The use of N-acetylcysteine (NAC) remains the standard of care for treatment of acetaminophen (APAP) toxicity and overdose. Currently, there is growing evidence to suggest that massive acetaminophen overdose is associated with increased hepatotoxicity despite timely administration of NAC. This raises the question as to whether an increased dose of intravenous (IV) NAC should be used in the setting of massive APAP ingestion. This study aimed to evaluate the rate of hepatotoxicity after massive APAP overdose treated with 3 different NAC treatment regimens. This was a retrospective cohort study conducted by electronic medical record review of cases reported to a statewide poison control system between 2007 and 2020. Inclusion criteria were single APAP or APAP combination-medication ingestion; acute massive acetaminophen (APAP) ingestion (defined as APAP concentration ≥ 2 times above the Rumack-Matthew 150 nomogram); received one of the three NAC regimens: standard dose IV NAC, oral (PO) NAC, or high dose IV NAC. The risk of hepatotoxicity was evaluated using a multivariate logistic regression model with standard dose IV NAC as the base variable for comparison. A total of 373 patients met inclusion for the study. Of those, 135 cases were treated with standard dose IV NAC, 121 cases treated with PO NAC, and 117 cases treated with high dose IV NAC. The risk of developing hepatotoxicity was not statistically significant between the high dose IV NAC (OR 1.05, 95% CI 0.52 − 2.09) or oral NAC (OR 0.69, 95% CI 0.33 − 1.46) when compared to standard dose IV NAC. When adjusted for APAP combination medications, initial APAP ratio, initial elevated AST/ALT, and treatment within 8 h, there remained no difference between treatment regimens. This study was unable to detect a large absolute reduction in the rate of hepatotoxicity after massive APAP ingestion in patients treated with high dose IV NAC or PO NAC when compared to standard dose IV NAC; even when treatment was initiated within 8 h of ingestion. Tetramesa romana is a biological control agent of the giant reed, Arundo donax (Poaceae: Arundinoideae), which is an invasive weed in the riparian habitats of the Rio Grande Basin of Texas, the southwestern U.S.A. and northern Mexico. Field evaluations were conducted in the native range of T. romana in Mediterranean Europe and in the introduced ranges of Texas, California, and South Africa to compare densities of the wasp. Population densities and percentage parasitism levels for the 2017 year are compared to meteorological variables (average temperature, precipitation, and heat units). In the introduced ranges of Texas (intentional) and South Africa (adventive) T. romana population densities were 39 and 10-fold higher than in the native range, respectively. Percentage parasitism of T. romana in Texas and in the native range of Thessaloniki, Greece were 2.0% and 34.3%, respectively. Annual heat unit accumulation was 1.3-2.7-fold higher at Texas sites than at other introduced or native sites, and heat units were positively associated with exit hole counts at introduced sites. Annual precipitation was 2-fold higher at Texas and South African sites than in California and the native range sites. Favourable weather conditions and reduced parasitism in Texas along the Rio Grande, as compared to the native range, allows T. romana to reach higher population levels and cause considerable damage to A. donax. The high liquid content in fruit and vegetable wastes makes it convenient to mechanically separate these wastes into mostly liquid and solid fractions by means of pretreatment. Then, the liquid fraction can be treated using a high-rate anaerobic biofilm reactor to produce biogas, simultaneously reducing the amount of solids that must be landfilled. In this work, the specific composition of municipal solid waste (MSW) in a public market was determined; then, the sorted organic fraction of municipal solid waste was treated mechanically to separate and characterize the mostly liquid and solid fractions. Then, the mesophilic anaerobic digestion for biogas production of the first fraction was evaluated. The anaerobic digestion resulted in a reduced hydraulic retention time of two days with high removal of chemical oxygen demand, that is, 88% on average, with the additional benefit of reducing the mass of the solids that had to be landfilled by about 80%. When electrical stimulation is used on wounds, the electrical current has difficulty penetrating areas where there is necrotic tissue. Further, for an irregularly shaped wound, current distribution is poor in some areas of the wound since conventional two-electrode delivery systems provide the greatest current in a line directly between the electrodes. A new stimulator and electrode system is described which uses three electrodes spaced around a wound to disperse current more evenly. The stimulator senses tissue impedance and then redirects current by altering its Thevenin's output impedance for each electrode; each of the three electrodes becomes the active one in sequence while the remaining are the sink electrodes. Eight subjects were examined to test the stimulator. Electrical stimulation was applied to the skin above the quadriceps muscle at currents of 15 mA in six subjects without wounds and in two subjects with wounds. The relationship between electrode position and current dispersion on the skin was examined with a two-electrode vs. a three-electrode system to set stimulation parameters for the computer. The results showed that the three-electrode system could (1) detect areas of the skin with high impedance; (2) compensate by altering the Thevenin's output impedance at each of the three electrodes to shift current to high impedance areas; (3) provide uniform current across the skin as assessed by skin current and blood flow measurements with a laser Doppler flow imager. Rainwater samples were collected in the western sector of Mexico City (MC) and at Rancho Viejo (RV), 80 km west-south-west of MC, from 2001 to 2005, and Orizaba City (OC), about 90 km from the Gulf of Mexico, where rainwater collections were only possible on some weekends in 2001. Rainwater samples were treated in the field, and analysed by fluorescence at the laboratory. The volume-weighted mean concentration (VWMC) of H2O2 was 13.2 μM at RV, and 11.2 μM in MC, for the period 2001-2005. The highest VWMC was observed in OC (21.6 μM). The VWMCs for each year were 9.5, 14.4, 11.5, 16.7, and 14.3 μM at RV, and 12.2, 12.2, 14.3, 11.8, and 9.9 μM in MC, for 2001-2005, respectively. Hydrogen peroxide in rainwater correlated significantly and negatively with sulfate in both MC and RV, but not, however, in OC. This study confirmed that H2O2 concentration in rainwater is controlled by a complex combination of rain intensity, washout processes and in-cloud formation of H2O2, acting simultaneously. This was suggested by the fact that rain intensity seemed to predominate in certain rain fractions of a rain event, while washout processes seemed to predominate in other fractions of the same rain event. Contrary to the English /r/, which has been extensively analysed, there is very little information on the trilled consonants in Spanish. These sounds are in general difficult to produce for young Spanish children and occur later (than other consonant sounds) in normal development. This paper describes acoustic measurements made on the trill productions in Spanish children with varying degrees of speech intelligibility. The spectral (frequency, amplitude, C-V ratio) and temporal characteristics (duration of the trill, number of open and closed periods-apertures and occlusions-and duration of these periods) were studied in 45 children (Granada, Spain) between 3.0 and 9.6 years old, divided into 5 groups. The results reveal differences in spectral and temporal acoustic correlates of trill /r/ among the five speaker groups (e.g., the duration of the first aperture period was longer for the articulatory disordered group than for the normal control group). They seem to indicate, in particular, that children who have trouble learning to make the trill sound do little more than tap the alveolar ridge. Moreover, these children seem to use more of their tongues to make these gestures; also, they do not make many trills or make them very quickly. [ABSTRACT FROM AUTHOR]
