Research indicates that sex offenders’ relationships are often unstable and superficial. Sex offenders are portrayed as showing little empathy toward their partners and being incapable of sharing intimacy. This study aimed to explore sex offenders’ intimate relationships and identify potential stabilizing factors in an outpatient setting. In an exploratory pilot study, the authors used standardized instruments to assess 17 heterosexual sex offenders and their intimate partners. Results indicate that both partners rate high in attachment anxiety. The authors found a high level of correlation between both partners with regard to attachment avoidance. The sex offender couples demonstrate similarly low scores for “dominance, pugnaciousness, and aggression” in intimate relationships, and the female partners revealed high neuroticism and conscientiousness scores. Controversies exist about the diagnostic concept of "sexual addiction." The aim of this study was to investigate if German specialized sex therapists are confronted with patients who possess sexual addiction symptoms and to obtain information about diagnoses, comorbidity and treatment. A 12-item questionnaire was sent to all members of the German Society of Sex Research asking them about diagnoses, comorbidity and treatment of patients who experienced sexual addiction problems. Forty-three out of 149 members (28.9%) responded to the survey and reported about 97 patients with sexual addiction symptoms. Most common were pornography dependence, compulsive masturbation, and protracted promiscuity. Using ICD-10 definition "excessive sexual drive" was diagnosed more often in women while "disorder of sexual preference" was mainly diagnosed in men. In women neurotic (e.g., anxiety disorders) and eating disorders were most commonly diagnosed as comorbid disorders, while in men substance disorders were most commonly diagnosed. Most patients were treated by individual psychotherapy. We discuss the difficulties of the "sexual addiction" concept critically and propose an algorithm to facilitate the diagnostic process. This algorithm differentiates between the paraphilic and non-paraphilic (we use the term paraphilia-related disorder) types of excessive sexual behavior first. Up to now there have been no published results of therapy of paraphilia (for example, pedophilia or sadism) and sexual aggressive impulsiveness with luteinizing hormone-releasing hormone (LHRH) agonists in the German-speaking countries. After a short intoduction about physiologic features and the present state of investigations in treatment of paraphilia with LHRH agonists we describe 11 patients who were treated with the LHRH agonist Leuprolide Acetate over a period of 12 months. The patients showed no tendency toward sexually aggressive behavior and reported an evident reduction of penile erection, ejaculation, masturbation, sexually deviant impulsiveness, and fantasies. One patient died from suicide. In combination with other treatments, LHRH Agonists seem to be a very promising alternative to cyproterone acetate and its possible carcinogene effects. Since that time, The Standards of Care were reviewed and revised by a group of professionals and unanimously endorsed by voice vote by the participants in the Third International Congress on the Treatment of Sex Offenders held in Minneapolis, Minnesota, September 20–22, 1994. These Standards were again published in the Journal of Offender Rehabilitation (Coleman, Dwyer, Abel, Berner, Breiling, Hindman, Honey-Knopp, Langevin, and Pfäfflin, 1996). Those Standards have been subsequently reviewed by the current authors at the Fifth International Conference on the Treatment of Sex Offenders held in Caracas, Venezuela, March 22–27, 1998 and minor modifications and changes were incorporated into this version. In a sample of 30 consecutively admitted sex-offenders at Justizanstalt Mittersteig (a treatment facility inside the Austrian prison system, located in Vienna) we found 23 patients with a diagnosis of personality disorder (according to DSM-111-R). The distribution of diagnoses did not differ significantly From other samples found in a general outpatient clinic with the exception of a relatively high rate of "sadistic personality disorder" (9 patients).
