Abstract

症例は61歳の男性で，膵頭部癌に対して膵頭十二指腸切除術（Pancreatoduodenectomy；以下，PDと略記）を施行した．術後10日目に膵空腸吻合部ドレーンから大量出血を認め，出血性ショックとなった．血管造影検査で上腸間膜動脈（Superior mesenteric artery；以下，SMAと略記）根部に径1 cmの仮性動脈瘤を認め，同部からの出血と判明した．仮性動脈瘤がSMA根部にあったため，Interventional radiology（以下，IVRと略記）による止血は不可能と判断し，緊急手術を施行した．手術は残膵全摘，SMA縫合閉鎖，左大伏在静脈による右総腸骨動脈-SMAバイパス術を施行した．術後CTではSMA本幹からの血流はなく，腸管血流はグラフト経由で供給されていた．PD術後の仮性動脈瘤大量出血に対しIVR施行困難例では躊躇なく積極的な外科手術を考慮すべきと考えられた．

Abstract

症例は74歳の男性で，S状結腸癌の同時性多発肝転移に対してS状結腸切除術および拡大肝左葉切除術を施行した．14か月後，下大静脈腫瘍栓を伴う肝転移および腹膜播種を認めた．化学療法を開始するも効果なく，肺線維症を併発したため化学療法の継続を断念した．腹膜播種および肝転移に対する粒子線治療を希望し来院した．腫瘍は胃と近接しており粒子線による直接治療は不可能であったためスペーサー留置術＋粒子線照射による2段階治療を勧めた．まず，開腹下にスペーサーを留置し腫瘍と近接消化管の間に最低1.0 cmの距離を確保した．術後CT画像において安全な照射が可能であったため，手術1か月後より陽子線治療64 GyE/8 Frを施行した．治療後12か月時点で照射部腫瘍の再発は認めず，下大静脈腫瘍栓は消失した．本法は従来治療不能と考えられていた転移性肝癌に対して新たな治療手段になると考え報告した．

肝切除術中の高カリウム (K) 血症の発生頻度, 成因および臨床上の問題点を検討した.肝切除例64例のうち術中の血清カリウムの最高値が5mEq/L以上を示した高K群は7例 (11%) で, うち3例 (4.7%) が6mEq/L以上の上昇を認め, T波増高 (2例) や心室細動 (1例) などの心電図異常を来した.高K群, 非高K群 (5mEq/L未満) の背景因子を比較すると, 術前因子ではGOT, GPTが高K群で有意に高かった (p
