Bone margin analysis in cases of osteomyelitis, osteoradionecrosis, and medication-related osteonecrosis of the jaw is a controversial topic. There is little evidence to guide treatment and the interpretation of bone margin results. This article examines the significance of margin status and any possible effect on progression of the disease process. A review of various treatment adjuncts used for intraoperative margin analysis during removal of affected tissue is provided. Literature on the role of imaging is also discussed with regards to treatment planning for surgical resection. The histology of the three separate entities including the approach to surgical and pathologic evaluation of margins is also discussed., (Copyright © 2017 Elsevier Inc. All rights reserved.)

Purpose: Surgery of irradiated tissue has an increased complication rate because of the development of hypovascular, hypocellular, and hypoxic tissue. This study was undertaken to perform histopathologic and histomorphometric analyses of irradiation tissue injury in bone and the surrounding soft tissues., Material and Methods: The histopathologic findings of 40 human mandibular bones and the surrounding soft tissue specimens obtained from different patients who underwent surgical procedures for treatment of osteoradionecrosis of the jaws were reviewed., Results: Histopathologic examination showed 7 processes in the following order of appearance: hyperemia, endarteritis, thrombosis, cell loss, hypovascularity, increase of fat in the bone marrow cavity, and fibrosis. Histomorphometric analysis showed significant hypocellularity (P = .007), hypovascularity (P < .001), and fibrosis (P < .001) in irradiated specimens compared with control specimens., Conclusion: These results showed that radiation injuries affect the bone and surrounding soft tissues. However, the irradiation-induced injuries, such as cellular loss (hypocellularity) and fibrosis, were more expressive in bone tissue than in the surrounding soft tissues., (Copyright © 2016 American Association of Oral and Maxillofacial Surgeons. The purpose of this report is to describe a case of osteonecrosis that occurred after arthroscopic meniscectomy associated with a radiofrequency device. The patient developed increasing knee pain and disability 6 months after arthroscopic meniscectomy. Magnetic resonance imaging showed subchondral osteonecrosis of both femoral condyles with cyst formation.

Because the diagnosis of mandibular ORN can often essentially be based on clinical grounds, radiology is used for confirmation and evaluation of the extent of the bone involvement. The localization and extent of the bone destruction can be better evaluated with CT than with conventional occlusal or panoramic films. Nevertheless, plain films often provide sufficient information for patient management. Based on the clinical evaluation and plain imaging findings, a decision can be made to treat patients conservatively or surgically.

Purpose: This study was undertaken to evaluate a new method in the treatment of mandibular osteoradionecrosis., Patients and Methods: Eight patients, seven male and one female, with a mean age of 64 years (range, 43 to 67 years), suffering from osteoradionecrosis of the mandible, two bilaterally and six unilaterally, were treated. Five initially had hyperbaric oxygen (HBO) followed by sequestrectomy, and three had sequestrectomy alone. Because healing failed to occur, all patients were treated by removal of the necrotic bone in the affected part of the mandible and filling the defect with compressed particulated cancellous bone and marrow from the tibia. The patients were observed for an average period of 39 months (range, 20 to 93 months)., Results: Primary healing was achieved in two patients with unilateral osteoradionecrosis and HBO treatment. In another patient treated with HBO primary healing occurred on one side while the other healed secondarily. In the remaining five patients, complete secondary healing took place, but it was complicated in three patients by fistulas, two of which were associated with fractures of the mandibular body. The fistulas were excised, and complete healing of the soft tissues occurred, but the fractures resulted in pseudarthrosis., Conclusions: The technique presented in this study can be useful in the treatment of osteoradionecrosis as an alternative to continuity resection and reconstruction with free osteocutaneous flaps, but, whenever possible, it should be proceeded by HBO treatment.

Osteoradionecrosis is a severe complication of radiation therapy for cancer. Prevention of osteoradionecrosis is most important as the condition may be chronic, progressive and lead to pathologic fracture. The clinical experience at the Cancer Control Agency of British Columbia with 26 cases in approximately 1000 patients treated with radical radiation therapy to the head and neck region is presented and discussed. A clinical classification system is proposed to provide a guide for treatment selection, and to allow classification for research purposes.
