "Initiatives that, from a religious inspiration, aim at organizing collective action for and/or providing support or services to people in vulnerable positions." Environmental health remains a niche topic in bioethics, despite being a prominent social determinant of health. In this paper we argue that if bioethicists are to take the project of health justice as a serious one, then we have to address environmental injustices and the threats they pose to our bioethics principles, health equity, and clinical care. To do this, we lay out three arguments supporting prioritizing environmental health in bioethics based on bioethics principles including a commitment to vulnerable populations and justice. We also highlight and advocate for environmental law efforts that align with these priorities, focusing specifically on the need for a right to a healthy environment. Our intention is to draw attention to the legal and ethical concepts that underlie the importance of a healthy environment, and urge bioethicists to prioritize both legal and ethical advocacy against environmental injustices in their practice. Our society gives certain rights and protections to those who are punished and incarcerated, deeming them a vulnerable group in need of additional protections because they are under state control. Despite these protections, prisoners are still susceptible to mistreatment and abuse. This paper delves into one area in which prisoners are particularly vulnerable—the use of prison labor to fight wildfires in the western United States. In this paper, I first broadly discuss prison labor, before going into the current ethical principles and guidelines in the field of corrections that address the treatment of prisoners and prison labor. I argue that these principles and guidelines should be augmented to help those in corrections to make decisions about what is ethical; suggesting that how researchers evaluate proposed research projects could serve as a model for decision-making. I then introduce prison fire camps, considering whether they are ethical by research ethics principles, and suggesting how they can be changed to better reflect the values professed by our society. Objectives This paper examines health profiles and work environments of hired U.S. farmworkers to understand the risk to essential workers and their employers, to the food supply, and to rural health systems such as what is possible with the COVID-19 pandemic. Methods Large-sample statistical methods and proprietary data from the National Agricultural Workers Survey from 2000 to 2018 were used to assess factors associated with exposure to COVID-19 and vulnerabilities associated with medical complications. Results An aging workforce and increased access to health care within the crop worker population has been associated with a higher reported incidence of diabetes, asthma, and heart disease among workers over time. These trends confirm a vulnerable, but essential, workforce with higher risks for COVID-19 complications than would have been true of U.S. farmworkers as a group in earlier years. Conclusions Increasing age and disease burden in the U.S. agricultural labor force puts workers at increased risk for developing COVID-19 complications. Limits to field sanitation and housing quality inflate the probability of the development of COVID-19 hotbeds in rural communities that could further compromise the physical health of workers, the economic health of farm establishments, the agricultural supply-chain, and rural health capacities. The COVID-19 pandemic has been stressful for many, increasing levels of anxiety, depression and other mental health concerns worldwide. With the spread of the virus, many youth found themselves physically isolated from their peers and confined to their homes, and medical and mental health services previously provided in person had to adapt by providing virtual sessions. The transition to virtual care created many new challenges for clinicians and patients, including some specific to transgender youth and their families. Pre-pandemic, transgender youth comprised a marginalized and vulnerable population, with elevated risk for adverse mental health outcomes. However, community support, strong group identification and family affirmation can serve as important mitigating factors. In this paper, we will discuss unique challenges encountered in working with the parents and caregivers of transgender youth during virtual visits that have the potential to interfere with development of a therapeutic alliance and the movement toward increased family acceptance. This paper examines the implications of recent developments in U.S. intercountry adoption (ICA) policy for vulnerable children. We review policy and practices from 2004-2018, including (1) the 2008 implementation of the Hague Convention and (2) the 2017 changes in Hague accrediting entities for adoption agencies. By analyzing the ICA contexts of the top five States of origin, we argue the decline in ICA is from factors within States of origin rather than U.S. policy. Though ICA benefits individual children's development, it can cause harm at a systems level, so the decline in ICA has mixed implications for vulnerable children. [ABSTRACT FROM AUTHOR]

'Evidence-based' is the key criterion for decision making not only in medicine and other health professions, where it originated and revolutionized what was previously subjective, opinion-based practice, but also across policy and service development. The 'evidence-based' epistemology uses the hierarchy of evidence - a qualitative ranking of information for consideration, as to its cogency - based on a biomedical view of human health. Evidence-based medicine originally proposed that it would contribute to justice by providing the means to make objective, and therefore, fair and equitable decisions, as well as helping to stop the use of ineffective treatments, thus minimizing costs. In this paper, we argue, however, that due to the epistemological ranking of evidence and its implications, only one type of question can ever be answered using this framework and that some groups are unjustly disregarded or excluded because of the limitations inherent in its epistemology; therefore, evidence-based decision making, whether in medicine, other health professions, or policy settings, using the hierarchy of evidence, in fact, can contribute to injustice. This injustice occurs through preferenced evidence generation set against a highly pressurized and competitive research environment, where funding is restricted and the mantra 'publish or perish' creates separate drivers and considerations for research design other than the research problem. We provide evidence of the lack of evidence generation for three specific minority groups and suggest that the epistemological foundations of an evidence-based strategy be revised to include measures of quality that reflect a connectedness not only between the researchers and the affected stakeholders (patient population, service providers, etc.) but also between the methodology/analysis and the problem trying to be solved. Contemporary research ethics policies started with reflection on the atrocities perpetrated upon concentration camp inmates by Nazi doctors. Apparently, as a consequence of that experience, the policies that now guide human subject research focus on the protection of human subjects by making informed consent the centerpiece of regulatory attention. I take the choice of context for policy design, the initial prioritization of informed consent, and several associated conceptual missteps, to have set research ethics off in the wrong direction. The aim of this paper is to sort out these confusions and their implications and to offer instead a straightforward framework for considering the ethical conduct of human subject research. In the course of this discussion I clarify different senses of autonomy that have been confounded and present more intelligible justifications for informed consent. I also take issue with several of the now accepted dogmas that govern research ethics. These include: the primacy of informed consent, the protection of the vulnerable, the substitution of beneficence for research's social purpose, and the introduction of an untenable distinction between innovation and research. Nature-based interventions hold promise for vulnerable youth experiencing mental, emotional, developmental, behavioral, or social difficulties. This scoping review examined wilderness therapy, animal assisted therapy, care farming, and gardening and horticultural therapy programs to raise awareness and guide future development of research and treatment options. Studies included in this review were identified through a systematic search of the literature informed by a scoping review framework. Studies were examined by design, sample, intervention, and key findings. The majority of studies were quantitative using repeated measures designs and were conducted primarily in the United States. Sample sizes were generally small. Interventions were residential and community based with varying degrees of duration. Outcomes were largely positive across a wide range of psychosocial and behavioral measures and often maintained post-treatment. We emphasize the importance of robust empirical designs, comprehensive description of the interventions and surrounding therapies, and identification of target groups. [ABSTRACT FROM AUTHOR]

These findings from focus groups explore attitudes, beliefs, perspectives, and experiences relevant to workplace sexual harassment (WSH) among men and women farmworkers in California, USA, and Michoacán, Mexico. Focus groups are stratified by country and gender, with two in California (10 men and 10 women) and two in Michoacán (8 men and 5 women). This community-based participatory research includes Community Advisory Boards (CABs) consisting of farmworkers, academicians, non-profit organizations, attorneys, industry personnel, and community leaders who took part in strategy and the development of materials. Themes are related to the experience of, responses to, and farmworkers' recommendations for prevention of WSH. Although men and women faced WSH, women's experiences were more severe and frequent. Participants condemned WSH as contrary to principles of caballerosidad, cortesía, respeto – cultural values promoting respect for others and protection for vulnerable persons. Participants endorsed the notion that women are responsible for WSH. Although farmworkers try to resolve WSH on their own with help from co-workers, family, and leadership, they face significant barriers that silence victims and allow WSH to persist. All farmworkers recommended that management set a good example and enforce consequences for offenders. Implications include directly appealing to cultural values (emphasizing respect), incorporating bystander education, and countering the myth that women are responsible for WSH in workplace training. WSH is a recognized occupational hazard that affects all directly or indirectly exposed workers. We emphasize that employers are ultimately responsible for their workers' safety, supported by a governmental regulatory role. Enforcement of existing policy is needed in California , whereas awareness and policy development is needed in Michoacán. While enormous strides have been made in the representation of women in clinical trials, the percentage of women enrolling in Phase I trials still remains low, which both raises public health concerns about the safety of new drugs and social justice concerns regarding their inclusion in research. As part of a longitudinal study of healthy volunteers in the United States, our inquiry aimed to examine impediments to women enrolling in Phase I trials as well as their experiences participating in these studies at residential research clinics. We analyzed 111 semi-structured interviews conducted with 47 women who had enrolled in at least one Phase I trial. Our study indicates that women face discrimination during all stages of their participation in Phase I trials from their ability to qualify for studies, the treatment they receive in the clinic facilities, and a lack of social support. Specifically, we found that (1) study designs disadvantage participants of childbearing potential, (2) women feel vulnerable in the clinic space when outnumbered by men, and (3) heterosexual women are often discouraged from participation by their husbands or significant others. Placing these findings within the scholarly literature on barriers to women's clinical trial participation, we argue that diverse strategies attending both to physiological and social factors are needed to combat inequalities in U.S. Phase I trial participation. Following the rise and fall of lobotomy, a majority of U.S. states took legislative aim at psychosurgical procedures. This article canvasses, organizes, and analyzes the existing body of United States statutes and regulations mentioning psychosurgery. Many states regulate psychosurgery without defining the term; existing definitions are imprecise, but many would arguably apply to contemporary procedures like deep brain stimulation. Common to many states are restrictions on surrogate consent to psychosurgery, codifications of patients' consent or refusal rights, and situation-specific bans on the practice targeting certain contexts of vulnerability. Many states have only a handful of scattered laws bearing on psychosurgery, but a few have wide-ranging and well-integrated regulatory regimes. In reviewing these laws we perceive much room for harmonization and modernization. Greater consistency in protecting vulnerable persons from troubling uses of psychosurgery is achievable even alongside an effort to remove undue legal obstacles impeding patient access to potentially therapeutic procedures. This systematic literature review aimed to gain an overview of studies using the Middle Childhood-Home Observation for Measurement of the Environment Inventory (MC-HOME Inventory), and to compare mean scores between the studies. PubMed, PsycINFO, EMBASE and SCOPUS were searched to find eligible studies. The literature search yielded a total of 532 articles. Seventeen studies investigating 5083 families were included. Studies were divided into population based samples and high-risk samples. Results showed that the MC-HOME Inventory is a widely used interview for the assessment of the home environment, and as documented by the studies of this review the HOME Inventories will help identify families in need of support and stimulation. Studies have shown internal validity and consistency of the Inventory. The finding of overlapping total mean scores between studies of general populations and high-risk populations could be due to the broad definition of high-risk populations as well as methodological differences between the studies. To strengthen the methodology and enable comparison it is recommended that studies follow the guidelines provided in the HOME Administration Manual and systematically report on methods and findings. Objective: Medical care alone cannot adequately improve population health or eliminate inequities; social determinants of health (SDH) must be addressed. This study's purpose was to describe the research, teaching, service, and clinical practice activities implemented by RWJF Nurse Faculty Scholars to act on the SDH. Design: A cross-sectional survey design was used with a sample of RWJF Nurse Faculty Scholars, chosen because they were provided specialized mentoring, grants, and other support that allowed them to explore SDH. Results: Respondents (n = 57) addressed SDH in their research (86.0%), teaching (68.4%), service (66.7%), and clinical practice (33.3%). Leading research foci were quality of health care (56.1%), social and physical environmental stressors (54.4%), and access to health care services (49.1%). Leading SDH areas in teaching were discrimination in society against vulnerable populations (54.4%), quality of health care received by vulnerable populations (50.9%), and vulnerable populations' access to health care services (50.9%). Service activities included addressing discrimination against diverse populations. Leading SDH areas in clinical practice were quality of health care received by vulnerable populations (28.1%), vulnerable populations' access to health care services (22.8%), and discrimination in society against vulnerable populations (19.3%). Respondents also addressed SDH through personal mentoring (71.9%); efforts to recruit and/or retain underrepresented faculty (59.6%); developing a diverse pipeline of nurses (59.6%); and participation on a diversity committee (40.4%). Conclusion: The RWJF Nurse Faculty Scholars were able to leverage their awards to address SDH; however, further research is needed to assess the impact of the SDH work conducted. Knowledge from this study can be used as a road map for SDH elements and areas of professional work that nurses and other health professionals could address SDH in research, teaching, service, and practice. This article reflects on the relevance and applicability of the Belmont Report nearly four decades after its original publication. In an exploration of criticisms that have been raised in response to the report and of significant changes that have occurred within the context of biomedical research, five primary themes arise. These themes include the increasingly vague boundary between research and practice, unique harms to communities that are not addressed by the principle of respect for persons, and how growing complexity and commodification in research have shed light on the importance of transparency. The repercussions of Belmont's emphasis on the protection of vulnerable populations is also explored, as is the relationship between the report's ethical principles and their applications. It is concluded that while the Belmont Report was an impressive response to the ethical issues of its day, the field of research ethics involving human subjects may have outgrown it. A growing number of cities and towns are implementing “friendly” initiatives to change the community’s social and physical infrastructure to address the needs of a particular vulnerable segment of their population. Three prominent examples are aging-friendly, child-friendly, and immigrant-friendly initiatives. Although the limited but growing literature describes these initiatives as promising strategies to improve the quality of life of the targeted population, to date there has been little written about the underlying goals, values, and assumptions of these efforts. Using the value-critical approach, this article aims to better understand these aspects of population-friendly initiatives by examining the scholarly and grey literature. Social workers engaged in community and policy practice can play a key role in these efforts and call attention to the ways they may alleviate or exacerbate inequities. The article traces the social life of a policy that aimed to define and circumscribe the ambiguous and contested category of ‘orphaned and vulnerable children’ (or OVC) in South Africa at the height of the ‘emergency response’ to HIV/AIDS. Drawing on several months of institutional ethnographic research conducted over the course of five years with South African organisations receiving funding from the US President's Emergency Plan for AIDS Relief to provide services to ‘OVC’, the project interrogates the influence of governmental forms of counting and accounting on health policy and practice in South Africa. Focusing on the experiences of one organisation, the article describes a process of policy ‘translation’ typified by a series of disconnects between the intentions of a policy and the exigencies of implementation, structured by the ambiguous and flexible nature of the ‘OVC’ category. In this context, the article argues that the uncertainty produced by the implementation of the guidelines was not simply an artefact of a poorly designed policy, but rather signals an underlying epistemological tension in the practice of ‘global health’, in which quantitative metrics designed for monitoring and evaluation are often incapable of approximating the complexities of everyday life. The Black Adoption Research and Placement Center is a nonprofit organization delivering culturally specific adoption and foster care services. The organization developed as a response to concerns in the African-American community about the high numbers of African-American children entering and not exiting the public foster care system. The organization has undergone significant transformations over its 25-year history in relation to social, political, and economic changes that have altered the ways that the agency finances and delivers services. The history of Black Adoption Research and Placement Center presents an organization that has weathered many challenges because of its strong leadership, its committed governing body, its external relationships, and its internal operations. Alcohol abuse and dependence is recognized as one of the leading causes of morbidity and mortality in the United States. Alcohol is the most widely used substance among working adults, and almost 80% of risky drinkers are employed. Health care costs associated with alcohol problems exceed major chronic diseases. Employers bear economic and other burdens from untreated alcohol problems: productivity losses, increased absenteeism, tardiness, and poor work quality. There is overwhelming scientific evidence of the effectiveness of alcohol screening, brief intervention, and referral to treatment (SBIRT) for detecting and treating people with alcohol problems in medical settings (e.g., emergency), a setting that presents numerous “teachable moments.” The effectiveness in nonmedical work-related settings (e.g., employee assistance, work-life) is unclear. A review of the literature suggests there has been little attention paid to adaptation of alcohol SBIRT for the workplace, a setting where millions of working adults spend most of their day. This research aims to identify and describe employer and vendor SBIRT practices and promising approaches feasible for adaptation and dissemination in a variety of work-related settings.
