The Boko Haram insurgency is one of the most devastating violent crises that Nigeria has encountered in recent years. This paper compares the relative effect of the Boko Haram insurgency, as compared to other conflict events, on vaccination uptake among Nigerian children. To evaluate the effect of conflict events on vaccination, the children's birth months and the months of armed conflict were matched. The data on vaccination uptake was from the Nigeria Demographic and Health Survey (DHS) conducted in 2018, while the information on conflict events was from the Uppsala Conflict Data Program (UCDP) Georeferenced Event Dataset (UCDP GED). The sub-group analysis was conducted, Boko-Haram affected sample vs. total sample, to evaluate the differential effect of armed conflict events on vaccination. The Boko Haram insurgency had a significant and negative effect on vaccination uptake in the conflict-affected states; Borno, Adamawa, and Yobe. On the other hand, other forms of conflicts that took place in Nigeria had significantly weaker effects on vaccination uptake. The negative effect of Boko Haram-related conflict events was particularly strong on Penta3/DPT3 uptake, while its effect on BCG was weak. [ABSTRACT FROM AUTHOR]

Background: Teen pregnancy and childbearing are common in Nigeria, and understanding the complexities, such as sociodemographics and economic factors including sexual and reproductive health knowledge and awareness among adolescents over time can trigger innovative approaches and interventions. This study intends to capture the patterns and associated factors of teen motherhood among sexually active adolescents (15–19 years) between 2008 and 2018. Methods: The study data was extracted from 2008, 2013, and 2018 Nigeria Demographic and Health Surveys. Descriptive analysis was presented using frequencies and percentages; multivariable analysis was conducted using log-binomial logistic regression at a p-value <0.05. All analyses were performed using Stata 15.0, weighted and adjusted for the complex survey design and population size. Results: The prevalence of teen motherhood increased between the three successive survey waves (50.9% vs. 52.4% vs. 55.2%) from 2008, 2013, and 2018. Although, the pooled adjusted analysis revealed no significant change over the 10-year period. Knowledge of modern contraceptive methods, primary education, non-Catholic Christians, residing in the South-South region, and those currently or formerly married were associated with increased risk of teen motherhood. There was an inverse relationship between teen motherhood and wealth status; lower wealth status was associated with high adolescent pregnancy and childbearing. Conclusion: This study revealed an increase in the proportion of teen pregnancy and childbearing in Nigeria. Notably, there exist variations across age groups, geographic location, educational level, religious belief, marital and economic status. Interventions that ensure comprehensive sexuality education, girl child education, and economic empowerment especially for school dropouts are advocated to reduce teen motherhood. Ending child marriage and early childbirths would reduce total fertility rates and population growth especially in countries with a high incidence of child marriage, early childbirths, or both. Savings for public budgets could be large. This article relies on demographic projections and a UNESCO costing model for the provision of education by governments to estimate savings that could result from ending child marriage and early childbirths for public education budgets. Introduction: Prelacteal feeding practice contravenes the recommendation of World Health Organisation that breastfeeding be initiated within an hour of childbirth. Consequently, the health, social, emotional and economic benefits of optimal breastfeeding are limited. Therefore, to break this vicious cycle of prelacteal feeding and suboptimal breastfeeding, factors associated with the practice must be identified. Objective: To assess prelacteal feeding practices and its associated factors in a rural community with the view to generate data for community-level interventions that will promote optimal breastfeeding. Methods: Data was collected during a community-based surveillance for maternal, newborn and child health project in Tsibiri, a rural community in north-western Nigeria. The survey questionnaire was uploaded into mobile devices running on an android operating system. Trained female interviewers collected the data over a period of one week in 2011. Results: A total of 270 out of 309 interviewed women had experienced childbirth and were included in the analysis. Majority (85.2%) of respondents utilised prelacteal feeds for their newborns. Plain water was the most common prelacteal feed (44.7%). Prelacteal feeding was associated with births assisted by unskilled birth attendants (AOR 5.322, 95%CI 1.634-17.333); while operative delivery reduced the likelihood of the practice (AOR 0.168, 95%CI 0.060-0.470). No statistically significant association was found between use of prelacteal feed and women’s age, education or access to income. Conclusion: The predominance of prelacteal feeding practices underscores the need for innovative strategies that create awareness among mothers and health care providers, with emphasis on health facility deliveries, advantages of breastfeeding and risks of prelacteal feeding. The objective of this study was to examine regional variations in the self-reported causes of choosing not to give birth at a health facility in Nigeria. Cross-sectional data on 24,033 community dwelling women aged 15-49 years were collected from the Nigeria Demographic and Health Surveys (NDHS) conducted in 2008 and 2013. In both surveys a large proportion of the women mentioned facility childbirth as not necessary: 55.6% (53.3-57.8) in 2008 and 35.6% (34.0-37.2) in 2013. Poor transportation, not being customary, and costliness were each reported by around 10% of the respondents. The regression analysis showed significant regional variations in the prevalence of the self-reported causes of non-utilization. Women in the northern regions of the country were more likely to regard facility childbirth as unnecessary and to complain about transportation and financial difficulties. [ABSTRACT FROM AUTHOR]

Healthcare experiences among women living with HIV/AIDS (WLHA), determine their utilisation of sexual and reproductive health services, which ultimately influences their decisions on childbearing. This study aimed to understand the importance of healthcare support in the childbearing decision-making processes of WLHA, and its impact on eliminating new paediatric HIV infections. We conducted in-depth interviews between July and August 2012 with 15 WLHA receiving clinical HIV care at a teaching hospital in Lagos. Using PEN-3 cultural model, as a guide we explored perceptions of healthcare support pre- and post-partum. Findings indicate that faith in God for the delivery of a healthy child is significant during the pre-partum period, while the advice of healthcare workers concerning childbearing and access to available healthcare services carry more weight post-partum. Our findings have important implications for HIV treatment and care programmes geared towards WLHA considering childbearing, and ultimately the UN Global plan to eliminate mother-to-child transmission of HIV, as we move towards the 2030 agenda for sustainable development. Objective To determine the history of resumption of intercourse after childbirth and associated contraceptive practices among women in the southwest region of Nigeria. Methods A cohort of 181 women with live births was followed up for 6 months after delivery. Enquiry about the time of first intercourse after childbirth, associated dyspareunia, use of contraception, etc was made during the postnatal clinic visits and/or by telephone contact. Results Fifty (27.6%) had coitus within six weeks of childbirth, it increased to 115 (63.3%) at three months and 127 (70.2%) by six months post-delivery. Prevalence of dyspareunia was 36.2%. Eighty three (65.4%) of sexually active women practiced contraception which was predominantly use of male condom and withdrawal method. Co-habitation with husband (adjusted OR: 6.30; 95% CI: 2.56-17.01; p = 0.001) and mode of delivery (adjusted OR: 2.45; 95% CI: 1.30-4.73; p = 0.006) were strong predictors of commencement of sexual intercourse within six months postpartum. Significantly fewer women who had Caesarean section resumed coitus within six months when compared with those who had vaginal deliveries (59.2% versus 78.4%). Perineal injury did not predict resumption of coitus or experience of dyspareunia. Conclusion In contrast to the norm, more women in southwest Nigeria are resuming coitus soon after childbirth. A prospective study on the maternal deaths in public and private health facilities in Ogun State was undertaken for a period of 9 months (November 2003 – July 2004). While data were collected from all of the primary, the secondary and tertiary health facilities, it could only be collected from 123 (18.6%) of the private health facilities in the state. Of the 20,831 live births during the period, 37 maternal deaths occurred giving a maternal mortality ratio of 177.6 deaths per 100,000 live births. Some 84% of maternal deaths were not registered for delivery in the health facilities they delivered. The maternal mortality ratio is comparable with previously reported data for South-West Nigeria. A total of 81% of the deaths were due to three main causes: haemorrhage, eclampsia and infection. The maternal mortality ratio was almost three-fold greater in women above the age of 35 years. There is therefore a need to enhance activities to reduce maternal mortality. Private health facilities registration must be tied to sending data to authorised government departments. There is also a need to capture childbirth and attendant complications and death occurring outside orthodox health facilities. [ABSTRACT FROM AUTHOR]

Background: Infant mortality (IM) is high in Nigeria. High-risk birth can limit a newborn’s survival chances to the first year of life. The approach used in investigating the relationship between high-risk birth and IM in this study is yet to be documented in Nigeria. Objectives: The Intra-Demographic Birth Risk Assessment Scheme (IDBRAS) was generated and its relationship with IM was examined. Methods: 2013 Nigeria demographic and health survey data were used. Mothers who gave birth in the 5 years before the survey were investigated (n = 31,155). IDBRAS was generated from information on maternal age at childbirth, parity and preceding birth interval and was disaggregated into low, medium and high. Data were analysed using the Cox proportional hazard and Brass 1-parameter models (α = 0.05). Results: Infant mortality rate was 88.4, 104.7 and 211.6 per 1000 live births among women with low, medium and high level of IDBRAS respectively. The rate of increase of reported infant deaths between low and high IDBRAS was 0.1932 (R2 = 0.5326; p < 0.001). The prevalence of medium- and high-risk birth was 24.6 and 4.2% respectively. The identified predictors of IM were place of residence, marital status and size of the child at birth. The hazard ratio of IM was higher among women with medium (HR = 1.35; 95% CI = 1.22–1.48,p < 0.001) and high IDBRAS (HR = 1.73; 95% CI = 1.48–2.02,p < 0.001) than among those with low IDBRAS. Controlling for other correlates barely changed this pattern. Conclusions: The risk and level of IM increased as the level of IDBRAS increases in Nigeria. IDBRAS was an important predictor of IM. Maintaining a low level of IDBRAS will facilitate a reduction in IM rate in Nigeria. [ABSTRACT FROM PUBLISHER]
