Conservation biological control uses habitat management to enhance the survival and impact of arthropod natural enemies for pest control. Its advantages are that it relies on native or established invertebrate populations that are adapted to local agricultural ecosystems and conditions. We surveyed 300 farmers in three agro-ecological zones of Kilimanjaro Region, Tanzania to assess farmers' knowledge of natural enemies, insect pests and pesticide use and ways of accessing agricultural information to identify hurdles to the adoption of conservation biological control measures. Data were collected through face to face interviews using questionnaires and pictures and by using a novel voice-response mobile phone survey. The farmers surveyed regarded almost all insects as pests, with data analyses revealing that 98.7% of farmers were completely unaware of natural enemies. After completing a short training course, however, awareness was transformed, with 80% of farmers recognising beneficial insects and expressing an intention to change farming practices to enhance their survival within the crop. Access to information about synthetic pesticide alternatives was a limiting factor to uptake of biological control measures with 8.7% of farmers reporting no access to agricultural information, while others were mostly dependent on agricultural officers. These findings identified a severe lack of knowledge among smallholder farmers about beneficial insects which will impact adoption of conservation biological control. We recommend improved access to information and knowledge among the technical officers and the smallholder farmers with direct training on agro-ecological intensification for wider adoption of conservation biological control. Medical students’ early clinical encounters may influence their perceptions of geriatrics. This study examines reflective essays written by 3rd-year medical students on required clinical rotations. Using content analysis, the authors analyzed the essays’ thematic content. The authors then used chi-squared analysis to compare themes with geriatric patients (age 60+) to themes with other age groups. One hundred twenty out of 802 essays described a geriatric patient. The most common geriatric themes were (1) death and dying, (2) decision making, (3) meaningful physician-patient interactions, (4) quality of care, and (5) professional development. Geriatric essays were more likely to discuss death/dying and risk-benefit themes and less likely to discuss abuse. Geriatric essays were more likely to describe students’ moral distress. Geriatric essays with moral distress were more likely to include empathy themes compared to geriatric essays without moral distress. This article examines experiences and perceptions related to the September 11, 2001, attacks among members of two immigrant groups (Spanish-speaking and Mandarin-speaking) in New York City. Focus groups were conducted 1–2 years after the attacks. Qualitative analysis was conducted using ATLAS.ti. Three major themes emerged: (a) Immediate Experience of the Attacks, (b) Evolving Psychological Adjustment, and (c) Long-Term Issues of Public Concern. The groups’ discussions diverged with temporal progression across these themes, reflecting increasing congruence with broad conceptualizations of their respective cultures. The timing of first sexual intercourse is often defined in terms of chronological age, with particular focus on “early” first sex. Arguments can be made for a more nuanced concept of readiness and appropriateness of timing of first intercourse. Using data from the third National Survey of Sexual Attitudes and Lifestyles (Natsal-3), conducted in 2010–2012, this study examined whether a context-based measure of first intercourse—termedsexual competence—was associated with subsequent sexual health in a population-based sample of 17-to 24-year-olds residing in Britain (n = 2,784). Participants were classified as “sexually competent” at first intercourse if they reported the following four criteria: contraceptive protection, autonomy of decision (not due to external influences), that both partners were “equally willing,” and that it happened at the “right time.” A lack of sexual competence at first intercourse was independently associated with testing positive for human papillomavirus (HPV) at interview; low sexual function in the past year; and among women only, reported sexually transmitted infection (STI) diagnosis ever; unplanned pregnancy in the past year; and having ever experienced nonvolitional sex. These findings provide empirical support for defining the nature of first intercourse with reference to contextual aspects of the experience, as opposed to a sole focus on chronological age at occurrence. Establishing the clinical significance of symptoms of sexual dysfunction is challenging. To address this, the fifth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) introduced two new morbidity criteria (duration and symptom severity) to the existing criteria of distress. This study sought to establish the impact of these three criteria on the population prevalence of sexual function problems. The data come from a national probability survey (Natsal-3) and are based on 11,509 male and female participants aged 16–74, reporting at least one sexual partner in the past year. The key outcomes were: proportion of individuals reporting proxy measures of DSM-5 problems, and the proportion of those meeting morbidity criteria. We found that among sexually active men, the prevalence of reporting one or more of four specific sexual problems was 38.2%, but 4.2% after applying the three morbidity criteria; corresponding figures for women reporting one or more of three specific sexual problems, were 22.8% and 3.6%. Just over a third of men and women reporting a problem meeting all three morbidity criteria had sought help in the last year. The Natsal-SF is a psychometrically validated measure of sexual function for use in community health surveys, derived from 17 questions reflecting three components of sexual function. Scoring requires knowledge of complex statistical modeling and, given the methodological complexities, we assessed the validity of two simplified scoring methods calculated using the factor loadings produced when originally modeling the Natsal-SF items. Method 1 uses these factor loadings to three decimal places, while method 2 assigns whole numbers to each item based on the factor loadings. Scores from these simplified methods are compared to the original score using correlation coefficients, by comparing the distributions and the scores of each method in a linear regression model with key variables. We found scores from the simplified methods both correlate highly with the original score, and the distributions of scores closely match. The simplified methods result in different regression coefficients for gender and relationship context but estimate the coefficients of all other variables similarly to the original method. While the Natsal-SF should ideally be scored using latent variable modeling, the simplified methods perform well so can be used in similar contexts, increasing the utility of the Natsal-SF and enabling future studies to measure sexual function more comprehensively. Background. The American Heart Association encourages trained and untrained bystanders to perform, at a minimum, chest compressions on anyone who suddenly collapses. It is possible that people who are not in cardiac arrest may receive bystander cardiopulmonary resuscitation (CPR), from which the potential for injury is unknown. Objectives. To determine the number of victims who received bystander CPR but were not in cardiac arrest and to identify any injuries resulting from receiving bystander CPR. Methods. Retrospective review of patient care records from a countywide emergency medical services (EMS) database. All patients treated by EMS between March 2003 and February 2009 who received bystander CPR were queried. Victims who were determined not to be in cardiac arrest upon EMS personnel assessment were identified as likely not in cardiac arrest. Hospital medical records for transported patients were reviewed for injuries possibly related to CPR. Patient demographics were collected and descriptive statistics were used for analysis. Results. Six hundred seventy-two incidents of bystander CPR occurred, with 77 (11.5%) cases not being identified as cardiac arrests by EMS. Twenty-three percent of the patients were less than 19 years of age. Emergency medical services arrived in less than 6 minutes for 68% of patients. Seventy-two patients were evaluable for injury; of those, 53% were admitted to the intensive care unit. One patient (1.4%) had an injury that was documented in the medical record as possibly CPR-related: rhabdomyolysis. Conclusions. Bystanders provide CPR for patients who are not in cardiac arrest at a relatively low frequency. Short-duration bystander CPR caused injury in less than 2% of victims. Our results suggest that the benefits of bystander CPR for adults who suddenly collapse outweigh the risk of injury for those not in cardiac arrest. [ABSTRACT FROM AUTHOR]

This paper reports on a case study which examined mental health service users and carers, public housing tenants and non-government organisations' (NGO) participation and representation within policy processes. A particular focus was influence upon policies aimed at achieving housing outcomes for people with a psychiatric disability. The research was a case study of a period of mental health system reform in South Australia (2000–2005) and involved primarily qualitative research; interviews, focus groups and participant observation, as well as document analysis. Participants (n = 92) included service user and carer representatives, public servants from across the health, housing and disability sectors, and NGO professionals from across sectors. The study found that the community participation that existed during the reform period was largely an end in itself rather than a means for groups to shape policy processes, including housing policy. A number of barriers to service users and NGOs influencing policy agenda setting and implementation were determined and described. A range of strategies and processes to enhance community participation, representation and accountability in policy processes are recommended. These include cross-sectoral policy development and accountability mechanisms, advocacy coalitions across sectors for policy agenda setting, public forums involving both service users and professionals, and challenging ‘consumerist’ discourses on participation. Such strategies could facilitate policy debates on housing for people with a psychiatric disability, counter dominant professional interests within policy networks, and help to address community stigma. Objective. To obtain a better understanding of how stroke events are communicated to 9-1-1 telecommunicators, and how telecommunicators and emergency medical services (EMS) personnel respond to such calls. Methods. The authors identified 104 patients with a hospital discharge diagnosis of stroke or transient ischemic attack who were transported to hospital by ambulance in two North Carolina counties during 1999 and 2000. Ambulance call reports were abstracted and linked to 9-1-1 call center audiotapes, which were transcribed and verified. Results. Of the 104 calls, 44 were made by medical personnel, 38 by a family member, eight by a bystander or neighbor, five undetermined, and three by other nonmedical personnel. In only six instances (6%) was the call placed by the patient. The most common symptoms reported were altered mental status (40%), trouble walking (32%), impaired speech (27%), and abnormal breathing (27%). Although the word “stroke” was often used (45%), 9-1-1 telecommunicators classified the calls as a stroke in only 31% of cases. However, in the majority of cases (79%), paramedics were dispatched at the highest priority. The median time from dispatch of EMS to patient arrival at the hospital was 41 minutes, approximately half of which was spent at the scene. Conclusion. Although typical stroke symptoms are commonly described, calls are often not classified as “strokes” by telecommunicators. Nevertheless, because of the symptoms reported during the calls, the majority of cases are treated as high priority by telecommunicators. [ABSTRACT FROM AUTHOR]
