This thesis explores how Australian gay men negotiate their regular relationships, specifically in reference to coupled monogamy and non-monogamy and enacting HIV prevention strategies. The data on which this thesis is based were collected during a period of change in the socio-political and HIV prevention landscapes in Australia. Same-sex marriage was under consideration politically and biomedical HIV prevention was becoming more widely available and used among gay men. In consideration of these changes, it has become increasingly important to rethink understandings of gay men’s regular relationships and the contexts in which they are negotiated and experienced. This thesis draws on three data sets. The first utilises semi-structured interviews with Australian gay men to explore their understandings of the enactment of monogamy in relationships. The second utilises semi-structured interviews with men in serodiscordant relationships (those of mixed HIV status) to explore how understandings of HIV and risk affect practices within relationships. The third was a cross-sectional survey exploring understandings and beliefs about relationships among Australian gay men. The findings show that gay men’s relationships are unique and complex, both mirroring and departing from broad normative understandings of relationships. In particular, non-monogamy is common among gay men’s relationships and gay men may at some point be challenged to consider alternatives to monogamy, potentially reshaping their values regarding relationships. Such challenges present tensions and opportunities regarding how men negotiate fidelity and intimacy with relationship partners. In the context of these negotiations, biomedical prevention provides opportunities for gay serodiscordant couples to reconsider the meaning of the HIV to their lives, including how they practice HIV prevention. The findings also demonstrate diversity in relationship types that have implications for how research captures data about relationships. This thesis contributes a renewed understanding of how gay men understand and negotiate regular relationships in the context of substantial changes in Australia, both in how their relationships are perceived socially and politically, and in how HIV prevention is enacted.

Migration of Chinese-identified people is an emerging area of enquiry. However, sexuality and health among the same-sex attracted ‘Chinese diaspora’ is still a highly under-research field. In the context of sexual health research, Chinese-identified people living outside the national border of China are often homogenised as part of the umbrella group ‘Asian’, and together are commonly stereotyped as the somewhat passive victims of racism and health inequalities. Few studies to date have taken into account the ripple effects of migration and globalisation in understanding the diverse sexual health experiences of Chinese diasporic people living in Western countries. To address this gap, an in-depth qualitative study was conducted in Sydney, Australia, to understand Chinese diasporic gay men’s health and sexual experiences. Two rounds of semi-structured in-depth interviews were conducted with twenty-two Chinese- and gay-identified men recruited to the study. In order to develop a better understanding of the implications of sociocultural, temporal and spatial factors on men’s lived experiences, insights from intersectionality theory, Mannheim’s concept of social generations, and assemblage theory were utilised. Data were analysed interpretatively and thematically to describe men’s lived experiences and to identify key issues focused upon in their accounts. This thesis provides original insights to the understanding of the complex experiences of Chinese diasporic gay men living in Australia. Study findings emphasise the importance of shifting the focus of attention from the racial oppression of diasporic subjects to a more dynamic and relative understanding of the intersection between race/ethnicity, migration, sexuality and health. In the contemporary context of migration, Chinese diasporic gay men’s lives are highly transnational, and their experiences are transgressive of existing racial and national boundaries. Moreover, there exists a strong generational aspect to men’s experiences with HIV and AIDS that is not only constituted by their experience in Australia, but also by the time of migration and their memories of the disease in their homeland. Most importantly, men’s experiences are constantly changing and transforming, and cannot be fully understood in terms of static conceptions of culture, sexuality, race/ethnicity and health. Theorisations of gay diasporic men’s health needs constantly to be revisited and refined.

In Australia,cases of HIV by the year 2020 is encapsulated through the Ending HIV campaign, with the ambitious aim of the virtual elimination of HIV transmission in Australia. The experiences of individuals recently diagnosed with HIV offer unique insight into the contexts in which HIV is transmitted. In this thesis, I examined the experiences of gay men in Australia who had recently been diagnosed with HIV using a mixed method approach of online cross-sectional surveys and in-depth interviews. The technique of using an online platform to collect data from individuals newly diagnosed with HIV provided reliable and valuable results. Through the lens of their experiences, I reflect on how the Ending HIV community engagement platform addresses current challenges to HIV prevention. Perhaps unsurprisingly, the men in our sample did not appear to be applying non-condom based HIV risk reduction strategies prior to their diagnosis. Most men attributed their HIV infection to a partner with whom they had little prior acquaintance. Prior to their against HIV transmission. Initiating antiretroviral therapy (ART) presented a difficult choice for some men, but those who were more connected to peers had greater faith in the benefits of early initiation of ART. Together, these analyses suggest that gay community, peer support, and the personal and social connections between gay men, remain essential components of HIV prevention in Australia.

We are arguably at a critical point in the history of the HIV epidemic. HIV prevention has entered a new era with the emergence of biomedical interventions such as ‘treatment as prevention’ (TasP). However, the dialogue related to these developments is polarised. Terms like ‘revolution’ have been used by some leaders in the field to forecast the end of the epidemic and this optimism is influencing the development of prevention strategy. Yet TasP has also generated criticism, particularly the perception that the public health benefits of treatment are being prioritised before individual well-being. Significantly, people living with HIV have been largely absent from the public discourse on TasP, with little opportunity to discuss their views about its potential impact on their lives. In response to this absence, the study for this thesis explored the perceptions that HIV-positive gay and bisexual men had towards TasP when the debate was beginning to influence HIV prevention strategy in Australia. A qualitative methodology was employed, which involved interviews with 22 HIV-positive gay and bisexual men in New South Wales. A modified interpretative phenomenological analysis was used to examine the data, an approach which explores how participants make sense of their world and give meaning to their experiences (Smith and Osborn, 2003). By exploring perceptions of TasP, this thesis has generated new insights into HIV-positive gay and bisexual men’s relationships with HIV treatment and their feelings about the role of treatment in HIV prevention. Trust in treatment’s effects on health was stronger than trust in its role in prevention, and most men still perceived themselves as potentially infectious despite TasP’s message that treatment reduces infectiousness and the risk of HIV transmission. However, there was also an anticipation of the potential opportunities that TasP might present for serodiscordant relationships. The implications of the study are discussed in terms of the complex challenges and opportunities that TasP presents for HIV-positive gay and bisexual men and what these mean for the HIV sector. The thesis also underlines the critical need to engage with those individuals most affected by this prevention technology who are instrumental to its success.

The aim of this study was to explore how living with HIV and being a migrant from an ethnic minority background interact and how these contingencies intersect with gender and sexual orientation in shaping the lives of individuals. It was a follow-up study to an earlier study which investigated the interrelationship of living with HIV and belonging to culturally and linguistically diverse groups outside the Anglo-Australian mainstream., Monograph 1/2013

This thesis explores how kinship is enacted in the context of gay men pursuing parenthood through surrogacy. These men represent an important first generation to access assisted reproductive technologies for this purpose and also make up an increasing proportion of gay men becoming parents outside a heterosexual relationship. Data were collected from three sources: surrogacy stories in the print media; websites of surrogacy agencies; and in-depth interviews with 30 gay men living in Australia and the United States, who had achieved or were pursuing parenthood through surrogacy. The analytic approach is influenced by insights from the field of science and technology studies (STS). The main findings relate to parenthood desire, biogenetic connection, and value. These gay men ‘enacted’ parenthood and family life in ways that both challenged and reinforced dominant notions of kinship and masculinity. Like most people, they had grown up with parenthood expectations, although these had been challenged when they assumed a gay identity. Parenthood desires were re-animated by social discourses that emphasised the prospect of parenthood and procreation for gay men as a conceptual and material reality through the influence of partners, media, peers, and in particular, the promotional activities of surrogacy agencies. The men also reworked the symbols of Western kinship—such as bilateral genetic inheritance—to create a family structure that felt meaningful. A range of inventive strategies was employed in this process both to instantiate and obscure conventional ideas about biology and kinship. The findings demonstrate that parenthood desire is socially produced: men come to experience parenthood desire largely because of the new narratives and opportunities being made available to them today. The creative and strategic negotiation strategies of gay men in family formation also demonstrate that biogenetic connection is an important and potentially flexible fact to be negotiated. The study contributes to the growing field of international research on the many complex and unexpected ways in which parenthood desires are informed by contemporary understandings of gender and sexual identity, but also the multiple ways in which the social construction of kinship influences the experience of desiring and conceiving a ‘child of one’s own.’

For many gay, bisexual and other men who have sex with men, the Internet is a central aspect of their sexual and romantic lives. Although often trumpeted as a convenient way to meet people, online sex and dating present unique challenges for those looking to connect. Among these is how racial concepts are used to define desire or disinterest, which in Australia is a contentious issue among men who look for sex or dates online. Although recent media attention has propelled what is sometimes called online sexual racism into mainstream debates, very little is known about this concept, its potential impact or men s perceptions of it. This thesis addresses that gap by exploring concepts of race and racism on sex and dating webservices for same-sex attracted men. A mixed methods approach was adopted, which consisted of a content analysis of sex and dating profiles posted online, a national online survey of gay and bisexual men in Australia, and in-depth interviews with gay men who use sex and dating webservices. The findings across these methods suggest a diversity of attitudes exists towards race online, accompanied by individual and often situationally-dependent understandings and expressions of racism and online etiquette. The ways in which men talk about race and their online practices differ among racial groups and reflect broader Australian discourses of race. There seems to be, however, interest among some of the gay men who use these webservices to approach concepts of race (and racism) from a critical position. Nevertheless, widely differing opinions and experiences related to race remain, as does the need for further work to engage men and ask important questions about the role and impact of race among these online communities. Although many men are troubled or conflicted about ideas of race and racism as they intersect with their search for sex or romance, the overwhelming response to this as an issue was one of resignation. This thesis provides a first analysis of the complex tension between ideals of sexual liberty, the normalisation of racial prejudice, and the increasingly important online social spaces that are negotiated by gay and bisexual men.

This study explores the role of alcohol and drug use on the diagnosis and management of depression among gay men attending Australian general practice clinics. The study was conducted with the goal of assisting GPs to address alcohol, drug use and depression in gay men and to add to the knowledge of these issues in the general practice setting. Three research analyses were made from the study relating to patterns of alcohol- and other drug-use associated with depression, comparison of doctor and patient beliefs about drug use and depression, and the impact of alcohol and other drug use on the agreement between doctors’ assessments of major depression and patients’ scores in depression screening tools.

This thesis is a qualitative study investigating the communication about sexual health between HIV-positive gay men and their doctors and other health care providers. The study was conducted in 2007 and 2008 in Sydney, Australia. Data was collected through in-depth interviews with eleven HIV-positive gay men, and analysed according to a modified grounded theory approach. The study aims to explore the men’s needs, expectations and experiences in relation to how sexual health is addressed in clinical consultations. While previous research has documented that HIV/AIDS is a diagnosis that poses significant physical, emotional and social challenges for People Living With HIV/AIDS (PLWHA) which may impact on their sexual health and well-being, this thesis examines whether, and to what extent this is communicated between HIV-positive gay men and their doctors. In doing so the study identifies and describes the ways in which HIV-positive gay men understand sexual health, how they currently talk to their clinicians about sexual health issues, the priority that such issues have for these men, and barriers and ‘incentives’ to sexual health being discussed within HIV treatment settings. The study reveals that the men’s communication about sexual health with their doctors is essentially limited to discussions about sexually transmissible infections (STIs), which represent a common understanding of what constitutes sexual health. The emotional and social aspects of sexual health and well-being are rarely discussed in doctor-patient communication. Instead, the men rely on sources outside the health system, such as friends, peers, partners, gay and HIV community organizations and the gay and HIV media for sexual health information, advice and support. This indicates that broad, holistic sexual health has a low priority in discussions between patients and their doctors, and that social networks therefore play an important role in how HIV-positive gay men take care of their sexual health.

Male couples place the issue of extra–relational sex (ERS) in their committed relationships at various points on a continuum between monogamy and non-monogamy. Further, many couples move about on this continuum over the course of the relationship, according to their changing wishes and desires. Making the right choice or transitioning from one relationship style to another is not always straightforward and unproblematic. Some male couples may benefit from therapeutic support at these crucial times. This research seeks to contribute to knowledge that could inform sensitive and respectful practice on the part of therapists and others who work with the relational concerns of gay men. Using qualitative methodology the research questions explored how gay men negotiate the place of ERS with their partners, the strategies they use to manage ERS, and their beliefs about the effects of their choices on themselves and their relationship. A purposive sample of 24 Sydney gay men (six single and 18 partnered) aged between 22 and 68 participated in the research. Data were collected through semi–structured interviews and subjected to thematic analysis. Despite diverse experiences of ERS, a core set of processes appeared central to relationship satisfaction and function. The knowledge produced by this research provides a number of ways for therapists to orient themselves to the therapeutic terrain of work with gay men and ERS while remaining alive to the unique and particular characteristics of the male couple relationship before them.

Commencing in April 2006 this three-year-project adopted a comprehensive and multi-method approach to investigate the prevalence, nature, clinical management and self-management of depression among men, particularly homosexually active men, attending HIV-caseload general practice clinics.

This report describes key findings from the e-male survey, a national, online survey of men who have sex with men (MSM) in Australia. The Qualitative Interviews Concerning Key Issues and Experiences (QUICKIE) project was a qualitative study that investigated key aspects of gay men's lives in Sydney. Sexually active gay men in Sydney were interviewed in 2007 and 2008 about a range of topics, including their use of licit and ilicit substances. During analysis of mens accounts of condom use, drug use and sexual activity, it was noted that a number of me described using Viagra to maintain condom use and/or counteract the adverse effects of illicit drugs on erectile funtion.

This project critically examines the question Are HIV/AIDS prevention campaigns more effective at engaging gay men if they use colloquial language and sexually explicit imagery? by investigating the impact of sexually explicit campaigns on the Sydney gay community. The project approaches the question in three parts: 1. A review of literature exploring the circumstances in which and for what reasons sexually explicit HIV/AIDS campaigns are produced. 2. An analysis of seven interviews with producers of HIV prevention campaigns exploring how, why, where and for what reasons explicit campaigns are produced. 3. An analysis of eight interviews with homosexually active Sydney men exploring how HIV prevention campaigns are viewed, consumed and understood. The interviews were in-depth, semi-structured and conducted over one hour per interviewee. The findings are dominated by two interrelated and at times conflicting themes. First, that there is a strongly held belief by campaign producers that the most effective HIV prevention campaigns targeting gay men are produced by the community and reflect that community and as a result must at times be sexually explicit. Second, that it is no longer sufficient for HIV prevention campaigns to rely on explicit sex to attract attention. In an environment where visual images, and sexually explicit visual images especially, are becoming more accessible, and media and communication is more prevalent and complex, safe sex campaigns are forced to deliver ever more sophisticated and stimulating creative materials in order to maintain the engagement of gay men. The interviews revealed that both campaign producers and consumers participated in the production of a visual literacy of safe sex campaigns. This literacy was necessary to both effectively produce and comprehend the campaigns. The interviews showed that while HIV prevention campaigns that use sexually explicit language and imagery can be highly effective at engaging gay men, the success of sexually explicit campaigns is contextual that is, dependent on where, to whom and in what circumstances the materials are delivered. The audience’s accumulation of knowledge and cultural experience when viewing the campaigns, in other words their safe sex campaign literacy, significantly influenced their capacity to understand, appreciate and be engaged with sexually explicit HIV prevention campaigns.
