Introduction: This article is part of the Focus Theme of Methods of Information in Medicine on "Biosignal Interpretation: Advanced Methods for Studying Cardiovascular and Respiratory Systems"., Objectives: The aim of this study is to assess the reliability of the estimated Nocturnal Heart Rate (HR), recorded through a bed sensor, compared with the one obtained from standard electrocardiography (ECG)., Methods: Twenty-eight sleep deprived patients were recorded for one night each through matrix of piezoelectric sensors, integrated into the mattress, through polysomnography (PSG) simultaneously. The two recording methods have been compared in terms of signal quality and differences in heart beat detection., Results: On average, coverage of 92.7% of the total sleep time was obtained for the bed sensor, testifying the good quality of the recordings. The average beat-to-beat error of the inter-beat intervals was 1.06%. These results suggest a good overall signal quality, however, considering fast heart rates (HR > 100 bpm), performances were worse: in fact, the sensitivity in the heart beat detection was 28.4% while the false positive rate was 3.8% which means that a large amount of fast beats were not detected., Conclusions: The accuracy of the measurements made using the bed sensor has less than 10% of failure rate especially in periods with HR lower than 70 bpm. For fast heart beats the uncertainty increases. This can be explained by the change in morphology of the bed sensor signal in correspondence of a higher HR.

Background: Physiological sleep is characterized by different cyclic phenomena, such as REM, nonREM phases and the Cyclic Alternating Pattern (CAP), that are associated to characteristic patterns in the heart rate variability (HRV) signal. Disruption of such rhythms due to sleep disorders, for example insomnia or apnea syndrome, alters the normal sleep patterns and the dynamics of the HRV recorded during the night., Objectives: In this paper we analyze long-term and complexity dynamics of the HRV signal recorded during sleep in different groups of subjects. The aim is to investigate whether the calculated indices are able to capture the different characteristics and to discriminate among the groups of subjects, classified according sleep disorders or cardiovascular pathologies., Methods: Parameters, able to detect the fractal-like behavior of a signal and to measure the regularity and complexity of a time series, are calculated on the HRV signal acquired during the night. Different groups of subjects were analyzed: healthy subjects with high sleep efficiency, healthy subjects with low sleep efficiency, subjects affected by insomnia, heart failure patients, subjects affected by obstructive sleep apnea., Results: The evaluated parameters show significant differences in the groups of subjects considered in this work. In particular heart failure patients have significant lower entropy and complexity values, whereas apnea patients show an increased irregularity when compared with normal subjects with high sleep efficiency., Conclusions: This work proposes indices that can be used as global descriptors of the dynamics of the whole night and can discriminate among different groups of subjects.

Objectives: The aim of the work was to compare two different approaches - one model-dependent, the other data-dependent - for "deblurring" EEG data, in order to improve the estimation of Event-Related Desynchronization/Synchronization., Methods: Realistic Surface Laplacian filtering (SL) and Infomax Independent Component Analysis (ICA) were applied on multivariate scalp EEG signals (SL: 128 electrodes with MRI-based realistic modeling; ICA: a subset of 19 electrodes, no MRI) prior to beta Event Related Synchronization (ERS) estimation after finger movement in 8 normal subjects. ERS estimation was performed using standard band-pass filtering. ERS peak amplitudes and latencies in the most responsive channel were calculated and the effect of the two methods above was evaluated by one-way analysis of variance (ANOVA) and Sheffe's test., Results: Both methods and their combination significantly improved ERS estimation (greater ERS peak amplitude, p <0.05). The results obtained after ICA on 19 electrodes were not significantly different than the ones obtained with Realistic SL using 128 electrodes and MRI for scalp modeling (p >0.89)., Conclusions: The "low cost" of ICA (19 electrodes, no MRI) imposes such method as a valid alternative to SL filtering. The employ of ICA after SL filtering suggests that the "ideal EEG deblurring method" would unify the two approaches, depending on both the scalp model and the data.

During a visual-motor task the movement strategies and the learning processes are investigated. A group of 10 normal young volunteers underwent the experiment. The EEG signal was recorded through the 10-20 acquisition system during the execution of a task after a visual input. Each subject repeated the movement several times in three different conditions: i) without knowledge of the performance; ii) with visual feedback; iii) with knowledge of the result. The signal was transformed through Laplacian operator in order to eliminate the spurious coherence and then time-variant coherence was calculated. Different trends of the coherence function have been evidenced in subjects learning and not learning the better movement strategy. In particular, relations have been found between frontal, central and occipital electrodes in medium and high frequency ranges.

Spectral parameters extracted from the heart rate variability signal are obtained on a beat-to-beat basis by means of autoregressive recursive identification. In this paper a whale forgetting window is introduced, instead of the classical exponential one, in order to reduce the noise influence on the estimated parameters. After proper simulation it was found that the whale forgetting window markedly reduces the noise in the identification, but maintains a good response to abrupt changes in the signal. The algorithm was thus applied to the analysis of the HRV data recorded during different transient situations in physiological and pathological conditions. The spectral parameters were obtained on a beat-to-beat basis and their trends were smoother and more accurate with respect to the traditional exponential window also in presence of noise or artifacts in the time series (sudden and short time changes, ectopic beats, etc.), without losing the signal variations of physiological interest.

Spectral parameters extracted from the heart rate variability (HRV) signal are obtained on a beat-to-beat basis, following a procedure which uses two recursive algorithms. In the first step of the procedure the set of the AR model coefficients is updated each time a new RR value is available. Then from the estimated AR model parameters, the new position of the poles of the model transfer function in the complex z-plane is evaluated and, finally, through a residual calculation, it is possible to calculate the spectral parameters which quantify the control of the autonomic nervous system in assessing the cardiac frequency (i.e., power and frequency of LF and HF components). The whole procedure has first been tested on a simulated time series, in order to evaluate its performance in tracking the dynamic changes during different conditions; next the algorithms were employed in the study of the HRV signal for continuous monitoring of non-stationary conditions.
