Background: Professional soccer athletes are exposed to repetitive head impacts and are at risk of developing chronic traumatic encephalopathy., Objective: To evaluate regional brain glucose metabolism (rBGM) and gray matter (GM) volume in retired soccer players (RSPs)., Methods: Male RSPs and age and sex-matched controls prospectively enrolled between 2017 and 2019 underwent neurological and neuropsychological evaluations, brain MRI and [18F]FDG-PET in a 3.0-Tesla PET/MRI scanner. Visual analysis was performed by a blinded neuroradiologist and a blinded nuclear physician. Regional brain glucose metabolism and GM volume were assessed using SPM8 software. Groups were compared using appropriate statistical tests available at SPM8 and R., Results: Nineteen RSPs (median [IQR]: 62 [50-64.5] years old) and 20 controls (60 [48-73] years old) were included. Retired soccer players performed worse on mini-mental state examination, digit span, clock drawing, phonemic and semantic verbal fluency tests, and had reduced rBGM in the left temporal pole (pFDR = 0.008) and the anterior left middle temporal gyrus (pFDR = 0.043). Semantic verbal fluency correlated with rBGM in the right hippocampus, left temporal pole, and posterior left middle temporal gyrus (p ≤ 0.042). Gray matter volume reduction was observed in similar anatomic regions but was less extensive and did not survive correction for multiple comparisons (pFDR ≥ 0.085). Individual [18F]FDG-PET visual analysis revealed seven RSPs with overt hypometabolism in the medial and lateral temporal lobes, frontal lobes, and temporoparietal regions. We report results of a magnetic ressonance imaging (MRI) study of 146 Brazilian children, whose intelligence quotient scored less than 70. 50% of MRI examinations did not exhibit any signal of structural lesion (N group), whereas a focal thinning at the junction of the body and splenium of the corpus callosum; ventricular asymmetry; periventricular leukomalacia; gliosis and arachnoid cysts were among the most frequent findings in the remaining of subjects (L group). Maternal stress and altered blood pressure were the most frequent findings in the pre-natal history of both N and L children. Familial antecedents of mental deficiency were reported in 30% of both groups, whereas familiar history of alcoholism was important in N group (60% in N versus 0% in L groups). Neuropsychomotor development was delayed in 80% of the children in both groups. Aggressiveness is the most frequent finding in the post-natal children history.

Background/aims: Splenectomy with esophagogastric devascularization and distal splenorenal anastomosis are used for the treatment of bleeding esophageal varices in Mansoni's hepatosplenic schistosomiasis. Portal thrombosis followed by ascitis has been observed in the early postoperative phase, but there are no studies about the spontaneous and late postoperative prevalence of this vascular complication. The aim was to evaluate the spontaneous and late postoperative prevalence of total portal vein thrombosis and of ascitis in these patients., Methodology: US-Doppler examination was performed on 168 patients divided in three groups: 1) 92 non-operated (preoperative); 2) 62 after splenectomy with esophageal devascularization, 3) 14 after distal splenorenal anastomosis. The presence of ascitis was sought in all., Results: Group 1 had 5.43% (5/92) of spontaneous total portal vein thrombosis, groups 2 and 3, presented 19.35% (12/62) and 50.00% (7/14) in the late postoperative follow-up, respectively. The Fisher's test showed that group 3 had higher incidence of this vascular occlusive complication (p = 0.038). No ascites were found., Conclusions: Surgical treatment of portal hypertension boosts the natural tendency to total portal vein thrombosis in this disease. The difference in the occurrence of this vascular complication during the late postoperative phase suggests the existence of a hemodynamic factor determined by the surgical technique.

Background/aims: The purpose of this study was to describe the echographic features of Ascaris lumbricoides invasion of the alimentary tract, biliary system, liver and pancreas., Methodology: We studied 38 patients with roundworm heavy infection whose diagnosis were primarily supported by ultrasonographic examination., Results: Thirty-seven patients were admitted with the following clinical complications: bowel obstruction, intra and extrahepatic dilatation, acute cholecystitis, intrahepatic abscess, acute appendicitis, subphrenic collection and acute pancreatitis. Ultrasound scanning was able to recognize in 30 cases the echogenic, nonshadowing images of the worms as single or multiple strips; in 8 cases, coiled structures and amorphous material were detected. The "inner tube" sign was considered as the most specific one. In two cases the roundworms were displayed within the stomach, a curious finding, up to now never reported in medical literature., Conclusions: Real-time sonography represents an efficient, reliable and non-invasive diagnostic approach for hepatobiliary, enteric and pancreatic ascariasis.

Pre-surgical evaluation of epileptic patients consists of neurological examination, intensive electroencephalographic (EEG) monitoring and anatomical studies (CT and MRI). Functional methods such as PET and SPECT imaging are now used more frequently. We have studied pre-operatively 15 adult epileptic patients (8 female, 7 male) using a rotational scintillation camera interfaced to a dedicated computer. The tomographic images were obtained 15 minutes after intravenous injection of 99mTc-HMPAO. All had MRI scanning and intensive EEG monitoring which generally included seizure recording. Five patients had progressive lesions (3 meningiomas, 2 astrocytomas). In 10 patients, neuroradiological studies did not show the presence of progressive lesions (2 normal scans and 8 cases with inactive lesions). Two patients with meningioma showed hypoperfusion at the lesion site while the third patient had a marked hyperperfusion which might correlate with the clinical diagnosis of epilepsia partialis continua. In the astrocytoma patients SPECT scans showed hypoperfusion at the lesion site. Data obtained from the 10 patients without progressive CNS lesions showed: (a) in 4, SPECT findings correlated well with the anatomical findings; (b) in 5 instances, SPECT was able to disclose additional functional deficits; (c) in one case, there was no SPECT correlate of a discrete anatomical lesion. In 5 of these cases with no progressive lesions (n = 10) SPECT findings were useful as a complementary tool in determining the clinical or surgical management of these patients. Despite the small number and heterogenicity of the present sample, SPECT seems to be an useful tool as part of the clinical workup of epileptic patients who are candidates for epilepsy surgery.

The ultrasonographic study of 33 patients with hepatolenticular degeneration showed the following main changes: disorders of the hepatic echotexture (29 cases), changes of the splenic dimensions (21), liver shrinkage (10), cholelithiasis (8), hepatomegaly and ascites (1). The disorders of liver echotexture exhibited different patterns, from slight to severe changes of the hepatic echogenicity, associated with anatomic distortions of the liver, such as alterations of outline and decrease of dimensions. Liver shrinkage was always accompanied by splenomegaly. The scarcity of hepatomegaly may be explained by the fact that the ultrasonography was performed after the onset of treatment with penicillamine, or by other factors still unknown, copper perhaps possessing a stronger fibrogenetic action than other hepatotoxic agents. Cholelithiasis was very frequent in females (6 of 13 patients), its incidence tending to increase with age. Regarding males there was no increase when the incidence was compared to the general population. The two sole males were young, an unusual finding in normal men.
