O envelhecimento do sistema imune est? entre os tr?s fatores mais importantes que influenciam a s?ndrome da fragilidade. A determina??o do comprimento telom?rico pode ser considerada um marcador de envelhecimento biol?gico. A reativa??o herpesv?rus humano tipo 6 (HHV-6) alcan?a a lat?ncia por integra??o na extremidade subtelom?rica dos tel?meros. A integra??o do genoma do HHV- 6 na regi?o telom?rica pode afetar no encurtamento e/ou na manuten??o do comprimento telom?rico. Objetivo: Investigar a influ?ncia da reativa??o do HHV-6 no comprimento telom?rico relativo e associa??o com a s?ndrome de fragilidade em idosos. M?todos: Estudo observacional, transversal, em amostras provenientes de um estudo multic?ntrico, realizado em idosos. A caracteriza??o da amostra foi realizada por meio de dados cl?nicos e sociodemogr?ficos. O ?cido desoxirribonucleico (DNA) foi extra?do do plasma usando protocolos padr?o, o comprimento dos tel?meros e o HHV-6 foram determinados atrav?s da rea??o em cadeia da polimerase (PCR) quantitativo em tempo real. A fragilidade foi classificada de acordo com fen?tipo proposto por Fried et al. (2001). O coeficiente de correla??o de Spearman foi utilizado para analisar a correla??o entre o comprimento dos tel?meros e a carga viral para o HHV-6. O teste Kruskal-Wallis foi usado para verificar se havia diferen?as significativas entre as categorias de grau de fragilidade: n?o-fr?gil, pr?-fr?gil e fr?gil. O teste de Mann Whitney para an?lise grupo a grupo. O n?vel de signific?ncia aplicado foi ?=0,05 para todas as vari?veis investigadas. Resultados: Neste estudo, o comprimento telom?rico relativo foi menor conforme aumenta a idade, entretanto, o comprimento telom?rico n?o foi diferente entre indiv?duos positivos ou negativos para HHV-6 e, tamb?m, n?o foi correlacionado com o grau de fragilidade e nem com a carga viral. A presen?a de HHV-6 no plasma foi frequente em idosos (~=40%) e a carga viral das amostras dos idosos foi correlacionada com o grau de fragilidade. Idosos classificados como fr?geis e pr?-fr?geis apresentaram carga viral pelo HHV- 6 significativamente maior (m?dia=12.597 c?pias/mL e 3.564 c?pias/mL, respectivamente) quando comparado com idosos n?o-fr?geis (m?dia=218 c?pias/mL) e o padr?o se manteve ap?s o ajuste por sexo. Houve uma maior frequ?ncia de idosos pr?-fr?geis com poucos fr?geis. Este ? o primeiro estudo que analisou a rela??o entre o comprimento telom?rico com a fragilidade e com a infec??o pelo HHV-6. S?o necess?rios outros estudos que investiguem esta tr?ade com n?mero amostral maior e mensura??o dos tel?meros em subtipos de c?lulas. Disserta??o (Mestrado) ? Frailty syndrome is characterized as a clinical condition of vulnerability to stress factors which decline the physiological reserves. The aging of the immune system is among the three most important factors which influence the frailty syndrome. The determination of telomere length can be considered a biological aging marker. Human herpesvirus type 6 (HHV-6) turns latent by integration at the subtelomeric end of telomeres. The integration of the HHV-6 genome into the telomeric region may affect the shortening and/or the maintenance of telomeric length. Aims: To investigate the influence of HHV-6 reactivation on the relative telomere length and association with frailty syndrome in the elderly. Methods: It was an observational, cross-sectional study including samples from a multicenter study. The sample was characterized by clinic and socio-demographic features. Deoxyribonucleic acid (DNA) was extracted from plasma using standard protocols, telomere length and HHV-6 were determined by real-time quantitative polymerase chain reaction (PCR). Frailty was classified according to criteria proposed by Fried et al. (2001). Spearman's correlation coefficient was used to analyze the correlation between telomere length and viral load for HHV-6. The Kruskal-Wallis test was used to check for significant differences between the categories of degree of frailty: non-frail, pre-frail and frail. Mann Whitney test for group-by-group analysis. The level of significance applied was ?=0.05 for all variables investigated. Results: In this study, the relative telomere length was shorter with increasing age, however, telomere length was not different between HHV-6 positive or negative individuals and it was also not correlated with the degree of frailty or viral load. The presence of HHV-6 in plasma was frequent in the elderly (~=40%) and the viral load in samples was correlated with the frailty degree. Elderlies classified as frail and pre-frail had a significantly higher HHV-6 viral load (mean=12,597 copies/mL and 3,564 copies/mL, respectively) when compared to non-frail elderly (mean=218 copies/mL) and the pattern remained after adjustment for sex. There was a higher frequency of pre-frail elderly with few frails. This is the first study that analyzed the relationship between telomere length and frailty and HHV-6 infection. Further studies are needed to investigate this triad with a larger sample size and measurement of telomeres in cell subtypes.
