As federal partners, the Veterans Health Administration (VA) and the Indian Health Service (IHS) agreed to share resources, such as education. The VA Geriatric Scholars Program, a workforce development program, provides one of its training programs on team-based primary care of elders to clinicians working in IHS and Tribal Health Programs. The practical impact of that training is described. A mixed methods approach was applied to the course's evaluation survey at five clinics in the Northwestern Plains, Southwest, Pacific Coast, and Alaska. Quantitative approaches assessed participants' self-reported intention to improve recognition and assessment of common geriatric syndromes. A qualitative approach applied to open-ended text responses revealed intensions to improve team-based care. Among the 51 respondents in our sample, we found significant improvements in self-reported ability to recognize previously unfamiliar potential risks to elders' health and safety, t(49) = 8.0233, p <.001, as well as increased comfort with conducting geriatric assessments and increased confidence in interprofessional team-based communication. Improvements to team-based care included enhanced clinical skills, organizational factors and the need to train additional employees. Background: In June 2020, the U.S. Preventive Services Task Force (USPSTF) issued a final recommendation on screening for unhealthy drug use in adults and adolescents. It assigned a "B" rating on a recommendation for screening in primary care for adults when services for accurate diagnosis, effective treatment, and appropriate care can be provided or referred, but declined to recommend the same for adolescents, concluding that current evidence is insufficient to assess the balance of benefits and harms. Objectives: To examine and provide recommendations to address gaps in the USP ST recommendation on screening for unhealthy drug use. Results: The lack of recommendation to screen adolescents represents a critical gap and highlights the need for research on adolescent substance use screening. While research is limited, available evidence shows short-term benefits from youth screening and early intervention and no evidence of harm. The lack of recommendation for youth leaves practitioners without guidance and incentive to intervene with youth who use substances, discourages expansion/support of youth screening, and reinforces treatment barriers. The statement also lacks guidance to address barriers to implementing screening in adults. Despite promotion of the practice, implementation in primary care is sporadic. Providers should be made aware of implementation barriers and the clinical guides and training available to facilitate implementation. Conclusions/Importance: Screening and intervention are vital for addressing the addiction crisis, and USPSTF recommendations are critical for making preventive health care services accessible and affordable. Heavy college drinking is associated with risky sexual behavior. It is therefore important to identify groups that are especially vulnerable to alcohol-influenced sexual risk (e.g., unplanned/unexpected sex). Undergraduates with elevated social anxiety represent one such vulnerable group given that social anxiety is associated with fear of intimacy and heavier drinking in intimate situations and situations with expected negative affect. Drinking to cope with negative affect induced by fear of intimacy might render socially anxious undergraduates vulnerable to risky sexual behavior, yet no known studies have examined this relationship. The current study tested whether social anxiety was related to alcohol-related sexual behaviors among current (past-month) drinking undergraduates (88.1% female; 77.6% non-Hispanic Caucasian) with higher (i.e., clinically elevated) social anxiety (HSA;n = 40) or lower (more normative) social anxiety (LSA;n = 94). Coping motives were examined as a moderator of the social anxiety-risky sexual behavior relationship. Gender was a covariate. HSA students reported more frequent alcohol-influenced sexual risk including regretted sexual situations, unprotected sex, sex with unwanted partners, unwanted sex, pressured/forced to have sex, and pressured someone to have sex. Coping motives significantly interacted with social anxiety group in the prediction of risky sexual behaviors except regretted sexual situations, such that HSA students with greater coping motives experienced more frequent sexual risk when drinking. Findings indicate that HSA students may be particularly vulnerable to risky sexual behaviors and suggest that coping motivated drinking may be an important target for therapeutic interventions geared toward reducing risky sexual behaviors among this high-risk population. The growth of distance education has necessitated strong evidence of quality for institutions of higher education, and numerous standards and principles of quality have been developed, such as Quality Matters™ (Quality Matters). These systems are often considered only at the course level to guide design and improve student outcomes, but they can also help to pull the institution together and galvanize advancement in online development. Adopting online quality standards can be a complex process that requires changes to institutional culture. This article describes how the use of the Community of Practice (CoP) framework engaged a campus-wide network of individuals in adoption of the Quality Matters training, rubric, and review process to advance distance education support and online course development. Using Social Network Analysis, researchers were able to identify the strengths and weakness of the CoP during the early adoption phase of Quality Matters to quantitatively measure the connections among members of the community. Based on a survey administered to every participant who attended the Applying the Quality Matters Rubric™ (APPQMR) workshop, researchers analyzed connections across the Quality Matters™ (Quality Matters) Community of Practice (CoP). This analysis indicated strong cohesion of the network based on work related to distance education but low density for connections based on Quality Matters. Furthermore, it demonstrated that significant connections existed between members prior to the adoption of Quality Matters and identified key influencers within the network. The results also indicated that the Quality Matters leadership team at the institution is highly central to the Quality Matters CoP network. Background: The increasing popularity of non-cigarette nicotine products, especially among youth, highlights the need for greater attention to their potential risks, including nicotine addiction and other substance use and addiction.Objectives: To examine the extent to which nicotine product use co-occurs with other substance use and addiction among youth and adults, describe the demographic groups and types of nicotine products associated with an increased risk of such co-occurrence, and discuss implications for research, prevention, clinical practice, and policy.Methods: Analyzing 2014 data from two nationally representative US surveys, the National Survey on Drug Use and Health (NSDUH) and the Monitoring the Future (MTF) study, we examined the co-occurrence between nicotine product use and alcohol and other drug use and addiction.Results: Individuals of all ages who reported using nicotine products of any kind were significantly more likely than nonusers to report alcohol, marijuana, other drug, and poly-substance use and to meet diagnostic criteria for a substance-use disorder. Users of multiple nicotine products generally were the most likely to engage in alcohol and other drug use and to be addicted to these other substances.Conclusions: The substantial co-occurrence of all forms of nicotine use and other substance use and addiction underscores the need to control the growing use of non-cigarette nicotine products among youth and to incorporate all forms of nicotine product use into substance use and addiction research, prevention, clinical practice, and policy efforts. Background: Underage drinking and its effects have been researched extensively. However, no study to date has examined how the levels of drinking that have been defined as risky for adults might relate to youth who have a heightened physiological vulnerability to alcohol.Objectives: To examine a range of drinking measures that go beyond common measures of youth alcohol use to gain a more detailed understanding of the nature of underage drinking and its associated correlates and outcomes.Methods: Analyzing data from a 2013 nationally representative US survey, we examined a variety of measures of alcohol use among 24,445 youth (weighted N = 381,155,562), the demographic groups most likely to have reported drinking in these ways, and associations between these measures of drinking and a number of adverse outcomes.Results: On all measures of potentially risky drinking, including meeting diagnostic criteria for an alcohol use disorder, underage drinkers exceeded the rates found for adults. Independent of sex, race, and age, youth who reported drinking in ways that exceeded guidelines set for adults had increased odds of meeting diagnostic criteria for an alcohol, tobacco, or other drug use disorder, and of reporting a number of health problems.Conclusions: The high rates at which youth report engaging in a range of risky drinking behaviors suggest a need for a more nuanced approach to substance use and mental health screening and interventions in clinical practice. This article assesses the ability of local housing ordinances to prevent neighborhood destabilization, specifically that arising as a consequence of the most recent housing crisis. We evaluate the degree to which vacancy registrations and point-of-sale inspection requirements influenced housing market outcomes during the housing crisis. With comprehensive real property data from Cuyahoga County, Ohio, we measure outcomes that characterize housing market distress including foreclosures, sales below the tax-assessed value, bulk sales, flipping, and property tax delinquency. We evaluate outcomes across properties in regulated and unregulated municipalities using matching procedures on linked data containing property, neighborhood, loan, and transaction characteristics. We find evidence that vacancy registrations substantially reduce foreclosures. In contrast, we find little evidence that point-of-sale inspections reduce undesirable transactions. Rather, properties in cities with inspection requirements displayed higher levels of foreclosure and tax delinquency relative to the control group during the study period. [ABSTRACT FROM PUBLISHER]

In light of the dramatically aging workforces in many industrialized countries, age diversity management will become a major challenge in human resource management. To successfully handle an age-diverse workforce, it is crucial to understand how employees of different ages can be motivated. This paper analyzes age's moderating role in the relationship between situational job characteristics and job satisfaction. To control for the potential influence of the cultural and institutional context, we use data from the USA, Japan and Germany. Findings show that older employees' job satisfaction is driven by different factors than younger employees: older employees put more emphasis on good relationships with colleagues, while income, advancement opportunities, job security and having an interesting job are less important. The archaeological work at the Wray Carpentry Yard in Williamsburg, Virginia offers a unique venue for the contextualization of 18th-century trades and tradesmen in the British colonies. James Wray's craftsmen, both enslaved and free, included carpenters, joiners, glaziers and cabinet-makers working in a growing urban centre, providing services to a town that was quite literally under construction. The story these workers tell through the archaeology of their buildings and possessions is distinct in the archaeology of 18th-century Britain and its colonies, as no comparably intensive contextual examination of an urban artisan complex has been undertaken. This study proposes a new hypothesis: Debris-covered glaciers served as Pleistocene biological refugia. This is based on detailed studies of vascular plant growth on six debris-mantled glaciers, literally around the world, as well as many casual observations also across the globe. We find that such glaciers are quite common and are distributed globally. Using Carbon Glacier, Mount Rainier, U.S.A., as a type locality and case study, we show aspects of the floristic and structural diversity as well as spatial patterns of plant growth on the glacier surface. Migration strategies, root characteristics, and origin and dispersal strategies for vascular plant species are documented. Also reported are special microclimatic conditions in these areas allowing for this remarkable plant ecology. We find that alpine taxa can grow considerably below their usual altitudinal niche due to the cooler subsurface soil temperatures found on glacial debris with ice underneath, and that may have significantly altered the spatial distribution of such flora during full glacial conditions. This in turn creates previously undocumented areas from which alpine, and perhaps arctic, plant species reestablished in post-glacial time. This hypothesis is complementary to both the nunatak hypothesis and tabula rasa theory and possibly helps solve the ongoing controversy between them. [ABSTRACT FROM AUTHOR]

Dominant discourses in the United States paint the acts of prisoner 'abuse' committed by US soldiers in Abu Ghraib in 2003 as either the obscene but exceptional example of some low-ranking soldiers gone mad, or as the direct result of the suspension of the rule of law in the global 'war on terror'. Alternatively, feminist theorist Barbara Ehrenreich suggests that the pictures depicting female soldiers torturing prisoners are both horrifying and a sign of 'gender equality'. This article departs from all three of these positions. I argue that the micro-level violences shown in the Abu Ghraib pictures are neither just aberrations nor a sign of gender equality. Rather they follow a pre-constructed heterosexed, racialized and gendered script that is firmly grounded in the colonial desires and practices of the larger social order and that underpins the hegemonic 'save civilization itself'-fantasy of the 'war on terror'. I explore how the participation of some of the US Empire's internal Others, namely White western women, may disrupt some of the social processes of normalization underpinning this colonial fantasy, but nevertheless serves to re/produce the identity and hegemony of the US Empire and its heterosexed, racialized and classed World (Dis)Order. Most substance abuse treatment facilities do not routinely treat cigarette smoking even though most of their clients smoke and many will die from tobacco-related illness. This study is a secondary analysis of a national survey of the prevalence and type of tobacco treatment services available in methadone maintenance treatment. It presents qualitative and quantitative information on provider attitudes regarding tobacco treatment in drug treatment. One clinic leader (either a medical director, head nurse, or clinic director) from all 697 U.S. facilities was invited to participate in the study. Most (38%) clinic leaders thought the best time to treat patients for nicotine dependence was whenever the patient wanted treatment. One in four clinic leaders reported they or one of their staff had advised patients to delay quitting smoking cigarettes. Fewer (14%) believed that patients in some way benefited from cigarette smoking; these benefits included managing mood disturbances (calming, reducing anxiety, depression, stress, anger) and substituting for illicit drug abuse. This suggests that some providers might rely on cigarettes in lieu of psychotherapy or prescription medications to address patients' mood disorders such as depression, anger, and anxiety. One tactic for disseminating tobacco treatment in drug treatment might involve identifying the purported benefits of tobacco and training providers in alternative methods for delivering these benefits. Globalization of the U.S. textile and apparel industry has been significantly spurred in the last decade by trade agreements such as the North American Free Trade Agreement (NAFTA), and the Caribbean Basin Initiative (CBI). In terms of the U.S. textile and apparel manufacturers and retailers, the competitive pressure from markets and consumers has forced many firms to rely increasingly on global sourcing to sustain competitiveness. In the last decade, U.S. textile and apparel manufacturers and retailers have been actively involved in global sourcing to find suppliers who could meet the criteria of low cost, high quality, reliable delivery, quick response time, and flexibility. The purpose of this study is to investigate the changes in US. global sourcing patterns for yarn, fabric, and apparel. Cluster analysis is used to analyze trade data in 1993 and 2002 in order to investigate the patterns of U.S. global sourcing of textile and apparel products. Results indicate that US. textiles and apparel sourcing patterns have shifted, with a substantially increased emphasis on apparel imports from Mexico and the CBI countries in 2002 as compared to imports from mainland China and Hong Kong in 1993. Sourcing patterns of yarn and fabric indicate that Mexico upgraded its place in U.S. imports of yarn and fabric in 2002. In addition, further longitudinal analysis of US. apparel imports from mainland China, Hong Kong, CBI countries, and Mexico, which were the major players in the U.S. global sourcing of apparel, shows that there was a dynamic change in the status of these four regions in terms of their respective percentage shares in U.S. apparel sourcing. The paper concludes that the U.S. textile and apparel global sourcing patterns in last decade reflect the dramatic changes in the competitive and dynamic global textile and apparel business environment. Successful binational planning and management of water resources is a complex process dependent on informed decision-making across diverse economic, social and political sectors. Additional technical and scientific information is often required as a part of this process. A critical factor in this process is how effectively social and physical scientists can help build collaboration and trust among stakeholders, water and land managers, and policy-makers. Within the international San Pedro River Basin, disparities between Mexico and the USA regarding economic development and political orientation, combined with a highly variable and complex physical setting, suggest that the successful engagement of scientists with communities and stakeholders will be essential for addressing challenges in water management. Based upon concepts associated with collective action theory, adaptive management and conflict resolution, the present paper proposes a process for fostering collaborative binational water management in basins such as the San Pedro that span international borders. We analyze a random sample of 300 U.S. House member Web sites, organized around four topics: personal information, communication and feedback, constituency services, and issue information. With respect to the first three, the sites are well-developed, though we anticipate some further diffusion in the provision of basic information. With regard to issues, there is little interactivity, and it is uncertain that interactive devices will catch on. Even with respect to conveying information about issues, Web sites could be made more informative, both by providing members' party affiliation more prominently and by providing simpler, more direct statements of members' issue positions. The US decision to withdraw from the Anti-Ballistic Missile (ABM) Treaty in June 2002 was a watershed development in international politics. Deciding that the threat from rogue states outweighed concerns about how Russia (and the larger international community) would respond, the Bush administration revealed that in the post-September 11 environment, it was not prepared to give other states an effective veto over US security policy. However, the announcement had enormous repercussions for allies as well as adversaries. This paper will examine how two close US allies—Canada and the UK—have reacted to the missile defense program. It will demonstrate that, while both have moved progressively closer to formal participation, the two countries have resisted making specific commitments because of political considerations. With initial deployment of the missile shield scheduled for later this year, the paper reveals that the US can expect allies to continue to react with caution, but that wariness should not necessarily be confused with opposition and/or defiance. This article presents a historical analysis of published data regarding the exposure of brake mechanics to asbestos as a result of doing brake work. Concerns about this possible hazard were first raised in the late 1960s. This analysis focuses on 30 years of data collected during the brake repair event (e.g., a brake job) and 8-hour time-weighted average (TWA) personal samples. A brake job TWA represents the average concentration a mechanic experienced during brake servicing, rather than throughout the workday, and an 8-hour TWA represents the average airborne concentration of asbestos for the entire workday (which would involve brake work and other activities). Nearly 200 brake job and 8-hour TWA airborne asbestos samples were analyzed to assess how asbestos concentrations varied by type of vehicle serviced, country in which mechanics worked, time period, and brake-cleaning method. To facilitate comparisons, brake job TWAs were converted to estimated 8-hour TWAs using the durations and number of brake jobs performed per mechanic each day. Estimated and measured 8-hour TWAs for mechanics servicing automobiles and light trucks ranged from <0.002 to 0.68 f/cc, with a mean of 0.04 f/cc. In contrast, the 8-hour TWAs for mechanics servicing heavy trucks and buses ranged from 0.002 to 1.75 f/cc, with a mean of 0.2 f/cc, suggesting that these mechanics experienced higher daily asbestos exposures than automobile and light truck mechanics. Brake job and 8-hour TWAs for brake mechanics worldwide were found to be similar during the same time periods, and they were consistently below contemporaneous occupational health standards in the United States. The increased use of brake-dust control measures in some garages resulted in at least a 10-fold decrease in the TWA airborne concentrations of asbestos from the 1970s to the late 1980s. Six focus groups were conducted in South Carolina with African American women (n = 42) aged 19-51 years to identify factors that influence physical activity. Transcripts were analyzed using NUD*IST. Cultural influences were seen as more important in determining the type of physical activity than its level. Barriers to and enablers of physical activity were identified in the social and physical environments, as were policy issues affecting physical activity in the community and at the work site. Objective. The purpose of this study was to assess grade, race, socioeconomic status and gender differences in perceptions of body size, weight concerns, and weight control practices between 4th and 7th grade students in South Carolina. Design. Two random samples, consisting of a total of 1,597 children (53.1% white, 51.97% female, 44.9% 4th graders) participated in two questionnaire surveys. Both surveys included a series of seven female and seven male body size drawings, body image and weight concern questions, and questions pertaining to weight control practices. Responses to the questionnaire were analyzed using chi-square analysis and the General Linear Model. Results. Using socioeconomic status (SES), race, gender, and grade as independent variables, differences in ideal adult body size, opposite gender ideal adult body size, weight concerns, perceptions of family/peer weight concerns, perceptions of own body size and weight control practices were studied. Analyses revealed that 4th grade males select a larger ideal adult body size and opposite gender ideal adult body size when compared to 7th grade males (p < 0.0001 and p = 0.0078, respectively). Fourth grade females indicated less personal concern about their weight than 7th grade females (p = 0.0009). Fourth graders also perceived less family/peer concern about weight than 7th graders (p = 0.0027) and 7th graders described themselves as being more overweight than 4th graders (p = 0.0039). Blacks selected a significantly larger body size than white children for ideal adult (p = 0.0287) and ideal opposite gender adult body size (p < 0.0001 for males, p = 0.0030 for females). Blacks also perceived less personal and family/peer concern about weight when compared to whites (p = 0.0083). More whites reported that they were trying to lose weight, as compared to blacks (p = 0.0010). Males also selected significantly larger body size silhouettes than females for ideal adult body size (p = 0.0012). Males expressed less personal concern about weight (p < 0.0001), perceived less family/peer concern about weight (p < 0.0001), and were less likely than females to be engaged in weight loss (p < 0.0001). Females in the high SES category selected a significantly smaller ideal male adult body size than females in the low SES group (p = 0.0124) and more females in the high SES category were trying to lose weight when compared to females in the low SES group (p = 0.0055).) Conclusion. This study indicates that early in a child’s sociocultural development, grade level, gender, race, and SES are influential in the perception of ideal adult body size and opposite gender ideal adult body size. These factors are also influential in determining concerns about weight and weight control practices. The findings of this study support the need to begin health and wellness education efforts early in childhood while taking into account racial, gender, age, and SES disparities. The article examines the results of a study of the opposite-sex ideal person of adolescents in four countries. Opposite-sex ideal person of adolescents in each country is presented with mean ranks of qualities of the opposite-sex ideal. Six hundred young adolescents (11 to 16 years (old) from 4 countries (Guatemala, Iceland, Mexico and the United States) ranked the importance of 10 qualities of the opposite sex ideal person. Those from the United States responded in an individualistic fashion; they ranked being fun, being sexy. and having considerable money as important for the ideal. Those from Guatemala responded in a collectivistic fashion; they ranked liking children as important, but being fun and good looking as unimportant. Adolescents from Mexico and Iceland reported patterns of values not clearly associated with either collectivism or individualism. The ranks were analyzed by means of a KruskaI-WaIIis test, with the four nationalities as the independent variable.

Medicaid waivers play an important role in the health policy process. Various health care policy initiatives have encouraged states to experiment with their Medicaid programs by expanding coverage to populations not traditionally covered by Medicaid, or by delivering Medicaid services in non traditional ways. Lessons learned by individual states' Medicaid waivers could be an important part of the health care reform debate. This paper presents an evaluation of a Medicaid waiver operating in South Carolina. The South Carolina Community Long-term Care Waiver for the Elderly and Disabled is evaluated for cost effectiveness, quality of care, and client satisfaction. This evaluation shows that the waiver is highly effective in offering individuals who are medically and financially eligible for Medicaid supported nursing home care a viable, cost effective, high quality alternate to institutionalization.
